Abstract:STUDY OBJECTIVES:This study evaluated differences in upper airway, soft tissues and craniofacial structures between Asians from China and Europeans from Iceland with OSA using three-dimensional magnetic resonance imaging (MRI).METHODS:Airway sizes, soft tissue volumes, and craniofacial dimensions were compared between Icelandic (N = 108) and Chinese (N = 57) patients with oxygen desaturation index (ODI) ≥ 10 events/h matched for age, gender, and ODI. Mixed effects models adjusting for height or BMI and residual differences in age and ODI were utilized.RESULTS:In our matched sample, compared to Icelandic OSA patients, Chinese patients had smaller BMI (p < 0.0001) and neck circumference (p = 0.011). In covariate adjusted analyses, Chinese showed smaller retropalatal airway size (p ≤ 0.002), and smaller combined soft tissues, tongue, fat pads, and pterygoid (all p ≤ 0.0001), but male Chinese demonstrated a larger soft palate volume (p ≤ 0.001). For craniofacial dimensions, Chinese demonstrated bigger ANB angle (p ≤ 0.0196), differently shaped mandibles, including shorter corpus length (p < 0.0001) but longer ramus length (p < 0.0001), and a wider (p < 0.0001) and shallower (p ≤ 0.0001) maxilla.CONCLUSIONS:Compared to Icelandic patients of similar age, gender and ODI, Chinese patients had smaller retropalatal airway and combined soft tissue, but bigger soft palate volume (in males), and differently shaped mandible and maxilla with more bony restrictions. Results support an ethnic difference in upper airway anatomy related to OSA, which may inform targeted therapies.
[Localization of Upper Airway Stricture by CT Scan in Patients with Obstructive Sleep Apnea Syndrome During Drug-Induced Sleeping].

Abstract:OBJECTIVE:To evaluate the feasibility of multi-slice spiral CT scan to localize upper airway stricture in patients with obstructive sleep apnea syndrome (OSAS) during drug-induced sleeping.METHODS:One hundred and fourteen patients diagnosed as OSAS by polysomnography were included in the study. Multi-slice spiral CT scan covering upper airway was performed at the end of inspiration and clear upper airway images were obtained in waking. After injecting 5 mg of midazolam intravenously slowly in 109 patients, CT scan was performed at apnea and clear upper airway images were obtained in sleeping. Cross-section area and minimal diameter of airway were measured and the parameters were compared under those two states. Upper airway was displayed intuitionisticly by using post-processing techniques.RESULTS:One hundred and nine patients with OSAS finished the examination with a success rate of 100 %. Airway obstruction at retropalatal level was observed in 62 patients, among whom 26 were associated with airway obstruction at retroglossal level, 27 with narrower airway at retroglossal level in sleeping compared with that in waking, and 9 with no significant change of the airway at retroglossal level after sleeping. Narrower airway at retropalatal level in sleeping compared with that in waking was observed in 40 patients, among whom 20 were associated with narrower airway at retroglossal level in sleeping compared with that in waking, 10 with complete airway obstruction at retroglossal level in sleeping, and 7 with no significant change of the airway at both retropalatal and retroglossal levels before and after sleeping. Minimal mean cross-section area of airway at retropalatal level was (72.60 +/-45.15)mm(2) in waking and (8.26 +/-18.16)mm(2) in sleeping; and minimal mean cross-section area of airway at retroglossal level was (133.21 +/-120.36)mm(2)in waking and (16.73 +/-30.21)mm(2) in sleeping (P <0.01). Minimal mean diameter of airway at retropalatal level was (6.91 +/-2.23) mm in waking and (1.18 +/-2.14) mm in sleeping; and minimal mean diameter of airway at retroglossal level was (8.68 +/-4.32) mm in waking and (1.68 +/-2.22) mm in sleeping (P <0.01).CONCLUSION:Multi-slice spiral CT with post-processing techniques can display the shape of the upper airway in patients with OSAS in sleeping, and can localize the upper airway stricture and assess its range accurately.
A comparison study of upper airway among different skeletal craniofacial patterns in nonsnoring Chinese children.

