Abstract:Coronary bypass grafting (CBG) is the most effective method of treating coronary artery disease and venae saphena magna is commonly used as vein graft for surgical revascularization;however,the incidence of vein graft restenosis is more than 60％ in ten years after CBG,which threatens the prognosis and long-term survival of the patients.The vein graft restenosis has multiple factors and levels,including de-endothelialization,thrombosis,re endothelialization and arterialization of vein graft in the early stage,intimal hyperplasia in the middle stage,and atherosclerosis and plaque rupture in the late stage.This review summarized the latest researches of the mechanisms of vein graft restenosis after coronary bypass grafting and discussed the prevention methods.

Abstract:OBJECTIVE:To explore the infrequent complications and treatment after endoluminal stent graft implantation for aortic diseases.METHODS:Review of the characters and complications for five cases of aortic diseases by stent graft implantation.RESULTS:The complications of 4 cases have been relieved by operation or re-stent implantation, 1 case was died.CONCLUSION:The complications are difficult to forecast for stent graft placement in aortic diseases, the surgeon should be well practiced in surgical and interventional technique, so as to treat the complications in time.
Progression of research on re-stenosis of saphenous vein grafts after coronary artery bypass grafting

Abstract:Coronary artery bypass grafting is one of the conventional treatment of coronary heart disease.Though the artery grafts have its own superiority,autologous saphenous vein is still commonly used.The restenosis of vein grafts which result from neointima and atherosclerosis has became an urgent problem to be resolved.Drug,external stent,expanding solution and gene therapy are the main methods of prevention of grafting occlusion,of which the gene therapy sheds new light on the resolution of vein grafts restenosis.

Abstract: BACKGROUND:It is may become a reality to prevent vein graft stenosis effectively with in-depth study of the mechanism of restenosis,further improvement of various treatment methods and the joint application.OBJECTIVE:To introduce the multiple causes of vein graft lesions and to explore new method for the prevention of delayed vein graft stenosis.METHODS:Using computer technology to retrieve the articles in VIP periodical full-text database,CNKI database,highwire full-text database and PubMed database published between January 1995 and December 2009 with key words of "heart surgery,vein transplantation,restenosis,prevention".Literatures with regard to vein graft transplantation were included.Irrelevant and repetitive contents were excluded.Totally 40 literatures were selected for the further analysis.RESULTS AND CONCLUSION:Surgical coronary artery bypass graft surgery is the main treatment for the patients suffered from left main coronary artery and severe multi-vessel lesions.However,great saphenous vein is a kind of graft material,and the postoperative restenosis rate is relatively high,which brings a lot of inconvenience for the clinical treatment.This paper summarizes the multiple causes of vein graft lesions and to explore new method for preventing delayed vein graft stenosis from the aspects of conservative treatment,surgery prevention,extravascular stent,drug therapy,gene therapy and laser and radiation therapy.

Abstract:Coronary artery disease (CAD) is a significant global health problem, contributing to significant morbidity and mortality. Percutaneous coronary intervention (PCI) is an efficient therapy for treating CAD, but it carries the risk of iatrogenic endothelial injury, which contributes to vessel inflammation and induction of in-stent restenosis. Therefore, developing novel methods for enhancing re-endothelialization after PCI is highly needed. Endothelial progenitor cells (EPCs) can differentiate into mature endothelial cells, and cell therapy with EPC may offer a novel way for accelerating reendothelialization. In this review paper, we aimed to briefly describe EPCs and highlight their potential therapeutic roles in in-stent re-stenosis and endothelial injury.

Abstract:Cell therapy is an important topic in the field of regeneration medicine that is gaining attention within the scientific community. However, its potential for treatment in coronary heart disease (CHD) has yet to be established. Several various strategies, types of cells, routes of distribution, and supporting procedures have been tried and refined to trigger heart rejuvenation in CHD. However, only a few of them result in a real considerable promise for clinical usage. In this review, we give an update on techniques and clinical studies of cell treatment as used to cure CHD that are now ongoing or have been completed in the previous five years. We also highlight the emerging efficacy of stem cell treatment for CHD. We specifically examine and comment on current breakthroughs in cell treatment applied to CHD, including the most effective types of cells, transport modalities, engineering, and biochemical approaches used in this context. We believe the current review will be helpful for the researcher to distill this information and design future studies to overcome the challenges faced by this revolutionary approach for CHD.

