Abstract:Sj(o)gren's syndrome(SS)is a chronic autoimmune inflammatory disease characterized by predominant infiltration of lymphocytes in exocrine glands as well as presence of several autoantibodies in sera.Primary SS(pSS)is the condition in the absence of other definitively diagnosed autoimmune diseases.The pathogenesis of pSS remains unclear.Currently,pSS is considered to be induced by multiple environmental factors and endogenous antigens,which trigger a self-perpetuating inflammatory response in susceptible individuals,lead to abnormal cellular and humoral immunity followed by the impairment of exocrine glands,production of autoantibodies and damage to target organs.Although no ideal therapy is available,biological agents have offered new clues to the treatment of pSS.
Mycophenolate for the Treatment of Primary Sjögren's Syndrome.

Abstract:Primary Sjögren's syndrome (pSS) is a systemic autoimmune disease with exocrine gland dysfunction and multi-organ involvement. Currently, there is an increasing trend toward non-steroid therapy for the treatment of autoimmune diseases. Some biological agents or immunosuppressive drugs may be the ideal choices. In real-world practice, as patients have severe systemic complications or organ damage, they will have a bad prognosis even if they are treated with high-dose steroids and strong immunosuppressive drugs. However, if we can start early intervention and prevent progressive development in advance, the patient may have a good prognosis. Mycophenolate is an immunosuppressive drug with minor side effects. Here, we conduct a systemic review and find supporting evidence that patients with pSS benefit from early mycophenolate therapy. Mycophenolate may be the first-line treatment for pSS patients in the future.
Clinicopathologic Characteristics and Prognosis in Primary Sj(o)gren's Syndrome with Renal Involvement

Abstract:Objective:To analyze clinical manifestations,immunological characteristics,pathological changes and prognosis in primary Sjgren's syndrome patients with renal involvement. Methodology:Sixty-two primary Sjgren's syndrome patients with renal involment,female/male 60/2 cases and average age of(36.8±10.7)years old,were enrolled in this retrospective study.Their clinical manifestations,renal pathological features and prognosis were analyzed. Results:88.1% cases showed a decrease in urine osmosis,49 cases(79%) renal tubular acidosis(RTA),including type Ⅰ 44 cases(89.8%),type Ⅱ 2 cases(4.1%) and type Ⅲ 3 cases(6.1%).47 cases(75.8%) presented with hypokalemia.9 cases(14.5%) with nephrogenic diabetes insipidus and one case(1.6%) with Fanconi syndrome.43 cases(69.4%) had urinary protein excretion from 0.42～7.51 g/d,of which 41(95.3%) had proteinuria less than 2 g/d,and 2(4.7%) more than 3.5 g/d.4 cases(6.5%) had microscopic hematuria,9(14.5%) chronic renal insufficiency,and 45(72.6%)had hyperglobulinaemia.The positive rate of serum autoantibody levels were as follows: anti-SSA(86.0%),ANA(77.4%),anti-SSB(52.6%),RF(39.5%),anticardiolipin antibodies(21.6%),and anti-RNP antibody(15.3%).In histological examinations,56 cases(90.3%) presented with interstitial nephritis,6 combined with mesangial proliferation and IgA deposition,2 combined with membranous nephropathy,and 6(9.7%) presented with normal.56 cases were followed up(37.5±38.0) months,all of 5 and 10 years renal and patient survival were 100%. Conclusion:Urine concentration ability decline and RTA with hypokalemia were most common in primary Sjgren's syndrome patients with renal involvement.The interstitial nephritis was the major pathological changes and could be combined with mesangial proliferation and IgA deposition,membranous nephropathy.Their long-term prognosis was good.
Clinicopathological Features and Prognosis of Kimura’s Disease with Renal Involvement in Chinese Patients

