Abstract:The purpose of this study was to investigate the clinical value of plasma thrombomodulin (PTM) in different diseases or in different severity or complications of diseases, PTM in 979 patients and 60 healthy controls was determined by ELISA method. The results showed that the PTM level in the control group was 20.40 ± 7.72 μg/L, there was no difference in sex and ages. In chronic primary glomerular disease, the PTM level in chronic renal failure (CRF)group was higher than that in non-CRF group (P ＜ 0.01 ). PTM level ＞ 70 μg/L was defined as its positive criterion.The sensitivity, specificity and positive predictive value in PTM were 85.7%, 82.4% and 77.8% respectively. The PTM level in septemia group was higher than that in non-septemia group ( P ＜ 0.01 ), the sensitivity, specificity and positive predictive value were 86.6%, 89.5% and 76.5% respectively ( ＞ 50 μg/L as its positive criterion). With respect of multiple trauma, the PTM level in multiple organ failare (MOF) group was higher than that in non-MOF group ( P ＜0.01 ), while the sensitivity, specificity and positive predictive value were 77.8%, 77.3% and 73.7% respectively ( ＞40 μg/L as its positive criterion). For systemic lupus erythematosus (SLE), the PTM level in the patients with albuminuria was higher than that in the patients without albuminuria (P ＜ 0.01 ), and the sensitivity, specificity and positive predictive value were 77.8%, 92.3% and 93.3% respectively ( ＞ 35.54 μg/L as its positive criterion). For diabetes, the PTM level in complication group was higher than that in group without complications, the sensitivity, specificity and positive predictive value were 53.4% ,97.1% and 98.6% respectively ( ＞ 35.54 μg/L as its positive criterion).The PTM level in microangiopathy group was higher than that in macroangiopathy group (P ＜ 0.01 ). The sensitivity,specificity and positive predictive value were 71.2%, 97.1% and 97.9% respectively. Acute leukemia (AL) and multiple myeloma (MM) had higher PTM level and PTM level was extremely high when renal failure developed ( P ＜ 0.01 ). As compared the acute stage with the restoration stage in stroke, pre-chemotherapeutics with post-chemotherapeutics in AL and MM, and pre-operation with post-operation in cancer, the PTM level was connected with clinical development. The PTM level in the patients with microangiopathy was higher than that in the patients with macroangiopathy (P＜ 0.01 ). The defined PTM level was higher than its normal upper limit as PTM positive criterion in microangiopathy diseases, the sensitivity, specificity and positive predictive value were 77.7%, 71.2% and 75.6% respectively. It is concluded that PTM level is a good criterion in evaluating the microangiopathy, and PTM is also a valuable indicator in prediction or assessment of the severity of diseases, or evaluation of therapeutic effectiveness.
The Significance of Plasma Thrombomodulin in Appraising Vascular Lesions and Complication in Diseases

Abstract:Objective To explore significance of plasma thrombomodulin(PTM) in appraising vascular lesions and complication in diseases.Methods PTM concentration was measured by ELISA.Results The PTM level,in macrovascular cardio-cerebrovascular diseases(coronary heart disease,cerebral infarction and cerebral hemorrhage) was slightly increased,average concentration was under 35 μg/L. The PTM levels of microangiopathy(primary chronic glomerulonephritis,urine protein positive group in SLE,complication group in diabetes mellitus,MOF and DIC) were all markedly increased,average concentration was40 μg/L and significantly higher than that in other groups of cardio-cerebrovascular diseases(P0.01).PTM levels of disease stages or complication in renal failure group of primary chronic glomerulonephritis were markedly higher than those in non-renal failure group(124.10±61.47 vs 51.13±35.68 μg/L,P0.01),SLE in urine protein positive group was markedly higher than that in urine protein negative group(55.81±38.49 vs 22.26±8.55 μg/L,P0.01),microangiopathy group in diabetes mellitus was markedly higher than in macrovascular disease group(43.10±13.28 vs 31.89±8.25 μg/L,P0.01). The PTM level of DIC(126.25±62.06 μg/L)was markedly higher than that in other groups(P0.01).Conclusion Test of PTM concentration can appraise vascular lesion types and complication in diseases.It has special significance in clinic and can provide new thoughts in studying the relation of pathological change and angiopathy.
Relation of Thrombomodulin, TFPI and Plasma Antioxidants in Healthy Individuals and Patients with Coronary Heart Disease.

