Transarticular screws and C1 hooks fixation for os odontoideum with atlantoaxial instability

Abstract:Objective The goal of this study was to introduce a modified three-point fixation device composed of C1-C2 transarticular screws and C1 hooks,and evaluate the clinical outcome for the treatment of os odontoideum with C1-C2 instability by using this device.Methods Eighteen patients with os odontoideum were treated from October 2004 to February 2007.Among them,there were 7 men and 11 women.The average age was 35.2(range 13-69)years.All the patients had atlantoaxial instability with local symptoms,and 15 of them had myelopathy and underwent this fixation for C1-C2 fusion.The pre-operative neurological function of each patient were assessed by the American Spinal Injury Association(ASIA)score system,there were B degree in two cases,C degree in seven cases,D degree in six cases and E degree in three.Clinical and radiographic analyses were performed when follow-up was made at 3,6,and 12 months,and annually thereafter.Results The device was well placed without incident and no neurological or vascular complications occurred in these cases.These patients were followed up for average,21 months(6 to 58 months)with complete clinical and radiographical assess.All patients gained solid bony fusion with a good alignment of C1 and C2 at 3 months postoperatively as were evaluated with plain X-rays and three-dimensional reconstruction of CT.No hardware failure,pseudarthrosis and instability were noted at the follow-up.All patients had complete relief of pain within 3 months,and neurological symptoms were substantially improved with C degree in four cases,D degree in seven cases and E degree in seven.Conclusion C1-C2 transarticular screws and C1 hooks could be used to effectively treat os odontoideum with atlantoaxial instability.
