Abstract:Abstract 4888 Background: Cytogenetic analyses of chronic myeloid leukemia (CML) have been performed previously in a large number of reports, but systematical research based on large sample sizes is seldom available. In order to further elucidate the cytogenetic nature of CML, we analyzed retrospectively the cytogenetic profiles of 1863 Ph/BCR-ABL-positive CML patients from a research center in China. Results: Of 1266 newly diagnosed CML patients, the median age was 41 years, which is younger than the median age of diagnosis in western populations. The incidence of additional chromosome abnormalities(ACAs) was 3.1% in newly-diagnosed chronic phase(CP), 9.1% in CP after therapy, 35.4% in accelerated phase(AP) and 52.9% in blast phase(BP), reflecting cytogenetic evolution with CML progression. 5.3% patients harbored a variant Ph translocation. A higher prevalence of ACAs was observed in variant Ph translocations than in classical t(9;22) in the disease progression, especially in BP(88.2% vs. 50%, p =0.002). Moreover, a hyperdiploid karyotype and trisomy 8 were closely correlated with myeloid blast crisis(BC) while a hypodiploid karyotype and monosomy 7 were associated with lymphoid-BC. Among subsets of myeloid-BC, compared with myeloid-BC with granulocytic differentiation or monocytic differentiation, myeloid-BC with minimal differentiation had higher ACAs rate (80% vs.46.8%, p =0.009 and 80% vs. 42.9%, p =0.006). Conclusion: CML tends to afflict younger population in China. In the disease progression, the incident of ACAs was higher in variant Ph translocations than in classical t(9;22). Among subsets of myeloid-BC, myeloid with minimal differentiation had distinct cytogenetic features. Disclosures: No relevant conflicts of interest to declare.

Abstract:OBJECTIVE:To evaluate the cytogenetic and molecular genetic features of chronic myeloid leukemia (CML) in Chinese.METHODS:A total of 1193 CML patients were retrospectively studied. Chromosome preparation of bone marrow cells was made using direct and short-term culture. Karyotype and bcr-abl fusion genes were analyzed by R-banding, RT-PCR, respectively.RESULTS:In the 1193 cases, 98.07% was Ph chromosome positive (Ph+) and 1.93% negative (Ph-). In the Ph+ patients, 95.64% was classical Ph and 4.36% variant rearrangements. Additional genetic changes were demonstrated in 11.88% of classical Ph cases. Cytogenetic clonal evolution was found in 7.94% of patients in chronic phase (CP), 27.78% in accelerated phase (AP), and 49. 04% in blast crisis (BC). Among the classical Ph cases, +Ph, +8, -21 were found in 14.62%, 10.77% and 7.69% of them respectively. In patients in BC and AP, the most common additional chromosome changes were + Ph (28.57%), +8 (16.67%) and +19 (7.14%), while in CP, -21 (10.26%), +Ph (8.97%), and +8 (8.97%). The combination of +Ph and +8 (3.60%) was the most frequent of combination pattern. 524 cases were investigated for bcr-abl fusion gene, and 54.01% was b3a2 (+) and 27.67% b2a2 (+).CONCLUSION:In Chinese CML patients seem to have their unique features in terms of cytogenetic clonal evaluation.
Decitabine for the Treatment of Atypical Chronic Myeloid Leukemia: A Report of Two Cases

Abstract:Atypical chronic myeloid leukemia (aCML) is a hematopoietic stem/progenitor cell disorder, predominantly involving neutrophils. At present, a limited number of studies regarding the treatment of aCML have been published, and the therapies that are currently available exhibit unsatisfactory outcomes. In the present study, the cases of two aCML patients treated with decitabine (DCA) therapy who achieved remission are presented. A 48-year-old male, who presented with fatigue and a cough that had lasted two months, and a 69-year-old male who presented with dizziness, fatigue and shortness of breath with exercise, were diagnosed with aCML following bone marrow examination, flow cytometry and chromosome banding analysis. The two patients were treated, with four cycles of DCA chemotherapy (20 mg/m(2), days 1-5) and remission was achieved in each patient. The present study evaluated the clinical manifestations, diagnostic criteria and relevant treatment regimens of aCML, which may provide insights for the treatment of affected patients. Routine blood and bone marrow examinations were performed weekly prior to each cycle. Symptoms were relieved in both patients after the first cycle and the two patients were followed up for 3 months after completion of the final cycle. The findings of the current case report indicate that DCA may present an efficacious treatment for aCML.
Clinical and Biological Characteristics of Acute Myeloid Leukemia with 20-29% Blasts: a Retrospective Single-Center Study

