An Open, Multicenter, Phase 2 Study of Thoracic Three-Dimensional Radiation Therapy with Concurrent Chemotherapy for Stage IV Non-Small Cell Lung Cancer

Abstract:To investigate the survival outcomes and feasibility of thoracic three-dimensional radiation therapy with concurrent chemotherapy in patients with stage IV non-small cell lung cancer in a multi-institutional setting. The patients were expected to receive at least 2 cycles of chemotherapy with concurrent thoracic three-dimensional radiation of ≥36 Gy in 20 fractions. The primary objective was to test the overall survival (OS) and acute toxicity. From May 2008 to May 2012, 198 eligible patients were enrolled from seven cancer centers. All 198 patients were included in the survival analysis according to the intent-to-treat principle. The main cause of death was distant metastasis; only 10% of the patients died of isolated primary recurrence. The median time of OS and progress-free survival was 13.0 months (95% CI, 11.7–14.3) and 9.0 months (95% CI, 7.7–10.3), respectively, and the median time of local control was not attainable. Multivariate analysis showed that gross tumor volume (GTV) <134 cm3, a stable or increased posttreatment Karnofsky Performance Status (KPS) score and the delivery of at least 63 Gy to the primary tumor were independent prognostic factors for better OS. Acute grade 3 to 4 hematologic toxicities were as follows: leukocytes, 37.9%; thrombocytopenia, 10.1%; and anemia, 6.9%. Acute grade 3 radiation pneumonitis was 2.5%, and radiation esophagitis was 6.6%; no grade 4 or 5 radiation-related toxicities developed. Thoracic three-dimensional radiation therapy with concurrent chemotherapy yielded satisfactory survival outcomes with acceptable toxicity. Primary tumor volume, posttreatment KPS, and radiation dose are prognostic factors for survival.