Abstract:Objective -The role of antioxidants in preventing endothelial dysfunction has been discussed for many years and still is a controversial issue. This study is to determine the relation between markers of endothelial damage and plasma antioxidants in healthy individuals and patients with coronary heart disease (CHD).Methods and results - Plasma concentration and activity of antioxiclant b-carotene (P-CAR), superoxide dismutase (SOD), vitamin C (VC) and vitamin E (VE) were measured in patients with coronary heart disease and healthy volunteers. Markers of endothelial damage, thrombomodulin and tissue factor pathway inhibitor (TFPI) were also measured. Levels of thrombomodulin in CHD patients and healthy individuals were 29.8 +/- 24.2 ng/mL and 21.7 +/- 10.8 ng/mL respectively. Levels of TFPI in CH D patients and healthy individuals were 114.1 +/- 19.4 ng/mL and 93.5 +/- 14.1 ng/mL, respectively. Both thrombomodulin and TFPI levels were significantly higher in CHD patients than healthy volunteers (P < 0.05 and P < 0.01, respectively). Levels of VC, VE, SOD and P-CAR showed no significant differences between the two groups. In CHD patients, plasma thrombomodulin levels correlated negatively with plasma VC, VE and P-CAR. TFPI levels correlated negatively with plasma VC,VE, P-CAR and SOD. No significant relations could be found in healthy individuals between plasma thrombomodulin,TFPI and plasma antioxidants levels.Conclusions - In CHID patients, serum antioxidant levels correlated significantly and negatively with endothelial damage.

Abstract:Growing evidence has suggested the feasibility and effectiveness of blood coagulation with low temperature plasma (LTP) at atmospheric pressure. In the present study we examined morphological changes and contents released out from platelet after LTP treatment to investigate the mechanism of LTP accelerate blood coagulation. Changes of isolated rat platelets treated with LTP had been detected by flow cytometry, transmission electron microscopy (TEM) and scanning electron microscopy (SEM). Additionally, a scheme that the mixture of different color fluorescent stained platelets treated by LTP and then detected by flow cytometry had proposed to quantitatively assess plasma triggered platelets aggregation. Furthermore, we had used label free quantitative mass spectrometry to identify changes in the supernatant proteome released from LTP treated platelets. On the surface of LTP treatment hastened whole rat blood clots, a layer of semi-transparent film had formed, which had been demonstrated to form from platelet-like membrane structure by ultrastructural analysis with TEM. Flow cytometry combined with fluorescent staining confirmed that LTP treatment promoted platelet aggregation. The observation of plasma treated stained platelets under fluorescence confocal microscopy also confirmed this judgment. We separated the supernatant from the platelets treated with LTP, and then analyzed their differences using mass spectrometry. Compared with control group, Gene ontology (GO) analysis showed that the concentration of 16 protein molecules in the experimental group was increased. LTP treatment can promotes platelet aggregation and the release of platelet activating proteins, which is the potential mechanism of speeding up blood coagulation.

