Laparoscopic cholecystectomy in patients with history of abdominal surgery:a report of 103 cases

Abstract:Objective To investigate the feasibility and surgical maneuver of laparoscopic cholecystectomy(LC)in patients with history of abdominal surgery.Methods The clinical data of 103 cases of LC with previous abdominal surgery,admitted during the period from July 2003 to June 2006,were reviewed.Among these patients,32 cases had a history of upper abdominal surgery(Group A)and 71 had a history of lower abdominal surgery(Group B).Results LC was successfully performed in 99 cases without postoperative complication,and 4 cases were converted to open cholecystectomy.Group A experienced a longer operating time(61.4 min)and a longer period of hospitalization(5.3 days) than those of Group B(42.5 min and 3.2 days,P<0.01).There was no significant statistical difference in the rate of convertion to open surgery between these 2 groups(6.3% vs 2.8%,P>0.05).Conclusions Previous lower abdominal surgery has little influence on LC operation.History of upper abdominal surgery is not an absolute contraindication for LC, but might increase some difficulty for LC maneuvering.The key points of LC procedure for those patients are: carefully locating the insertion point of the first trocar;dissecting minutely the existing intraabdominal adhesions;and rendering clearance to the anatomy of Calot' s triangle.
