Abstract:Objective The surgical approach scar excision and repair by laparoscopy combined hysteroscopy has been carried out and achieved good results.This study compared it with estrogen and progesterone cycle therapy to explore the value of it.Methods Between February 2009 and January 2012,the 145 patient in Obstetrics and Gynecology Hospital of Fudan University has been collected and been divided into laparoscopy combined hysteroscopy group and the estrogen and progesterone cycle therapy group.The improvement of vaginal bleeding and wound healing situation etc were analyzed.Results The rate of incision healing well in the study group after surgery accounted for 73.5%,and the patients monthly vaginal bleeding time≤7 days accounted for 38.8%,and that for 814 days accounted for 51.0%.The patients menstruation returned to normal accounted for 24.5%.The patients incision completely healed accounted for 73.5%,and that significantly reduced and no change were 20.4% and 6.1% respectively.In control group monthly vaginal bleeding time≤7 days and 814 days were observed in only 8.5% and 44.7% of patients respectively;The patients menstruation returned to normal accounted for 4.3%.Incision defect of patients had no significant improvement.The differences between two groups were significant(P<0.01).But the difference of abdominal pain between the two groups was not significant(P>0.05).Conclusion The effectiveness and thoroughness of the surgery were significantly higher than estrogen and progesterone cycle therapy,so the method should be widely applied.

A Systematic Review and Meta-Analysis of the Efficacy and Safety of Hysteroscopic Electric Resection Versus Vaginal Surgery in the Treatment of Uterine Scar Defects after Cesarean Section

Abstract:Background: Cesarean sections are increasingly likely to be applied; however, uterine scar defects (USD) often remain after delivery.The two existing treatment methods, hysteroscopic electric resection and vaginal surgery, are still controversial in terms of efficacy and safety.So, this paper to compares the effectiveness and safety of hysteroscopic electric resection and vaginal surgery in the treatment of USD after cesarean section.Methods: We performed a related literature search from main databases.According to the PICOS principles inclusion criteria were adult female USD patients to evaluate the efficacy of hysteroscopic resection and vaginal surgery for the treatment of USD, outcome data could be extracted to compare the efficacy and safety of the two procedures.Subsequently, according to the titles, abstracts, and full texts of the retrieved articles, studies that did not meet the inclusion criteria were eliminated.The RevMan 5.20 software was used for meta-analysis and Cochrane Risk of Bias 2 (RoB 2.0) was used to assess the risk of bias.The effectiveness and safety of hysteroscopic resection and vaginal surgery in the treatment of USD patients after cesarean section were compared.Results: Eight articles were finally included, with a total of 191 patients in the hysteroscopic electric resection group and 212 patients in the vaginal surgery group.Compared with hysteroscopic resection and vaginal surgery, there is less intraoperative blood loss [mean difference (MD) is -25.23,P<0.00001], shorter operation time (MD is -29.45,P<0.00001), and shorter hospital stay (MD is -1.87,P<0.00001), but menstrual improvement risk ratio (RR) is 0.71 (P=0.51) and diverticulum recovery RR is 0.60 (P=0.43)there were no significant differences.Discussion: Hysteroscopic electric resection provides a more satisfactory outcome than vaginal surgery in terms of intraoperative blood loss, operation time, and hospital stay.However, the sample size of the study was not large enough and some studies had high risk of bias, more large-sample multi-center high quality studies are needed for further comprehensive comparative analysis.
Comparison of Therapeutic Effects of Three Surgical Approaches for Cesarean Scar Diverticulum