Abstract:Abstract Abstract 4527 Background: Transplantation-associated thrombotic microangiopathy (TA-TMA) is lifethrewten though rare complication of allogeneic hematopoietic stem cell transplantation (HSCT). Diagnosis of TA-TMA is difficult due to its variation of criteria. Recently two new diagnositic criteria for TA-TMA have recently been proposed: the Bone Marrow Transplant Clinical Trials Network (BMTCTN) and the International Working Group (IWG) criteria. Our purpose is to investigate the indence and risk facter of TA-TMA in our institute. Study design and methods: All 8 cases of TA-TMA previously diagnosed at our institution between October 2005 and August 2010 were retrospectively evaluated and analysised. Table I showed the transplant details and patients characteristics. Results: Six patients performed Matched Unrelated Donor HSCT, two were haploidentical donor HSCT. Four patients complicated with hypohepatia, but only three patients had renal insufficient. The incidence of central nervous symptom abnormalities or dysfunction was very high (sever of eight patients). Four of eight patients were CSA linked TA-TMA, withdrawing CSA resulted in complete response. The other four patients were no CSA linked TA-TMA and developed aGVHD or CMV infection before TA-TMA. They had badly response to common treatment including plasma exchange (PE), steroid treatment and immunosuppression decreased. All of them died of multiple organ failure. Table II showed TA-TMA manifestations and associated circumstance of the patient. Conclusion: Our experience suggests that CSA linked TA-TMA is totally different from no CSA linked TA-TMA. The former had good response and prognosis, the later were always associated with GVHD, hypohepatia and virus infectious. The mortality of no CSA linked TA-TMA is high, they had poor prognosis and badly response. Disclosures: No relevant conflicts of interest to declare.
Plasma Level Of Thrombomodulin Is An Early Indication Of Pancreatic Necrosis In Patients With Acute Pancreatitis

Abstract:Background The potential to predict pancreatic necrosis within the first 48 h by using plasma soluble thrombomodulin (sTM) in 104 patients with acute pancreatitis (AP) was analyzed in a prospective 5-year investigation performed at a single institution.Methods According to Balthazar CT grade, pancreatitis was classified as no necrosis in 72 patients, one-third necrotic in 18 patients, one-half necrotic in 10 patients and more than one-half necrotic in 4 patients. Blood was collected at the first 48 hours after the onset of pain and analyzed for sTM.Results In the healthy volunteers, plasma levels of TM were 16.49 +/- 5.24 mu g/L. By comparison, the mean plasma levels of TM in each group of pancreatitis patients were as follows: CT grade A group, 34.21 +/- 10.73 mu g/L; CT grade B group, 36.18 +/- 12.50 mu g/ L; CT grade C group, 49.39 +/- 18.38 mu g/ L; CT grade D group, 114.46 +/- 39.44 mu g/ L; CT grade E group, 100.22 +/- 15.97 mu g/ L (p<0.01). And for the patients, the Pearson correlation coefficient between the CT grade and TM values was 0.784 (p<0.01). No necrosis group, 39.22 +/- 13.75 mu g/ L; one-third necrotic group, 71.44 +/- 18.02 mu g/ L; one-half necrotic group, 123.50 +/- 28.57 mu g/ L; more than one-half necrotic group, 129.00 +/- 33.28 mu g/ L (p<0.01); And for the patients, the Pearson correlation coefficient between the degree of necrosis and TM values was 0.888 (p<0.01). ROC analysis indicated the area under the ROC curve (AUC +/- SE) for sTM was 0.949 +/- 0.020, clearly supportive of the high accuracy of this index in predicting the necrosis of AP.Conclusion Plasma soluble thrombomodulin (sTM) is a potential marker to predict pancreatic necrosis within the first 48 h, and further investigation in a multicentre study is necessary.
The Potential of Plasma Thrombomodulin As a Biomarker of Portal Vein Tumor Thrombus in Hepatocellular Carcinoma

