Abstract:Objective To evaluate the effects of different strategies on short-and long-term clinical outcomes of renal transplantation in Chinese subjects.Methods 2520 renal transplantations were retrospectively evaluated,including 2490 first renal transplantations and 30 second renal transplantations.Triple-immunosuppressant including cyclosporine A,azathioprine or myeophenolate mofetil(MMF)and prednisone(Pred)was adopted.Patients receiving kidney transplantation were given low dose immunosuppressants since 2000.Immunosuppressants including tacrolimus,MMF and Pred were adopted in some patients since 2000.Risk factors leading to graft loss and patients'death were analyzed.Results Until the cut date of June 30,2009,135 patients lost follow-up,and the follow-up rate was 94.6%.Incidence of acute(within 6 months post-transplantation) rejection was 18% among 2520 patients.Incidence of acute rejection (within 6 months post-transplantation) was 25.7% in panel reactive antibody (PRA) positive patients,significantly higher than 17.0% in PRA negative patients(P<0.05).Incidence of acute rejection within 6 months post-transplantation was 16.9% in HLA mismatches<4 patients,significantly lower than 23.7% in HLA≥4 patients (P<0.01).Total patient/death censored graft 1-,3-,5- and 1O-year survivals were 94.5%/96.0%,91.6%/93.1%,88.5%/90.1% and 81.7%/80.6%,respectively.Acute rejection and immunosuppressant regimen were independent risks for allograft loss.1mmunosuppressant regiment,pulmonary infection,cardio-brain-vessel accident, hepatic failure and tumor were independent risks for patients' death.Conclusion Renal allograft and patient survival appeared to be improved by optimal immunosuppressant regimen,strict HLA match and efficient post-transplant complication prophylaxis.
Outcomes of Liver–kidney Transplantation in Patients with Primary Hyperoxaluria: an Analysis of the Scientific Registry of Transplant Recipients Database

Abstract:Primary hyperoxaluria (PH) is an inherited disease lacking of hepatic oxalic acid metabolic enzymes which could lead to irreverisible renal damage. Currently, liver–kidney transplantation is a curative but highly invasive therapy used to treat patients with PH. However, limited studies have focused on combined liver–kidney transplantation (CLKT) and sequential liver and kidney transplantation (SLKT) in patients with PH. The present study included 201 patients with PH who received both liver and kidney transplants and who were listed on the Scientific Registry of Transplant Recipients from 1987 to 2018. According to the liver–kidney transplant procedure, patients were separated into a CLKT group and a SLKT group. Patient demographics and transplant outcomes were assessed in each group. Compared with the SLKT group, The CLKT group got a worse pretransplant dialysis condition in both the proportion of patients under pretransplant dialysis (p = 0.048) and the duration of the pretransplant dialysis (p < 0.001). The SLKT group got higher human leukocyte antigen mismatch score of kidney donor (p < 0.001) and liver donor (p = 0.003). The CLKT group utilized higher proportion (98.9%) of organs from a single deceased donor, while the SLKT group utilized 75.0% of organs from deceased liver donors and only 35.0% of organs from deceased kidney donors (p < 0.001). Kidney function measured by serum creatinine concentration before liver transplantation (LT) or CLKT was similar (p = 0.305) between groups. Patient survival was not significantly different between the two groups (p = 0.717) and liver (p = 0.685) and kidney (p = 0.464) graft outcomes were comparable between the two groups. SLKT seems to be an alternative option with strict condition for CLKT, further exploration about the SLKT is still required.
Renal Transplantation in China: Ten Years of Experience at Nanjing Jinling Hospital.

Abstract:The current are evidence of success in renal transplantation in this center, and this is consistent with data from the UNOS study. However, the current study also suggests that transplantation in China is quite different from that in Europe and America. The undergoing diseases in China are mainly primary glomerular diseases; IgA nephropathy accounts for a quarter of ESRD, while diabetes nephropathy accounts for only 3.13% of ESRD. Only small doses of MMF and Tac can inhibit rejection and result in a decreased incidence of infection. The leading causes of death after renal transplantation are severe pneumonia and liver failure. With the exception of late rejection, death with a functional graft is another significant cause of graft loss.