Abstract:Objective To compare the therapeutic effects of three surgical approaches for cesarean scar diverticulum (CSD). Methods From January 2013 to January 2018, 80 patients diagnosed with CSD by transvaginal ultrasound at the First Affiliated Hospital of Zhejiang University were selected and divided into three groups: 40 patients who underwent hysteroscopic electrical incision of the diverticulum (hysteroscopic group), 30 patients who were treated by hysteroscopic-laparoscopic diverticulum repair (hysteroscopic-laparoscopic group), and 10 patients who underwent transvaginal repair of CSD (transvaginal group). For age, frequency of cesarean section, preoperative menstrual period, operative time, intraoperative bleeding, postoperative hospital stay, hospitalization expenses, and other indicators of the three groups, ANOVA was used for comparison among multiple groups, and LSD method was used for pairwise comparison. The efficacy and recurrence in the three groups within 12 months of follow-up were evaluated by the Chi-square test, pairwise comparison was performed by Chi-square segmentation, and they were finally corrected by the BONFERRONI method. Results The operative time, intraoperative blood loss, postoperative hospital stay, and hospitalization cost of the hysteroscopic group were significantly less than those of the hysteroscopic-laparoscopic group and the transvaginal group (F=198.300, P 0.05). In addition, there was no statistically significant difference in the efficacy or recurrence among the three groups within 12 months of follow-up (P>0.05). Conclusion All the three surgical methods have good effectiveness in the treatment of CSD, and hysteroscopic electrical incision of the diverticulum has higher clinical application value. Abstract:Objective This study aimed to determine the risk factors associated with the necessity of laparoscopic scar defect repair for cesarean scar pregnancy (CSP). Methods We retrospectively analyzed 237 patients with CSP who were treated by ultrasound-guided suction curettage and/or laparoscopy in our hospital from April 2012 to November 2019. A total of 199 of these patients underwent ultrasound-guided suction curettage without uterine scar defect repair, while 38 of these patients underwent laparoscopic resection and uterine scar defect repair. We analyzed various clinical variables and compared the efficacy of treatment between the two groups. Results Gestational age, the maximum transverse diameter (MTD) of the gestational sac, myometrial thickness, the operation time, intraoperative blood loss, and the duration of the hospital stay were significantly different between the two groups. Gestational age, the MTD of the gestational sac, and myometrial thickness were independent risk factors for laparoscopic repair. Conclusions Gestational age, the MTD of the gestational sac, and myometrial thickness are important factors associated with the necessity for laparoscopic repair of a uterine scar defect.

Study on Comparison of Clinical Effects of Two Kinds of Operational Procedures in Treatment of Heavy Rectocele.

Abstract:Objective To analyze and compare the effectiveness of modified Block's repair procedure and transvaginal incision and repair procedure in treatment of heavy rectocele.Methods Eighty patients with heavy rectocele were randomly allocated into study group(n=40) and control group(n=40).Patients in study group received modified Block's repair procedure and patients in control group received transvaginal incision and repair procedure.The effect of treatment and complications were compared between these two groups.Results Scores on symptoms were lower in patients of study group than those of control group.The short term effect was similar in patients between these two groups.There were significantly less operating time,operative bleeding and postoperative pain in patients of study group in comparison with those of control group.Conclusion Modified Block's repair procedure for heavy rectocele is better than transvaginal incision and repair procedure in short term and long term efficacy.

Abstract:Objective: To explore the possible pathogenesis of uterine scar defect after cesarean section, and to investigate the effect of laparoscopic surgical repair on the defect. Methods: Fourteen patients of uterine scar defect after cesarean section who underwent hysteroscopic examination and laparoscopic repair in Obstetrics and Gynecology Hospital of Fudan University since Oct., 2009 were reviewed retrospectively and followed. Results: All the patients had a prolonged period, and the duration was (14.0±4.4) days (8-25 days), 4 cases had a history of infertility, and 4 cases complained of abdominal pain during menstruation. Hysterosalpingography (HSG) showed the width of the defect was (19.3±5.0) mm (13-30 mm), and the depth was (11.6±3.2) mm (5-19 mm), both significantly larger than thosed measured by trans-vaginal ultrasonic examination. The indications of cesarean section were uterine scar (previous section, 6 cases), premature rupture of fetal membranes (PROM, 3 cases), dystocia (3 cases), social factor (2 cases) and fetal distress (3 cases). In the previous cesarean section, the uterine was sutured by 1 layer of absorbable material in 7 cases, 2 layers in 4 cases, and unknown in the other 3 cases. Repair of the uterine defect was successfully achieved in all 14 patients. There was no intra-operative complications and none of the patients required blood transfusion. The mean operation time was (130±47) min (75-225 min), and the mean blood loss was (102±79) mL (30-300 mL). The cases was followed for an average of 4.1 months (2-12 months). In the 12 patients who had a recovery of menstruation, 11 patients had an obvious shortened period, and the duration was (8.6±3.2) days (6-10 days), and the clinic effective rate was 91.7% (11/12). Nine patients had trans-vaginal ultrasonic examination, and small defect was found in 2 patients (6 mm×2 mm and 7 mm×5 mm, respectively). Conclusions: HSG is better than transvaginal sonography to evaluate the defect before the surgery. Multiple cesarean section and PROM are the possible high risk factors of uterine scar defect after cesarean section. Laparoscopic surgical repair of the defect may be performed with satisfying effective rate.