Abstract:Purpose: To study the relationship between thrombomodulin (TM) plasma levels and the formation of portal vein tumor thrombus (PVTT) in patients with hepatocellular carcinoma (HCC). Methods: Pre- and-postoperative plasma TM levels of 45 patients with HCC and six patients with benign liver-occupying lesion were measured by enzyme-linked immunosorbent assay (ELISA), and the expression of TM in human HCC tissues was determined by immunohistochemistry assay. Results: The preoperative plasma TM level of patients with HCC (10.2±5.7 ng/ml) was significantly higher than that of those patients with benign liver-occupying lesion (6.1±2.2 ng/ml) and that of normal controls (5.7±1.0 ng/ml), respectively (P<0.05). The postoperative TM level of 40 patients with HCC whose tumors had been removed decreased significantly than the preoperative TM level (10.8±5.3 ng/ml versus 7.6±4.2 ng/ml, P<0.05), whereas there was no significant difference between the preoperative and postoperative TM level of six patients with benign liver-occupying lesion (6.1±2.2 ng/ml versus 5.9±1.8 ng/ml, P>0.05). The preoperative plasma TM level of patients with single HCC (11.5±5.9 ng/ml) or no PVTT (11.4±5.6 ng/ml) was significantly higher than that of those patients with multiple HCC (8.1±4.6 ng/ml) or PVTT (6.9±4.5 ng/ml), respectively (P<0.05). The preoperative plasma TM level of the patients with HCC tissue that stained positive for TM was significantly higher than those with tissue that stained negative for TM (12.2±6.5 ng/ml versus 8.7±4.6 ng/ml, P<0.05). The postoperative plasma TM level showed no difference between the patients with HCC tissue stained positive and negative for TM (8.3±4.1 ng/ml versus 7.6±4.4 ng/ml, P>0.05). There was also no significant difference between the plasma TM level and other clinicopathological futures. Conclusions: Plasma TM increases in patients with HCC and can be a biomarker of the formation of PVTT.

Effective biomarkers are required for its early diagnosis and prognosis. This study aimed to determine the diagnostic and prognostic value of thrombomodulin (TM) in patients with HBV-ACLF. Methods: The expression of TM during disease progression was evaluated through transcriptomics analysis. The plasma TM concentrations of 393 subjects with HBV-ACLF (n=213), acute-on-chronic hepatic dysfunction (ACHD, n=50), liver cirrhosis (LC, n=50) or chronic hepatitis B (CHB, n=50), and normal controls (NC, n=30) from a prospective multicenter cohort, were measured to verify the diagnostic and prognostic significance of plasma TM for HBV-ACLF patients by enzyme-linked immunosorbent assay (ELISA). Results: TM mRNA was highly expressed in the HBV-ACLF group compared with the ACHD group (AUROC=0.710). High expression of TM predicted poor prognosis for HBV-ACLF patients at 28/90 days (AUROCs=0.823/0.788). Functional analysis showed that TM was significantly associated with complement activation and the inflammatory signaling pathway. External validation confirmed its high diagnostic accuracy for HBV-ACLF patients (AUROC=0.796). Plasma TM concentrations were correlated with organ failure, including coagulation and kidney failure. Plasma TM concentrations showed a potential prognostic value for 28-day mortality rates (AUROC=0.702). Risk stratification specifically identified HBV-ACLF patients with a high risk of death as having a plasma TM concentration of ≥ 8.4 ng/mL. Conclusion: This study reveals that the plasma TM can be a candidate biomarker for early diagnosis and prognosis of HBV-ACLF, and might play a vital role in coagulation and inflammation. Relationship Between Plasma Thrombomodulin Level and Angiopathy in Patients with Diabetes Mellitus

Abstract:Objective To study the clinical significance of plasma thrombomodulin(PTM) level in diabetes mellitus(DM)patients with or without complication.Methods PTM level was measured by ELISA.Results The PTM level in complication group (38.14±12.89) μg/L was significantly higher than that in non complication group(22.11±6.58) mg/L,P0.01,The PTM level in non complication group was similar with normal controls(20.23±7.54) μg/L. ln complication group ,The PTM level in microangiopathy group(43.51±13.28) μg/L was significantly higher than that in macroangiopathy group(31.89±8.25) μg/L,P0.01.Moreover,the level of PTM increased with the elevation of urinary albumin excretion. PTM was not correlated with fasting blood glucose (FBS,r=0.0362, P0.05),cholesterol(r=-0.0229, P0.05),triglyceride(r=0.0325,P0.05), fibrinogen(r=0.2112,P0.05),fibrin degradation products (r=-0.0701, P0.05)and age (r=0.2341,P0.05).Conclusion PTM level can reflect the degree of endothelial injury and determine if complication exist in DM patients. Clinical Significance in Determination of Plasma Thrombomodulin Protein C Antigen and D - Dimer in Liver Cancer Patients.