Clinical Presentations and Outcome Analysis of Renal Transplant in Elderly Patients——a Single Center Retrospective Study

Abstract:Objective To analyze the characteristics and prognosis of elderly renal transplant recipients.Methods The authors included 130 recipients older than or equal to 60 years at the time of operation in elderly group,and the paired 130 patients receiving contralateral renal transplants from the same donors and younger than 60 years in control group.All the patients received renal transplant in Kidney Disease Center of the First Affiliated Hospital of Zhejiang University from Nov 1994 to Dec 2013.Results The average age of the patients of elderly group was (63±3) years old,whereas the patients in control group were (41± 10) years old.There was no significant difference in sex,type of dialysis,number of mismatched HLA,level of panel reactive antibodies (PRA),percentage of receiving induction therapy or immunosuppressive regimen between elderly group and control group,except that the patients in old group had a longer duration of dialysis.The patients of elderly group had a lower level of serum creatinine than control at the follow up times from 6 months to 24 months after transplant.The doses of immunosuppressives were lower in elderly group compared with the control group whereas the concentration of tacrolimus or cyclosporine was same.The dose of prednisone in old group was lower compared with control after 6 months post-transplantation.The patients of elderly group had high percentages of pulmonary infection and new-onset diabetes mellitus compared with the control group.Until June 2014,the follow-up rate of all patients was 85.4％;the median follow-up time was 70.4 months in elderly group and 79.9 months in control group.There was no significant difference in mortality rate or graft loss rate between elderly group and control group.Pulmonary infection (HR=2.981,P=0.018),hepatitis C virus infection (HR=5.797,P=0.003) and malignancies (HR=5.228,P=0.005) were correlated with the survival rate of the elderly group.Conclusions Elderly renal transplant recipients have a similar prognosis compared with the younger ones.Pulmonary infection,hepatitis C virus infection and malignancy are related risk factors for the survival rate of elderly patients.
Improved Clinical Outcomes in Chinese Renal Allograft Recipients Receiving Lower Dose Immunosuppressants

Abstract:Background. The application of potent immunosuppressants has decreased the incidence of acute rejection and increased short- and long-term graft survival; however, these drugs cause a variety of complications. In China, many transplant centers have adopted the immunosuppressive protocols based on the white population, neglecting the differences between the races. The purpose of this study was to explore a suitable immunosuppressive regimen for Chinese renal allograft recipients.Methods. Two hundred cadaveric renal allograft recipients who underwent transplantation between July 1999 and October 2001 were observed. Before October 2000, 104 recipients received the conventional dose of immunosuppressants; thereafter, 96 recipients received lower dose treatment. Doses of immunosuppressive agents, the incidence of acute rejection and pulmonary infection, and patient and graft survival rates were compared between the two groups.Results. Doses of mycophenolate mofetil (MMF) and cyclosporine A (CsA) administered in the conventional dose group were significantly higher than in the lower dose group at 3 months posttransplant, as was prednisone at 6 months posttransplant. The incidence of acute rejection and subclinical rejection that was biopsy-proven or diagnosed by clinical manifestations was 17.3% and 19.8%, respectively, in the conventional dose group and the lower dose group within the first 6 months, and no significant difference was noted (P=0.55). The incidence of pulmonary infection, especially severe infection, was much higher in the conventional treatment group (40.1% and 26.9%, respectively) than that in the lower dose group (11.5% and 5.2%, respectively), and the differences were statistically significant (P<0.001). The corresponding 1-year survival rate of patients was 87.4% and 97.9% (P<0.01), and that of renal grafts was 85.5% and 96.9% (P<0.01), for patients receiving conventional dose and lower dose immunosuppressive drugs, respectively. The rate of death with a functioning allograft caused by infection in the conventional dose group was significantly higher than that in the lower dose group (12.5% vs. 0%, P<0.01).Conclusions. The regimen of lower dose MMF, CsA, and prednisone in combination can significantly reduce the incidence of pulmonary infection, especially severe pulmonary infection, without increasing the incidence and severity of allograft rejection.

Abstract:During the last 29 years the dramatic changes that have taken place in our transplant programs and in solid organ transplantation permit us to face tomorrow with courage and confidence. In the future we will be deeply concerned that the long-term benefit of more effective immunosuppression or novel strategies might be unfavorably impacted by the tendency to use organs whose intrinsic lifespan may be limited. We are deeply concerned by the inexorably increasing discrepancy between supply and demand for deceased kidneys, especially for the added morbidity that this inevitably entails for those patients with ESRD who do not have live donors. Our program continues to seek better ways to improve survival and the quality of the life of our patient population.
Postreperfusion Renal Allograft Biopsy Predicts Outcome of Single-Kidney Transplantation: A 10-Year Observational Study in China

