Abstract:Some people with mental illness in China do not receive treatment. We explored how stigma and familial obligation influenced accessibility of social support for patients with depression in China and the potential acceptability of peer support programs. Semi-structured qualitative interviews were conducted with five psychiatrists and 16 patients receiving care for depression from a large psychiatric hospital in Jining, Shandong Province of China. Patients with mental illness reported barriers that prevented them from (a) receiving treatment and (b) relying on informal social support from family members, including stigma, somatization, and community norms. Circumventing these barriers, peer support (i.e., support from others with depression) was viewed by patients as an acceptable means of exchanging information and relying on others for support. Formative research on peer support programs to examine programming and activities may help reduce the burden of unmet mental health care needs in China.

Abstract:Background: Self-stigma exerts a range of adversities for persons with severe mental illness (SMI), however, little is known about the association between peer contact, social support and self-stigma. Aims: This study aimed to explore the mediating role of social support on the relationship between peer contact and self-stigma among persons with SMI in Hong Kong. Methods: A total of 159 persons with SMI (schizophrenia and mood disorder) in community service centres participated in the study through completing a survey on self-stigma, social functioning, social support, perception of peer contact and mass media. Logistic regression was utilised to explore the influencing factors of self-stigma among the participants. Results: The results showed that 81.1% of participants reported moderate to severe levels of self-stigma. Self-stigma was significantly associated with diverse factors (e.g. social functioning). Importantly, positive peer contact was significantly associated with lower self-stigma of persons with SMI. Social support acted as a mediator between peer contact and self-stigma. Conclusion: The results of this study suggest that contact-based interventions, such as enhancing positive peer-to-peer contact, should be conducted for reducing self-stigma among persons with SMI.

Abstract:People with mental illness are challenged by self-stigma, leading to a sense of behavioral futility and reduced goal seeking. Honest, Open, Proud (HOP) program is a peer service program that utilizes strategic disclosure to combat self-stigma. HOP was developed and predominantly examined in western culture, leaving unknown its effectiveness in East Asian countries. The current study reports adaptation and evaluation of HOP for Chinese people living with serious mental illness. Adaption underwent a community-based participatory research approach to assure cultural responsiveness. A pilot randomized controlled trial with 135 participants revealed that HOP diminished self-stigma in participants. Satisfaction and feasibility assessments indicated HOP was well received within the focal community. Results suggested HOP had benefits on disclosure self-efficacy and self-stigma for those participating in the program. Future research should further examine cultural mediators of peer support and disclosure on stigma and empowerment.

Abstract:From existing empirical research, we identified that Chinese college students commonly experience stigma surrounding mental illness and found some factors that support them in resisting the stigma and achieving psychological health. However, less research provides qualitative data involving individual experiences and insights on mental illness within this group of college students. This study, based on Linking Hearts (an internationally cooperative research-sharing project between China and Canada), was conducted in Shandong, Jinan, and aims to promote the mental health of college students by empowering interdisciplinary professionals and students. Through the research project, this study analyzed the materials from 24 focus groups, explored the understanding of mental illness and prevalence of mental illness stigma in Chinese colleges at the present time, administered a background questionnaire, and provided statistical support for some revealed themes. The final themes are as follows: mental illness is stereotyped as “severe, pathetic, and complicated”; the misconception of “visiting a psychological counselor is scary”; from public stigma to self-stigma; barriers deterring students from seeking help or accessing services; two sides of the same coin: peer support versus peer pressure.

Application of mental illness stigma theory to Chinese societies: synthesis and new directions

Abstract:The rapidly-evolving literature concerning stigma towards psychiatric illnesses among Chinese groups has demonstrated pervasive negative attitudes and discriminatory treatment towards people with mental illness. However, a systematic integration of current stigma theories and empirical findings to examine how stigma processes may occur among Chinese ethnic groups has yet to be undertaken. This paper first introduces several major stigma models, and specifies how these models provide a theoretical basis as to how stigma broadly acts on individuals with schizophrenia through three main mechanisms: direct individual discrimination, internalisation of negative stereotypes, and structural discrimination. In Chinese societies, the particular manifestations of stigma associated with schizophrenia are shaped by cultural meanings embedded within Confucianism, the centrality of "face", and pejorative aetiological beliefs of mental illnesses. These cultural meanings are reflected in severe and culturally-specific expressions of stigma in Chinese societies. Implications and directions to advance stigma research within Chinese cultural settings are provided.