Abstract:BACKGROUND:Vein graft restenosis and occlusion seriously affected postoperative long-term effect of coronary artery bypass grafting, which is the difficult medical problem to be solved at present.OBJECTIVE:To review the therapeutic gene, vectors and transplanted pathway of gene therapy for vein graft restenosis.METHODS:The relevant articles between January 1995 and July 2009 were retrieved from PubMed (http://www.ncbi.nlm.nih.gov/PubMed) and database of Chinese knowledge (CNKI http://ckrd.cnki.net) using keye words of "vein graft, gene, therapy", and restricting the language as English and Chinese by computer.Literatures addressing therapeutic gene, vectors and transplanted pathway of gene therapy for vein graft restenosis were included.Old or duplicated studies were excluded.RESULTS AND CONCLUSION:Vein graft restenosis seriously affected postoperative long-term effect of coronary artery bypass grafting.The traditional drug therapy brings little effect.Gene therapy can precisely target specific gene transfection to vascular wall and achieve the expression of exogenous gene, playing the maximum biological effect in the local, reducing systemic side effects.It is considered as a very promising approach.Gene therapy for vein graft restenosis will be broad prospects for development thank to its unique advantages.With the further study of the mechanism of restenosis and the appearance of safe and efficient vectors, the application of multi-gene therapy, drug therapy and physical therapy combination therapy will be the prevention of vein graft restenosis development.
Therapy approaches in treatment of recurrent ischemic angina with internal mammary artery bridge after coronary artery bypass grafting

Abstract:Objective To investigate the clinical therapeutic methods and their curative effects in recurrent ischemic angina for internal mammary artery resterosis after coronary artery bypass grafting (CABG).Methods We enrolled the patients who had recurrence of ischemic angina for restenosis of internal mammary artery graft after CABG as research subjects in the Affiliated Hospital of Yan'an University from January 2014 to January 2016.The 42 patients were divided into three groups according to the different treatment approaches for recurrence of ischemic angina:Group A (n=22)who received internal mammary artery interventional therapy;Group B (n=12)who received coronary artery bypass grafting treatment;and Group C (n=8)who received left subclavian artery proximal stent treatment.Then we compared the clinical therapeutic effects in the three groups.Results The success rate in Group C was 100%,which was the highest in the three groups,and the post-operative restenosis rate was 0.The hospitalization time was significantly shorter in Group A than in Group B (P＜0.05).However,the two groups did not significantly differ in mortality,success rate or restenosis rate (P＞0.05).Conclusion We should select the appropriate treatment according to the patient's specific situation for recurrent ischemic angina. Endovascular treatment has evident therapeutic effects,rapid postoperative recovery,and lower treatment risk, making it the preferred treatment when possible.

Abstract:Due to dominant atherosclerosis etiology, cardiovascular diseases (CVDs) remain the leading cause of morbidity and mortality worldwide. In clinical trials, advanced atherosclerotic plaques can be removed by angioplasty and vascular transplantation, but the severe risk of vascular restenosis remains, which needs to be urgently improved. After understanding and generalizing the pathological processes and cellular mechanism of certain atherosclerosis and restenosis, their essential pathogenic factors show noteworthy similarity, primarily including endothelium injury, excessive proliferation of smooth muscle cells (SMCs), and pathogenic inflammatory responses. These common pathogenic factors inspire researchers to develop co-target treatments for these atherosclerosis and restenosis. Numerous therapeutic agents such as nucleic acids, proteins, small molecule drugs and gas signaling molecules for co-target treatments are summarized in this paper. To improve the safety and effectiveness of therapeutic agents, high-performance nanodelivery strategies have been developed for anti-atherosclerosis via systemic administration and anti-restenosis via local administration. They can augment blood circulation time and targeting ability for systemic administration, as well as efficient nanodrug load and precise release. Overall, this review aims to recapitulate the emerging therapeutic molecules and nanodelivery strategies for the treatment of atherosclerosis and restenosis, with the goal of mutual enlightenment in exploring therapeutic approaches for these diseases.

