Abstract:Noninvasive diagnosis of bone density and mechanical properties using non-radiation imaging modality is an emerging area with promising in early prediction of osteopenia and treatment effectiveness in the clinic and functional disuse, i.e., long-term bedrest and space mission. Advances in quantitative ultrasound have shown advantages in measuring both bone density and mechanical strength, non-radiation, imaging capability, and easy to use. The challenge that remained is the poor penetration of ultrasound signals passing through trabecular and cortical bones and acoustic energy scattering. A new scanning confocal ultrasound technology is developed in this lab to detect the alteration of bone to provide diagnostic results in bone density and structure properties. A software-controlled flexible ultrasound system with 2-D dual array transducer is developed and proposed for the purpose of noninvasive bone density diagnosis and assessment of bone loss. Transmitting (Tx) transducer elements are divided into sub-blocks to excite the ultrasound signals in sequence to decrease the system complexity while maintaining beam pattern properties by the signal processing procedure at receiving (Rx) side. Apodization is also applied to reduce acoustic side lobes and to make the resolution in the ultrasound field of view (FOV) more uniform. This study may provide basic understanding of modulated confocal ultrasound beam forming for tissue characterization, such as trabecular bone structual and strength properties.

Abstract:Intravascular ultrasound (IVUS) imaging is a minimally invasive medical technology that plays a critical role in diagnosis, treatment guidance, and post-treatment assessment of coronary artery diseases. As a crucial component of the IVUS system, conventional IVUS transducers are designed to be planar and unfocused to adequately cover the region of interest. However, this design comes at the cost of spatial resolution. Here, we developed a high-performance focused IVUS transducer using Pb(In1/2Nb1/2)O3-Pb(Sc1/2Nb1/2)O3-PbTiO3 (PIN-PSN-PT) textured ceramics with both high electromechanical performance (thickness-mode electromechanical coupling factor kt: ∼60%) and high Curie temperature (TC: ∼250 °C). Benefiting from the relatively low clamped dielectric constant (ε33S/ε0: ∼450) of PIN-PSN-PT-textured ceramics in contrast to currently used soft piezoelectric ceramics (>1000), we designed a relatively large aperture for the focused IVUS transducer, with a goal of enhancing lateral resolution across a larger depth of field, ranging from 1 to 5 mm. The developed focused IVUS transducer operates at 42 MHz with an −6 dB bandwidth of 72%, featuring a 0.6 × 0.6 mm2 aperture while maintaining an electrical impedance of approximately 40–60 Ω. The axial and lateral resolutions characterized by wire phantom imaging are 45 and 208 μm, respectively. The acoustic pressure generated by the focused IVUS transducer is 1.4 times higher than that of its planar counterpart. Ex vivo porcine coronary artery imaging demonstrates that our focused IVUS transducer offers improved image quality and uniformity for the visualization of intravascular structures. Our work shows great potential of PIN-PSN-PT-textured ceramics for creating high-frequency miniaturized focused transducers.

Abstract:We have developed a simple approach for mechanically scanning a focused bowl ultrasound (US) transducer for either hyperthermia or tissue ablation therapies called the 3-point support (3PS) mechanical steering technique. The scanning involves translation of the required 3D motion of the ultrasound transducer to the more manageable linear movement of three support rods. It is a cost-effective alternative, especially compared with electronic scanning and other previous implementations of mechanically scanned systems. The 3PS approach is particularly well suited for integration with our microwave breast imaging technique--the combination of which could be an effective, low-cost thermal therapy/monitoring approach. The results show that the US focus can be moved laterally in a spiral pattern 3 cm below the surface in a gel phantom and that similar patterns can be moved to multiple locations within the phantom volume in succession. The feasibility of simultaneously acquiring microwave thermal images is also demonstrated.

Abstract:We present a back-to-back (BTB) structured, dual-mode ultrasonic device that incorporates a single-element 5.3 MHz transducer for high-intensity focused ultrasound (HIFU) treatment and a single-element 20.0 MHz transducer for high-resolution ultrasound imaging. Ultrasound image-guided surgical systems have been developed for lesion monitoring to ensure that ultrasonic treatment is correctly administered at the right locations. In this study, we developed a dual-element transducer composed of two elements that share the same housing but work independently with a BTB structure, enabling a mode change between therapy and imaging via 180-degree mechanical rotation. The optic fibers were embedded in the HIFU focal region of ex vivo chicken breasts and the temperature change was measured. Images were obtained in vivo mice before and after treatment and compared to identify the treated region. We successfully acquired B-mode and C-scan images that display the hyperechoic region indicating coagulation necrosis in the HIFU-treated volume up to a depth of 10 mm. The compact BTB dual-mode ultrasonic transducer may be used for subcutaneous thermal ablation and monitoring, minimally invasive surgery, and other clinical applications, all with ultrasound only.