Abstract:In 1995, Morris first described cesarean scar defect as an "isthmocele" by macroscopy following hysterectomy in women with prior cesarean delivery. Cesarean scar defect is associated with gynecological symptoms such as abnormal uterine bleeding, secondary infertility, pelvic pain, and obstetrical complications such as cesarean scar pregnancy, placenta accreta, and uterine rupture. Surgical treatment techniques include hysteroscopic resection, transabdominal repair (laparotomy, laparoscopic, and robotic), and vaginal repair. If the residual myometrial thickness is <3 mm and a patient is symptomatic, consideration is made for defect repair from above rather than hysteroscopic resection. The advantages of laparoscopic repair include anatomic restoration of myometrial thickness, correction of uterine retroflexion, exploration of other causes of infertility and pelvic pain, and pathological diagnosis of scar tissue with endometriosis. Cesarean scar defect often cannot be visualized on the side of the abdominal cavity; therefore, it is difficult to identify the extent of the defect laparoscopically. Herein, we introduce laparoscopic cesarean scar defect repair through a surgical video with narration. This technique uses a uterine manipulator to distend and help delineate the defect, and a laparoscopic support suture within the defect as a "handle" to place the scar tissue on tension to ensure complete resection of the fibrotic tissue. Temporary uterine artery occlusion can be included to reduce bleeding in the surgical field to support visualization for complete fibrotic tissue removal and to achieve good apposition with a double-layer suture to promote proper anatomic wound healing. Symptom relief was achieved, and the patient became pregnant one year postoperatively. This video demonstrated a feasible, safe, effective procedure for laparoscopic cesarean scar defect repair in the patient.
Combined Laparoscopy and Hysteroscopy Vs. Uterine Curettage in the Uterine Artery Embolization-Based Management of Cesarean Scar Pregnancy: A Cohort Study

Abstract:Aim: To evaluate the efficacy of combined laparoscopy and hysteroscopy compared with traditional uterine curettage in removing the ectopic conceptus and repairing the tissue defect following uterine artery embolization (UAE) management of cesarean scar pregnancy (CSP). Design: A prospective cohort study. Setting: Three large obstetrics and gynecology centers in Shanghai, China. Sample: CSP patients diagnosed between March 2009 and August 2010 who had received no prior treatments, were hemodynamically stable, and had no contraindications for UAE were enrolled. Methods: Patients were divided into two cohorts to undergo the intra-arterial methotrexate (MTX), UAE, and one of the following treatments: combined laparoscopy and hysteroscopy (research group, 25 cases) and uterine curettage (control group, 33 cases). Main Outcome Measures: The conceptus removal rate, the severity of intra-and postoperative complications, surgical time, and duration of hospital stay. Results: The single-surgery conceptus removal rate reached 100% in the research group, which was significantly higher than the 82% (P=0.024) observed in the control group (with one hysterectomy). The average volume of intraoperative blood loss was 78.0 mL in the research group, which was much less than the 258.5 mL (P=0.004) in the control group. Moreover, the research group had significantly shorter hospital stays and beta-hCG regression times, as well as lower rates of postoperative abdominal pain, uterine bleeding and menstruation abnormalities. Conclusions: Combined laparoscopy and hysteroscopy is much safer and more effective than uterine curettage as a supplementary measure following UAE.

