Abstract:INTRODUCTIONOncologic resection of the head and neck malignant tumor often results in complex reconstructive problems that require a reliable flap to restore both the form and function in single-stage operation. The free flap transfer has revolutionized head and neck reconstruction. However, forehead flap pedicled with superficial temporal artery (STA) or supraorbital artery (SOA)/supratrochlear artery (STrA) is a new option for reconstruction of head and neck with its own specific advantages.METHODThe forehead flap was applied to reconstruct the defects of the posterior hypopharyngeal wall, external perioral skin and buccal mucosal, and nose and periorbital tissues after malignant tumor resection. Nine of the patients were reconstructed with forehead flap pedicled with STA, 7 of whom underwent postoperative radiation. Eight of the patients were pedicled with SOA (and/or STrA), 1 of whom underwent postoperative radiation.RESULTSWe applied this method in a total of 17 male patients. The mean age of patients was 61 years (range, 47-69 years) with a follow-up time of 12 to 180 months. The biggest defect covered by forehead flap pedicled with STA was measured 10.0 × 6.5 cm. The biggest defect covered by forehead flap pedicled with SOA and/or STrA was measured 9.0 × 7.0 cm. In all 17 patients, no vascular crisis happened and no partial or total flap necrosis was observed. All skin graft area and donor site healed well. No patient experienced flap strictures or fistulas after operation or after radiotherapy. No local recurrence developed during the follow-up time. Functions were assessed by specialist as optimal as no limitation of eyes or mouth opening, and no limitation of food swallowing.CONCLUSIONSThe forehead flap provides thin pliable tissue with reliable blood supply and high tolerance to radiotherapy. These advantages make it a valuable option to simultaneously reconstruct the defects after head and neck malignant tumor resection in certain selected patients.
[Free Tissue Transfer for Skull Base Reconstruction of Advanced Oncological Defects].

Abstract:OBJECTIVE:To analyze the results and the indications of free flaps in the skull base reconstruction.METHODS:A retrospective review was performed of 25 free flaps reconstructions for skull base tumor ablation defects between March of 1999 and March of 2009. There were 9 latissimus dorsi flaps, 6 rectus abdominis flaps, 4 deep inferior epigastric artery perforator flaps, 3 anterolateral thigh flaps, 2 radial forearm flaps and 1 fibular flap. The defects were classified as cranial bone (100%), dura (40%), brain (12%), sino-nasal cavity (52%), mucosa (56%) and skin (80%).RESULTS:Twenty-three of 25(92%) free flaps survived. One total flap failure and 1 partial flap failure occurred. Five patients (20%) had central nervous system related postoperative complications which including 1 mortality, 2 central nerve system infection and 2 cerebrospinal fluid fistula.CONCLUSIONS:Free flaps transfer is a reliable reconstruction technique for cranial base surgery. This provides a solution to the select advances disease otherwise surgical contraindications. Free flap is the choice for reconstruction of advanced oncological defects, especially when skin and mucosa are violated.
Free perforator flap transfer for reconstruction of skull base defects after resection of advanced recurrent tumor.

Abstract:BackgroundSkull base reconstruction is challenging due to its proximity to important anatomical structures. This report evaluates the use of perforator flaps for reconstruction of skull base defects after advanced recurrent tumor resection. MethodsFourteen free perforator flaps were transferred to reconstruct skull base defects in 14 consecutive patients, from October 2004 to May 2011. All patients had advanced recurrent neoplasms that were previously treated with either radiation therapy or surgery. The surgical defects were reconstructed using various perforator flaps mainly the deep inferior epigastric artery perforator flaps, anterolateral thigh (ALT) flaps, or thoracodorsal artery perforator flaps. The outcomes following reconstruction and associated complications were evaluated. ResultsThe overall free flap success rate was 93% (13/14). One ALT flap was lost. Three patients (20%) had a cerebrospinal fluid fistula, and two of them developed meningitis. No complications were observed at the donor site. ConclusionsThe use of perforator flaps may be a viable option for reconstruction of skull base defects after the resection of advanced recurrent tumor. (c) 2014 Wiley Periodicals, Inc. Abstract:Free tissue transfer is a great method for the reconstruction of craniofacial defects, especially in complicated situations such as calvarial defects and dura exposure. Latissimus dorsi muscle flap, anterolateral thigh flap, and radial forearm flap are 3 commonly used flaps that have distinct characteristics. The objective of the present study is to share our experiences with microvascular free flap reconstruction of cranium exposure or defects and to compare distinct microvascular free flap options. The authors reviewed 8 cases that received resection and reconstruction of craniofacial tumors. Perioperative and reconstructive techniques employed to improve flap survival, esthetic outcome, and prevent complications in these patients were retrospectively detailed. In addition, the advantages and disadvantages of latissimus dorsi muscle flap, anterolateral thigh flap, and radial forearm flap were illustrated in this study. The authors believe that taking advantage of these flaps according to the characteristics of defects and flaps will contribute to a good functional and esthetic outcome.

