Abstract:Objective To invesigate the relationship of plasma homocysteine levels to fibrinolytic system in acute coronary syndrome patients.Methods A total 180 patients were diagnosed coronary artery disease by coronary artery angiography for evaluation.135 cases of acute coronary syndrome(ACS) patients,45cases of stable angina pectoris(SAP) patients and 30 normal subjects were enrolled to the study.Plasma total homocysteine(tHcy) levels were measured using high performance liquid chromatography with fluorescence detection,meanwhile,tissue plasminogen activator(t-PA) and plasminogen activator inhibitor(PAI-1) were measured.Analysing the connection among those parameters of patients in every group.Results Plasma tHcy,t-PA,PAI-1 and PAI-1/t-PA levels in SAP patients and ACS patients were higher than that in normal subjects(P<0.05).While plasma tHcy PAI-1 and PAI-1/t-PA levels in ACS patients were higher than that in SAP patients((P<0.05)),and t-PA levels in ACS patients were lower than that in SAP patients.Plasma tHcy levels were found significant association with PAI-1 and t-PA(r-value=0.439 and r-value=(-0.398),P<0.001).Conclusion There was hyperhomocysteinemia in SAP and acute coronary syndrome patients.Hyperhomocysteinemia play an important role in fibrinolytic system in patients with acute coronary syndrome.
Novel Distribution Pattern of Fibrinolytic Components in Rabbit Tissues Extract: a Preliminary Study

Abstract:Objective The purpose of this work was to investigate the distribution pattern of fibrinolytic factors and their inhibitors in rabbit tissues. Methods The components of the fibrinolytic system in extracts from a variety of rabbit tissues, including tissue plasminogen activator (tPA), plasminogen activator inhibitor-1 (PAI-1), plasminogen (Plg), plasmin (Pl) and α 2 plasmin inhibitor (α 2 PI), were determined by colorimetric assay. Results The tissue extracts in renal, small intestine, lung, brain and spleen demonstrated strong fibrinolytic function, in which high activity of tPA, Plg and Pl was manifested; whereas in skeletal muscle, tongue and stomach, higher activity of PAI-1 and α 2 PI showed obviously. Also excellent linear correlations were found between levels of tPA and PAI-1, Pl and α 2 PI, Plg and Pl. In related tissues, renal cortex and renal marrow showed distinctly higher activity of tPA and lower activity of PAI-1, with the levels of Plg and Pl in renal cortex being higher than those in renal marrow, where the α 2 PI level was higher than that in renal cortex. Similarly, the levels of tPA, Plg and Pl in small intestine were higher than those in large intestine, but with respect to PAI-1 and α 2 PI, the matter was reverse. In addition, the fibrinolytic activity in muscle tissue was lower, however, the levels of tPA, Plg, and Pl in cardiac muscle were obviously higher than those in skeletal muscles, and the levels of PAI-1 and α 2 PI were significantly lower than those in skeletal muscle. Conclusion Our data demonstrate that a remarkable difference of the fibrinolytic patterns exists in rabbit tissues, which has probable profound significance in understanding the relationship between the function of haemostasis or thrombosis and the physiologic function in tissues.
Importance of Fibrinolytic Activity in Patients with Coronary Artery Diseases

Abstract:Objective To study the correlation of fibrinolytic activity to the area,extent,and stability of coronary artery culprit lesion.Methods In 256 patients undergoing coronary angiography,tissue plasminogen activator(t-PA) plasminogen activator inhibitor-1(PAI-1) were measured with usual procedures.Results ①There was a positive association between PAI-1 and unstable angina (13 64±2 32 vs 6 22±1 78, P 0 01);②A strong relationship between PAI-1 and the area and extent of coronary artery disease was found (11 67±2 12 vs 6 38±1 72 12 69±1 82 vs 8 38±1 82, P 0 01);③There was no relation between t-PA and the severity of coronary artery culprit lesion.Conclusion Elevated PAI-1 may predisposes to the area,extent and destabilization of culprit lesion in coronary artery.