Abstract:Objective: Patients with OSAS (obstructive sleep apnea syndrome) demonstrate decreased upper airway dimension and craniofacial skeletal abnormalities. The study was performed to analyze whether upper airway dimensions differed among Chinese nonsnoring children of different sagittal and vertical skeletal facial morphologies. Materials and Methods: Lateral cephalometric records were used to measure the dimensions of the upper airway. Two groups of subjects were studied. A group of subjects with a normodivergent facial pattern (n = 190; FH-MP angle between 23.5 degrees and 30.5 degrees) was divided into three subgroups according to ANB angle (Class I, II, or III). A second group of subjects with a normal sagittal facial pattern (n = 180; ANB angle between 0.7 degrees and 4.7 degrees) was divided into three subgroups according to the FH-MP angle (low angle, normal angle, or high angle). All subgroups were matched for age and sex. Results: In the group of subjects with a normodivergent facial pattern, a significant tendency for reduced upper airway dimension in the inferior part (palatopharyngeal and hypopharynx) was found in the Class III, Class I, and Class II subgroups, in that order. In the group of subjects with a normal sagittal facial pattern, the superior part of the airway (nasopharyngeal and palatopharyngeal) decreased with increasing mandibular plane angle. Conclusion: The sagittal and vertical skeletal patterns may be contributory factors for the variation of the inferior and superior part of the upper airway, respectively. Skeletal deficiency of nonsnoring Chinese children may predispose them to upper airway obstruction. (Angle Orthod. 2010;80:267-274.)

Abstract:OBJECTIVES:We identified the primary symptoms leading to Chinese patients presenting at hospital with suspected obstructive sleep apnea (OSA) and studied the prevalence and characteristics of OSA in confirmed cases. METHODS:We collected data on 350 consecutive patients (302 males and 43±11 years old) with suspected OSA who underwent overnight polysomnography (PSG). RESULTS:Among all patients, rankings of primary symptoms that led to the patients presenting at hospital for PSG were observed apnea (33%), snoring alone (29%), choking/gasping (13%), daytime sleepiness (5%) and other (20%). For severe OSA, prevalence rate was 61%, apnea hypopnea index (AHI) was 64±18, age was 44±10 years old, body mass index (BMI) was 28±3.5 kg/m(2), and hypertension rate was 28%. CONCLUSIONS:Self-awareness of symptoms led a majority of the patients to present at hospital in China. Compared to currently available case series studies, our results suggest that OSA patients in East Asian countries are characterized by higher prevalence and more severe apnea, younger age, poorer sleep quality, but less obesity and less comorbidity with hypertension, relative to countries in North America, South America and Europe.
Differences Of Craniofacial Characteristics In Oral Breathing And Pediatric Obstructive Sleep Apnea

Abstract:Background: Oral breathing (OB) was considered associated with specific craniofacial structures and same for pediatric obstructive sleep apnea (OSA). This study aimed to investigate the differences of craniofacial structures between OB and OSA. Methods: In this retrospective study, 317 children under age 18 years were recruited and divided into OB group, OSA group, and control group. OSA group (15 boys, 4 girls) were referred from qualified sleep center and diagnosed as pediatric OSA with full-night polysomnography. OB group (10 boys, 10 girls) were mostly referral from pediatric or ENT department, some of whom undertook polysomnography and were not OSA. Control group consisted of orthodontic patients within the same period. Lateral cephalograms were obtained in all groups and their parameters were compared with Chinese normal values and each other. Results: R-PNS of OB group (18.04 +/- 2.49 mm) was greater than OSA group (14.27 +/- 4.36 mm) and even control group (16.22 +/- 3.91 mm) (P < 0.01). U1-NA was also the greatest in OB group (7.15 +/- 2.92 mm), followed by OSA group (4.88 +/- 2.66 mm), while control group was the smallest (5.71 +/- 2.94 mm) (P < 0.05). In addition, OB group presented the smallest adenoids and tonsils among three groups. Bony nasopharynx development, mandibular length and growth direction of mandible of OB group were all better than OSA group. Conclusion: Despite of oral breathing, anatomical morphology (well-developed dentoalveolar structures; mild adenotonsillar hypertrophy) might protect children from developing OSA.

Abstract:Objectives: To identify craniofacial and upper airway morphological characteristics associated with the presence and severity of obstructive sleep apnea (OSA) in children. Methods: This study consisted of 82 OSA children and 77 controls (age 5–10 years). All subjects underwent cephalograms and were divided into a 5–7 age group and an 8–10 age group. Cephalometric variables were compared between OSA children and controls, and hierarchical regression analysis was performed to examine the relationship between cephalometric variables and OSA severity [expressed by the obstructive apnea–hypopnea index (OAHI)] in different age groups. Results: Increased A/N ratio, narrowed posterior airway space, decreased SNA and SNB angles, and shortened ramus height were observed among OSA children in different age groups. In the 5–7 age group, the A/N ratio and a lower gonial angle explained 40.0% and 14.7% of the variance in the OAHI, respectively. In the 8–10 age group, the BMI z-score and A/N ratio explained 25.2% and 6.6% of the variance in the OAHI, followed by a lower gonial angle and the hyoid-retrognathion distance (19.1% in total). Conclusions: Adenoid hypertrophy was a major factor associated with OSA in preschool children, whereas obesity replaced adenoid hypertrophy as the main contributor to OSA in late childhood. Several craniofacial skeletal variables such as the SNB angle, ramus height, lower gonial angle, and hyoid position are also associated with the presence and/or severity of OSA, which could be used to help recognize children at a higher risk for OSA.