Abstract:Background: Reconstruction of head and neck soft tissue defects with bone exposure is both challenging and technically demanding for plastic surgeon. Objectives in head and neck soft tissue defects with bone exposure reconstruction are consistent restoration of functionality while also improving appearance. This study retrospectively analyzed the results of head and neck reconstructions using various types of free flaps over the past 4 years. Methods: A retrospective analysis was conducted from June 2019 to June 2023 on 12 patients treated at our hospital for head and neck soft tissue defects with bone exposure due to various causes. These included 4 cases of trauma from car accidents, 1 burn case, and 7 postoperative malignant tumor removals. The defect sizes ranged from 4 × 6 to 15 × 45 cm. Different free flaps were used for repair based on the defect, including 6 anterolateral thigh flaps, 3 forearm flaps, 2 latissimus dorsi flaps, and 1 dorsalis pedis flap. Flaps were designed and harvested to match the defect size and transplanted via anastomosed vessels. Results: All 12 flaps survived successfully. One patient required flap thinning surgery postoperatively. All patients were followed up for over 3 months, showing good color and texture of the transplanted flaps, satisfactory healing, and significant aesthetic improvement. Donor sites showed significant scarring without functional impairment. Conclusion: Free flap repair for head and neck soft tissue defects with bone exposure is feasible and yields good results.

Abstract:Objective:To study the surgical skills of reconstruction of the skull base after resection of skull base tumors.Methods:Thirty-one cases of skull base reconstruction were studied.Galeo-pericranial flaps were used for anterior cranial base defects and tempero muscle flaps for the middle skull base respectively.Results:All the 31 patients with skull base tumors,underwent reconstruction of the skull base of whom,30 experienced no complications,but 1 died of CSF leak.Conclusion:Galeo-pericranial flaps and tempero muscle flaps are reliable materials for the reconstruction of the skull base.

Abstract:Objective: Revision endoscopic endonasal skull base surgery for failure of skull base reconstruction is most often successful using a variety of surgical techniques, from the simplest flap repositioning, grafting reinforcement and lumbar drain placement to the more advanced application of alternative vascularized flaps, such as the nasal floor flap, inferior turbinate flap, or contralateral nasoseptal flap. Although uncommon, failure of the skull base reconstruction in spite of all previous measures may result in an extreme situation with potentially devastating outcome. At this point, the temporoparietal fascia (TPF) flap is an alternative in the absence of the nasoseptal flap, but its detailed anatomic analysis and technical nuances remain rarely reported. We aimed to investigate the anatomy and surgical application of TPF flap transposition in endoscopic endonasal surgery.

Abstract:INTRODUCTION: Reconstruction with a free flap is routine in head and neck surgery because of better functional outcomes, improved esthetics, and generally higher success rates.OBJECTIVE: To evaluate the clinical outcomes in patients undergoing different microvascular free flap reconstructions.METHODS: This was a retrospective study of 93 patients undergoing reconstructions with free flaps from 2007 to 2015. Four types of free flap were performed: anterolateral thigh (76.3%), radial forearm (16.1%), fibula (4.3%) and jejunum (3.3%). Patients' demographic data were collected, and the outcomes measured included flap survival and complications. Postoperative functional and oncological outcome were also analyzed.RESULTS: The patients included 73 men and 20 women, with a mean age of 56.1 years. The most common tumor location was the tongue. Squamous cell carcinoma represented the vast majority of the diagnosed tumors (89.2%). The most common recipient vessels were the superior thyroid artery (77.4%) and the internal jugular vein (91.4%). Nine patients required emergency surgical re-exploration and the overall flap success rate was 90.3%. Venous thrombosis was the most common cause for re-exploration. Other complications included wound infection (5.4%), wound dehiscence (1.1%), partial flap necrosis (9.7%), fistula formation (10.8%), and 1 bleeding (1.1%). The majority of patients had satisfactory cosmetic and functional results of both donor site and recipient site after 46.7 months of mean follow-up.CONCLUSION: Microsurgical free flap is shown to be a valuable and reliable method in head and neck surgery. It can be used safely and effectively with minimal morbidity in selected patients. The reconstruction can be performed by appropriately skilled surgeons with acceptable outcomes. Success rate appears to increase as clinical experience is gained.

