Abstract:Congenital adrenocortical hyperplasia(CAH) is a group of monogenic autosomal recessive hereditary diseases due to congenital deficiency of certain enzyme in steroidal hormonal biosynthesis.Typical clinical features of CAH are salt loss, dehydration, ambiguous external genitalia, accelerated growth and typical hormonal abnormalities. The disease is prone to be missed diagnosis or be misdiagnosed.Commonly, there are many kinds of deficient enzymes in CAH,of which,CYP21(21-hydroxylase) deficiency is the most common, accounting for 90%~95% of the total of the disease. The differentiation of different phenotypes depends on detection of hormonal levels and genotype analysis. At present, the diagnosis of the disease is at molecular level, and deficient genes' diagnosis can be made directly.Prenatal diagnosis and neonatal screening become possible and feasible through molecular genetic analysis of DNA.In addition to surgical treatment, oral glucocorticoid therapy is a classic treatment of the disease, however, appropriate dose is most important.
