Effect and Mechanism of Safe Reuse of High Permeability Dialysis--study in Vivo and Vitro

Abstract:Objective To investigate the effect and mechanism of safe reuse of high permeability dialysis through experimentation in vivo and vitro.Methods In vivo study: twenty stable hemodialysis patients were divided into two groups, using F6 dialyzer or F60 high permeability hemofilter. Acute complications were recorded during treatments. Pre- and post-dialysis circulating plasma IL-1β、TNFα、IL-6 levels were determined by ELISA both in first-use and in reuse patients. Post-dialysis plasma endotoxin levels were also measured in these two groups. In vitro: a closed-loop dialysis system was established to assemble hemodialysis. Specimens were obtained from blood compartments before(60 minutes) and after(120 minutes) a sterile filtrate of Pseudomonas maltophilia, which is one of the most popular bacteria grown from dialysate, adding to the dialysate compartments. All specimens and different levels of standard endotoxin were monitored for levels of interleukin-1 receptor antagonist (IL-1ra), which were determined in the supernatants of whole blood incubation.Results Hypotension and pyrogenic reactions occurred more frequently in patients treated with reused F60 hemofilters than those with reused F6 dialyzer(P0.05). The post-dialysis circulating plasma IL-6 levels in reuse high permeability group were significantly higher than predialysis levels(P0.05). No differences were found with respect to post-dialysis plasma endotoxin levels in these two groups, as for the membrane of high permeability dialyzer (F60 Polysulfone) and that of common hemodialyzer (Cuprophan and F6 Polysulfone ). During first-use, IL-1ra in all blood compartments increased conspicuously after (120 minutes) adding filtrate of Pseudomonas maltophilia, showing significant difference as compared with that adding the filtrate before (60 minutes),but there was no difference among the three membranes; during reuse, the increase of IL-1ra levels in F60 Polysulfone was much higher than those in Cuprophan and F6 Polysulfone.Conclusions High permeability dialysis is safe in first-use, but it causes more serious acute complications in reuse practice. Reuse might damage the membranes of high permeability dialyzer. Transfer of endotoxin and other cytokine-inducing substances across the high permeability membranes might play important role in causation of acute complications