Abstract:Introduction: Biopsy findings often lead to the discard of many donor kidneys although their clinical value is not fully understood. We investigated the predictive value of postreperfusion biopsy on long-term allograft outcome after single-kidney transplantation. Methods: We retrospectively evaluated the significance of histologic findings, read by experienced renal pathologists, in 461 postreperfusion biopsy specimens collected from 2010 to 2017 after deceased donor renal transplant; and performed time-to-event analyses to determine the association between histology and hazard of death-censored graft failure. Recipients were followed-up with over a median time of 6.8 (range, 0.2-11.9) years. We assessed specimens using the Remuzzi score (scale of 0-12) and categorized them into low-score (<= 3) and high-score (>3) groups. Kappa coefficients were calculated to assess agreement in procurement versus reperfusion biopsies. Results: High Remuzzi score kidneys came from older donors with a higher incidence of hypertension, higher final creatinine, death from cerebrovascular disease, expanded criteria donor, and a higher kidney donor risk index (KDRI) (all P < 0.001). In adjusted analyses, Remuzzi score was independently associated with death-censored graft failure (hazard ratio [HR] 1.389 for each 1 score rise in Remuzzi score, 95% confidence interval 1.181-1.633, P < 0.001). Overall histologic agreement (procurement biopsy versus reperfusion biopsy) was kappa = 0.137. Conclusion: Our findings suggest that postreperfusion biopsy is associated with long-time graft outcomes after transplant from a deceased donor. Agreement between procurement and reperfusion biopsy was found to be low. Prospective trials are necessary to optimize procurement biopsy practices.
Outcome of Single Kidney Transplant from Pediatric Donors Younger Than 5 Years of Age after Cardiac Death in China: A Single Center Experience

Abstract:OBJECTIVES:This study was conducted to evaluate the outcomes of single kidney transplant from pediatric donors in China younger than 5 years old after cardiac death.MATERIALS AND METHODS:We retrospectively reviewed single renal transplants from pediatric donors of cardiac death between January 2012 and June 2013 at the First Affiliated Hospital of Zhejiang University, China.RESULTS:Eleven recipients received single kidney transplant from 6 pediatric donors of cardiac death donors at our center (aged, < 5 years). The median donor age was 39 months (range, 17-56 mo). The median donor weight was 14.5 kg (range, 11-20 kg). While the median recipient age was 35 years (range, 12-55 y). The median recipient weight was 42.5 kg (range, 21-55 kg). The recipient serum creatinine level gradually decreased and the 1-year serum creatinine was 77.7 ± 17.6 μmol/L. Delayed graft function occurred in 5 recipients (45.5%), and all of them recovered within 1 month after transplant. No episodes of acute rejection occurred. No vascular thrombosis or stenosis after ureteroneocystostomy was seen. One patient had urine leakage on the eighth day after transplant, and was cured through a ureter reimplantation. The graft size increased significantly the first month after transplant compared with that recorded immediately after reperfusion (P = .011). The 1-year patient/graft survival was 100%.CONCLUSIONS:Use of single kidney from pediatric donors after cardiac death (aged, < 5 y) is a safe and effective procedure and can greatly expand the donor pool.
High Serum Level of the Soluble CD30 Identifies Chinese Kidney Transplant Recipients at High Risk of Unfavorable Outcome

Abstract:Background. We sought investigate the relationship between serum level of sCD30 and recipient/graft survival rates, rejection types, as well as other prognostic factors among Chinese kidney transplant patients. Materials and methods. We performed enzyme-linked immunosorbent assays of serum sCD30 levels in duplicate among retrospective cohort of 707 renal transplant patients. Results. The incidences of rejection increased in relation to the pretransplant sCD30 level. The reversal rates of rejection were 100%, 90.6%, and 78.6% for the low, intermediate, and high sCD30 groups. This observation suggested that high levels of sCD30 and pretransplant panel-reactive antibody (PRA)-positive patients are risk factors for acute rejection with odds ratios of 6.862 and 1.756. High sCD30 was an independent risk factor for functional graft survival. The 5-year graft survival rates were 99.39% +/- 6.1%, 93.11% +/- 1.93%, and 82.07% +/- 3.97% among the low, intermediate, and high sCD30 groups, while the 5-year recipient survival rates were 89.25% +/- 2.41%, 91.82% +/- 1.64%, and 88.85% +/- 2.36%, respectively. Increased sCD30 levels were observed among patients who were PRA-positive, cytomegalovirus antigens or antibodies positive, on long-term dialysis, and <= 20 years old. Conclusions. Pretransplant sCD30 serum levels reflect immune status.