Analysis of the Aerodynamic Characteristics of the Upper Airway in Obstructive Sleep Apnea Patients

Abstract:Background/purpose: This study was designed to explore upper airway aerodynamic characteristics in individuals diagnosed with obstructive sleep apnea (OSA) and to evaluate correlations between these characteristics and other anatomical upper airway findings in these patients. Materials and methods: This was a retrospective study of 40 OSA patients (22 male, 18 female) who were stratified into groups with mild, moderate, and severe disease based upon overnight polysomnographic (PSG) recording results. Newtom5G cone-beam CT scans (CBCT) were conducted for all patients, and the resultant images were used to reconstruct threedimensional images of the upper airways which were used to calculate aerodynamic characteristics. Differences in these characteristics between groups were evaluated with one-way ANOVAs, while relationships between anatomical and aerodynamic characteristics were assessed through Pearson correlation analyses. Results: The aerodynamic of the upper airway has typical characteristic in severe group. There was a significant negative correlation in severe group between resistance during inspiration (Rin) and volume (V) (r = -0.693, P = 0.013), minimum axial area (MMA) (r = -0.685, P = 0.014), and lateral dimension (LAT) (r = -0.724, P = 0.008), resistance during expiration (Rex) and LAT (r = -0.923, P < 0.001). Conclusion: This study showed that airway resistance during inspiration and expiration are most closely associated with upper airway collapse in OSA patients, with repetitive collapse occurring during both of these breathing processes. [Relationship Between Craniofacial and Airway Structure and Pediatric Obstructive Sleep Apnea and Hyponea Syndromes: a Case-Control Study].

Abstract:OBJECTIVE:The children obstructive sleep apnea and hyponea syndromes has become a global research hot spot, but the research of craniofacial features in obstructive sleep apnea and hyponea syndromes (OSAHS) children was scarce. To evaluate the craniofacial features of obstructive sleep apnea and hyponea syndromes children.METHODS:The subjects involved in this study fell into two groups: the patient group and the control one. The patients and controls were strictly matched by age and sex. Lateral head radiographs and cephalometric measurements were obtained and then compared between the two groups.RESULTS:The findings demonstrated marked differences in terms of SNB, PG-NB, lower facial height, H-C3Me and A&T/P. The SNB angle (75.8±4.3) in the patient group was smaller than that in the control one (78.7±2.6) and the P value was 0.035; the PG/NB value in the patient group (1.3±0.8) mm was higher than that in the control one (0.6±0.6) mm and the P value was 0.02. The anterior face height was (65.1±5.9) mm in the patient group (P=0.04), while the anterior face height in the control group was (61.5±3.2) mm. The position of hyoid was lower in the patient group(5.3±3.7) mm, compared with the control one (2.6±2.6) mm, and the P value was 0.03. Furthermore, the patients of OSAHS had more swelled adenoids and tonsils than the controls.CONCLUSION:The patient group differed from the control group in the length of mandible, lower facial height, position of hyoid and the chin, and the size of the adenoids and tonsils.
Study on measurements of upper airway in obstructive sleep apnea hypopnea syndrome during natural sleep by cine magnetic resonance