Abstract:Objective: To find more safer and effective treatment methods for caesarean scar pregnancy (CSP) after uterine artery embolization (UAE), we compared laparoscopy and hysteroscopy-combined surgery with uterine curettage. Methods: Fifty-eight patients were enrolled from three medical centers in Shanghai, China from Mar. 2004 through Aug. 2010, who were divided into two cohorts: 25 received intra-arterial methotrexate (MTX)+UAE+laparoscopy and hysteroscopy-combined surgery (research group), and 33 received intra-arterial methotrexate (MTX)+UAE+uterine curettage (control group). The baseline characters, surgical clearance rates, complications, operation times, durations of hospitalization and recovery qualities of patients from these two groups were compared. Results: The first-time clearance rate of the patients in the research group reached 100%, while the mean blood loss during operation was 78.0 mL. The patients in the control group were cured with single or multiple uterine curettages except for one case whose uterus was removed due to the uterine perforation. The first-time clearance rate was 82% (P=0.024) and the mean blood loss was 258.5 mL (P=0.004) in the control group, which were both significantly inferior to those of the research group. On the hospital stay and β-hCG regression time, the research group was much better than the control group (P=0.43 and P=0.033, respectively). On the post-surgical recovery qualities, the patients from the research group had significantly lower rates of abnormal abdominal pain, vaginal bleeding and menstruation compared with those of the control group. Conclusions: As a successive medical measure following the UAE in the treatment of CSP, the safety, effectiveness and therapeutic radicalness of the laparoscopy and hysteroscopy-combined surgery are much higher than those of the traditional uterine curettage, indicating it can be used a routine scenario in the clinic.

Abstract:Objectives: We described laparoscopic repair of previous lower uterine segment caesarean scar defect (PCSD) in a series of 146 patients in our hospital. Methods: From April 2010 to July 2015, 146 patients with PCSD in the Obstetrics and Gynecology Hospital of Fudan University underwent laparoscopic repair of PCSD. The surgical and pregnancy outcome were followed, and the risk factors for successful healing of the repair were analyzed. Results: The patients in the incomplete and complete excision group both had an obviously shortened period after surgery. Single factor analysis showed that the thickness of the residual myometrium, suturing material, and estimated blood loss were correlated with the effects of surgery. Multivariate logistic regression analysis showed that the thickness of the residual myometrium and suturing material were independent risk factors for successful healing of the repair. Thirty-two patients desired for fertility in this study, and 12 of them got pregnant in 13-32 months after surgery, including 8 term cesarean delivery, 2 preterm cesarean delivery, 1 artificial abortion, 1 cesarean scar pregnancy, and 2 were pregnant when this study was summarized. Conclusions: Laparoscopic complete excision of CSD and repair with delayed absorbable material may be performed with fair symptom relief and acceptable postoperative anatomic and functional outcomes.

Abstract:Background: There is no consensus on a standardized therapy for type II cesarean scar pregnancy (CSP II). The objective of the present study was to evaluate the efficacy and safety and compare costs associated with transvaginal removal and repair (TRR) of uterine defect for CSP II to those of uterine artery embolization (UAE) and curettage. Methods: We conducted a retrospective study that included 87 patients diagnosed with CSP II and treated by performing UAE in combination with curettage and hysteroscopy ( n = 53), or TRR ( n = 34). Clinical data and outcomes were analyzed. Results: UAE and TRR groups exhibited similar success rates. The TRR group had significantly lower complication rates (30.19 vs. 8.82%, P < 0.05) and lower total costs (13,765.89 ± 2,029.12 vs. 9,063.82 ± 954.67, P < 0.05) than the UAE group. The anterior myometrium of the lower uterine segment was relatively thicker after performing TRR, and no patient suffered from recurrent CSP II. The proportion of patients in the TRR group who had full-term delivery without uterine rupture was 88.24% (30/34), while four patients failed to pregnancy. Conclusion: TRR is a safe and effective treatment method for patients with CSP II and presents a highly cost-effective outcome, especially for patients with future fertility desire.

