Comparison Between Cisatracurium and Vecuronium on Neuromonitoring During Thyroid Surgery

Abstract:Objective To compare the effect of both cisatracurium and vecuronium on neuromonitoring during thyroid surgery.Methods A total of 112patients were randomized to receive 2times ED 95 dose 0.15mg / kg of cisatracurium 0.15mg / kg ( group A ) or vecuronium 0.10mg / kg ( group V ) with 56cases each to facilitate endotracheal tube insertion and electromyography ( EMG ) . Evoked potentials were obtained every 5 min by stimulating the vagus or recurrent laryngeal nerve between the period of 30and 70min after administration of cisatracurium or vecuronium.The tracheal intubating conditions , the time required for each group with adductor pollicis muscle twitching 0% and the magnitude of evoked potentials at each time point were compared between the two groups. Accelerometry ( twitch ) was used to monitor the quantitative degree of neuromuscular transmission at the adductor pollicis muscle.Results No significant difference was observed between the two groups in the overall intubation conditions.At 30and 35 min after administration of cisatracurium or vecuronium , the twitch in both groups were 0% , but the different rate of positive EMG response was considerable between the two groups ( P0.05 ) .The mean amplitude of EMG from the time point of 30to 70min was greater in the group A than the comparison.Conclusion Both cisatracurium and vecuronium at 2 times ED 95 doses can be applied to tracheal intubation for intraoperative neuromonitoring during thyroid surgery , but cisatracurium is superior to vecuronium.