Abstract:[ABSTRACT]OBJECTIVETo assess the upper airway and related structures in different patterns of the upper airway obstruction on cine magnetic resonance (CMR) imaging in patients with obstructive sleep apnea hypopnea syndrome (OSAHS).METHODSCMR images of upper airway were obtained in 30 male patients with severe OSAHS during waking state and natural sleep. The midsagittal plane images were obtained. Patients were classified into 3 groups based on the upper airway obstruction patterns at apnea events and the reference data of the upper airway were measured.RESULTSDuring natural sleep, the diameter of retropalatal region, retroglossal region, retroepiglottic region and the length of suprahyoid muscles were significantly shorter than those during waking state (P<0.01). The maximum angle between the suprahyoid muscles and the apex of the tongue during natural sleep was significantly larger than that during waking state (P<0.01). During wakefulness, there was a significantly difference in the diameter of retropalatal region among 3 obstruction patterns (P<0.01). During natural sleep, there were statistical difference in the diameter of retropalatal region and the upper tongue, the angle between the hard palate and soft palate, the maximum angle between the suprahyoid muscles and the apex of the tongue, and their change values among 3 obstruction patterns (P<0.05). CONCLUSIONThe measurements of the upper airway and related structures on CMR imaging in OSAHS patients could provide useful information in assessing upper airway.
CT with 3D Reconstruction of Upper Respiratory Tract in Obstructive Sleep Apnea Hypopnea Syndrome Application

Abstract:Objective To investigate the relationship between the obstructive sleep apnea hypopnea syndrome (OSAHS) patient's structure of the upper airway and severity of OSAHS.Methods A total of 77 cases (22 cases of mild,moderate 22 cases,33 cases of severe) of OSAHS patients and normal controls were 22 cases,in the three-dimensional structure of the downstream CT airway remodeling,measuring soft palate length,thickness,soft palate roots,uvula,tongue,epiglottis away from the posterior pharyngeal wall distance relationship with the severity of OSAHS.Results (1) In patients with OSAHS,the soft palate length [mild group (41.77 ±9.14) mm,moderate group (40.30 ±7.74) mm and severe group (40.64 ± 9.64)mm] and normal control group [(40.25 ±7.55)mm] compared,the difference was not statistically significant (P ＞0.05);the thickness of soft palate [mild group (10.00 ± 3.40) mm,moderate group (10.31 ±2.53)mm] and normal control group [(9.88 ±2.02)mm] compared,the difference was not statistically significant (P ＞ 0.05),severe group [(11.49 ± 2.62) mm] and normal control group compared,the difference was statistically significant (P ＜ 0.05).(2) In calm inspiratory breath hold and severe OSAHS patients epiglottis from posterior pharyngeal wall distance was longer than the moderate OSAHS group [(10.75 ±5.18)mm vs (7.61 ±3.57)mm],the difference was statistically significant (P ＜0.05);after deep inspiratory breath hold,OSAHS patients in the severe group uvula from posterior pharyngeal wall distance was shorter than in normal control group [(4.82 ±3.33)mm vs (7.10 ±4.64)mm],the difference was statistically significant (P ＜ 0.05).(3) In patients with OSAHS thickness of soft palate with body mass index (BMI) and apnea hypopnea index (AHI) were positively correlated (r =0.333,P ＜ 0.01;r =0.226,P ＜ 0.05),and with lowest pulse oxygen saturation (LSpO2) and mean pulse oxygen saturation (MSpO2) were negative correlation (r =-0.283,P ＜0.05;r =-0.400,P ＜0.01);in calm breathing and forced inspiratory breath hold,the soft palate roots from the posterior pharyngeal wall distance and BMI were negatively related (r =-0.297,P ＜ 0.01;r =-0.232,P ＜ 0.05);in calm inspiratory breath hold root of the tongue from the posterior pharyngeal wall distance and MSpO2 was negatively related (r =-0.225,P ＜ 0.05);in calm inspiratory breath hold epiglottis from the posterior pharyngeal wall distance and AHI was positively correlated (r =0.277,P ＜ 0.05),and negatively related (r =-0.289,P ＜ 0.05) with MSpO2;in forced inspiratory breath hold uvula from retropharyngeal wall distance and MSpO2 was positively correlated (r =0.260,P ＜ 0.05).Conclusions OSAHS patients through upper airway CT reconstruction with polysomnography (PSG) indicators combine to determine the severity of OSAHS disease pathogenesis studies,can be used as indicators to determine the therapeutic effect,have clinical significance.
Cephalometric studies on the upper airway space in normal Chinese

Abstract:A detailed cephalometric analysis of the soft and hard tissues of the upper airway was conducted with lateral cephalographs from 116 normal Chinese, aged between 18 and 25 years. Normal values and deviation range were preliminarily established for the size of the tongue, soft palate, nasopharynx, oropharynx, and hypopharynx, and for the relative position of the hyoid bone and vallecula in both sexes. Statistical findings showed that there were significant differences between the sexes. Significant relationships (P < 0.001) were observed 1) between the hypopharyngeal depth and the position of the hyoid bone and the vallecula, in which the horizontal position of vallecula appears to be the best predictor of the hypopharyngeal depth, as confirmed by the multiple regression equation; 2) between the upper airway depths at four different levels; and 3) between the naso-oropharyngeal area and tongue, soft palate, and oral area. The present data can be used to investigate further the upper airway in abnormal states.
Impact of Upper Airway Characteristics on Disease Severity and CPAP Therapy in Chinese Patients With OSA: An Observational Retrospective Study