Abstract:Objective To document the impact of conversion to mycophenolate mofetil (MMF)at different time points after transplantation on the renal function of renal function.Methods A longterm,multicenter,non-interventional and observational study was done.Two cohorts were included:One was Switch cohort (340 cases) including renal allograft recipients who switched to MMF at least 6 months after renal transplantation and followed up for 4 years after switch; The other was Stay cohort (123 cases),including renal allograft recipients who received MMF treatment after transplantation and followed up for 4 years after enrollment.Results GFR values of patients in Switch cohort was significantly increased after switch,and the change in GFR slope was 3.1 mL· min-1 · year-1 (P＜0.01).GFR values of patients in Stay cohort kept steady before and after enrollment,and the change in GFR slope was 0.44 mL·min-1 ·year-1 (P＞0.05).Statistically significant difference in the onset time of GFR decline (defined as 20％ decline from the baseline) was observed among subgroups within Switch cohort (P＜0.01),but there was no significant difference among subgroups within Stay cohort (P＞0.05).Stay cohort was 12％ higher than in Switch cohort every year.Conclusion Conversion to MMF ＞6 months or even many years after transplantation can obviously improve the renal function of recipients.The earlier conversion can benefit improvement of the renal function.

Abstract:Background: There have been some gradual changes in the distribution of renal biopsy pathological diagnoses during recent years. This study aimed to show changes in renal disease prevalence in China by investigating 10 patients diagnosed at our Kidney Disease Centre during the last 15 years. Methods and results: All patients aged 15-year-old or older who underwent renal biopsy at the First Affiliated Hospital, Zhejiang University, from 2001 to 2015 were enrolled. There were 5 common types of primary glomerulonephritis: IgA nephropathy (IgA N), membranous nephropathy (MN), mesangial progressive glomerulonephritis (MsPGN), minimal change disease (MCD), and focal segmental glomerulosclerosis (FSGS), which represented 50%, 16.8%, 15.9%, 8.1% and 2.5% of total cases, respectively. IgA nephropathy was the most common type of primary glomerulonephritis (PGN). Conclusions: Our results mostly showed a new trend that the diagnosis of IgA nephropathy was not increasing and the prevalence of membranous nephropathy had increased, becoming the second most common type of primary glomerulonephritis.Key POINTS Distinguished with other domestic studies, IgA nephropathy did not show a trend of continuous growth although it still had about the half proportion of PGN, whereas membranous nephropathy kept rising and became the second common PGN. Concerning SGN, LN peaked in the younger-age and middle-age groups with a significant female prevalence, DN, BANS and SV had a male predominance peaking in the middle-age and old-age groups.

Abstract:Objective: To investigate the prognostic factors and outcomes in patients with anti-neutrophil cytoplasmic antibody-associated glomerulonephritis (ANCA-GN) in Southern China Methods: A retrospective analysis of medical records of patients with ANCA-GN admitted to Shenzhen Hospital of Southern Medical University and Nanfang Hospital of Southern Medical University between September 2011 and September 2021 was performed. The clinical presentation, biological, and renal pathology were collected. In addition, the risk factors for end-stage renal disease (ESRD) and short-term overall survival in patients with ANCA-GN were analyzed. Results: A total of 93 patients with ANCA-GN were included in the study. Of them, 91.4%, were perinuclear anti-neutrophil cytoplasmic antibodies (MPO-pANCA)-positive. Approximately one-quarter (24.7%) of patients had progressed to ESRD, and 7.5% died within six months. Most patients presented with hematuria (94.6%), proteinuria (78.5%), elevated serum creatinine (86.0%), anemia (90.3%), and increased erythrocyte sedimentation rate (ESR) (44.1%). The majority (94.6%) of patients presented with crescent formations at histopathological examination. Serum creatinine, hemoglobin, and Birmingham vasculitis activity score (BVAS) were all independent factors for ESRD (P 0.05). The effect of Berden's histopathologic classification on ESRD has not been confirmed. Age at onset, ESR and cardiovascular involvement were all independent factors affecting short-term overall survival of patients with ANCA-GN (P<0.05). Conclusions: Serum creatinine, hemoglobin, and BVAS were all independent risk factors of ESRD, while ARRS and Berden's histopathologic classification were not. Abstract:OBJECTIVE:To investigate the influence of pre-transplant sCD30 level on the long-term survival rates of kidney transplant recipients and grafts among Chinese.METHODS:A retrospective cohort of 707 patients undergoing cadaver renal transplants between Dec.1998 and Aug 2003, 467 males and 240 females, aged 40 +/- 11, with their blood samples preserved was studied. The plasma levels of sCD30 were determined by ELISA.RESULTS:The 5-year graft survival/functional rates of the high sCD30 group were 77.7% +/- 3.5%/85.0% +/- 3.2%, significantly lower than those of the low and intermediate groups, 84.7% +/- 2.1%/98.9% +/- 1.1% and 88.1% +/- 2.9%/95.1% +/- 1.6% respectively (all P < 0.05). The 5-year recipient survival rate of the intermediate sCD30 group was 92.4% +/- 1.6%, higher than those of the low and high sCD30 groups, 84.7% +/- 3.9% and 87.1% +/- 2.7% respectively with a significant difference between the intermediate and high sCD30 groups (P = 0.032).CONCLUSIONS:Pre-transplant serum level of sCD30 reflects the immune status. Recipients with high sCD30 are prone to rejection while those with low sCD30 are prone to infections.