Abstract:AIMS:Kimura's disease (KD) with renal involvement is a rare disease. Optimal treatments are still not well established. It is necessary to analyze clinicopathological features, treatment responses, and prognosis for improving KD diagnosis and treatment.MATERIALS AND METHODS:Clinicopathological data, treatment responses, and prognosis were collected and analyzed retrospectively.RESULTS:The patients consisted of 27 males and 2 females, with an average age of 35.5 ± 15.1 (13 - 61) years. 27 exhibited proteinuria ranging from 0.730 to 14.1 g/24 h (5.98 ± 3.40 g/24 h). Hypertension, renal insufficiency (serum creatinine (Scr) > 1.24 mg/dL), and microhematuria occurred in 4 (13.8%), 11 (37.9%), and 13 (44.8%) cases, respectively. Light microscopy (LM) identified mesangium proliferation, minimal change, focal and segmental glomerulosclerosis (FSGS), membranous glomerulonephritis, membranoproliferative glomerulonephritis (MPGN), and acute tubular necrosis in 14, 8, 3, 2, 1, and 1 cases, respectively. All were treated with Tripterygium wilfordii (TW), prednisone, leflunomide (LEF), tacrolimus (FK506), myophenolate mofetil (MMF), or renin-angiotensin system blockers (RASI). 26 patients were followed up for 1.60 - 108.7 months (39.6 ± 28.7). After treatments, urinary red blood cells (RBC) decreased in all. The amount of 24-hour urinary protein (24-hUPE) decreased in 24 patients. 22 reached complete remission (CR), 4 partial remissions (PR). The patients who did not relapse were younger than those who relapsed.CONCLUSIONS:KD with renal involvement occurs predominantly among 35 - 50 year old Chinese patients with male predilection. The most common features are proteinuria, hypertension, micro hematuria with minimal change, and mesangial proliferative glomerulonephritis. Most were responsive to treatment, but could relapse. Gender, age, and hypertension are associated with KD recurrence. The prognosis is good mostly.
Effect of Chinese Herbal Medicines for Nourishing Yin, Supplementing Qi, and Activating Blood on Reproductive Endocrine Activity and Immune Functions in Patients with Primary Sjogren's Syndrome.

Abstract:OBJECTIVE:To investigate the effect of Chinese herbal medicines for nourishing yin, supplementing qi, and activating blood on the reproductive endocrine-immune network and its mechanisms in patients with primary Sjogren's syndrome (pSS). METHODS:Seventy pSS patients were randomly assigned to two groups using a randomized digital table: the integrative therapy group (36 cases) and the control group (34 cases). Thirty healthy subjects were taken as a normal group. The control group was treated with hydroxychloroquine sulfate tablets alone, and the integrative therapy group was treated by Chinese herbal medicines for nourishing yin, supplementing qi, and activating blood combined with hydroxychloroquine sulfate tablets. The treatment course was 6 months for both groups. Before and after treatment, serum estradiol (E2), testosterone (T), luteinizing hormone (LH), prolactin (PRL) by radioimmunoassay and immunoglobulin (IgG) by immunodiffusion, erythrocyte sedimentation rate (ESR) by Westergren, interferon-γ (IFN-γ) and interleukin-4 (IL-4) by enzyme linked immunosorbent assay were determined. RESULTS:E2 and T levels in all patients were lower than those of normal subjects before treatment (P<0.05) and were increased significantly after 6-month treatment (P<0.05). ESR, FSH, LH, IgG, IFN - γ, IL - 4 and ratios of E2/T, and IFN -γ/IL in the patients were higher than those of normal subjects before the treatments (P<0.05), and were reduced significantly after the treatments (P<0.05). The T and IFN - γ levels and E2/T ratio in the patients treated with integrative therapy were reduced significantly compared with the control group (P<0.05). However, the PRL levels before and after treatment were not significantly changed in the two groups (P>0.05). The ratios of E2/T and IFN -γ/IL-4, and levels of IgG and ESR were positively correlated before and after treatment (P<0.05). CONCLUSIONS:The ratios of E2/T and IFN -γ/IL-4 might be used as indicators of pSS activity. Chinese herbal medicines for nourishing yin, supplementing qi, and activating blood combined with Western medicine could improve the therapeutic effect by regulating the reproductive endocrine-immune network in pSS patients.