Abstract:Objective The characteristics of the upper airway (UA) are important for the evaluation and treatment of obstructive sleep apnea (OSA). This study aimed to investigate the association of UA characteristics with OSA severity, titration pressure, and initiation of and 3-month compliance with continuous positive airway pressure (CPAP). Methods This retrospective study included consecutive patients examined using a semi-quantitative UA evaluation system (combination with physical examination and awake endoscopy) during 2008–2018 at the Department of Respiratory and Critical Care Medicine, Peking University First Hospital. First, the differences in UA characteristics were compared between patients with simple snorers and mild OSA and those with moderate-to-severe OSA. Then, the effect of UA characteristics on the initiation to CPAP therapy and 3-month adherence to CPAP was conducted. Results Overall, 1,002 patients were included, including 276 simple snorers and patients in the mild OSA group [apnea-hypopnea index (AHI) <15] and 726 patients in the moderate-to-severe OSA group (AHI ≥15). Tongue base hypertrophy, tonsillar hypertrophy, mandibular recession, neck circumstance, and body mass index (BMI) were independent risk factors for moderate-to-severe OSA. Among those patients, 119 patients underwent CPAP titration in the sleep lab. The CPAP pressures in patients with thick and long uvulas, tonsillar hypertrophy, lateral pharyngeal wall stenosis, and tongue hypertrophy were higher than those of the control group ( P < 0.05, respectively). The logistic regression analysis showed that nasal turbinate hypertrophy, mandibular retrusion, and positive Müller maneuver in the retropalate and retroglottal regions were independent predictors for the initiation of home CPAP treatment. Conclusion Multisite narrowing and function collapse of the UA are important factors affecting OSA severity, CPAP titration pressure, and the initiation of home CPAP therapy. Clinical evaluation with awake endoscopy is a safe and effective way for the assessment of patients with OSA in internal medicine.

Abstract:Objective To investigate the configuration index related to the degree of the state of an OSAS,bivariate correlation analysis was performed between the craniofacial structures,upper airway and the sleeping-respiratory disturbance of the OSAS.Methods The craniofacial skeletal and upper airway morphology of 86 male adult patients with AHI≥15 was measured by cephalometry and bivariate correlation and Multiple regression analysis was performed between the indexes of craniofacial structures,upper airway and AHI,AI,HI,LSaO_2 which represent the degree of sleeping-respiratory disturbance of the OSAS.Results Multiple regression analysis showed that the equation were obtained from AHI,HI,LSaO_2.Conclusion The variables of craniofacial structure and upper airway correlated with respiratory disturbance were: mandibular body length,the angle between upper and lower central incisor,back gap of the soft palate,the diameter of the nasopharynx,the sagittal position of hyoid.
CT Measurements of Upper Airway in the Patients of Obstructive Sleep Apnea - Hypopnea Syndrome Associated with Micrognathism

Abstract:OBJECTIVE The dimensions of upper airway in a group of micrognathic patients associated with obstructive sleep apnea-hypopnea syndrome (OSAHS) were measured using CT to explore the mor- phologic characteristics and improve the measurement method. METHODS Nineteen normal volunteers (10 males, 9 females), whose average age was 30.37 years, and 12 micrognathic patients associated with OSAHS (10 males,2 females) were included in this study. All the objects undertook CT scan in head and neck region with 16 array CT machine, 20 axial images were recon- structed from the palate level to the upper edge of arytenoids,anterior-posterior dimension, transverse dimension,and cross section area(CSA) of the upper airway in each image were measured. The maximum, minimum and mean anterior-posterior dimension, trans- verse dimension and CSA in velopharyngeal region, glos- sopharyngeal region and larynpharyngeal region were calculated respectively. RESULTS The differences be- tween OSAHS group and control group were significant on anterior-posterior dimension at all the 20 images, on CSA at 12/20 images, but on transverse dimension only at a few images while anterior-posterior dimensions were quite narrow. All the measurements except the maxi- mum transverse dimension in glossopharyngeal regionshowed significant differences between two groups. CONCLUSION The majority of the narrow upper airway attributes to the decrease of anterior-posterior dimen- sion in micrognathic patients associated with OSAHS, glossopharyngeal region is the narrowest region in all three regions.
Upper airway morphology and function study of middle-age and old women with OSAHS