Digitally reconstructed severe trauma-induced oro-maxillofacial defects with free vascularised composite tissue flaps

Abstract:The aim of this study was to outline the characteristics of severe trauma-induced oro-maxillofacial defects and explore the application of microsurgical reconstruction of these defects by selecting appropriate free vascularised composite tissue flaps. A retrospective analysis was performed in 11 cases with severe trauma-induced oromaxillofacial defects from 2005 to 2018, All cases were reconstructed with free vascularised composite flaps using a supermicro-surgical/micro-surgical and digital technology. This study consisted of 11 patients (8 men, 3 women) with a median age (range) of 28 (6-64) years. Patients with long-standing trauma experienced severe scars and displacements. The trauma-induced oro-maxillofacial defects ranged in size from 4.0 × 5.0 cm2 to 7 × 15 cm2. Six radial forearm flaps, three fibular osteocutaneous flaps, a lateral thoracic flap, and an anterolateral thigh flap were harvested individually. All patients were treated effectively and followed up for over two years, with a 100% survival rate of these free vascularised composite tissue flaps. No uncontrollable complications occurred, and all patients were satisfied with the postoperative shape and function. The trauma-induced oro-maxillofacial defects display unique features, and microsurgical techniques play an important role in the reconstruction of shapes and function. In addition, some patients may need multi-stage surgeries to attain aesthetic refinement.

Abstract:Background The reconstruction of large anterior skull base defect is one of the major challenges after endoscopic endonasal resection of anterior skull base. The nasoseptal and pericranial flaps have been used to reconstruct the anterior skull base defect, which has significantly decreased the incidence of postoperative cerebrospinal fluid leakage. Abstract:Abstract: Reconstruction of complete bilateral maxillary defects (CBMDs) can be challenging due to the extensive loss of bone and soft tissues. This is a retrospective case series of 46 consecutive patients with CBMDs that were reconstructed with different micro-vascular free flaps. The authors aimed to evaluate the surgical outcomes and discuss the different reconstruction options in this case series. Thirty-six patients underwent reconstruction following ablation surgery for malignant tumors, 6 for benign tumors, 3 patients were treated for osteomyelitis, and 1 patient underwent free flap reconstruction for posttraumatic defects. Free fibula flap (n = 26) is the most commonly used reconstruction method in this case series, which was used in all defect types. This is followed by anterolateral thigh flap (n = 10), 5 rectus abdominis myocutaneous free flap, 3 radial forearm free flaps, and 2 composite free flaps. In this series, 44 free flaps survived, whereas only 2 flaps were lost. All patients could resume a soft diet postoperatively. Reconstruction of CBMDs with vascularized free flaps is a safe and reliable procedure.

Abstract:OBJECTIVE:To choose suitable free flaps for reconstructing head and neck defects caused by tumor resection.METHODS:A retrospective analyses was made in 86 cases of head and neck defects treated with four kinds of free flaps between January 1999 and January 2002. The head and neck defects were caused by tumor resection. The locations were oral cavity (n=32), hypopharynx (n=27), mandible (n=12), skull base (n=5), scalp and skin (n=6) and midface (n=4). The donor sites of free flaps included the rectus abdominis (n=32), anterolateral thigh (n=10), jejunum (n=25), fibula (n=11), latissimus dorsi (n=4), forearm (n=3) and scapula (n=1). The sizes of the cutaneous/musculocutaneous flaps ranged from 4 cm x 5 cm to 14 cm x 24 cm. The lengths of the fibula were 4-16 cm, of jejunum 9-20 cm.RESULTS:The overall free flap success rate was 92% (79/86). Of 32 oral cavity defects, 22 were reconstructed by rectus abdominis (69%) and 10 by anterolateral thigh flaps (31%). Of 27 hypopharyngeal defects, 25 were restored by jejunum flaps (93%). Eleven of 12 mandibular defects were reconstructed by fibula flaps (92%). Four of 5 defects of skull base were reconstructed by rectus abdominis flaps (80%). The free flaps of rectus abdominis, anterolateral thigh, jejunum and fibula were most frequently used, accounting for 91% (78/86) of all flaps in head and neck defect reconstruction.CONCLUSION:Although head and neck defects represent a complicated spectrum of subsites and loss, these four free flaps can manage most reconstruction problems.
[A Preliminary Study on Free Medial Sural Artery Perforator Flap for Head and Neck Reconstruction Following Tumor Ablation].

