Abstract:Laparoscopic cholecystectomy (LC) is the gold standard for gallbladder removal. Innovative techniques such as single-incision laparoscopy and natural orifice endoscopic surgery (NOTES) have been applied as a step towards more less-invasive procedures. Transvaginal, transgastric and transcolonic accesses in humans and animal models have been described, but mostly with the assistance of laparoscopic guidance, or with another device such as a second endoscope or transabdominal instrument for extraction. We hereby present the successful treatment of a case regarding endoscopic transgastric cholecyctectomy (ETGC) with a single scope and without laparoscopic guidance or other transabdominal instruments. A 55-year-old gentleman was admitted for recurrent abdominal pain. Ultrasound and MRI confirmed chronic cholecystitis with gallstones. He had ERBD for acute cholangitis with choledocholithiasis two months ago. Pre-procedure preparations were the same as LC. The anterior wall of gastric antrum was actively perforated with a sterile gastroscope. The gallbladder was then exposed and dissected from the fundus to the neck. Intermittent injection of normal saline to the connective tissues was necessary to create working space. Then, a thorough dissection of the hepatocystic triangle, bounded by the gallbladder wall, cystic duct, and common hepatic duct, was performed to obtain the "critical view of safety". The cystic artery was dissected and electrocoagulated. Once the cystic duct was fully dissected, it was closed with clips and then incised, and the gallbladder was extracted from the mouth through a snare. To prevent bile leakage, the cystic duct stump was ligated with a nylon snare. Finally, the gastric wall was closed with clips. The procedure took 240 minutes. We have finished 5 ETGCs in our center. The operation time has decreased to around 3 hours. Two patients had pre-operative ERBD. In one case with severe intra-abdominal adhesion, extraction with dental floss was adopted. The patients were all uneventful after the procedure. NOTES offers the potential for scarless operations and reduces pain. However, endoscopic retraction of tissue still remains challenging with current instruments. For ETGC, the greatest challenge is the visualization and dissection of the hepatocystic triangle, failure of which could bring risk of damage of the common bile duct and severe bleeding. Pre-operative ERBD may bring benefits for avoiding damage to the common bile duct. We are only at the beginning of a new minimally invasive revolution, and at the current stage the procedure should only be performed on selected patients without severe inflammation or abdominal adhesion, by experienced hands with surgeons standby. Modifications in the technological aspects and innovations in endoscopic instruments will likely yield better outcomes in NOTES.