Abstract:Objective To understand the impact of primary Sjogren's syndrome(PSS)on kidney by following up 113 patients with PSS in 1996 to 2006.Methods One hundred and thirteen patients of PSS with renal impairment were analyzed by routine,clinical features,tubular function and immunoassay.Results Among 113 PSS,89 patients were diagnosed as renal tubular acidosis(RTA),of whom 76 cases presented with Myastheina,69 with polydipsia and polyuria,57 with ostealgia,9 with diabetesinsipidus,and 21 hypokalenic paralysis.Of 113 patients,24 patients presented with glomerulonephritis,36 patients initially presented with renal impairment,significant elevation of IgG was noticed in 82.3%,87 patients responded to the combination of steroid and immunosuppressors,and 28 had renal function completely recovered.Conclusion PSS patients with renal involvement developed varied syndrome,mainly tubular acidosis and glomerulonephritis.The combination of steroid and immunosuppressors can significantly improve the renal function of patients with PSS.

Abstract:Primary Sj(o)gren' s Syndrome (pSS)is a chronic autoimmune disease with complex and diverse clinical manifestations.It is characterized by lymphocyte infiltration of exocrine glands such as the salivary gland and lacrimal gland leading to insufficient secretion of the gland,manifested as dry mouth and dry eyes.The disease in addition to exocrine glands cause damage,but also involving organs outside the gland,resulting in systemic systemic injury.The pathogenesis of pSS is not clear yet.Currently,symptomatic treatment is the mainstay of pSS,and the exact treatment is lacking.However,there are some new treatments in recent years.

Treatment Outcome of 301 Aplastic Anemia Patients in China: a 10-Year Follow-Up and Real-World Data from Single Institute Experience

Abstract:OBJECTIVE:This study was carried out to explore clinical treatment and prognosis of patients with AA with different economic status. Methods: We retrospectively analyzed the clinical outcome of 301 patients with AA in our center from April 2008 to November 2017.RESULTS:Treatments included anti-thymocyte globulin (ATG) or anti-lymphocyte globulin (ALG) combined with cyclosporineA (CsA) (9%), allogeneic hematopoietic stem cell transplantation (allo-HSCT) (7%), CsA combined with androgen or CsA alone (hereinafter referred to as CsA group) (77%), no specific therapy (7%). The 5-year overall survival (OS) was higher in patients with non-severe AA (94.6%) compared with those with severe AA (SAA) (66.6%, P <.001), very severe AA (VSAA) (41.3%, P <.001). The 5-year OS was 76.5% in patients with SAA/VSAA treated with ATG/ALG combined with CsA, 75% in allo-HSCT group(P =.936), 63.6% in CsA group (P =.557), which was significantly higher than no specific therapy group (21.8%, P =.002). For those who responded to CsA , the duration of CsA (median follow-up time: 27 months, 1-101 months) was positively correlated with progression-free survival (r=0.603, P <.001). Multivariate analysis revealed that 36-65 years of age, SAA/VSAA, and no specific therapy were independent risk factors for inferior survival.CONCLUSION:The treatment of elderly patients with AA still faces challenges. CsA is benefit to the survival of SAA/VSAA patients. AA patients, who responded to initialy CsA treatment, may benefit from prolonged CsA treatment. In view of the side effects of CsA, the timing of withdrawal is worth further exploration.
AN ONE-CENTER EPIDEMIOGRAPHIC ANALYSIS OF MALIGNANCY IN RENAL ALLOGRAFY RECIPIENTS

Abstract:OBJECTIVE To analyze the epidemiographic features of malignancy in renal allograft recipients in a single center.METHODOLOGY This analysis included 898 patients who received renal allografts between July 1997 and December 1999 andanti-rejection treatment for at least 3 months. RESULTS After transplantation among the 898 recipients, 5 (a incidence about0.6% )were diagnosed with malignancies that included bladder transitional cell carcinoma, squanous bladder cell carcinoma, pri-mary liver cell carcinoma,non-Hodgkin' s lymphoma, Kaposi' s sarcoma. The mean age at diagnosis of malignancy was 56.6+ 3.7(range 51-56)years,and the mean duration of immunosuppressive treatment 27 ± 17(range 8-48)months. Four of the patients wereon cyclosporin-azathioprine-prednisolone,and one on cyclosporin-mofetil-prednisolone regime. The treatments for malignancies con-sisted of completely removal of tumor mass, decrement of immunosuppressive treatment (cyclosporin by 1/2-1/3, azathioprine/mofetil by 1/2 or withdrawal),chemotherapy or radiotherpay, and traditional Chinese medicine. All the five cases were cured andhave been followed-up for 28 ± 15(range 11-50)months without malignance recurrence, allograft dysfunction, or episode of rejec-tion. CONCLUSION Malignance is an important complication in renal transplantation. Strict follow-up and early diagnosis arecritical.