Comparison of transvaginal repair and transabdominal repair for vesicovaginal fistula

Abstract:Objective To compare the outcome of transvaginal repair of vesicovaginal fistula with that of transabdominal repair.Methods A total of 72 patients were managed for vesicovaginal fistula between January 2001 and December 2011.Fifty-seven cases underwent surgical repair,39 of which were repaired by a vaginal approach using flap splitting techniques,18 of which underwent transabdominal repair.The baseline statuses of the 2 group were equal.After 3 months of follow-up,the operative and outcome data were abstracted and analyzed with statistical methods.Results Transvaginal repair could result in a significantly higher success rate than transabdominal procedure (95％ versus 72％,P=0.027).The transvaginal operative time was shorter than transabdominal ones (142± 35 min versus 212± 79 min,t\u0027 =-3.592,P =0.002).The postoperative hospital stay of transvaginal group was also shorter than that of transvaginal group (14±6 d versus 27±10 d,t\u0027=-4.983,P=0.000).Conclusion For simple vesicovaginal fistula,transvaginal repair could be a minimal invasive procedure,and provide favorable outcomes.\r\n\r\nKey words: \r\nVesicovaginal fistula; Transvaginal repair; Transabdominal repair; Treatment outcome
Evaluation of Short-Term Outcomes of Laparoscopic and Open Total Mesorectal Excision for Mid and Distal Rectal Cancer

Abstract:Objective:To evaluate the difference of short-term outcomes between laparoscopic total mesorectal excision and open operation for mid and distal rectal cancer.Methods: A total of 138 patients with mid-lower rectal cancer were randomly assigned to laparoscopic surgery group(n=67) and open surgery group(n=71) from September 2008 to July 2011.Data about the patients' demographics,operative details and postoperative outcomes were collected retrospectively.Comparison of the short-term outcomes was performed between the two groups.Results: Anterior resection was performed in 65.7%(44/67)patients of the laparoscopic surgery group and 60.6%(43/71)patients of the open surgery group.The operating time of the laparoscopic surgery group [(176.4±68.3) min] was longer than the open surgery group [(122.7±42.5) min],and the difference is significant(P<0.05).The blood loss in the laparoscopic surgery group [(86.9±37.6)ml] was less than the open group [(119.1±32.7)ml],and the difference is significant(P<0.05).Patients undergoing laparoscopic resection had a shorter analgesic time,an earlier return of bowel function,a shorter indwelling catheter time,as well as a shorter hospital stay.There were no significant differences in the number of lymph nodes harvested,specimen length,the rate of positive circumfeential margins,as well as the length of distal margin between the two groups.Conversion to conventional surgery was necessary in two cases(2.99%).There were no significant differences in the rate of perioperative complications between the two groups,four in the laparoscopic surgery group and six in the open surgery group.Conclusions: Compared with open operation,laparoscopic TME for mid and distal rectal cancer is a safe and effective option for less blood loss,faster recovery,less pain,less postoperative complications and short hospitalization,while the short-term outcomes of the two methods is similar.