Abstract:Objective To investigate the application of serum levels of interleukin-18(IL-18) and matrix metalloproteinase-9(MMP-9) detection in acute coronary symptoms.Methods The enzyme-linked immunosorbent assay(ELISA) determination of 150 patients with serum IL-18,MMP-9 levels.Results Serum IL-18,MMP-9 levels in 9 patients with acute coronary syndrome was significantly higher in the control group and stable angina pectoris difference between the two groups was statistically significant(P<0.05);Double support and three vascular disease in patients with serum IL-18,MMP-9 levels higher than those of the control group,the difference between groups was statistically significant(P<0.05).Conclusions ACS serum IL-18 and MMP-9 levels increased plaque stability is expected as a predictor of ACS.Serum IL-18,MMP-9 in patients with acute coronary syndrome has important clinical value.
Differential Proteome Profiling of Pleural Effusions from Lung Cancer and Benign Inflammatory Disease Patients

Abstract:The pleural effusion proteome has been found containing information that directly reflects pathophysiological status and represents a potential diagnostic value for pulmonary diseases. However, the variability in protein composition between malignant and benign effusions is not well understood. Herein, we investigated the changes of proteins in pleural effusions from lung adenocarcinoma and benign inflammatory disease (pneumonia and tuberculosis) patients by two-dimensional difference gel electrophoresis (2D-DIGE). Twenty-eight protein spots displayed significantly different expression levels were positively identified by MALDI-TOF-MS representing 16 unique proteins. Five identified protein candidates were further validated and analyzed in effusions, sera or tissues. Among them, hemopexin, fibrinogen gamma and transthyretin (TTR) were up-regulated in cancer samples. The effusion concentration of serum amyloid P component (SAP) was significantly lower in lung cancer patients than in benign inflammatory patients, but no differences were found in sera samples. Moreover, a Jumonji C (JmjC)-domain-containing protein, JMJD5, was observed to be down-regulated in malignant effusions, lung cancer tissues and cancer cells. These results shed light on the altered pleural effusion proteins as a useful and important complement to plasma or other routine clinical tests for pulmonary disease diagnosis.

Abstract:Objective To explore the differential diagnostic value of major fibrinolytic parameters in pleural fluid.Methods Tissue-type plasminogen activator(t-PA) and plasminogen activator inhibitor-1(PAI-1) in pleural fluid at the first thoracentesis were measured with ELISA and D-dimer was measured with immunoturbidimetry.Results Eighty-four patients with pleural effusion were enrolled,among which 40 with malignant effusion,33 with infectious effusion and 11 with transudative effusion.t-PA level was higher in malignant and transudative pleural fluid than that in infectious pleural fluid[(52.49±31.46) ng/mL and(58.12±23.14)ng/mL vs(37.39±22.44)ng/mL,P<0.05],but was not statistically different between malignant pleural fluid and transudative(P>0.05).PAI-1 level was higher in malignant and infectious pleural fluid than that in transudative[(164.86±150.22) ng/mL and(232.42±175.77) ng/mL vs(46.38±16.13)ng/mL,P<0.01],but was not statistically different between malignant and infectious pleural fluid(P>0.05).D-dimer levels in the three types of pleural fluid were significantly different,which was(23.66±25.18) mg/L,(6.36±10.87) mg/L and(66.90±42.17) mg/L in malignant,transudative and infectious pleural fluid,respectively.As single-item detection for malignant pleural fluid,the cutoff of t-PA was >38.7 ng/mL(area under ROC curve was 64.0),with sensitivity of 60.0%,specificity of 63.6%,positive predictive value of 66.7%,negative predictive value of 56.8% and accuracy of 61.6%.The cutoff of D-dimer was <27.0 mg/L(area under ROC curve was 85.5),with sensitivity of 84.8%,specificity of 72.5%,positive predictive value of 85.3%,negative predictive value of 71.8% and accuracy of 78.1%.The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of combined examination(t-PA+D-dimer) were 92.5%,60.6%,74.0%,87.0%,78.1%,respectively.Conclusions The t-PA,PAI-1 and D-dimer levels are significantly different in the three types of pleural fluid.The detection of fibrinolytic parameters in pleural fluid,especially the value of D-dimer,may be helpful in the differential diagnosis of pleural effusion.
Efficacy of intrapleural instillation of fibrinolytics for treating pleural empyema and parapneumonic effusion: a meta-analysis of randomized control trials