Abstract:It is controversial whether acute myeloid leukemia (AML) patients with 20-29% bone marrow (BM) blasts should be considered AML or myelodysplastic syndromes (MDS). We retrospectively studied 382 patients, including 108 AML with 20-29% BM blasts (AML20-29), 210 AML with >= 30% BM blasts (AML >= 30), and 64 MDS with 10-19% BM blasts (MDS-EB2). We found that AML20-29 were more similar to MDS-EB2 in terms of advanced age, less blood count, the increased presence of poor-risk cytogenetics. The frequency of mutated genes in AML20-29 had both the characters of AML and MDS. Median overall survival of AML20-29 and MDS-EB2 were similar and shorter than those of AML >= 30 (p = .045). Multivariate analysis showed inferior survival with increased age, low platelet count and FLT3 mutations. Our findings suggest that AML20-29 have clinical features more similar to MDS than AML.

All the patients were signed informed consents to agree to provide their clinical information to this study. Informed consents were approved by Ethics Committee. Peripheral white cell count at diagnose; complete remission (CR) rate; the percentage of progressing to refractory leukemia and the response to allogenic stem cell transplantation (allo-SCT) were analyzed. Abstract:The purpose of this study was to investigate 135 cases of chronic myelogenous leukemia with non-simple Philadelphia chromosome and to analyze their cytogenetic date. Chromosome preparations in 135 cases of patients were performed by using direct method and/or short-term culture, and karyotyping was performed with R-banding technique. The results showed that the overall frequency of chronic myelogenous leukemia with non-simple Philadelphia chromosome (based on 1210 cases of chromosome detection in chronic myelogenous leukemia) was 11.16%, which included 87 cases of chronic phase, 21 cases of accelerated phase and 27 cases of blastic phase. Among 87 cases of patients in chronic phase, 14 cases were with simple variant translocation and 22 cases had complex variant translocation while the others were with other chromosomal abnormalities including 4 cases of +8, 4 cases of + Ph and 2 cases of i (17); among 21 cases of patients in accelerated phage, 4 cases were with +8 and 4 cases were with + Ph while 3 cases were with i (17); among 27 cases of patients in blastic phage, 2 cases were with simple variant translocation and 3 cases had complex variant translocation while the others were with other chromosomal abnormalities including 5 cases of +8, 5 cases of + Ph and 2 cases of i (17). The detection rate of extra chromosomal abnormalities in this group of 135 cases patient were + Ph, +8, i (17), -Y, +19 and +21 in order. There were 16 cases with simple variant translocation and 25 cases with complex variant translocation in in this group of 135 cases. It is concluded that karyotype analysis is helpful in diagnosis, prognosis, pathogenesis and treatment selection for chronic myelogenous leukemia.
The role of peripheral blood, bone marrow aspirate and especially bone marrow trephine biopsy in distinguishing atypical chronic myeloid leukemia from chronic granulocytic leukemia and chronic myelomonocytic leukemia

Abstract:Objectives: To better realize the features of peripheral blood (PB), bone marrow (BM) aspirate and especially BM trephine biopsy in atypical chronic myeloid leukemia (aCML). Methods: We studied PB, BM smears in 35 cases of aCML and compared with 84 cases of chronic granulocytic leukemia chronic phase (CGL-CP), 39 cases of chronic myelomonocytic leukemia (CMML). In addition, we evaluated characteristics of BM trephine biopsies in 21 cases of aCML and compared with 68 cases of CGL-CP, 20 cases of CMML. Results: All aCML patients presented with leukocytosis (median WBC 17.3 x 10(9)/L), 48% had moderate anemia, and 85% had thrombocytopenia. Values of monocytes, eosinophils, basophils, percentage of immature granulocytes and monocytes (0.63 +/- 0.41 x 10(9)/L, 0.18 +/- 0.16 x 10(9)/L, 0.09 +/-0.08 x 10(9)/L, 6.27 +/- 3.09%, and 2.46 +/- 1.75%, respectively) were useful in distinguishing aCML from CGL-CP and CMML groups. The BM smears showed that striking dysgranulopoieis (100%), dyserythropoiesis (48.6%), percentage of blasts, nucleated erythrocytes, monocytes, eosinophils, and basophils (2.45 +/- 2.06%, 7.76 +/- 2.89%, 1.30 +/- 1.21%, 1.47 +/- 1.60%, and 1.15 +/- 1.08%, respectively) were all important parameters for a diagnosis of aCML. On BM trephine sections, aCML was characterized as hypercellularity, a moderate degree of reticulin fibrosis (71.4%), lymphocytopenia (76.2%), plasmacytopenia (90.5%), abnormal localization of immature precursors (28.5%), and absence of eosinophilia, basophilia, monocytosis. Furthermore, BM imprints, immunohistochemical, and cytochemical staining findings provided important morphological reference to BM trephine sections and made the identification of nucleated cells more convenient. Conclusions: Besides the findings observed in PB and BM aspirate, features of BM trephine biopsy (including BM trephine section, BM imprint, immunohistochemical, and cytochemical staining) can also aid in the diagnosis of aCML.
Atypical chronic myeloid leukemia as defined in the WHO classification is a JAK2 V617F negative neoplasm