Abstract:Objective: To compare the incidence of postmenstrual bleeding after hysteroscopic resection versus laparoscopic repair of previous cesarean scar defect (PCSD).Materials and methods: Retrospective analysis of computerized patient records. For the diagnosis of PCSD, patients underwent transvaginal ultrasound first without and then with saline-assisted sonohysterography. Hysteroscopic PCSD resection was performed under sonographic guidance, while laparoscopic repair was guided by hysteroscopy for the confirmation of scar margins.Results: Records of 62 patients presenting with PCSD-related postmenstrual bleeding were included in analysis. Hysteroscopic surgery had significantly shorter operative time compared to the laparoscopic approach (Mean =30.9 vs 71.0 minutes; p < 0.001). Blood loss and hospital stay were significantly less (p < 0.001) in hysteroscopic resection (10.4 ± 4.6 ml and 2.1 ± 0.4 days) than in laparoscopic repair (36.6 ± 4 ml, and 4.6 ± 1 days). After surgical interventions, the postmenstrual bleeding was resolved or improved. The effectiveness rates of hysteroscopic resection and laparoscopic repair were 91.4% and 96.3%, respectively. Incidence of post-treatment postmenstrual bleeding was not significantly different between hysteroscopy and laparoscopy (OR= 1.29 [95% confidence interval 0.367, 4.86]; p = 0.662). Pretreatment postmenstrual bleeding was associated with time since cesarean section (B= −0.091 [−0.158, −0.023]; p = 0.01) and PCSD length (B = 0.502 [0.085, 0.919]; p = 0.019).Conclusion: Both hysteroscopic resection and laparoscopic repair of PCSD yield comparable efficacy in reducing postmenstrual bleeding. However, hysteoroscopic resection of PCSD is associated with comparatively shorter operative time, less blood loss, and shorter hospital stay.

Abstract:Objective To compare the efficacy and safety between transabdominal preperitoneal and total extraperitoneal laparoscopic inguinal hernia repair.Methods From August 2005 to August 2008,in our laparoscopic surgery center,the patients who had underwent laparoscopic inguinal hernia repair were randomly divided into two groups according to surgical method,via transabdominal preperitoneal laparoscopic hernia repair(TAPP) group of 55 patients,via totally extraperitoneal laparoscopic hernia repair(TEP) group of 64 cases,make a comparative analysis of clinical data between TAPP group and TEP group.Results Patients were followed up from 6 months to 3 years;TEP operative time was shorter(P=0.000),lower costs(P=0.035),blood loss and postoperative hospital stay has no significant difference;preperitoneal injury,hematoma,serum swelling,intestinal obstruction,urinary retention,intestinal injury,after 6 weeks for pain,infection and recurrence rates were not significantly different.Conclusion TAPP with TEP each have advantages,but with the TAPP comparison,TEP operation time was shorter in lower costs.There is no difference between the selection of indications.
Prospective Evaluation of Five Methods Used to Treat Cesarean Scar Defects.

Abstract:OBJECTIVE:To evaluate operative and nonoperative therapies for cesarean scar defects (CSDs).METHODS:A prospective study was conducted among patients who underwent treatment of CSDs at a hospital in Shanghai, China, between April 1, 2010, and December 31, 2014. Treatment included laparoscopy (group 1), vaginal surgery (group 2), hysteroscopy (group 3), combined oral contraceptives (group 4), and the levonorgestrel intrauterine system (group 5).RESULTS:Among 142 participants, 119 patients underwent surgical repair and 23 received nonsurgical (conservative) treatment. Operation time, blood loss, and length of hospital stay reported for patients in group 3 (n=19) were all lower than those reported for patients in group 1 (n=86) and group 2 (n=14; P<0.001 for all comparisons). Patients who underwent surgery and those in group 4 (n=18) experienced shortened menstrual periods after treatment (P<0.001 for all comparisons). For group 5 (n=5), the duration of menstruation was similar before and after therapy (P=0.89). All 32 women who desired fertility underwent laparoscopy; 12 (37.5%) became pregnant after this procedure.CONCLUSION:All treatments for CSDs other than the levonorgestrel intrauterine system shortened menstrual periods. Laparoscopic surgery was effective for patients with fertility requirements.
A Comparison of Treatment Options for Type 1 and Type 2 Caesarean Scar Pregnancy: A Retrospective Case Series Study