Abstract:OBJECTIVE To study the role of anatomic structure and compliance change of the upper airway in the pathogenesis of obstructive sleep apnea hypopnea syndrome (OSAHS) in middle-age and old women. METHODS Twenty-three OSAHS women over 40 years old were divided into three groups. The cephalometric items of different groups and the volume change of each pharyngeal subdivision measured by CT three-dimensional reconstruction were compared. Then the correlation between the measuring items and degree of the OSAHS was studied. RESULTS There was a statistic difference between group AHI<5/h and group AHI>20/h in the items of PAS, SNA, SNB and narrowest pharyngeal distance. In the regression equation that AHI was as the dependent, only SNA and narrowest pharyngeal distance had statistic significance, while only MP-H was significant when the dependent was LSaO2. In the study of pharyngeal volume measurement, velopharyngeal volume during different breath states in the group 5≤AHI≤20/h was statistically different, while the volume of glossopharynx and velopharynx showed the same change in group AHI>20/h. In the regression equationthat AHI was as the dependent, only volumetric change rate of velopharynx had statistic significance. CONCLUSION Middle-age and old women with OSAHS have obvious anatomy abnormalities which are the morphological foundation of the OSAHS. Higher compliance of upper airway plays an important role in the OSAHS pathogenesis in middle-age and old women.

Abstract:Objective To evaluate the value of CT scan in the localization of obstruction sites in the upper airway in subjects with obstructive sleep apnea hypopnea syndrome(OSAHS).Methods Spiral CT scan was performed in 25 snoring subjects and 25 OSAHS patients during resting and Müller respiration respectively.The scanning range was from the upper edge of the first cervical vertebra to the lower edge of the sixth cervical vertebra.The parameters including the cross-sectional area,sagittal anteroposterior diameter and coronal transverse diameter of the upper edge of the second cervical vertebra,uvula root,the upper edge of the third cervical vertebra and the lower edge of the third cervical vertebra in resting and Müller respiration were measured and compared by using the softwares of the three-dimensional reconstruction workstation.Results Both under the resting and Müller respiration,the transverse diameter and the cross-sectional area of the upper edge of the second cervical vertebra and the uvula root in the OSAHS patients was smaller than that in the snoring subjects respectively(P<0.05).During the resting respiration,the transverse diameter,anteroposterior diameter and cross-sectional area of the upper edge of the third cervical vertebra in OSAHS patients was smaller than that in the snoring subjects respectively(P<0.01).Under the Müller respiration,the transverse diameter of the upper edge of the third cervical vertebra and the anterioposterior diameter of the lower edge of the third cervical vertebra in OSAHS patients was smaller than that in the snoring subjects respectively(P<0.01).Conclusions Multi-planar obstruction could be observed in OSAHS patients.Spiral CT could measure the diameter and the cross section area of the upper airway accurately and repeatedly.

Abstract:To investigate the correlation between upper airway CT measurement value and severity of patients with obstructive sleep apnea hypopnea syndrome (OSAHS).The upper airway of 82 patients with OSAHS were scanned by CT during normal respiration. The 3-D CT measurement were made in lateral and anterior-posterior diameters, cross-section areas and volumes of retropalatal and retroglossal region.The lateral diameters and cross-section areas of every research plane were negatively correlated with apnea and hypopnea index (AHI), in particular the minimal lateral diameter of the whole airway (r = -0.558, P < 0.01). For volumes, AHI decreased with larger volume of nasal cavity, nasopharyngeal cavity and velopharyngeal cavity (P < 0.05 or P < 0.01). The negative correlation between AHI and proportion of cavity volume to the bone frame of velopharyngeal area and whole upper airway were significant (P < 0.05 or P < 0.01). But the correlation between AHI and volume of velopharyngeal soft tissue, the proportion of soft tissue to the bone frame volume of velopharyngeal area, the proportion of soft tissue to the bone frame volume of whole upper airway was significant positive, as well as the correlation between AHI and the proportion of soft tissue to the cavity volume of velopharyngeal area and whole upper airway (P < 0.05 or P < 0.01).To evaluate the severity of OSAHS with upper airway CT measurement, there is no difference between 3-D parameters and 2-D parameters. In terms of the whole airway, the lateral diameters, the cross-section area, the cavity volume, the soft tissue of the upper airway, are meaningful to evaluate the severity of disease, in particular the measurement of velopharyngeal area.