Abstract:OBJECTIVE To investigate the application of free medial sural artery perforator flap to reconstruct the defect following head and neck tumor ablation. METHODS From April 2010 to January 2011, the defects after head and neck tumor resection were reconstructed with free medial sural artery perforator flaps in 16 patients. The clinical data were collected and analyzed, including site and histology of tumor, the skin paddle's size of the flap, the length of pedicle, the number of major perforators and donor-site morbidity. RESULTS Medial gastrocnemius territories were evaluated before operation and free medial sural artery perforator flaps were used to reconstruct the defect after tumor ablation in 16 cases. One flap partially survived due to venous thrombosis. All of the other 15 flaps succeeded. The donor site was primarily closed in 15 cases and skin graft was applied in one case. All donor sites except one case healed in first intention. Long-term follow-up showed satisfying receipt-site reconstruction results with good functional and cosmetic outcome in the donor site. There was only minor sensory deficit in the donor site. CONCLUSIONS The medial sural artery perforator flap is reliable for reconstruction of the small to medium-sized defect in head and neck region with less donor site morbidity.
[Management of Vascular Crisis of Free Flaps after Reconstruction of Head and Neck Defects Caused by Tumor Resection].

Abstract:OBJECTIVE:To discuss the management of vascular crisis of free flaps after reconstruction of head and neck defects caused by tumor resection.METHODS:A total of 259 cases of free flap reconstruction performed in the Cancer Hospital of Chinese Academy of Medical Sciences from 2010 to 2013 were retrospectively analyzed, including 89 cases of anterolateral thigh flaps, 48 cases of radial forearm flaps, 46 free fibula flaps, 5 cases of inferior epigastric artery perforator flaps, 5 cases of free latissimus dorsi flaps, one case of lateral arm flap, and one case of medial femoral flap. The surveillance frequency of free flaps was q1h on post-operative day (POD) 1, q2h on POD 2 and 3, and q4h after POD 3. Vascular crises were reviewed for analysis.RESULTS:The incidence rate of vascular crisis was 8.1% (21/259), with 15 males and 6 females. The average age was 54.8 years old (17-68), and the average time of vascular crisis was 100.8 h post-operation (3-432). There were 7 cases of free jejunum flaps and 14 dermal free flaps. Seven of these 21 cases with vascular crisis were rescued by surgery. The success rate of salvage surgery within 72 hours from the primary operation was 54.5% (6/11), significantly higher than that of salvage surgery performed later than 72 hours from primary operation (10.0%, 1/10, P=0.043). There were 14 cases of flap necrosis, two of which died of local infection.CONCLUSION:Early detection of vascular crisis can effectively improve the success rate of salvage, so as to avoid the serious consequences caused by free flap necrosis.

Abstract:Objective To evaluate the application of perforator flaps to the skull base reconstruction of advanced oncological defects.Methods Clinical data of 14 patients received reconstruction of skull base tumor ablation defects with perforator flaps from Oct.2004 to May 2011 was analyzed retrospectively.There were 14 flaps including 8 deep inferior epigastric artery perforator(DIEA) flaps,5 anterolateral thigh(ALT) flaps and 1 thoracodorsal artery perforator(TAP) flap.All patients had recurrent advanced neoplasms with previous radiation therapy or surgery history.Results The overall success rate was 93 %(13/14).One ALT flap got necrotic.Three patients(20 %) had cerebrospinal fluid fistula and 2 of them developed infection of central nervous system.No complications such as wound dehiscence,weakness and hernia were observed in the donor sites.Conclusions Perforator flaps are new and reliable for reconstruction of advanced oncological skull base defects.With advantages of minimal donor site morbidity,the perforator flap may be one of the preferred choices.
Reconstruction of Maxillofacial Defect by Means of Vascularized Fibular Free Flaps and the Implants Immediately Placed into the Fibula