Abstract:Introduction: The effects of intrapleural fibrinolysis for treating pleural empyema and parapneumonic effusion remain uncertain. Objectives: We conducted a meta-analysis of published randomized controlled trials (RCTs) to evaluate the efficacy of intrapleural instillation of fibrinolytics for treating pleural empyema and parapneumonic effusion. Methods: Medline, Web of Science, Ovid and regulatory documents up to June 10, 2012 were searched. We selected RCTs on intrapleural fibrinolysis vs placebo control treatment for pleural empyema and parapneumonic effusion. The meta-analysis was used to determine the odds ratios (OR) for death, surgical intervention and severe side effects, and weighted mean differences were used to estimate lengths of hospital stays. Results: Ten trials with a total of 977 patients were included. Compared with a placebo, intrapleural fibrinolytic therapy decreased the OR for surgical intervention [OR = 0.24; 95% confidence interval (CI): 0.10-0.60] and the length of hospital stays (weighted mean difference = -6.47; 95% CI: -8.87, -4.08). Intrapleural fibrinolysis was associated with a non-significant reduction in mortality rate (OR = 1.16; 95% CI: 0.71-1.89) and a non-significant increase in severe side effects (OR = 1.92; 95% CI: 0.87-4.21). Subgroup analyses indicated that urokinase agents had marked positive effects on reducing surgical intervention (OR = 0.33; 95% CI: 0.14-0.78), but neither streptokinase nor tissue plasminogen activator did. Conclusions: The present results show that intrapleural fibrinolysis with urokinase may be potentially effective for reducing the need for surgery. Intrapleural fibrinolytic therapy is effective for shortening the lengths of hospital stays without increasing the incidence of severe side effects.

Abstract:Objective:The antigens as molecular markers of coagulation and fibrinolysis systems were measured in patients with malignant tumors in order to investigate their changes among the patients with acute leukemia(AL),malignant lymphoma(ML) and solid tumors.Methods:By enzyme-linked immunosorben assay(ELISA),4 plasma molecular markers of the coagulation and fibrinolysis systems were measured in 30 healthy volunteers and 47 cases of malignant tumors that included 27 patients with AL,in which 21 newly-diagnosed AL,6 relapsed cases,10 patients with ML and 10 patients with solid tumor.Their blood samples were taken from their veins at fasting in the morning,with anticoagulant of 3.8% sodium citrate.These molecular markers of coagulation and fibrinolytic system were as followings: tissue factor(TF),Thrombin——antithrombin complexes(TAT),tissue-type plasminogen activator(t-PA),plasminogen activator inhibitor-1(PAI-1).Results:1.The levels of TF in 47 cases of malignant tumors were all significantly higher than that in control group,TF level in ML group was significantly higher than that in AL group;2.TAT levels in the group of AL,ML group and solid tumor group were all significantly higher than that in control group respectively;3.The levels of t-PA in 3 groups of malignant tumors were all significantly higher than that in control group,the solid tumor group was significantly higher than AL group.4.The PAI-1 levels in AL group or in solid tumor group were significantly higher than that in control group.Conclusion:In patients with acute leukemia,lymphoma and solid tumor,there are the similar coagulation and fibrinolysis aberrancy that are predisposed to blood coagulation and inhibition of fibrinolysis.

Abstract:Objective To study the fibrinolytic abnormalities in acute leukemia (AL). Methods A series of fibrinolytic parameters were measured among 93 cases using ELISA or chromogenic substrate assay. Results Plasma tissue plasminogen activator activity and D dimer (D D) were significantly elevated at diagnosis, while the plasminogen (PLG), α 2 antiplasmin (α 2 PI) and plasminogen activator inhibitor (PAI) activity decreased. Conclusion There existed the activation of fibrionolytic system in AL, some of which were related to AL types, bleeding symptom and prognosis.

