Abstract:OBJECTIVE:To evaluate the application, indications and outcomes of free flaps for ora-maxillofacial traumatic defects reconstruction.METHODS:Twenty consecutive cases of ora-maxillofacial the traumatic defects reconstruction with free flaps were reviewed. All clinical data including causes of injuries, the type of defects, selection of free flaps, perioperative complications and the follow-up were analyzed.RESULTS:All the cases underwent free flap reconstruction for ora-maxillofacial traumatic defects: 8 cases with soft tissue defects, 12 cases with soft and hard tissue defects. Fifteen patients received two-stage operation and 5 patients underwent primary reconstruction at the time of debridement or fracture reduction. Twenty free flaps were applied for the reconstruction, 11 cases with fibula flap, 1 case with iliac crest free flap, 7 cases with radial forearm flap and 1 case with scapula flap. No flap failure occurred. The successful rate of free flaps transfer was 100%.CONCLUSIONS:The free flaps transfer is reliable and can reconstruct the ora-maxillofacial traumatic soft and hard tissue defects. Fibula and radial forearm free flap are the most common used flaps. Early aggressive surgery with free flaps transfer for traumatic defects can prevent the scar contracture and tissue displace, which can shorten the treatment period and improve the final outcome.

Abstract:Objective To evaluate the use and indications of forearm flap,rectus abdominus myocutaneous flap and fibula osteocutaneous flap used for reconstructing the maxillary defects.Methods Sixty-five consecutive cases of maxil-lary reconstruction with free flaps were reviewed.All the clinical data including primary diseases,the type of maxillary de-fect,the select of free flap,perioperative complications and the follow-up were analyzed.Results All the cases under-went the three free flaps reconstruction of the maxillary defects.50cases with malignant tumors,10cases with benign tu-mors,2cases with radioosteonecrosis,2cases with trauma and1case with inflammation.Only one flap failure was noted.The successful rate was98.5%.All patients had oral diet with good separation of the oral and sinonasal cavities,and had no difficulty in speech intelligibility.The age and the type of maxillary defects in different free flap groups were analyzed statistically,the results indicated there were significant difference in the three free flap groups(P0.05).Conclusions The three free flaps can be used to reconstruct maxillary defects,which can improve patient's outlook and function and contribute to the overall quality of life.The forearm flap is suitable for the old patients with small maxillary defect.The fibula osteocutaneous flap is an ideal choice for ClassⅠandⅡmaxillary defect reconstruction.The rectus abdominus myocutaneous flap is fitted for large maxillary defects.

Abstract:Objective To analyse the rationale and feasibility of maxillary reconstruction using free flaps.Methods Twenty consecutive cases of maxillary reconstruction using free tissue transfers were reviewed.Data concerning the operation included date of surgery,defect description and site,stage and histology of tumor,flap type,recipient vessel and complication.Results Three types of free flaps were used for maxillary reconstruction.The rectus abdominis flap was most commonly used,followed by free fibula flap and free radial forearm flap.All the flaps survived with the overall success rate of 100%.The overall complication rate was 30 percent.Apart from two hernia in our earlier cases,all the complications were minimal and neglectable.Conclusion Maxillary reconstruction using free tissue transfer is safe and reliable.

Abstract:BACKGROUND:The objective of this study was to evaluate the outcomes of using the free fibula flap in the reconstruction of maxillary defects.METHODS:Thirty-four consecutive cases of maxillary reconstruction with the free fibula flap were reviewed. All clinical data were analyzed, including primary diseases, types of maxillary defect, free fibula flap design, perioperative complications, and follow-up results. The main postoperative functional indices, including oral diet, speech, type of dental restoration, and aesthetic results, were evaluated.RESULTS:Of the 34 patients who underwent maxillary reconstruction with the free fibula flap, the primary diseases were malignant tumor in 20 patients, benign tumor in 11 patients, and trauma in three patients. Free fibula flap transfer was successful in all cases. Postoperative complications occurred in five patients. Recipient-site wound infection occurred in two patients and donor-site wound dehiscence occurred in three patients. One patient with donor-site wound dehiscence had postoperative lameness. The oral and nasal cavities were separated well by the flap in all patients. The patients were able to take food orally and had no problems with speech intelligibility. Osseointegrated implants were placed in four patients, and complete conventional prostheses were applied in 19 patients. Excellent cosmetic results were obtained in 22 patients.CONCLUSIONS:Alveolar arch defects can be reconstructed successfully using free fibula flaps. This procedure also allows for dental implant rehabilitation, which can improve the patient's appearance and oral function and enhance the overall quality of life. The fibula free flap transfer has a high success rate and low perioperative complication rate, making it an ideal choice for maxillary defect reconstruction.
[Repair and reconstruction of oral and maxillofacial defect--clinical analysis of 1973 cases]