Abstract:Piezoelectric ultrasonic transducers have shown great potential in biomedical applications due to their high acoustic-to-electric conversion efficiency and large power capacity. The focusing technique enables the transducer to produce an extremely narrow beam, greatly improving the resolution and sensitivity. In this work, we summarize the fundamental properties and biological effects of the ultrasound field, aiming to establish a correlation for device design and application. Focusing techniques for piezoelectric transducers are highlighted, including material selection and fabrication methods, which determine the final performance of piezoelectric transducers. Numerous examples from ultrasound imaging, neuromodulation, tumor ablation to ultrasonic wireless energy transfer are summarized to highlight the great promise of biomedical applications. Finally, the challenges and opportunities of focused ultrasound transducers are presented. The aim of this review is to bridge the gap between focused ultrasound systems and biomedical applications.

Abstract:Rationale: Focused ultrasound-stimulated microbubbles have been shown to be capable of inducing blood flow shutdown and necrosis in a range of tissue types in an approach termed antivascular ultrasound or nonthermal ablation. In oncology, this approach has demonstrated tumor growth inhibition, and profound synergistic antitumor effects when combined with traditional platforms of chemo-, radiation- and immune-therapies. However, the exposure schemes employed have been broad and underlying mechanisms remain unclear with fundamental questions about exposures, vessel types and sizes involved, and the nature of bubble behaviors and their acoustic emissions resulting in vascular damage - impeding the establishment of standard protocols. Methods: Here, ultrasound transmitters and receivers are integrated into a murine dorsal window chamber tumor model for intravital microscopy studies capable of real-time visual and acoustic monitoring during antivascular ultrasound. Vessel type (normal and tumor-affected), caliber, and viability are assessed under higher pressure conditions (1, 2, and 3 MPa), and cavitation signatures are linked to the biological effects. Results: Vascular events occurred preferentially in tumor-affected vessels with greater incidence in smaller vessels and with more severity as a function of increasing pressure. Vascular blood flow shutdown was found to be due to a combination of focal disruption events and network-related flow changes. Acoustic emissions displayed elevated broadband noise and distinct sub- and ultra-harmonics and their associated third-order peaks with increasing pressure. Conclusions: The observed vascular events taken collectively with identified cavitation signatures provide an improved mechanistic understanding of antivascular ultrasound at the microscale, with implications for establishing a specific treatment protocol and control platform.

High-Intensity Focused Ultrasound Ablation Monitoring Using Ultrafast Doppler Imaging: A Preliminary Study

Abstract:High-intensity focused ultrasound (HIFU) has emerged as a promising and noninvasive technique for localized thermal ablation of pathological tissue. Ultrasound-based monitoring of HIFU treatments has been developed by detecting echogenicity changes induced by cavitation. However, due to the low contrast between the lesion and surrounding tissue, for instance, ablation may occur in the absence of significant bubble generation, there is still a high request for developing more sensitive monitoring methods. Ultrafast ultrasound Doppler imaging has advantages in monitoring HIFU treatment due to its superior spatiotemporal resolution and heightened sensitivity for small vessels. In this study, we present a synchronized protocol that harmonizes HIFU exposure with an ultrafast acquisition sequence. The proposed method was demonstrated on the rat cerebral cortex ablation, enabling simultaneous visualization of hemodynamic changes and small vasculature disruption during the HIFU treatment. Ultrafast Doppler monitoring holds the potential to improve the safety and accuracy of HIFU treatments, thereby promoting its broader clinical adoption and utilization.

Abstract:Advances in methodologies and tools often lead to new insights into cardiovascular diseases. Intravascular ultrasound (IVUS) is a well-established diagnostic method that provides high-resolution images of the vessel wall and atherosclerotic plaques. High-frequency (>50 MHz) ultrasound enables the spatial resolution of IVUS to approach that of optical imaging methods. However, the penetration depth decreases when using higher imaging frequencies due to the greater acoustic attenuation. An imaging method that improves the penetration depth of high-resolution IVUS would, therefore, be of major clinical importance. Modulated excitation imaging is known to allow ultrasound waves to penetrate further. This paper presents an ultrasound system specifically for modulated-excitation-based IVUS imaging. The system incorporates a high-voltage waveform generator and an image processing board that are optimized for IVUS applications. In addition, a miniaturized ultrasound transducer has been constructed using a Pb(Mg1/3Nb2/3)O-3-PbTiO3 single crystal to improve the ultrasound characteristics. The results show that the proposed system was able to provide increases of 86.7% in penetration depth and 9.6 dB in the signal-tonoise ratio for 60 MHz IVUS. In vitro tissue samples were also investigated to demonstrate the performance of the system.