Abstract:OBJECTIVE:To explore the long-term outcome and prognostic factors of primary focal segmental glomerulosclerosis (FSGS) in Chinese adults.METHODS:A retrospective study was conducted in 104 adults with primary FSGS. Clinical records and renal biopsies were reviewed. The primary endpoint was end-stage renal disease (ESRD) and death.RESULTS:The most frequent FSGS variant was not otherwise specified (45.2%), followed by tip lesion (20.2%). 57 patients presented nephrotic syndrome. Among the nephrotic patients with first episode, the total remission rate was 94.75% by prednisone treatment and 50% by Tripterygium wilfordii (TW) multiglycoside treatment. Steroid-resistant patients treated by TW multiglycoside achieved a total remission rate of 80%. Over a median follow-up of 72 months, 25 patients developed ESRD. The median renal survival was 116 months. The renal survival rates were 83.5% and 43.8% at 5 and 10 years after the biopsy, respectively. By multivariate Cox proportional analysis, the degree of proteinuria, acute kidney injury (AKI) with chronic kidney disease (CKD) Stage 3, chronic tubulointerstitial injury, and complete and partial remission were independent predictors of ESRD.CONCLUSION:TW is a new potential treatment for steroid-resistant nephrotic FSGS. The 5-year renal outcome in adults with primary FSGS in China is better in comparison to the west. Severe proteinuria, CKD with AKI, chronic tubulointerstitial injury, and no response were independent risk factors of prognosis.

Abstract:Since the first kidney transplantation was performed in 1976, 4,306 renal transplantations have been performed at Shanghai No. 1 People's Hospital and Fujian Oriental Hospital. Nearly all (99%) of the grafts were from deceased donors. Graft survival rates in the era of modern immunosuppression and HLA-based organ allocation have improved substantially. The one-, 5- and 10-year graft survival rates for 2,575 transplants performed during 1995-2004 were 93.9%, 71.8% and 53.9%, respectively. Chronic rejection accounted for 54.3% of graft failures and 54.2% of patient deaths resulted from graft failure. We performed 48 combined liver-kidney transplants since 2001 and the first simultaneous human islet-kidney transplantation last year.

Abstract:Objective:To investigate the long-term renal survival rate and related risk factors of progression to renal failure in Chinese adult patients with IgA nephropathy(IgAN) and to quantify the effects of proteinuria during the follow-up on outcome in patients with IgAN.Methodology:The patients with biopsy-proven primary IgAN in the Nanjing Glomerulonephritis Registry were studied.Renal survival and the relationships between clinical parameters and renal outcomes were assessed.Results:1 126 patients were enrolled in this study.The 10,15,and 20-year cumulative renal survival rates,calculated by Kaplan-Meier method,were 85%,76% and 67% respectively.At the time of biopsy,proteinuria>1.0 g/d(HR 3.3,P<0.001),eGFR<60 ml/min per 1.73 m2(HR 2.2,P<0.001),hypertension(HR 2.0,P<0.001),and hyperuricemia(HR 1.8,P=0.002) were the independent risk factors.Multivariate COX analysis showed the time-average proteinuria(TA-P) during follow-up was the most important risk factor of renal failure.The patients with TA-P>1.0 g/d were associated with a 9.8-fold risk than that patients with TA-P<1.0 g/d(P<0.001),and 67.7-fold risk than those with TA-P<0.5 g/d(P<0.001).The patients with TA-P<0.5 g/d were better than those with TA-P between 0.5 and 1.0 g/d(HR 13.1,P<0.001).Conclusion:33% of Chinese adult patients with primary IgAN have progressed to End Stage Renal Disease(ESRD) within 20 years.Four clinical features—higher proteinuria,hypertension,impaired renal function,and hyperuricemia are independent predictors of an unfavorable renal outcome.The basic goal of antiproteinuric therapy for Chinese patients is to lower proteinuria to <1.0 g/d,and the optimal goal is to lower proteinuria to <0.5 g/d.