Abstract:Atypical chronic myeloid leukemia (aCML) as defined by the WHO classification is a rare hematopoietic stem cell disorder, which shows both myeloproliferative as well as myelodysplastic features. Because of the presence of neutrophilic leukocytosis, aCML may resemble chronic myelogenous leukemia. However, in contrast with the latter, aCML lacks a Philadelphia chromosome or the BCR/ABL fusion gene. The molecular pathogenesis of aCML and its relationship to other myeloproliferative neoplasms is unknown. To clarify these points, the presence of JAK2 V617F was examined by a retrospective analysis of archival specimens obtained from two large medical institutions. Paraffin-embedded bone marrow (BM) trephines and clot sections were examined by an allele-specific TaqMan PCR suitable for use with decalcified tissue. Fifty-nine cases of Philadelphia (Ph) chromosome negative chronic myeloproliferative neoplasms (CMPN) and normal bone marrows (BM) served as controls. None of the nine amplifiable cases of aCML and none of the normal BM controls showed a JAK2 V617F mutation, in contrast to 45/59 (76%) of the Ph chromosome negative CMPN cases. Atypical CML should therefore be considered as a JAK2 negative chronic myeloid neoplasm that remains properly categorized, alongside chronic myelomonocytic leukemia and juvenile myelomonocytic leukemia, within the WHO group of myelodysplastic/myeloproliferative neoplasms.

Abstract:Background: Atypical Chronic Myeloid Leukemia (aCML) is a rare and aggressive myelodysplastic syndrome/myeloproliferative neoplasm. This study aimed to provide a comprehensive understanding of the epidemiology, clinical characteristics, and survival outcomes of aCML patients. Methods: The study utilized data from the Surveillance, Epidemiology, and End Results (SEER) database from 2001 to 2020. The age-adjusted incidence rate (AIR) of aCML was calculated, and survival outcomes were analyzed using the Kaplan-Meier method and accelerated failure time (AFT) regression analysis. Results: The AIR of aCML was found to be 0.024 per 100,000 person-years, with the highest rate observed in 2020. The incidence of aCML increased with age and was higher in males. The study cohort predominantly consisted of elderly White individuals, with an average age at diagnosis of 68.2 years. The median overall survival (OS) and disease-specific survival (DSS) were 1.4 years and 1.7 years, respectively. Older age was independently associated with worse survival outcomes. Notably, treatment delay and chemotherapy did not significantly impact OS or DSS. Conclusions: This study provides comprehensive insights into the epidemiology, clinical characteristics, and survival outcomes of aCML, highlighting its rarity, aggressive nature, and poor prognosis. Further research is needed to validate these findings and explore novel therapeutic strategies for improving outcomes in this challenging hematologic malignancy.