Abstract:OBJECTIVE:To explore the long-term outcome and prognostic factors of primary focal segmental glomerulosclerosis (FSGS) in Chinese adults.METHODS:A retrospective study was conducted in 104 adults with primary FSGS. Clinical records and renal biopsies were reviewed. The primary endpoint was end-stage renal disease (ESRD) and death.RESULTS:The most frequent FSGS variant was not otherwise specified (45.2%), followed by tip lesion (20.2%). 57 patients presented nephrotic syndrome. Among the nephrotic patients with first episode, the total remission rate was 94.75% by prednisone treatment and 50% by Tripterygium wilfordii (TW) multiglycoside treatment. Steroid-resistant patients treated by TW multiglycoside achieved a total remission rate of 80%. Over a median follow-up of 72 months, 25 patients developed ESRD. The median renal survival was 116 months. The renal survival rates were 83.5% and 43.8% at 5 and 10 years after the biopsy, respectively. By multivariate Cox proportional analysis, the degree of proteinuria, acute kidney injury (AKI) with chronic kidney disease (CKD) Stage 3, chronic tubulointerstitial injury, and complete and partial remission were independent predictors of ESRD.CONCLUSION:TW is a new potential treatment for steroid-resistant nephrotic FSGS. The 5-year renal outcome in adults with primary FSGS in China is better in comparison to the west. Severe proteinuria, CKD with AKI, chronic tubulointerstitial injury, and no response were independent risk factors of prognosis.

Abstract:Objective: To research and explore Shenwuyishen Tablets,the prescription of Professor Zou Yunxiang,in the clinical use of chronic renal failure(CRF).Methods: Using the methods of a multicenter,randomized,double blind double-dummy,parallel control for positive drug.The curative efficacy of Shenwuyishen Tablets for CRF with Qi and Yin deficiency and dampness syndrome was evaluated.Meanwhile,basic researches for Shenwuyishen Tablets,including pharmacology,pharmacodynamics and molecular cytology,were developed.Results: Shenwuyishen Tablets can significantly improve the CRF with Qi and Yin deficiency and dampness syndrome in clinical manifestations.It lowered BUN and SCr levels,reduced urinary protein loss,increased CCr.The results of the experiment group were better than that of the control group.The mechanism of Shenwuyishen Tablets may be associated with regulating hormone levels in a variety of ways,improving hemodynamics,inhibiting mRNA expression of cell enhancer factor and extracellular matrix components,inducing apoptosis,reducing the damage of glomerular,improving the functions of kidney tubules and tubulointerstitial,thus protecting the renal function and to delay the progression of chronic renal failure.

Abstract:Objectives: Primary Shigren's syndrome (pSS) is an important cause of pulmonary arterial hypertension (PAH), which remains insufficiently studied and needs attention. This study aimed to investigate the clinical characteristics, risk factors, prognosis and risk assessment of pSS-PAH.Methods: We established a multicentre cohort of pSS-PAH diagnosed by right heart catheterisation. The case control study was conducted with pSS-non-PAH patients as a control group to identify the risk factors for PAH. In the cohort study, survival was calculated, and risk assessment was performed at both baseline and follow-up visits.Results: In total, 103 patients with pSS-PAH were enrolled, with 526 pSS-non-PAH patients as controls. The presence of anti-SSB (p<0.001, OR 4.095) and anti-U1RNIP antibodies (p<0.001, OR 29.518), the age of pSS onset (p<0.001, OR 0.651) and the positivity of corneal staining (p=0.003, OR 0.409) were identified as independent risk factors for PAH. The 1-, 3- and 5-year survival rates were 94.0%, 88.8% and 79.0%, respectively. Cardiac index (p=0.010, hazard ratio (HR) 0.161), pulmonary vascular resistance (1)=0.016, HR 1.105) and Sjogren's syndrome disease damage index (p=0.006, HR 1.570) were identified as potential predictors of death in pSS-PAH. Long-term outcomes were improved in patients in the low-risk category at baseline (p=0.002) and follow-up (p<0.0001).Conclusion: The routine screening of PAH is suggested in pSS patients with early onset and positivity for anti-SSB or anti-U1RNP antibodies. Patient prognosis might he improved by improving reserved cardiopulmonary function, by achieving a damage-free state and especially by achieving low-risk category, which supports the treat-to-target strategy for pSS-PAH.