Abstract:Treatments of atherogenesis,one of the most common cardiovascular diseases(CVD),are continuously being made thanks to innovation and an increasingly in-depth knowledge of percutaneous transluminal coronary angioplasty(PTCA),the most revolutionary medical procedure used for vascular restoration.Combined with an expanding balloon,vascular stents used at stricture sites enable the long-time restoration of vascular permeability.However,complication after stenting,in-stent restenosis(ISR),hinders the advancement of vascular stents and are associated with high medical costs for patients for decades years.Thus,the development of a high biocompatibility stent with improved safety and efficiency is urgently needed.This review provides an overview of current advances and potential technologies for the modification of stents for better treatment and prevention of ISR.In particular,the mechanisms of in-stent restenosis are investigated and summarized with the aim to comprehensively understanding the pathogenesis of stent complications.Then,according to different therapeutic functions,the current stent modification strategies are reviewed,including polymeric drug eluting stents,biological friendly stents,prohealing stents,and gene stents.Finally,the review provides an outlook of the challenges in the design of stents with optimal properties.Therefore,this review is a valuable and practical guideline for the development of cardiovascular stents.

Abstract:Coronary artery bypass grafting (CABG) has witnessed remarkable progress in recent years, driven by innovative techniques and refined approaches that have transformed patient outcomes and treatment paradigms. This review article comprehensively explores the latest advances in CABG techniques and their consequential impacts on patient outcomes. The advent of minimally invasive CABG techniques has revolutionized traditional surgical approaches. Robotic-assisted surgery and small thoracotomy methods offer reduced invasiveness, yielding shorter recovery times and improved patient satisfaction. Integrating CABG with percutaneous coronary intervention (PCI), hybrid procedures have emerged as a versatile strategy, providing tailored treatment solutions for complex coronary anatomies. The paradigm shift to off-pump CABG, which preserves the beating heart during surgery, has shown promise in reducing perioperative complications and neurocognitive deficits. Advances in graft selection, particularly the utilization of arterial grafts such as the internal thoracic artery and radial artery, have significantly enhanced graft patency rates and long-term survival. Adjunctive technologies, such as intraoperative imaging and graft flow assessment, have bolstered the precision of CABG procedures. Pharmacological agents have demonstrated their potential to improve graft outcomes. Surgical decision-making is now optimized based on patient characteristics and disease severity owing to the development of patient selection and risk stratification tools. Long-term results have also significantly improved. Patients undergoing CABG have higher survival rates, less angina, and better quality of life. Developing more resilient grafts through tissue engineering, using bioresorbable materials in graft fabrication, and using gene therapy to improve graft patency and overall cardiac recovery are all exciting future research directions. This review's summary of current developments in CABG procedures highlights their profoundly positive effects on patient outcomes. These developments can change the face of cardiovascular care by giving clinicians more tools to treat coronary artery disease (CAD) and enhance patients' quality of life.

Abstract:Coronary artery bypass graft (CABG) procedures face challenges related to graft failure, driven by factors such as acute thrombosis, neointimal hyperplasia, and atherosclerotic plaque formation. Despite extensive efforts over four decades, the optimal antithrombotic strategy to prevent graft occlusion while minimizing bleeding risks remains uncertain, relying heavily on expert opinions rather than definitive guidelines. To address this uncertainty, we conducted a review of randomized clinical trials and meta-analyses of antithrombotic therapy for patients with CABG. These studies examined various antithrombotic regimens in CABG such as single antiplatelet therapy (aspirin or P2Y 12 inhibitors), dual antiplatelet therapy, and anticoagulation therapy. We evaluated outcomes including the patency of grafts, major adverse cardiovascular events, and bleeding complications and also explored future perspectives to enhance long-term outcomes for CABG patients. Early studies established aspirin as a key component of antithrombotic pharmacotherapy after CABG. Subsequent randomized controlled trials focused on adding a P2Y 12 inhibitor (such as clopidogrel, ticagrelor, or prasugrel) to aspirin, yielding mixed results. This article aims to inform clinical decision-making and guide the selection of antithrombotic strategies after CABG.