Abstract:Objective To study the clinical significance of plasma thrombomodulin(TM), protein C(PC) antigen and D - dime (D-D) before and after treatment in liver cancer patients. Methods 25 liver cancer patients were selected into the study, the levels of plasma thrombomodulin and protein C antigen were measured by ELISA, and D-D was tested by immunoturbidimetric method. Results There were differences among three periods(Ⅰ,Ⅱ,Ⅲ)of hepatocarcinoma, plasma levels of TM and D - D increasing and PC decreasing with upgrading of the stage. The PC levels of patients among two periods(Ⅰ,Ⅱ) after operation decreased far more, the DD levels increased, and the TM levels went down but these in Ⅱ periods were higher than that in the contrast groups. Conclusion The plasma levels of TM, PC antigen and D-D can be used as one of die indicator of the development, curative effect and prognosis of liver cancer.
Targeting thrombomodulin to circulating red blood cells augments its protective effects in models of endotoxemia and ischemia-reperfusion injury

Abstract:Endothelial thrombomodulin (TM) regulates coagulation and inflammation via several mechanisms, including production of activated protein C (APC). Recombinant APC and soluble fragments of TM (sTM) have been tested in settings associated with insufficiency of the endogenous TM/APC pathway, such as sepsis. We previously designed a fusion protein of TM [single-chain variable fragment antibody (scFv)/TM] targeted to red blood cells (RBCs) to improve pharmacokinetics and antithrombotic effects without increasing bleeding. Here, scFv/TM was studied in mouse models of systemic inflammation and ischemia-reperfusion injury. Injected concomitantly with or before endotoxin, scFv/TM provided more potent protection against liver injury and release of pathological mediators than sTM, showing similar efficacy at up to 50-fold lower doses. scFv/TM provided protection when injected after endotoxin, whereas sTM did not, and augmented APC production by thrombin ∼50-fold more than sTM. However, scFv/TM injected after endotoxin did not reduce thrombin/antithrombin complexes; nor did antibodies that block APC anticoagulant activity suppress the prophylactic anti-inflammatory effect of scFv/TM. Therefore, similar to endogenous TM, RBC-anchored scFv/TM activates several protective pathways. Finally, scFv/TM was more effective at reducing cerebral infarct volume and alleviated neurological deficits than sTM after cerebral ischemia/reperfusion injury. These results indicate that RBC-targeted scFv/TM exerts multifaceted cytoprotective effects and may find utility in systemic and focal inflammatory and ischemic disorders.-Carnemolla, R., Villa, C. Abstract:Objective:The antigens as molecular markers of coagulation and fibrinolysis systems were measured in patients with malignant tumors in order to investigate their changes among the patients with acute leukemia(AL),malignant lymphoma(ML) and solid tumors.Methods:By enzyme-linked immunosorben assay(ELISA),4 plasma molecular markers of the coagulation and fibrinolysis systems were measured in 30 healthy volunteers and 47 cases of malignant tumors that included 27 patients with AL,in which 21 newly-diagnosed AL,6 relapsed cases,10 patients with ML and 10 patients with solid tumor.Their blood samples were taken from their veins at fasting in the morning,with anticoagulant of 3.8% sodium citrate.These molecular markers of coagulation and fibrinolytic system were as followings: tissue factor(TF),Thrombin——antithrombin complexes(TAT),tissue-type plasminogen activator(t-PA),plasminogen activator inhibitor-1(PAI-1).Results:1.The levels of TF in 47 cases of malignant tumors were all significantly higher than that in control group,TF level in ML group was significantly higher than that in AL group;2.TAT levels in the group of AL,ML group and solid tumor group were all significantly higher than that in control group respectively;3.The levels of t-PA in 3 groups of malignant tumors were all significantly higher than that in control group,the solid tumor group was significantly higher than AL group.4.The PAI-1 levels in AL group or in solid tumor group were significantly higher than that in control group.Conclusion:In patients with acute leukemia,lymphoma and solid tumor,there are the similar coagulation and fibrinolysis aberrancy that are predisposed to blood coagulation and inhibition of fibrinolysis.