Detection of FLT3/ITD, JAK2(V617F) and NPM1 gene mutations in chronic myelomonocytic leukemia

Abstract:Patients with chronic myeloid leukemia (CML) have a t (9;22)(q34;q11.2) or variant translocation that results in a BCR-ABL1 fusion gene. For many years, conventional karyotyping has been used as the standard diagnostic tool for t (9;22) (q34;q11.2). However, it has several limitations that may lead to failure for detecting BCR-ABL1 gene rearrangements in around 5% of all CML patients. Although reverse transcription polymerase chain reaction (RT-PCR) has evolved as a sensitive method for detecting BCR-ABL1 translocation, this method fail to detect certain BCR-ABL1 fusion transcript type, such as e13a3 (also known as b2a3), as a result of many commercially available and laboratory-developed primer sets. Fortunately, these two rare situations rarely appear at the same time, therefore, the combination of two methods rarely misdiagnosed the patients with CML. In this study, we report a patient with CML who tested both negative by RT-PCR and cytogenetic analysis at the time of diagnosis. She was diagnosed as atypical CML (aCML) and allogeneic hematopoietic stem cell transplantation was suggested. Further fluorescence in situ hybridization (FISH) showed cryptic insertion of ABL into BCR gene on chromosome 22, and DNA sequencing with alternative primer sets demonstrated the presence of an e13a3 BCR-ABL1 fusion. She was diagnosed as CML and received imatinib 400 mg/day. A follow-up BCR-ABL1 FISH analysis demonstrated a markedly reduced BCR-ABL1 fusion rate of 0 after 6 months treatment, indicating a complete cytogenetic response.
[Role of Flow Cytometry in the Diagnosis of Chronic Myeloid Leukemia].

Abstract:OBJECTIVE:To analyze the immunological phenotype of chronic myeloid leukemia (CML), and explore its characteristics and significance.METHODS:The immunophenotypes of 40 CML children and 40 controls were analyzed by multicolor flow cytometry. CD45/SSC, as the basic gate, was used to delineate neutrophils. Then, the distribution of cluster differentiation (CD) molecules on the surface of granulocytes was analyzed in three ranges (≥1%, ≥5%, and ≥20%), and the expression rates of CD molecules (≥1% included in the statistical analysis) and the mean fluorescence intensity (MFI) were compared between the two groups.RESULTS:The proportion of granulocytes in the CML group was (82.1±6.4)%, which was significantly higher than (57.8±11.8)% in the control group (P <0.001). The expression rates of CD15/CD11b/CD33/CD13 in CML and control groups were high, and both distributed in the range of ≥20%. The differentiation trajectory of CD33/CD13 was normal and there were no significant differences in the expression rate and MFI between the two groups. However, both the expression rate of CD11b and CD15 MFI in the CML group were significantly lower than those in the control group (P <0.001). There were no significant differences in the expression rate and MFI of CD10 between the two groups, and the expression levels of CD10 between the two groups were consistent in different distributions. In the CML group, there was a large number of cases with abnormal high expression of CD56, 52.5% of the cases had a CD56 expression rate of ≥5%, and 42.5% had a CD56 expression rate of ≥20%, while the control group did not express CD56 (<1%). The expression distribution of CD117 was different between the two groups. In the range of expression rate ≥5%, there were 35.0% cases in the CML group, while only 2.5% in the control group. The expression rate of CD117 in the CML group was higher than that in the control group (P <0.001), though there was no significant difference in MFI.CONCLUSION:The immunophenotyping of CML is characterized by increased proportion of mature neutrophils, decreased CD15 MFI, decreased proportion of CD11b and abnormal high expression of CD56 and CD117. Flow cytometric analysis of immunophenotype can effectively distinguish normal granulocytes from chronic granulocytes, and help in the diagnosis of CML.

Abstract:Objective: To invistigate the characteristic and value of bone marrow morphology,immunology,cytogenetic and molecular biology(MICM) in patients with chronic myeloid leukemia in blastic crisis(CML-BC).Methods: 38 patients with CML-BC were studied by MICM analysis.Results: The accuracy rate of morphological diagonis based on FBA classification was 94.7%.Immunol-ogy results,the percentage of CML-AMLwas 71.0%,in which 37.0% patients expressed lympholytic associated antigen;The percentage of CML-ALL was 23.7% and all were B lypholytic immunophenotype,in which 66.7% patients expressed myeloid assuiated antigen;The percentage of CML-MALwas 5.3% with all expressing B lypholoid and myeloid immumphenotype.26 cases(68.4%) had CD34 expres-sion and 10 cases(26.3%) had CD7 and CD34 Co-expression.In cytogenetic result,the positive rate of Ph chromosome was 94.3%.Oth-er chromosome abnomality rate was 60.5% with +ph,+8 and i(17q) in high appearance rate.The positive rate of BCR /ABL gene by FISH reached 100% with 14.7% del(9).The positive rate of BCR /ABLgene by RT-PCR also reached 100% with b2a3(12/20),b3a2(8/20) and b2a3+b3a2(1/20).Conclusion: CML-BC is a kind of disease of stem cell.The differentiation of blast cell is blocked in the early stage.MICM has great value in diangnosis,therapy and prognosis judgement.
Distinct clinical profiles and patient outcomes in aCML and CNL