Abstract:Objective: To explore the feasibility and safety of using tissue-type plasminogen activator (t-PA) to prevent graft restenosis after coronary artery bypass grafting (CABG).Methods: In this prospective observational study, 37 patients underwent CABG between June 2009 and May 2013. These patients were grouped according to the anti-coagulation strategy after surgery: t-PA (n = 12) and conventional treatments (n = 25). In the t-PA group, the patients received acetylsalicylic acid (ASA) and clopidogrel plus intravenous infusion of t-PA (0.25 mg/kg/day) starting at 24 h after surgery and that lasted for 3 days. In the conventional group, the patients received only ASA and clopidogrel. 64-row spiral computed tomographic coronary angiography was performed at 1 week, 1, and 3 months after surgery to evaluate the patency of the graft vessel.Results: The mean stenosis severity of the saphenous vein grafts was lower in the t-PA group compared with the conventional group at 3 months after surgery (p < 0.05), but there was no significant difference at 1 week and 1 month (p > 0.05). The patency rate of the grafts was not significantly different between the two groups at 1 week, 1, and 3 months after surgery (p > 0.05).Conclusion: Early application of t-PA after CABG was feasible and safe, and might help prevent early restenosis of SV grafts. Additional clinical randomized trials are necessary to address this issue.
A study on restenosis after artificially grafting bypass for chronic ischemia of the lower extremities

Abstract:Objective To probe the etiology and management of restenosis after artificially grafting bypass for chronic ischemia of the lower extremities. Methods In this study 52 cases suffering from postoperative restenosis and obliteration were compared with 32 cases whose artificial grafts remain patent during the same postoperative follow-up period of 3～62 months.Possible risk factors that lead to restenosis were evaluated.Resuits FIB(4.48±1.68)g/L,CRP(9.5±2.6)mg/L and LDL(4.5±1.7)mmol/L were significantly higher in the restenosis group than FIB(3.50±0.72)g/L,CRP(4.0±3.2)mg/L and LDL(2.8±0.9)mmol/L in the patent group(P＜0.01).There were no significant difference between HDL(1.02±0.32)mmol/L in the restenosis group and HDL(1.12±0.28)mmol/L in the patent group (P＞0.05).Reoperation in these 52 cases found severe intima hyperplasia and secondary thrombosis within anastomosis in 42 cases and the remaining 10 cases were found with artificial vessel primary thrombosis.After reoperation,artificial graft remain patent in 28 cases,limb amputation was performed in 10 cases,the grafted bypass were removed due to infection in 3 cases. Five patients died postoperatively.Conclusion The main reason for restenosis after artificially grafting bypass is intima hyperplasia in vascular anastomosis.Higher levels of FIB,CRP and LDL maybe the major high risk factors that lead to intima hyperplasia and artificial graft obliteration.

Abstract:Coronary chronic total occlusion (CTO), which occurs in 18. 4–52% of all patients referred for coronary angiography, represents one of the last barriers in coronary intervention. Approximately half of all patients with prior coronary artery bypass graft (CABG), who undergo coronary angiography, are diagnosed with coronary CTO. In fact, these patients often develop recurrent symptoms and events, necessitating revascularization. Currently, there is neither a consensus nor developed guidelines for the treatment of CTO patients with prior CABG, and the prognosis of these patients remains unknown. In this review, we discuss current evidence and future perspectives on CTO revascularization in patients with prior CABG, with special emphasis on clinical and lesion characteristics, procedural success rates, periprocedural complications, and long-term outcomes.

Abstract:Coronary artery bypass grafting(CABG)is a highly effective method in the treatment of coronary heart disease(CAD).However,its effectiveness is impeded by the limited life expectancy of vein grafts,which are the most common types of conduits used.Many reports presented follow-up angiographic data on large cohorts of patients,demonstrating that approximately one-half of vein grafts fail within 10 to 15 years of surgery and that graft failure is associated with worse clinical outcomes.Therefore,understanding the venous-specifc pathophysiological and molecular mechanisms of vein graft adaptation is important for clinical vein graft management.This overview describes the normal development and regulation of vessel,compares the differences between artery and vein,introduces the adaptive response of vein graft to the arterial environment during the post-surgical process and its molecular mechanism,and discusses various therapeutic prospects.