Abstract:Microwave ablation (MWA) can rapidly lead to tumor cell necrosis by heating local tissues using electromagnetic field energy, making it a highly efficient modality widely utilized in clinical ablation surgeries. However, the lack of timely and accurate intraoperative monitoring methods is a critical issue that urgently needs to be addressed in MWA. This article introduces an innovative approach for real-time monitoring of ablation therapies by the combination of ultrasound radio frequency (RF) signal acquisition and singular value decomposition (SVD), which breaks through the limitations of current monitoring techniques during MWA procedures. Due to the difference in acoustic scattering properties between the gas generated in high-temperature ablated region and the non-ablated tissues, the current new method aimed to apply SVD analyses to ultrasound RF signals scattered from the tissues, through which the range of gas generation could be determined and then the ablation area could be estimated. Eventually, high-precision differentiation between ablated and non-ablated tissues could be achieved, enabling real-time adjustment and improvement of MWA strategies. Validated through ex vivo experiments on pig liver, this technique demonstrated a relative error in monitoring the size of ablation region of approximately 7%, indicating a strong correlation with actual ablation outcomes. The study indicated the proposed method should have great potential to improve the precision and safety of MWA therapy.

Asymmetric-Backed Multi-Frequency Ultrasonic Transducer for Conformal Tumor Ablation

Abstract:Objective: Minimally invasive ultrasound ablation transducers have been widely studied. However, conventional designs are limited by the single working frequency, restricting their conformal ablation ability (i.e., ablation size and shape controllability). Methods: New multi-frequency ultrasonic transducer design method is proposed based on the asymmetric backing layer, which divides the transducer into non-backing-layer region (i.e., front-piezoelectric region) and backing-layer region (i.e., front-piezoelectric-backing region) with multiple local thickness mode resonant frequencies. Ablation zone can be controlled by exciting the local resonance within or between the regions, and its control flexibility is further enhanced by driven under a frequency modulation signal. Experiments and calculations are combined for verifying the proposal. Results: The fabricated transducer with a Y-direction asymmetric backing layer shows five resonances, with two in each region and one resonance excited in both regions. Spatial ultrasound emission is demonstrated by acoustic measurements. Tissue ablation experiments verified spatial ablation zone control, and frequency modulation driving method enables the spatial transition of ablation zone from one region to the other, generating different ablation sizes and shapes. Finally, patient-specific simulations verified the effectiveness of conformal ablation. Conclusion: The proposed transducer enables flexible control of ablation zone. Significance: This study demonstrates a new method for conformal tumor ablation.
Design and ex vivo kidney evaluation of a high-intensity focused ultrasound transducer and 3D positioner

Abstract:This article describes the design, construction, and evaluation of a three-dimensional (3D) ultrasound system to be used for the treatment of kidney tumors using high-intensity focused ultrasound (HIFU). The system consists of a therapeutic ultrasound delivery module (driver, power amplifier, matching circuit, and transducer (1 MHz)) and a 3D positioning module (stepper motors and microcontroller). The system includes software that designs a treatment planning according to the tumor coordinates. In order to verify the capability of the system for moving the transducer, the positioning module was successfully driven to cover cylinders of dimensions as large as 20 cm (diameter) x 10 cm (height) with step sizes as low as 0.05 mm. Several exposimetry experiments were done which showed close match between experimental and theoretical results. Ex vivo experiments were performed and indicated the capability of the system to generate a single lesion of 0.27 cm diameter and 0.45 cm length, as well as five lesions (each with a diameter of 0.3 cm) at prespecified locations. During the ex vivo experiments, temperature monitoring indicated an increase in temperature from 37 degrees C to about 60 degrees C inside the lesion, while outside it the temperature remained below 39 degrees C.

Abstract:Beyond the implementation of the photoacoustic effect to photoacoustic imaging and laser ultrasonics, this study demonstrates a novel application of the photoacoustic effect for high-precision cavitation treatment of tissue using laser-induced focused ultrasound. The focused ultrasound is generated by pulsed optical excitation of an efficient photoacoustic film coated on a concave surface, and its amplitude is high enough to produce controllable microcavitation within the focal region (lateral focus <100 µm). Such microcavitation is used to cut or ablate soft tissue in a highly precise manner. This work demonstrates precise cutting of tissue-mimicking gels as well as accurate ablation of gels and animal eye tissues.

