Thoracic contrast-enhanced MR angiography: dose and flow rate correlation of the contrast medium

Abstract:Objective To optimize the dose and flow rate of contrast medium in thoracic contrast-enhanced MR angiography (CE MRA). Methods Sixty-one volunteers without apparent thoracic disease underwent breath-hold 3 D CE MRA of the thoracic vessels on a 1.5 T system. They were assigned to 12 groups, and the contrast medium was injected with different doses (0.08 mmol/kg, 0.17 mmol/kg, 0.25 mmol/kg, and 0.33 mmol/kg) by using the flow rates at 2 ml/s, 3 ml/s, and 4 ml/s, respectively. The effects were compared using the SPSS statistic analysis system. Results The highest parenchymal-vessel contrast ratios were acquired at the dosage of 0.25 mmol/kg and 0.33 mmol/kg, and the former was more stable than the latter. In groups with the flow rate of 2 ml/s and 4 ml/s, the parenchymal-vessel contrast ratio in group of 0.33 mmol/kg was lower than that of 0.25 mmol/kg, and this interpreted that the parenchymal-vessel contrast ratio did not increase with the dosage proportionally. On the contrary, it might decrease when the dose was added up to a specific level. Both the signal to noise ratio and contrast ratio of aorta were much more stable when compared with pulmonary arteries and pulmonary veins, and they had a linear relationship with the dosage. The enhancement and vessel contrast ratio were good enough for diagnosis when the dosage was up to 0.17 mmol/kg, especially when it was larger than 0.25 mmol/kg. In groups of 0.08 mmol/kg and 0.17 mmol/kg, although the vessel-pulmonary contrast ratio at the flow rate of 4 ml/s was significantly higher than that at 2 ml/s and 3 ml/s, the quality of MRA images in these two groups could not meet the need of diagnosis because the segment of bolus contrast medium was too short to display the entire vessels. Conclusion The best dosage of contrast agent for pulmonary artery 3D CE MRA was 0.25 mmol/kg, and the 3D CE MRA of thoracic vessels mainly correlated with the contrast dose but not the flow rate. 0.25 mmol/kg and 3 ml/s for vessel 3D CE MRA were recommended.