Abstract:Thrombomodulin (TM) is a type I transmembrane glycoprotein mainly expressed on the endothelial cells, where it binds thrombin to form the thrombin-TM complex that can activate protein C and thrombin-activable fibrinolysis inhibitor (TAFI) and induce anticoagulant and anti-fibrinolytic reactions, respectively. Cell activation and injury often sheds microparticles that contain membrane TM, which circulate in biofluids like blood. However, the biological function of circulating microparticle-TM is still unknown even though it has been recognized as a biomarker of endothelial cell injury and damage. In comparison with cell membrane, different phospholipids are exposed on the microparticle surface due to cell membrane ''flip-flop'' upon cell activation and injury. Liposomes can be used as a microparticle mimetics. In this report, we prepared TM-containing liposomes with different phospholipids as surrogates of endothelial microparticle-TM and investigated their cofactor activities. We found that liposomal TM with phosphatidylethanolamine (PtEtn) showed increased protein C activation but decreased TAFI activation in comparison to liposomal TM with phosphatidylcholine (PtCho). In addition, we investigated whether protein C and TAFI compete for the thrombin/TM complex on the liposomes. We found that protein C and TAFI did not compete for the thrombin/TM complex on the liposomes with PtCho alone and with low concentration (5%) of PtEtn and phosphatidylserine (PtSer), but competed each other on the liposomes with higher concentration (10%) of PtEtn and PtSer. These results indicate that membrane lipids affect protein C and TAFI activation and microparticle-TM may have different cofactor activities in comparison to cell membrane TM.

Abstract:Background: Thrombin activatable fibrinolysis inhibitor (TAFI) is one of the most important physiological fibrinolysis inhibitors. Its inhibitory efficacy under physiological conditions remains uncertain. Objectives: Elucidate the role of soluble thrombomodulin (sTM)/TAFI axis in the regulation of fibrinlysis. Methods: Since thrombin is required to generate activated TAFI (TAFIa) that targets the C-terminal lysine of partially digested fibrin, a clot lysis assay is suitable for evaluating its function. Using tissue-type plasminogen activator-induced plasma clot lysis time (tPA-PCLT) together with TAFIa inhibitor and recombinant sTM (rsTM), we evaluated the specific function of TM/TAFI in the plasma milieu. Results: tPA-PCLT values were significantly shortened by the TAFIa inhibitor. rsTM supplementation prolonged tPA-PCLT, which was shortened by the TAFIa inhibitor to a time similar to that obtained without rsTM and with the TAFIa inhibitor. Plasma obtained from patients treated with rsTM showed prolonged tPA-PCLT, which was shortened by the TAFIa inhibitor but not further prolonged by rsTM. However, no significant correlation was observed between tPA-PCLT and parameters of TM/TAFI system in the plasma. Conclusion: The role of the TM/TAFI system in regulating fibrinolysis was successfully evaluated using TAFIa inhibitor and rsTM. Trace amounts of soluble TM in normal plasma appeared sufficient to activate TAFI and inhibit fibrinolysis. Further, a therapeutic dose of rsTM appeared sufficient to activate TAFI and regulate fibrinolysis in the plasma milieu.

