Abstract:Background: China has played an increasing role in development aid across Africa. Most recently, China has increased its external investments through the Belt and Road Initiative, China's signature infrastructure and trade drive to link China to Asia and Africa. This is likely to result in continuing growth of China's investment in health in sub Saharan Africa. While conflicting opinions have been raised regarding the motivation and value of these investments, few data have been solicited from those directly involved in China-Africa health aid. We conducted a qualitative study to collect information on perceptions and opinions regarding Chinese-supported health related activities in Africa through in-depth interviews among local African and Chinese participants in Malawi and Tanzania. Results: Our findings reveal shared experiences and views related to challenges in communication; cultural perspectives and historical context; divergence between political and business agendas; organization of aid implementation; management and leadership; and sustainability. Participants were broadly supportive and highly valued Chinese health aid. However, they also shared common insights that relate to challenging coordination between China and recipient countries; impediments to communication between health teams; and limited understanding of priorities and expectations. Further, they share perspectives about the need for shaping the assistance based on needs assessments as well as the importance of rigorous reporting, and monitoring and evaluation systems. Our findings suggest that China faces similar challenges to those experienced by other longstanding development aid and global health donors. As it continues to expand cooperation across Africa and other regions, it will be important for China to consider the issues identified through our study to help inform collaborative and effective global health assistance programs. The insights garnered from this research are not only relevant to China's engagement in Africa but for other global health assistance donors as well.

Abstract:Health is one of the essential ties between China and Africa. As the Chinese economy is experien-cing rapid development and the strategic transformation, the China-Africa cooperation is entering the era of communi-tas in all directions and the health-related aids and exchanges between China and Africa are experiencing new oppor-tunities and challenges. Previously, the China-Africa health cooperation has experienced three transitional stages from the assistance, collaboration and communitas. During the current global fight against Ebola outbreak period, while the world feared and wondered the Ebola in the beginning, China’s fast action on the fight had showed that not only the relationship between China and Africa is seen from the previous health aid legacy to Africa, but also is coming to the economic development communitas stage. Moreover, there are two essential government and non-government col-laboration established between China and Africa. So, when we analyze the fast action of China towards Ebola out-break, it is clearly shown that the China’s response is based in the perspective of humanitarian, super state accounta-ble responsibilities, and also focused on health system resilience in the epidemic countries. China and Africa are stepping into the economic development communitas stage. Therefore, this change of cooperation reflects the change of China’s position in the global health governance.
Public health engagement: new opportunities and challenges in 60 years of China’s health aid to Africa

Abstract:Along with China’s active engagement in global affairs over the past decade, China’s public health engagement (CPHE) has become an essential element of China’s 60th anniversary of health aid and cooperation with Africa. Through analysis, we found that CPHE in Africa reflects a new shift in the content and form of China’s foreign health aid. Compared with China’s medical teams which only involve bilateral cooperation, CPHE has a number of substantive characteristics, mainly including the construction of public health infrastructure, malaria control, participation in public health emergencies and the provision of global public goods. This complexity necessitates that CPHE be implemented in a different form from China’s medical teams, by adopting inclusive multiparty cooperation involving African and international partners. However, CPHE in Africa still faces a number of challenges in its practices. In particular, these challenges arise from the cross-cultural practice process, the African sociocultural context, the experience of public health and its participants. While the reasons for these challenges are diverse, this study argues that the main reason is the lack of multidisciplinary engagement in the Africanisation of the Chinese experience that corresponds to cross-cultural practice. On the Chinese side, a systematic strategy is needed to critically refine these challenges.

Abstract:It is now about 15 years since the introduction of the market into health care in China. This produced fundamental changes in the way that health care is financed and resulted in the disappearance of universal free basic health care. Responsibility for provision of health services has been devolved to the provincial and county governments, and healthcare providers have been given considerable financial independence. A fee for service system has been introduced, and several different payment mechanisms are now in operation. The new financing and pricing structures are responsible for greater inequity of access to services and more inefficient use of resources. These problems are widely acknowledged, and a range of solutions is being developed and tested. Since the introduction of the reforms the measurable health status of the population has not declined, probably as a result of overall improved socioeconomic conditions and a continued emphasis on prevention.
Understanding China’s shifting priorities and priority-setting processes in development assistance for health

Abstract:Abstract Over the past two decades, China has become a distinctive and increasingly important donor of development assistance for health (DAH). However, little is known about what factors influence China’s priority-setting for DAH. In this study, we provide an updated analysis of trends in the priorities of Chinese DAH and compare them to comparable trends among OECD Development Assistance Committee (DAC) donors using data from the AidData’s Global Chinese Development Finance Dataset (2000–2017, version 2.0) and the Creditor Reporting System (CRS) database (2000–2017). We also analyse Chinese medical aid exports before and after the start of the COVID-19 pandemic using a Chinese Aid Exports Database. We further explore the potential factors influencing China’s shifting priority-setting processes by reviewing Chinese official documents following Walt and Gilson’s policy analysis framework (context–actors–process–content) and by testing our conjectures empirically. We find that China has become an important DAH donor to most regions if measured using project value, including but not limited to Africa. China has prioritized aid to African and Asian countries as well as to CRS subsectors that are not prioritized by DAC donors, such as medical services and basic health infrastructure. Chinese quarterly medical aid exports almost quintupled after the start of the COVID-19 pandemic. Noticeably, China has allocated more attention to Asia, eye diseases and infectious disease outbreaks over time. In contrast, the priority given to malaria has declined over the same period. Regarding factors affecting priority shifts, the outbreaks of SARS and Ebola, the launch of the Belt and Road Initiative and the COVID-19 pandemic appear to be important milestones in the timeline of Chinese DAH. Unlike stereotypes of China as a ‘lone wolf’ donor, our analysis suggests multilateral processes are influential in informing and setting Chinese DAH priorities.

Abstract:BACKGROUND:China has been described as a "rogue" donor suspected of using foreign assistance to obtain raw materials, promote exports, and strengthen its business links with aid recipient countries. However, the factors influencing China's development assistance policy have rarely been analysed, particularly those related to its health aid. This study explores the factors that could affect the allocation of China's development assistance for health (DAH) from three key aspects: recipient need, recipient merit, and donor interest.METHODS:Analysis was based on cross-sectional data of China's DAH from 2006 to 2014. This study explores the following characteristics of the 82 recipient countries identified in the data: the association between China's allocation of DAH and recipient need (measured by Gross Domestics Product [GDP] per capita and all-cause Disability-Adjusted Life Years [DALYs]), recipient merit (measured by government effectiveness and human rights violations) and donor interests (measured by China's export, United Nations voting alignment, and recipient natural resources). A stratified analysis was conducted to understand these associations in different development contexts and the factors that influenced each type of DAH.RESULTS:Multivariate Spearman correlation suggested that the most significant factors influencing China's allocation of DAH were the recipient countries' GDP per capita (r = -0.31941, P = 0.0049) and human rights conditions (r = -0.23227, P = 0.0435). Health workforce was associated with medical team deployment (r = -0.20929, P = 0.0715), while malaria DALYs was associated with anti-malaria center establishment (r = 0.46473, P < 0.0001). According to the sub-group analysis, donor interests such as trade and natural resources only slightly influenced DAH allocation.CONCLUSION:Recipient need and merit strongly influence China's DAH allocation while donor interests only slightly influence DAH allocation in certain development contexts.
