The diagnostic value of 99 Tcm-DMSA renal cortical scintigraphy in children with renal dysplasia

Abstract:Objective To study the diagnostic value of 99Tcm-dimercaptosuccinic acid (DMSA) renal cortical scintigraphy in children with renal dysplasia. Methods Twenty-nine patients with suspected renal dysplasia were included. The mean age of them was (72.4±43.3) months (2 months~13 years old). Of 29 patients, 7 cases were boys, 22 were girls. All patients underwent 99Tcm-DMSA renal scintigraphy and ultrasound scan (US). Based on Roach's criterion, the dysplastic kidney was assessed using a four-point scale (grade 0, no uptake; grade 1, uptake seen only with background enhancement; grade 2, lightly visualized without computer enhancement; grade 3, easily visualized). Results All of the results of 99Tcm-DMSA renal scintigraphy of the 24 patients showed unilateral renal dysplasia. 11 cases as grade 1, 7 cases as grade 2, 6 cases as grade 3. Differential function of involved kidneys ranged from 0 to 24.9% (mean 6.3%). Uptake was demonstrated in 24/29 dysplastic kidneys. Five cases with suspected dysplastic kidney of no uptake were diagnosed by other imaging methods. The sensitivity in diagnosing renal dysplasia was 82.76% by DMSA renal scintigraphy. Of 29 patients, 19 underwent surgical treatment, and their renal dysplasia were confirmed by pathology. Conclusions 99Tcm-DMSA renal cortical scintigraphy is a highly sensitive, reliable, practical technique for diagnosing renal dysplasia. It can be used for locating the dysplastic kidney, evaluating the renal function and guiding the appropriate treatment.