Abstract:Accessible SummaryWhat is known on the subject? Having a mental illness may undermine men's self‐image as a father while having children may facilitate their recovery. Many fathers who experience mental health issues experience stigma and are reluctant to seek support in their parental role. Most studies in this field focus on fathers from Western countries. What does the paper add to existing knowledge? Similar to Western fathers, Chinese fathers reported both negative parenting (parenting disengagement, harsh parenting and inconsistent discipline) and positive father–child interactions when ill. Unlike their Western counterparts, Chinese fathers relied on extended family support in their parenting role, though this support was problematic at times. What are the implications for practice? This research helps mental health nurses understand the experiences of Chinese fathers with mental illness. Findings suggest that specific cultural adaptions to family‐focused interventions are needed for the Chinese context including considering extended family members (e.g. grandparents). AbstractIntroductionParenting roles are important for those with a mental illness. While research on mothers is well established there is scant focus on fathers with mental illness, especially in non‐Anglophone countries.AimTo better understand the parenting and family experiences of fathers, who have a mental illness, from the People's Republic of China.MethodQualitative semi‐structured interviews were conducted with nine Chinese fathers with mental illness, using Interpretative Phenomenological Analysis.ResultsSeven superordinate themes were identified: (i) mental illness undermines idealized images of fatherhood; (ii) parenting in the context of mental illness; (iii) concerns about the negative impact of their illness on children; (iv) children as a burden and a source of hope; (v) stigma; (vi) relying on family support; and (vii) unmet professional and peer support needs.DiscussionChinese fathers held a gendered image of fatherhood, and experienced self‐stigma for not living up to their ideal sense of fatherhood, which resulted in a reluctance to seek support. This study adds to the existing literature by focusing on Chinese fathers. Although many of their experiences are similar to fathers from Western countries, some differences were found, notably around a reliance on extended family support.Implications for practiceNurses and other mental health professionals need to adopt a family‐focused, culturally informed approach to supporting Chinese fathers with mental illness and their families.

Abstract:The self-stigma of mental illness can undermine self-esteem leading to greater depression. This study examines the impact of varied models of self-stigma on Chinese with serious mental illness. First, we validate a progressive model of self-stigma. Then, we examine the mediating effects of self-esteem on stigma harm and depression. Finally, we examine mediating effects of disclosure and secrecy on stigma harm and depression. One hundred thirty-five Chinese with serious mental illness completed measures of the four progressive stages of self-stigma, self-esteem, depression, disclosure-self-efficacy, and secrecy-self-efficacy. Findings of structural equation modeling supported the progressive model of self-stigma. Self-esteem was found to positively mediate stigma harm on depression. While secrecy seemed to have no effect on the self-stigma and self-esteem path, disclosure was found to positively mediate stigma harm on self-esteem. These findings have implications for the development of programs meant to diminish self-stigma.
A longitudinal study on the effects of social support on self-stigma, psychiatric symptoms, and personal and social functioning in community patients with severe mental illnesses in China

Background:Few studies have examined whether social support contributes to better consequences among chronic patients with severe mental illnesses (SMI) in their community recovery stage and whether self-stigma would be a mechanism through which social support impacts psychiatric symptoms and personal and social functioning.Aims:This study aimed to examine prospective associations of social support with long-term self-stigma, psychiatric symptoms, and personal and social functioning, and to investigate whether self-stigma would mediate the associations of social support with psychiatric symptoms and personal and social functioning among patients with SMI.Methods:A total of 312 persons with SMI (schizophrenia and bipolar disorder) in their community recovery stage participated in the study. Social support, self-stigma, psychiatric symptoms, and personal and social functioning were evaluated at baseline. The follow-up assessment was conducted at 6 months with the baseline measures except for social support. Hierarchical linear regression and mediation analysis were performed.Results:The results showed that baseline social support predicted decreases in stigma (β = −.115, p = .029) and psychiatric symptoms (β = −.193, p < .001), and increases in personal and social functioning (β = .134, p = .008) over 6 months, after adjusting for relevant covariates. Stigma at 6 months partially mediated the association between baseline social support and 6-month psychiatric symptoms (indirect effect: β = −.043, CI [−0.074, −0.018]). Stigma and psychiatric symptoms at 6 months together mediated the association between baseline social support and 6-month personal and social functioning (indirect effect: β = .084, 95% CI [0.029, 0.143]).Conclusion:It is necessary to provide comprehensive social support services and stigma reduction interventions at the community level to improve the prognosis of SMI.