Abstract:Objective: To summarize clinical experiences of reconstruction of maxillofacial defect using vascularized fibular free flaps with implants immediately placed.Methods: Data of eleven patients,aged 33 years to 67 years,with maxillofacial defect undergoing reconstruction using vascularized fibular free flaps were analyzed.Of those 11patients,8 was squamous cell carcinoma(4 left mandible gum,2 right mandible gum and 2 mouth floor),1 was adenoid cystic carcinoma of right sublingual gland,1 was mucus epidermoid cancer of left mandible gum and 1 was adamantoblastoma.Results: Microvascular fibula transfers were completely successful,and all skin paddles survived without necrosis.In our cases,the paddles were harvested corresponding to the size of the mandibular defect.Prosthodontic treatment was completed in 2 patients with 6 implants implanted.In our cases,the oral function after reconstructive surgeries with the fibula osteoseptocutaneous flap was acceptable.Furthermore,the patients could walk without difficulty in 3 to 6 months.Conclusion: The vascularized free fibula flap has become one the most popular reconstruction methods after mandibular resection because of adequate bone graft length,easy contour achieving and antiinfection due to its reliable vascularization.

Abstract:Objective To explore the application of vascularized double-barrel fibula flap by microsurgery in the appearance and functional reconstruction of tumor related mandibular defects.Methods Thirteen patients with mandibular defects caused by mandibular tumor expanded excision were enrolled and treated with vascularized double-barrel fibula flap microsurgery,to improve the appearance and chewing function of the mandibular.Results All 13 patients received the surgery successfully.The fibular flaps grew well after transplantation.Patients were satisfied with their facial appearance,articulation capability,as well as the chewing and occlusion function.No surgery complication was observed.Follow-up examination at 6 months after surgery showed good bone union between fibula segments and mandible.Dental implants were placed with great stability.No recurrence was found during the follow up.Conclusion Vascularized double-barrel fibular flap restores mandible morphology,alleviates surgery wound,reduces the failure rate of free graft,facilitates subsequent bone graft,and effectively improves the appearance and chewing function of the patients.

Abstract:Objective: To explore the method and plausibility of using free rectus abdominis flap (FRAF) to reconstruct the major and complex defects in the craniofacial regions after ablation of advanced sinonasal carcinoma. Methods: From 2007 to 2018 at No. 980 Hospital of Joint Logistic Supportive Force of People's Liberation Army of China, 13 patients with advanced carcinoma of nasal cavity and paranasal sinuses, including 11 males and 2 females, aged from 33 to 67 years, were treated with FRAF to repair the invasion of skull base, face and orbit. Based on adequate and meticulous preoperative evaluations on patients and tumors, complete resection of tumor bulks was performed. According to the sites, characters and extents of the defects, FRAF was introduced in different ways into reconstruction of major and complex defects in the craniofacial regions after resection of advanced sinonasal carcinomas, restoring the structure and contour of the craniofacial region. Results: Complete resection of tumors was achieved in all cases. For repairing the major and complex defects resulted from tumor ablation, FRAF graft was conducted in 13 patients with advanced sinonasal carcinomas. The recipient vessels were facial artery and vein in 9 cases, superficial temporal artery and vein in 4 cases. Vascular bridging with the external jugular vein was carried out in 2 cases who underwent submandibular neck dissections, in which facial artery and vein were used as recipient vessels. Free fibular flap was used along with FRAF in a case undergone bilateral maxillectomy. Pedicled galea capitis and periosteal flap were applied simultaneously in 3 cases with dura defects at the skullbase resulted from tumor resection. All flaps used in 13 cases survived uneventfully without major complications, with successful repair of defects, reconstruction of structures and restoration of contour of craniofacial regions. Conclusion: With adequate evaluation of resectability of tumors as well as nature and extent of defects after tumor removal, FRAF transplantation is an ideal method and strategy of choice for reconstructing the major and complex defects in craniofacial regions resulted from tumor ablation of advanced sinonasal carcinomas.