Abstract:Background: There is currently no agreement on the optimal management of caesarean scar pregnancy. Caesarean scar pregnancy is currently categorised into two subtypes according to the site of implantation. This may consequently result in the difference in treatment options. However, the comparison of the success rate of each treatment option according to the subtypes has not been fully investigated. Methods: 71 patients who were treated by uterine curettage (D and C), or uterine artery embolization with curettage (UAE) or hysteroscopy in conjunction with laparoscopy between January 2016 and March 2020 were included. Data on maternal age, gestational sac age, the sac diameter, the interval between two pregnancies, the number of previous caesarean sections, amount of bleeding and β-hCG levels were collected and analysed dependent on the subtypes. Results: There was no difference in the clinical parameters of the cases who received different options of treatment, as well as no difference in the clinical parameters between type 1 and type 2 caesarean scar pregnancy. The primary success rate for type 1 caesarean scar pregnancy by D and C, or UAE, or hysteroscopy in conjunction with laparoscopy was 95, or 100 or 100%, respectively. The primary success rate for type 2 caesarean scar pregnancy by D and C, or UAE, or hysteroscopy in conjunction with laparoscopy was 27, or 67, or 95% respectively. Conclusion: Our data demonstrates that hysteroscopy in conjunction with laparoscopy for type 2 caesarean scar pregnancy was the most successful compared to other options, but for type 1 caesarean scar pregnancy, D and C could be the cost-effective option.

Obstetrical outcomes after vaginal repair of caesarean scar diverticula in reproductive-aged women

Abstract:Background: Although vaginal repair has been conducted to manage caesarean scar diverticula, most studies evaluated only the gynaecological outcomes post-surgery, and their obstetrical outcomes were unknown. This study aimed to evaluate the obstetrical outcomes in vaginal repair-treated caesarean scar diverticula patients. Methods: A series of 51 symptomatic women with caesarean scar defects or a thickness of the remaining muscular layer of less than 3 mm according to transvaginal ultrasound were included. We retrospectively evaluated the gynaecological and obstetrical outcomes after vaginal repair and histologically analysed the defect. Results: Transvaginal ultrasound revealed that the thickness of the remaining muscular layer significantly increased from 2.24 ± 0.81 mm to 6.10 ± 1.43 mm 3 months after vaginal repair. The duration of menstruation significantly decreased from 14.29 ± 3.13 days to 8.31 ± 2.14 days post-vaginal repair. Notably, 26 of the 51 (50.98%) women who were followed for more than 15.04 months post-surgery achieved pregnancy. A total of 6 of the 26 pregnancies (23.08%) resulted in miscarriages, including 5 early miscarriages and 1 late miscarriage. Among the 20 women who achieved pregnancy without miscarriage, 18 had term deliveries, 2 had preterm birth, and none reported uterine rupture. Histological analysis was performed in all 51 cases. Muscle fibre density was significantly lower in the scar than in the myometrium adjacent to the scar and collagen expression was markedly increased in the scar tissue. Conclusion: Satisfactory gynaecological and subsequent obstetrical outcomes can be achieved in vaginal repair-treated caesarean scar diverticula patients.
Comparison and analysis of comprehensive curative effect of gastric perforation repair by laparoscope and laparotomy operation

Abstract:Objective To explore comparison of comprehensive curative effect of gastric perforation repair by laparoscope and laparotomy operation. MethodsClinical data of 67 patients with acute gastric perforation who were admitted to our hospital were analyzed and they were allocated to the laparotomy group with 27 cases and the laparoscope group 40 cases according to different treatment methods.Results The intraoperative blood loss, leaving bed time, gastrointestinal function recovery time, hospitalization stays and The volume of rinsing of the laparoscope group were all lower than those of the laparotomy group(t=12.37, 7.40, 8.48, 8.32, 35.75,P<0.05). Infection, perforation recurrence and incidence of abdominal abscess of the laparoscope group were lower than those of the laparotomy group(x2=11.75,11.75, 11.75,P<0.05). The difference was statistical significant.Conclusion Gastric perforation repair by laparoscope has fewer trauma and postoperative complications worthy of clinical promotion and application.