Abstract:The classification of atypical chronic myeloid leukemia (aCML) and chronic neutrophilic leukemia (CNL) as a single disease entity remains a topic of debate. To elucidate the characteristics of both entities, this retrospective cohort study was conducted, encompassing 36 cases of aCML and 18 cases of CNL. We discovered that aCML and CNL presented distinct blood counts, genetics, molecular profiles and outcomes. Specifically, hemoglobin levels ( P < 0.001) and platelet counts ( P < 0.001) were significantly lower in aCML cases than in CNL cases, with no significant difference in mean white blood cells ( P = 0.637). The proportion of abnormal karyotypes was higher in aCML cases compared with CNL cases ( P = 0.010). Notably, we found that aCML and CNL showed distinct gene expression profiles by transcriptome sequencing technology. The median follow-up duration for the entire cohort was 8 months (rang 0.4 to 36.6 months), and the median overall survival (OS) was significantly shorter in aCML cases (7.3 months, 95%CI 5.4 to 20.5 months) than in CNL cases (median OS not reached). The one-year OS rate for aCML patients was 31.0% (9/29), compared to 92.9% (13/14) for CNL patients. In conclusion, our study supports the notion that aCML and CNL are indeed distinct disease entities characterized by unique hematological features and clinical outcomes.

Deciphering Potential Molecular Signatures to Differentiate Acute Myeloid Leukemia (AML) with BCR::ABL1 from Chronic Myeloid Leukemia (CML) in Blast Crisis

Abstract:Acute myeloid leukemia (AML) with BCR::ABL1 has recently been recognized as a distinct subtype in international classifications. Distinguishing it from myeloid blast crisis chronic myeloid leukemia (BC-CML) without evidence of a chronic phase (CP), remains challenging. We aimed to better characterize this entity by integrating clonal architecture analysis, mutational landscape assessment, and gene expression profiling. We analyzed a large retrospective cohort study including CML and AML patients. Two AML patients harboring a BCR::ABL1 fusion were included in the study. We identified BCR::ABL1 fusion as a primary event in one patient and a secondary one in the other. AML-specific variants were identified in both. Real-time RT-PCR experiments demonstrated that CD25 mRNA is overexpressed in advanced-phase CML compared to AML. Unsupervised principal component analysis showed that AML harboring a BCR::ABL1 fusion was clustered within AML. An AML vs. myeloid BC-CML differential expression signature was highlighted, and while ID4 (inhibitor of DNA binding 4) mRNA appears undetectable in most myeloid BC-CML samples, low levels are detected in AML samples. Therefore, CD25 and ID4 mRNA expression might differentiate AML with BCR::ABL1 from BC-CML and assign it to the AML group. A method for identifying this new WHO entity is then proposed. Finally, the hypothesis of AML with BCR::ABL1 arising from driver mutations on a BCR::ABL1 background behaving as a clonal hematopoiesis mutation is discussed. Validation of our data in larger cohorts and basic research are needed to better understand the molecular and cellular aspects of AML with a BCR::ABL1 entity.

Characteristics of 11 Patients with Acute Myeloid Leukemia Accompanied with Karyotype Aberration T(6;9)