Abstract:Background Pulmonary involvement is common in primary Sjögren’s syndrome (pSS), which seriously affects the prognosis and survival rate. However, the treatment of those patients has hardly been explored before. Most studies were specifically designed to evaluate sicca features. Objectives To analyze efficacy and safety of corticosteroid therapy combined with hydroxychloroquine (HCQ) or intravenous cyclophosphamide (CTX) in pSS-associated lung disease. Methods A total of 15 patients were recruited in the prospective study. Among them, 7 patients took oral prednisone combined with HCQ and 8 patients received oral prednisone combined with intravenous CTX. Prednisone was prescribed initially at a dosage of 30-40mg/day, and was tapered to 7.5mg/d within 4 months. This dosage of prednisone was maintained or tapered for the rest of the follow up as appropriate. HCQ was administered at a dosage of 200mg, twice a day. CTX was administered intravenously at an initial dosage of 400mg, every 2 weeks for 6 months, and then tapered to 400mg every 4 weeks. Results Fourteen patients receiving oral prednisone combined with HCQ or intravenous CTX were followed up for 11.57±4.91 months. One patient from CTX group withdrew from the study for stopping DMARDs to have surgery. Six of 7 patients (85.71%) were female of both groups. The mean age was 55.57±14.52 and 55.43±14.33 years old for HCQ group and CTX group, with median disease duration 120 and 180months, respectively. During the follow up, there was evidence of improvement in HRCT for most of the patients, and no obvious deterioration of PFT was observed. One patient in HCQ group presented hypoleukemia, which was ameliorated after reducing to 100mg, twice a day. Two patients had pneumonia and one had herpes zoster virus (HZV) infection in CTX group. No life-threatening side effect was observed in both groups. Conclusions Corticosteroid therapy combined with hydroxychloroquine or intravenous cyclophosphamide is administered with a favorable response seen in the majority of patients. Fundamental researches are providing scientific bases for the therapeutic effect of Chinese medicine. Professional Chinese medicine treatment can be integrated into the conventional management of primary Sjögren's syndrome.

Relationship between Traditional Chinese Medicine Syndrome Elements and Prognosis of Patients with IgA Nephropathy