Abstract:Objective To explore the influence of acute lung injury/acute respiratory distress syndrome(ALI/ARDS) on coagulation and fibronolysis system by examining the plasma levels of protein C(PC),thrombomodulin(TM) and plasminogen activator inhibitor-1(PAI-1). Methods The study was divided into two groups: ALI/ARDS patients(n=24) and normal volunteers(n=12).The plasma levels of PC antigen,TM antigen and PAI-1 antigen were determined by enzyme-linked immunosorbent assay(ELISA).The plasma level of active PC(APC) was determined by chromogenic substrate method.Furthermore,ALI/ARDS patients were estimated with APACHE Ⅱ score and multiple organ dysfunction syndrome(MODS) score.ALI/ARDS patients were studied by dividing patients into subsets based on the numbers of organ dysfunction and prognosis. Results The plasma levels of APC and PC antigen were significantly lower in patients with ALI/ARDS than those of the normal controls(P0.05);the levels of TM antigen and PAI-1 antigen were significantly higher in ALI/ARDS patients than those in the normal controls(P0.01).Furthermore,there was a significant negative correlation between the plasma level of APC in patients and APACHE Ⅱ score and MODS score(P0.01);a positive correlation was found between the level of PAI-1 antigen and APACHE Ⅱ score(P0.01) or MODS score(P0.05).However,there was no correlation between the levels of PC antigen,TM antigen and APACHE Ⅱ score or MODS score.The plasma level of APC in patients with single lung dysfunction was significantly higher than that in patients with MODS(P0.01);however,there was no difference in the levels of PC antigen,TM antigen and PAI-1 antigen between patients with single lung dysfunction and patients with MODS.The plasma levels of APC,PC antigen,TM antigen and PAI-1 antigen in patients who survived did not differ from those in patients who died. Conclusions The results suggest that there is an increased procoagulant activity and reduced fibrinolytic activity in patients with ALI/ARDS.Furthermore,there is a correlation between the extent of change in the coagulation and fibronolysis system and the seriousness of ALI/ARDS.
The significance of fibrin in synovial fluid for patients with internal derangement of the temporomandibular joint

Abstract:PURPOSE: To analyze the synovial fluid,and explore the significance of fibrin during the formation of intra-articular adhesion(IA).METHODS:Arthroscopic disc repositioning and suturing was performed on patients with temporomandibular joint(TMJ) internal derangement(ID) ranging from stage Ⅱ to Ⅴ who visited the TMJ clinic at the Ninth People's Hospital,Shanghai JiaoTong University School of Medicine,between Dec.27,2007 and Jun.16,2008.The patients' ID stages were classified based on the criteria of Wilkes & Bronstein's classification.125 samples of synovial fluids were collected before arthroscopic surgery and stored in cold environment.According to the fluorescence intensity of each sample,and the immunofluorescence confocal laser microscopic semi-quantitative analysis was used to determinate the content of fibrin.Student's t test was performed using SPSS16.0 software package,to compare the difference of the average fluorescence intensity of joint fluid in different groups(ID stage,different age,different sex and different joints).RESULTS:There was statistically significant difference in the synovial fluid immune fluorescence intensity in different sex and age group.The average synovial fluid immune fluorescence intensity of all 125 samples was 4.43±3.67(0-31.68).The average immune fluorescence intensity of each stage was stage Ⅱ: 2.38±1.17,stage Ⅲ: 4.68±3.98,stage Ⅳ: 4.80±2.80 and stage Ⅴ: 3.58±2.64.There was significant difference between the results of stage Ⅱ and stage Ⅲ(P=0.0065<0.05),and there was significant difference between the results of stage Ⅱ and stage Ⅳ(P=0.0207<0.05).The average immune fluorescence intensity of the male group was 4.05±2.70 and the female group was 5.57±4.71.There was significant difference between the results of male group and female group.The average immune fluorescence intensity of the synovial fluid of left TMJ was 4.37±3.04 and the right TMJ was 5.97±5.20.There was significant difference between the results of the left and right TMJ synovial fluids.The average immune fluorescence intensity of group 1 was 4.55±2.45,group 2 was 4.56±3.39,group 3 was 2.60±1.12 and group 4 was 5.68±6.71.There was significant difference between group 1 and group 3,group 2 and group 3,group 3 and group 4.CONCLUSION: The average intensity of TMJ increased with ID stage,which confirmed that fibrin in synovial fluid is one of the important protein factors during the formation of IA,and verified our hypothesis about the formation of IA.