Abstract:Coronary artery bypass grafting (CABG) is and continues to be the preferred revascularization strategy in patients with multivessel disease. Graft selection has been shown to influence the outcomes following CABG. During the last almost 60 years saphenous vein grafts (SVG) together with the internal mammary artery have become the standard of care for patients undergoing CABG surgery. While there is little doubt about the benefits, the patency rates are constantly under debate. Despite its acknowledged limitations in terms of long-term patency due to intimal hyperplasia, the saphenous vein is still the most often used graft. Although reendothelialization occurs early postoperatively, the process of intimal hyperplasia remains irreversible. This is due in part to the persistence of high shear forces, the chronic localized inflammatory response, and the partial dysfunctionality of the regenerated endothelium. "No-Touch" harvesting techniques, specific storage solutions, pressure controlled graft flushing and external stenting are important and established methods aiming to overcome the process of intimal hyperplasia at different time levels. Still despite the known evidence these methods are not standard everywhere. The use of arterial grafts is another strategy to address the inferior SVG patency rates and to perform CABG with total arterial revascularization. Composite grafting, pharmacological agents as well as latest minimal invasive techniques aim in the same direction. To give guide and set standards all graft related topics for CABG are presented in this expert opinion document on graft treatment.

Abstract:Cardiovascular diseases have become one of the leading causes of mortality worldwide. Stents and artificial grafts have been used to treat cardiovascular diseases. Thrombosis and restenosis seriously impact the clinical outcome of stents and artificial vascular grafts. For the purpose of antithrombosis and anti-restenosis, numerous strategies have been developed to construct highly hemocompatible surfaces, enhance endothelialization, and inhibit intimal hyperplasia. Rapid endothelialization and inhibited intimal hyperplasia play an important role in artery repair after stent implantation and coronary artery bypass graft surgery. This review focuses on the recently developed strategies for improving the hemocompatibility and endothelialization of cardiovascular devices. We also introduce drug, gene and RNA delivery technologies for inhibiting intimal hyperplasia. The challenges and future perspectives about promoting endothelialization are also briefly discussed with the hope to help inspire further innovations.
Computational Investigation of Hemodynamics in Fully Stenosed CABG

Abstract:Coronary Artery Bypass Graft (CABG) is an important surgical treatment for critically stenosed arteries. Unfortunately restenosis always occurs after CABG surgery, which bring about surgery failure, lntimal thickening in the CABG distal anastomosis has been implicated as the major cause of restenosis and long-term graft failure. The nonuniform hemodynamics including disturbed flows, recirculation zones, oscillating wall shear stress, and long particle residence time were thought to be the possible etiologies. Numerical simulation was proved to be of great help and guidance meaning for the biofluid mechanics research and the CABG surgical plan. The present study was based on the hypothesis that the geometry configuration of CABG could greatly influence the hemodynamics in the vicinity of anastomosis. The hemodynamic features of two geometry models of end-to-side CABG were studied and compared. One simulated a conventional CABG with 1-way bypass graft, and the other simulated a modified CABG with symmetric 2-way bypass graft. The numerical investigations of hemodynamics in these two models with fully stenosed coronary arteries were accomplished using finite element method. The temporal and spatial distributions of hemodynamics were analyzed and compared. Results showed that the presence of symmetric 2-way bypass graft was of reasonable and favorable hemodynamics than 1-way bypass graft. The modified CABG model created a more hemodynamically efficient streamlined environment with higher mean and maximum axial velocities and lower radial velocities than the conventional 1-way model. Meanwhile, the symmetric 2-way bypass graft was featured with low pressure near the wall, high and uniform WSS in the host artery. All of these were favorable for inhibiting the development of intimal thickening, restenosis, and ultimate failure of the CABG, and it could considerably improve the flow conditions and decrease the probability of intimal hyperplasia and restenosis of CABG.