Abstract:Focused ultrasound (FUS) lacks reliable real-time image guidance, which hinders the development of non-invasive ultrasound treatment in many important clinical applications. A dual-mode ultrasound array, capable of both imaging and therapy offers a new and reliable strategy for image-guided ultrasound therapy applications. The strategy has the advantages of real-time use, low cost, portability and inherent registration between imaging and therapeutic coordinate systems. In this work, a dual-mode two-dimensional (2D) matrix array with 1 MHz center frequency and 256 elements for ultrasound image-guided non-invasive therapy is reported. The array can provide three-dimensional (3D) volumetric ultrasound imaging and 3D focus control. Ultrasound imaging and therapeutic applications for the brain of small animals demonstrated the multi-functional capability of the dual-mode 2D matrix array. A method of rat brain positioning based on ultrasound imaging was proposed and verified. Transcranial ultrasound image-guided blood–brain barrier (BBB) opening of multiple-targets was achieved in vivo, using the proposed dual-mode 2D array. The obtained results indicate that the dual-mode 2D matrix array is a promising method for practical use in ultrasound image-guided non-invasive therapy applications.

Abstract:Acoustic cavitation plays a critical role in various biomedical applications. However, uncontrolled cavitation can lead to undesired damage to healthy tissues. Therefore, real-time monitoring and quantitative evaluation of cavitation dynamics is essential for understanding underlying mechanisms and optimizing ultrasound treatment efficiency and safety. The current research addressed the limitations of traditionally used cavitation detection methods by developing introduced an adaptive time-division multiplexing passive cavitation imaging (PCI) system integrated into a commercial diagnostic ultrasound platform. This new method combined real-time cavitation monitoring with B-mode imaging, allowing for simultaneous visualization of treatment progress and 2D quantitative evaluation of cavitation dosage within targeted area. An improved delay-and-sum (DAS) algorithm, optimized with a minimum variance (MV) beamformer, is utilized to minimize the side lobe effect and improve the axial resolution typically associated with PCI. In additional to visualize and quantitatively assess the cavitation activities generated under varied acoustic pressures and microbubble concentrations, this system was specifically applied to perform 2D cavitation evaluation for ultrasound thrombolysis mediated by different solutions, e.g., saline, nanodiamond (ND) and nitrogen-annealed nanodiamond (N-AND). This research aims to bridge the gap between laboratory-based research systems and real-time spatiotemporal cavitation evaluation demands in practical uses. Results indicate that this improved 2D cavitation monitoring and evaluation system could offer a useful tool for comprehensive evaluating cavitation-mediated effects (e.g., ultrasound thrombolysis), providing valuable insights into in-depth understanding of cavitation mechanisms and optimization of cavitation applications.
A Novel Dual-Frequency Imaging Method for Intravascular Ultrasound Applications.

Abstract:Intravascular ultrasound (IVUS), which is able to delineate internal structures of vessel wall with fine spatial resolution, has greatly enriched the knowledge of coronary atherosclerosis. A novel dual-frequency imaging method is proposed in this paper for intravascular imaging applications. A probe combined two ultrasonic transducer elements with different center frequencies (36 MHz and 78 MHz) is designed and fabricated with PMN-PT single crystal material. It has the ability to balance both imaging depth and resolution, which are important imaging parameters for clinical test. A dual-channel imaging platform is also proposed for real-time imaging, and this platform has been proven to support programmable processing algorithms, flexible imaging control, and raw RF data acquisition for IVUS applications. Testing results show that the -6 dB axial and lateral imaging resolutions of low-frequency ultrasound are 78 and 132 mu m, respectively. In terms of high-frequency ultrasound, axial and lateral resolutions are determined to be as high as 34 and 106 mu m. In vitro intravascular imaging on healthy swine aorta is conducted to demonstrate the performance of the dual-frequency imaging method for IVUS applications. (C) 2014 Elsevier B.V. Combined drug delivery and treatment monitoring using a single high frequency ultrasound system

Abstract:Ultrasound-mediated drug delivery is typically performed using transducers with center frequencies ≤ 1 MHz to promote acoustic cavitation. Such frequencies are not commonly used for diagnostic ultrasound due to limited spatial resolution. Therefore, delivery and monitoring of therapeutic ultrasound typically requires two transducers to enable both treatment and imaging. This study investigates the feasibility of using a single commercial ultrasound imaging transducer operating at 5 MHz for both drug delivery and real-time imaging. We compared a single-transducer system (STS) at 5 MHz with a conventional dual-transducer system (DTS) using a 1.1 MHz therapeutic transducer and an imaging probe. in vitro experiments demonstrated that the STS could achieve comparable extravasation depth and area as the DTS, with higher drug deposition observed at 5 MHz. Additionally, extravasation patterns were influenced by peak negative pressure (PNP) and duty cycle, with the narrower beam width at 5 MHz offering potential advantages for targeted drug delivery. in vivo experiments in a murine bladder cancer model confirmed the efficacy of the STS for real-time imaging and drug delivery, with cavitation dose correlating with drug deposition. The results suggest that a single-transducer approach may enhance the precision and efficiency of ultrasound-mediated drug delivery, potentially reducing system complexity and cost.