Abstract:Platelets, derived from a certain subpopulation of megakaryocytes, are closely related to hemostasis, coagulation, metastasis, inflammation, and cancer progression. Thrombopoiesis is a dynamic process regulated by various signaling pathways in which thrombopoietin (THPO)–MPL is dominant. Thrombopoiesis-stimulating agents could promote platelet production, showing therapeutic effects in different kinds of thrombocytopenia. Some thrombopoiesis-stimulating agents are currently used in clinical practices to treat thrombocytopenia. The others are not in clinical investigations to deal with thrombocytopenia but have potential in thrombopoiesis. Their potential values in thrombocytopenia treatment should be highly regarded. Novel drug screening models and drug repurposing research have found many new agents and yielded promising outcomes in preclinical or clinical studies. This review will briefly introduce thrombopoiesis-stimulating agents currently or potentially valuable in thrombocytopenia treatment and summarize the possible mechanisms and therapeutic effects, which may enrich the pharmacological armamentarium for the medical treatment of thrombocytopenia.

Abstract:Objective To evaluate the efficacy of recombinant human thrombopoietin (rhTPO) in patients with severe sepsis and thrombocytopenia. Methods Sixty-six patients with severe sepsis and hrombocytopenia between October 2013 and September 2015 were divided into the experiment group (35 cases) and control group (31 cases). All patients received primary treatment and infection control. And patients in the experiment group received rhTPO 300 U· kg-1·d-1 by hypodermic injection, and withdraw when platelet count (PLT)≥50 × 109/L, and the course did not exceed 14 d. Levels of PLT, C-reactive protein and alanine aminotransferase (ALT) were detected before and after the treatment on 1, 2, 3, 5, 7, 9 ans 14 d. The platelet transfusion cases, acute physiology and chronic health evaluation Ⅱ (APACHEⅡ), the time of temperature dropped to normal, time of clinical symptom disappearance, pulmonary imaging recovery time, 28-day fatality rate and length of ICU stay were compared between the two groups. The adverse reactions were recorded, and the activated partial thromboplastin time, levels of ALT, C-reactive protein and total bilirubin were compared before and after treatment in the experiment group. Results The PLT only increased markedly on 3 and 5 d after treatment in the experiment group as compared with those in the control group [(56 ± 19) × 109/L vs. (42 ± 18) × 109/L, t=3.112, P<0.05; (67 ± 22) × 109/L vs. (54 ± 21) × 109/L, t=2.520, P<0.05]. The platelet transfusion rate in the experiment group was much lower than that in the control group (5/35 vs. 11/31,χ2=4.022, P=0.045). However, the APACHEⅡ scores (t=0.692, P<0.05), the time of temperature dropped to normal (t=0.510, P<0.05), time of clinical symptom disappearance (t=0.262, P<0.05), pulmonary imaging recovery time (t=0.685, P<0.05), 28-day fatality rate ( χ2= 0.001, P < 0.05) and length of ICU stay (t=0.637, P<0.05) all showed no significant differences between the two groups after treatment. Meanwhile, there was no adverse reactions happened, and the activated partial thromboplastin time (t=0.697, P<0.05), levels of ALT (t=0.478, P<0.05), C-reactive protein (t=0.110, P<0.05) and total bilirubin (t=1.634, P<0.05) in the experiment group also showed no significant differences before and after the treatment. Conclusions The use of rhTPO combined with conventional treatment in patients with sepsis-associated thrombocytopenia is safe. It can significantly enhance the platelet count, reduce platelet transfusion and improve patient's prognosis.