Abstract:Objective. It is not clear which Traditional Chinese Medicine- (TCM-) related elements affect primary IgA nephropathy (IgAN) progression. Here, we explored the risk factors, based on TCM syndrome elements, related to the prognosis of primary IgAN patients. Methods. We analyzed patients with newly diagnosed, biopsy-proven IgAN at a single institution from December 2013 to September 2021. Basic clinical and pathological characteristics were assessed at the time of renal biopsy. The study endpoint was end-stage renal disease (ESRD: eGFR 30% from baseline. Kaplan‒Meier survival analysis was used to explore the role of TCM syndrome elements in IgAN progression. Multivariate Cox regression analysis with adjustment for traditional risk factors was performed to explore TCM syndrome elements that may influence patient prognosis. The factors correlated with TCM syndrome elements in IgAN patients were further evaluated by logistic regression analysis. Results. During a median follow-up of 22.0 months, 53 (12.5%) of the 423 included IgAN patients reached the study endpoint. The main IgAN disease location elements were the kidney, liver, and spleen. The main IgAN disease nature elements were Yin-deficiency and Qi-deficiency, dampness, Yang-deficiency, phlegm, and Blood-deficiency. Kaplan‒Meier analysis identified three disease locations (liver, spleen, and kidney) and four disease natures (Qi-deficiency, Yang-deficiency, phlegm, and dampness) as elements associated with poor renal survival in IgAN patients. In multivariate Cox regression analysis, baseline Yang-deficiency was an independent risk predictor of poor prognosis in primary IgAN patients (hazard ratio 2.338; 95% confidence interval [CI]: 1.208–4.525; P = 0.012 ) after adjustment for traditional risk factors. Furthermore, logistic regression analysis identified being female (odds ratio [OR] 2.518; 95% CI: 1.538–4.122; P < 0.001 ), older age (OR 1.043; 95% CI: 1.022–1.065; P < 0.001 ), low hemoglobin levels (OR 0.984; 95% CI: 0.971–0.996; P = 0.013 ), and cellular/fibrocellular crescents (OR 1.706; 95% CI: 1.068–2.728; P = 0.026 ) as factors affecting Yang-deficiency in IgAN patients. Conclusions. Yang-deficiency independently predicts the risk of poor prognosis in primary IgAN patients. Being female, older age, low hemoglobin levels, and cellular/fibrocellular crescents were independently associated with Yang-deficiency in IgAN patients.

Renal Lesions in Patients with Sjogren's Syndrome

Abstract:Objective:Renal involvement is an important clinical concern in patients with primary Sjgren's syndrome(pSS).In this study,we investigate the clinical features of the renal involvement and the pathological characteristics of renal lesion in pSS by retrospective analysis of 14 pSS patients who underwent renal biopsy in our clinic. Methodology:Fourteen pSS patients,all female,aged between 23 and 56,were included in this retrospective analysis.Renal biopsy was performed in all of the 14 patients (4～120) months after the onset of the disease.Clinical data with respect to tubulointerstitial function and autoimmunity,and renal pathological data with respect to grades of glomerular and tubulointertitial lesion were analyzed,and the co relationships among clinical and pathological parameters were investigated. Results:①Clinically,all the patients which were middle age women,and renal manifestation included hyposthenuria (14/14,100%),renal tubular acidosis(13/14,92 9%),mild proteinuria (8/14,57 5%) and microscopic hematuria (3/14,21 4%).In the 13 patients with renal tubular acidosis,8 were classified as Type Ⅰ,2 as Type Ⅱ,2 as mixed type and 1 as incomplete distal RTA.②Immunological study revealed that 13 of the 14 had multiple positivities(+) for autoantibodies.Increased serum IgG was found in 8 of 11 patients,elevated serum cryoglobulin in 6/12,hyperglobulinemia in 10/14,hypergamalobulinemia in 7/7.③Renal histology confirmed that the prominent lesion located in the tubulointerstital area,with immunodeposits neither in glomeruli nor along tubular basement membrane,nor on any vessel wall.Correlation analysis indicated that renal tubular lesion correlated significantly with the disease course,hyperglobinemia,and cryoglobinemia. Conclusion:Renal involvement in pSS patients is characterized clinically by prominent tubular acidosis and decreased urinary osmolarity,and pathologically by the chronic tubularinterstitial lesion as increased intertubular area and cellular deeper intersitial infiltration plus tubular cell degradation.

Abstract:Objective To investigate the clinical and X ray findings of renal tubular acidosis (RTA). Materials and Methods Clinical manifestations and X ray findings in 27 patients with RTA (mean age 30.3 years) were retrospectively analyzed. Imaging materials included KUB (n=18), IVU (n=6) and CT scan (n=4).Results Secondary type was composed of 70.37% cases, of which 63.16% was secondary to Sjgren's syndrome. Skeleton abnormality was seen in 45.8% of the cases, which was manifested as growth retardation and osteomalacia. Nephrocalcinosis was found in 38.9% of the cases, exhibiting calcifications in bilateral renal pyramids. Half patients with a course of disease over one year had nephrocalcinosis. Conclusion The diagnosis of RTA can be made when osteomalacia and nephrocalcinosis, especially calcifications in bilateral renal pyramids, are present.
