Hemifacial Spasm Caused by Vascular Compression: MRI Diagnosis

Abstract:Objective To evaluate MR diagnostic value of neurovascular compression in patients with hemifacial spasm(HFS). Methods MRI and MRA manifestations and operative results of eighteen patients with HFS were reviewed retrospectively.Results (1)The roots of the facial nerve involved sides were compressed by vessel in all cases.(2)There was statistical correlation between the vascular compression of the root exit zone(REZ)of facial nerves and the symptoms of HFS(P0.001).(3)There were 29 arteries resulting in neurovascular compression in patients with HFS,including 14 anterior inferior cerebellar arteriae (48.27%),10 posterior inferior cerebellar arteriae(34.48%),3 vertebral arteriae(10.35%),1 basilar arteriae(3.45%),1 superior cerebellar artery(3.45%).(4)The roots of nerve compressed by single artery in 10 cases (55.56%) and multi-arteriae in 8 cases(44.44%).(5) Neurovascular compression types appeared as contact in 14(48.28%) and encasement in 15(51.72%).)Conclusion MRI and 3D-TOF MRA are of important value in diagnosing hemifacial spasm caused by vascular compression.