Abstract:Objective To investigate coagulation fibrinolysis index in the patients with ovarian hyperstimulation syndrome(OHSS),and to explore the clinical value.Methods 64 patients undergoing invitro fertilization and embryo transferb(IVF-ET) were developed OHSS and 40 patients as control group who didn′t undergoing OHSS in the same period were analyzed.Plasma samples were collected,and the levels of plasma PT,APTT,TT,FIB and D-dimer were measured and analyzed by t test.Results Compared with control group,PT and TT were longer,but APTT was shorten.Their levels were lied in the reference limit.The levels of FIB and D-dimer of the OHSS patients were higher than control group(P<0.05),but their levels were decreased after the treatment(P<0.05).Conclusion Evaluation of the blood coagulation and fibrinolysis function may be important for predicting prognosis and therapeutic effect for OHSS patients.

Abstract:Background To conduct a comprehensive performance evaluation of a fully automated analyzer for measuring thrombomodulin (TM), thrombin-antithrombin complex (TAT), plasmin-alpha 2-antiplasmin complex (PAP), and t-PA: PAI-1 complex (tPAI-C). Methods According to the Clinical and Laboratory Standards Institute (CLSI) EP05-A2, EP06-A specifications, TM, TAT, PAP, and tPAI-C were analyzed to evaluate intraassay variability and interassay variability, linear range, carryover rate, reference range, sample stability, and interferences. Results The intraassay variability and interassay variability of the four factors were all below 5%. The carryover rates were below 1%. Linear verification analysis revealed correlation coefficients of 0.998-0.999. The recommended reference ranges of TM, TAT, and PAP were appropriate for our laboratory, whereas the reference of tPAI-C should be established by each laboratory. Stability assessment revealed that TM is stable for 2 days at room temperature but lacks stability at colder temperatures. In contrast, TAT is stable for 5 days at 4 degrees C and -20 degrees C but has poor stability at room temperature. PAP and tPAI-C are stable for 3 days at all three temperatures. The measurement of TM, TAT, PAP, and tPAI-C is not altered by the presence of 510 mg/dL hemoglobin, 1490 FTU triglycerides, or 21.1 mg/dL conjugated and free bilirubin. Conclusion The determination of TM, TAT, PAP, and tPAI-C using a high-sensitivity chemiluminescence analyzer performs well in terms of precision, carryover rate, linear range, and interference. Thus, this method is suitable for the detection of these substances in clinical specimens.
Detection of Thrombomodulin Level in Patients with Hepatocellular Carcinoma

Abstract:Purpose To detect the thrombomodulin(TM) level in patients with hepatocellular carcinoma(HCC) and explore its relation to the clinicopathological futures of HCC. Methods 45 cases of patients with HCC,6 cases of patients with benign liver occupying lesions(BLOL) and 10 adult control were enrolled in this study.Pre-and-postoperative plasma TM level of patients with HCC and (BLOL) was measured by enzyme linked immunosorbent assay,and the expression of TM in human HCC tissues was determined by immunohistochemistry assay. Results The preoperative plasma TM level of patients with HCC(10.2±5.7)ng/mL was significantly higher than that of those patients with BLOL(6.1±2.2)ng/mL and that of normal control(5.7±1.0)ng/mL,respectively(P0.05).The postoperative TM level of 40 patients with HCC whose tumors had been removed decreased significantly than the preoperative TM level[(10.8± 5.3) ng/mL versus (7.6±4.2)ng/mL,P0.05],whereas there was no significant difference between the preoperative and postoperative TM level of 6 patients with BLOL(P0.05).The preoperative plasma TM level of patients with single HCC(11.5±5.9)ng/mL or no portal vein tumor thrombus(PVTT)(11.4±5.6)ng/mL was significantly higher than that of those patients with multiple HCC(8.1±4.6)ng/mL or PVTT(6.9±4.5)ng/mL,respectively(P0.05).The preoperative plasma TM level of the patients with HCC tissue that stained positive for TM was significantly higher than those with tissue that stained negative for TM(P0.05).The postoperative plasma TM level showed no difference between the patients with HCC tissue stained positive and negative for TM(P0.05).There was also no significant difference between the plasma TM level and other clinicopathological futures. Conclusions Plasma TM increases in patients with HCC and correlates the formation of PVTT.