Abstract:This study was aimed to investigate the characteristics of 11 patients with acute myeloid leukemia (AML) accompanying with karyotype t(6;9). The laboratorial and clinical data were analyzed retrospectively, including immunophenotype analysis and result analysis of real-time quantitative PCR detection. The results showed that a high prevalence of M2 was observed. Among 11 cases, 6 of M2, 2 of M4, 2 of M5 and 1 of MDS-RAEBT were found according to FAB criteria. Ten patients had high counts of peripheral white blood cells. Bone marrow dysplasia was seen in only 2 cases, and basophilia occurred in 4 cases. Six patients carried additional cytogenetic aberrations apart from t(6;9). Immunophenotypic analysis showed that all patients were positive for CD117, CD33, CD13, HLA-DR, CD38 and CD123. No NPM1 mutation was observed in all patients and a high level of WT1 was detected in all patients (7/7), out of 7 patients FLT3-ITD mutation was detected in only 3 patients. Follow-up details were available for 7 patients, one of whom died before chemotherapy, and the remaining 6 patients all had no response to IA or DA regimen. Among the 6 patients, 3 did not response to subsequently chemotherapy and all died from infections in a short period after diagnosis, the other 3 patients achieved a complete remission after alternative chemotherapy, but 2 relapsed in a short time and died. It is concluded that AML with cytogenetic aberration of t(6,9) is a distinct disease with a very poor prognosis. The first line chemotherapy such as IA or DA regimen is not effective to such patients, and the effective treatment needs further study.
Three Novel Clonal Non-Characteristic Chromosome Aberrations in Acute Myeloid Leukemia Resembling Acute Promyelocytic Leukemia

Abstract:Abstract Abstract 4912 We report here three acute myeloid leukemia cases with novel clonal non-characteristic chromosome aberrations and morphologically resembling acute promyelocytic leukemia (APL). Case No.1: A 57-year-old woman came with mucosal bleeding, ecchymoses, and fever. Blood tests showed hemoglobin (Hb) of 9.3g/dL, platelet count (Plt) of 29×109/L and white blood cell count (WBC) of 1.3×109/L. D-dimer was 3.6ug/ml and Fibrin degradation product (FDPs)>5ug/ml. Bone marrow (BM) morphology showed 91% atypical hypergranular cells with Auer rods and resembling APL. The immunophenotype of peripheral blood (PB) blasts showed 83% positive for CD9, CD13, CD15, CD33, CD64, CD117 and negative for CD34, HLA-DR and B, T-cell markers. PML-RARA and t(15;17) negative but clonal t(1;16)(q44;q22) positive. The patient was initially treated with all-trans retinoic acid (ATRA), As4S4 and mitoxantrone and then switched to harringtonine and ara-C treatment but couldn't get remission. Case No.2: A 39-year-old woman came with asthenia, mucosal bleeding and ecchymoses. Blood tests showed Hb of 10g/dL, Plt of 49×109/L and WBC of 6.2×109/L. D-dimer >20ug/ml and FDPs >20ug/ml. BM morphology showed 84% atypical hypergranular cells with Auer rods and resembling APL. The immunophenotype of PB blasts showed 76% positive for CD13, CD33, CD64, CD117, cMPO and negative for CD9, CD34, HLA-DR and B, T-cell markers. PML-RARA and t(15;17) negative but clonal t(7;17)(p10;q10) positive. The patient was initially treated with ATRA and As4S4 and mitoxantrone but couldn't get a good response. He was then switched to idarubicin and ara-C treatment and got partial response. He is now waiting for allogeneic hematopoietic stem cell transplantation. Case No.3: A 62-year-old man came with asthenia. Blood tests showed Hb of 68 g/dL, Plt of 61×109/L and WBC of 3.3×109/L. D-dimer was 10.2ug/ml and FDPs was not tested. Bone marrow (BM) morphology showed 63.5% atypical hypergranular cells with Auer rods and resembling APL. The immunophenotype of Bone marrow showed 71.2% positive for CD13, CD15, CD33, CD117, cMPO and negative for CD34, HLA-DR,and B, T-cell markers. PML-RARA and t(15;17) negative but clonal (47,XY,+mar1) positive. The patient was initially treated with ATRA and As4S4 but couldn't get a good response. He was then swiched to Daunorubicin and ara-C treatment and got partial remission. APL should be distinguished from other subtypes of acute myeloid leukemia (AML) because of the increased risk of disseminated intravascular coagulation (DIC) and its response to ATRA and arsenic compounds. Some cases of AML seem morphologically similar to APL but lack the characteristic t(15;17) or other RARA-related translocations, and they generally didn't response to ATRA and arsenic compounds treatment. The 3 cases reported here reiterate the lack of specificity of morphologic and Immunophenotyping findings in APL-like AMLs. Karyotypic analysis, as well as FISH and RT-PCR, must be conducted on these cases to rule out APL and other chemotherapy regimens should be considered. APL-like AMLs that do not demonstrate RARA-related translocations may constitute a heterogeneous population of AML with diverse clonal non-characteristic chromosome aberrations, and the molecular mechanisms is worthy of further study.