Abstract:Objectives To investigate the dynamical changes of coagulation and fibrinolysis in surgical critical patients.Methods Coagulation and fibrinolysis changes were monitored for one week after patients enrolled and results were compared according to presence and absence of complications.Results All patients' AT Ⅲ and Plg were decreased whereas their GMP 140 and vWF were increased on admission.The t PA of patients with complications was lower whereas their PAI was higher,and the increases of their GMP 140 and vWF were more significant than those of patients without complications. During the study periods,AT Ⅲ and vWF of patients without complications gradually recovered but patients with complications had persistently abnormal levels.t PA and PAI in patients with complications remained abnormal but unchaged in patients without complications.No changes in PLT,Fg and α 2 PI were found in any patients.Conclusion Critical patients had activation of coagulation and damages of platelet or endothelium.Patients with severe complications also had fibrinolysis inhibition and more severe platelet and endothelium damage,which resuled in an imbalance between coagulation and fibrinolysis.But patients without complications had no fibrinolysis inhibition and their endothelial damages recovered gradually. Neutrophil-mediated Inflammatory Plasminogen Degradation, Rather Than High Plasminogen-Activator Inhibitor-1, May Underly Failures and Inefficiencies of Intrapleural Fibrinolysis

Abstract:Background: Complex pleural space infections often require treatment with multiple doses of intrapleural tissue plasminogen activator (tPA) and deoxyribonuclease, with treatment failure frequently necessitating surgery. Pleural infections are rich in neutrophils, and neutrophil elastase degrades plasminogen, the target substrate of tPA, that is required to generate fibrinolysis. We hypothesized that pleural fluid from patients with pleural space infection would show high elastase activity, evidence of inflammatory plasminogen degradation, and low fibrinolytic potential in response to tPA that could be rescued with plasminogen supplementation. Research question: Does neutrophil elastase degradation of plasminogen contribute to intrapleural fibrinolytic failure? Study design and methods: We obtained infected pleural fluid and circulating plasma from hospitalized adults (n = 10) with institutional review board approval from a randomized trial evaluating intrapleural fibrinolytics vs surgery for initial management of pleural space infection. Samples were collected before the intervention and on days 1, 2, and 3 after the intervention. Activity assays, enzyme-linked immunosorbent assays, and Western blot analysis were performed, and turbidimetric measurements of fibrinolysis were obtained from pleural fluid with and without exogenous plasminogen supplementation. Results are reported as median (interquartile range) or number (percentage) as appropriate, with an α value of .05. Results: Pleural fluid elastase activity was more than fourfold higher (P = .02) and plasminogen antigen levels were more than threefold lower (P = .04) than their corresponding plasma values. Pleural fluid Western blot analysis demonstrated abundant plasminogen degradation fragments consistent with elastase degradation patterns. We found that plasminogen activator inhibitor 1 (PAI-1), the native tPA inhibitor, showed high antigen levels before the intervention, but the overwhelming majority of this PAI-1 (82%) was not active (P = .003), and all PAI-1 activity was lost by day 2 after the intervention in patients receiving intrapleural tPA and deoxyribonuclease. Finally, using turbidity clot lysis assays, we found that the pleural fluid of 9 of 10 patients was unable to generate a significant fibrinolytic response when challenged with tPA and that plasminogen supplementation rescued fibrinolysis in all patients. Interpretation: Our findings suggest that inflammatory plasminogen deficiency, not high PAI-1 activity, is a significant contributor to intrapleural fibrinolytic failure. Clinical Significance of Changes of D-D, T-Pa and PAI in Acute Renal Failure Patients

Abstract:AIM: To investigate the clinical significance and the changes of coagulation and fibrinolysis function in acute renal failure patients. METHODS: The level of plasma D-D and the activity of tissue-type plasminogen activator (t-PA) and plasminogen activator inhibitor (PAI) were determined in 37 acute renal failure (ARF) patients while those of the healthy people were used as control. RESULTS: ① The plasma D-D level and the PAI activity in ARF patients were obviously higher and the t-PA activity was lower than those in control group(P0.05). ②Significant difference of the above changes was also found in post-renal failure groups, pre-renal and renal failure groups and between simple ARF patients and renal failure patients combined with multiple organ dysfunction syndrome (MODS)(P(0.05)). CONCLUSION: There was a disorder of coagulation and fibrinolysis system in ARF patients. ARF, caused by ischemic and toxicosis or combined with MODS, has the greatest effect on the changes on coagulation system. Plasma D-D, t-PA and PAI can be used clinically as effective and sensitive markers reflecting the abnormal changes of coagulation and fibrinolysis system.

Abstract:Objective To investigate the expression of fibronectin (Fn) in patients with acute myocar-dial infarction (AMI) and its clinical significance. Methods A total of 12 patients with AMI, and 13 healthy controls. The expression of Fn on peripheral blood platelet membrane were analyzed with flow cytometry. Soluble Fn in plasma was assayed by enzyme-linked immunoadsorbent assay (ELISA) . Results The expression of Fn on peripheral blood platelet membrance was 67. 88±19. 55% and the plasma Fn was 515. 17±83. 85 mg/ L in patients with AMI. While those in controls were 14. 35±5. 31% and 361. 54±68. 48 mg/L respectively, indicating a higher expression of Fn on peripheral blood platelet membrance and plasma Fn level in patients with AMI than in control group (P 0. 01). Conclusion It suggested that Fn might contribute to artery thrombosis in patients with AMI.
Determination and Clinical Significance of Plasma Fibrinogen, Lipoprotein (a) and Platelet Parameters in Coronary Heart Disease

Abstract:Objective:To investigate the correlativity among plasma fibrinogen(Fg) ,platelet count (PLT) ,mean platelet volume (MPV), Lipoprotein (LP) a and coronary heart disease (CHD). Methods: Fg, PLT, MPV and LP (a) were detected and compared in CHD patients (n = 64) and in controls (n = 36), then their correlativity with CHD were also analyzed. Results: Fg. MPV, LP (a) in CHD group were higher than those in control group (P0. 05), there was no significant difference of PLT between two groups. Multivariant gradual regressive analysis shows that Fg, MPV, LP (a) were significantly associated with CHD (P0. 01). Conclusion: Coronary heart disease is a multifactorial disease. Tt possesses important reference value that combine examine Fg, platelet index and LP (a) to evaluate the CHD risk.

Abstract:Immunothrombosis has emerged as a potential mechanistic link underlying the development and progression of acute respiratory distress syndrome (ARDS), but understanding its specific profile in patients, both locally and systemically, is limited. The objective of this study was to characterize and compare the immunothrombotic signatures in patients diagnosed with pneumonia-related ARDS (p-ARDS) at both the pulmonary and systemic levels and to evaluate their clinical relevance. The study included 23 consecutive patients diagnosed with p-ARDS admitted to the intensive care unit at a tertiary university hospital from July 2022 to May 2023, alongside 40 concurrently hospitalized patients with common pneumonia as controls. Paired bronchoalveolar lavage fluid (BALF) and serum samples were collected from the participants for the analysis of 15 biomarkers to assess and quantify the pulmonary and systemic immunothrombotic signatures. The study results revealed significant pulmonary inflammation and systemic endothelial injury in p-ARDS patients compared to pneumonia controls. These observations were maintained after adjustment for severity of illness (Acute Physiology and Chronic Health Evaluation II scores). In terms of clinical relevance, inflammatory biomarkers (interleukin [IL]-6, IL-8) in BALF were found to correlate with PaO2/FiO2 ratio, while serum levels of a disintegrin and metalloproteinase with thrombospondin type 1 motif 13 (ADAMTS-13) and thrombomodulin showed associations with Sequential Organ Failure Assessment and Disseminated Intravascular Coagulation scores. In conclusion, this preliminary investigation identified compartment-specific variations in the immunothrombotic signature between patients with p-ARDS and those with pneumonia alone, with inflammatory responses predominantly localized in the alveolar compartments and coagulation/endothelial injury biomarkers more pronounced in peripheral blood.

Abstract:Objective To investigate fibrinolytic activity and clinical significance in patients with essential hypertension.Methods The activities of tissue type plasminogen activitor(t PA),plasminogen activitor inhibitor 1(PAI 1),plasminogen(Plg) were measured by remant showing color assay in the serums of 42 patients with essential hypertension and normal persons.Results The hypertensive patients had significant higher activity of PAI 1 and Plg, whereas t PA activity was significantly lower.Conclusion These findings indicate that there is a endogenic fibrinolytic deficiency in essential hypertension.