Abstract:Objective: To compare the reconstructive method of oral and maxillofacial defect with free tissue flaps. Methods: The clinical materials were collected from 1 973 reconstructive cases between January 2000 and June 2004 and analyzed in terms of the distribution of age, gender, disease type, defect location, reconstructive method and the incidence of vascular crisis of free flaps as well as success rate of free flap respectively. SAS 6. 12 was adopted for statistical analysis. Results: A total of 1973 reconstructive cases included 764 in middle age (>45 years to < or =60 years, 38.72%), 527 in old age (>60 years, 26.71%), 450 young adults (>28 years to < or =45 years, 22.81%), 187 in young age (>14 years to < or =28 years, 9.48%) and 45 children (< or =14 years, 2.28%). The ratio of male to female was 1.5 : 1. The ratio of benign to malignancy lesion was 1 : 1.94. The tongue defect accounted for 20.63%, followed by mandibular defect(17.38%), parotid defect(13.74%), buccal defect(12.72%), maxillary defect (8.16%), oral pharynx defect (7.60%), floor of mouth defect (5.68%) and others (14.09%). Vascular free flap transfers accounted for 45.82%(904), followed by axial flap(38.17%, 753), random flap (10.19%, 201), a vascularized bone graft (1.52%, 30) and others(4.30%, 85). The most frequently used flap was the forearm flap(594 cases), followed by the fibula free flap(143 cases) and the pedicled pectoralis major myocutaneous flap(369 cases); these three flaps accounted for 56.06% (1106/1973). In 47 free tissue flaps (5.20%) having vascular crisis, 30 were saved (63.83%). The success rate of total free tissue flaps was 98.19% (923/940). Conclusion: The majority of reconstructive cases of oral and maxillofacial defects is the middle aged and the old aged male patients with malignancy. The tongue defect accounts for about one fifth of all the cases. The vascularized free flap has a high success rate, so it is a main method for reconstruction of oral and maxillofacial defects. The forearm flap, the fibular free flap and the pedicled pectoralis major myocutaneous flap are the main management for repairing oral and maxillofacial defects.

Abstract:Objective To review 12 cases with free flap transfer in the oral and maxillofacial region. Methods From October 2001 to October 2006,12 cases with free flap transfer were performed in our hospital. Among them,8 had fibula flaps and 4 had radial forearm flaps. Results Five mandiblular defects were reconstructed,3 maxillary defects were reconstructed,and 4 tongues were reconstructed with the free flaps All the free flaps survived completely with 100% success rate. Conclusions Free flap is reliable for the reconstruction of oral and maxillofaicial defect.

Abstract:Maxillary defects resulting from tumor resection or trauma can cause severe functional and cosmetic deformities. Maxillary reconstruction has long been a challenge for oral maxillofacial surgeons. Functional maxillary reconstruction with vascularized composite bone flap and osseointegrated implants is one of the most important improvements in head and neck reconstructive surgery. Since 1999, our research group has performed a comprehensive research on functional maxillary reconstruction with free composite fibula flap. Clinical data of the patients with maxillary reconstruction using free fibula flap were analyzed to describe the indications and principles of perioperative period of this technique. The modified free fibula flexor-hallucis longus myofascial flap was introduced, which could overcome the disadvantages of traditional free composite fibula flap. The donor site morbidity, post-operative speech outcome, mastication function, and quality of life were evaluated objectively. The biomechanical effects of stress distribution on maxilla reconstructed by free fibula composite flap were analyzed by three-dimensional finite element analysis. These studies demonstrated maxillary defects can be reconstructed successfully using free fibula flaps. This procedure also allows dental implant or conventional denture rehabilitation, which can improve the patient's appearance and oral function and enhance the overall quality of life. The fibula free flap transfer has a high success rate and low perioperative complication rate, making it an ideal choice for maxillary defect reconstruction.
Digitally reconstructed severe trauma-induced oro-maxillofacial defects with free vascularised composite tissue flaps

Abstract:The aim of this study was to outline the characteristics of severe trauma-induced oro-maxillofacial defects and explore the application of microsurgical reconstruction of these defects by selecting appropriate free vascularised composite tissue flaps. A retrospective analysis was performed in 11 cases with severe trauma-induced oromaxillofacial defects from 2005 to 2018, All cases were reconstructed with free vascularised composite flaps using a supermicro-surgical/micro-surgical and digital technology. This study consisted of 11 patients (8 men, 3 women) with a median age (range) of 28 (6-64) years. Patients with long-standing trauma experienced severe scars and displacements. The trauma-induced oro-maxillofacial defects ranged in size from 4.0 × 5.0 cm2 to 7 × 15 cm2. Six radial forearm flaps, three fibular osteocutaneous flaps, a lateral thoracic flap, and an anterolateral thigh flap were harvested individually. All patients were treated effectively and followed up for over two years, with a 100% survival rate of these free vascularised composite tissue flaps. No uncontrollable complications occurred, and all patients were satisfied with the postoperative shape and function. The trauma-induced oro-maxillofacial defects display unique features, and microsurgical techniques play an important role in the reconstruction of shapes and function. In addition, some patients may need multi-stage surgeries to attain aesthetic refinement.
Reconstruction of Maxillary Defects with Free Fibula Flap Assisted by Computer Techniques.

Abstract:Objective: Free fibula flaps are widely used for maxillary reconstruction. The three-dimensional position of the fibula flap is very difficult to control in conventional operations based solely on the surgeon's experience. We aimed to improve this surgery by using computerized techniques.Methods: Twenty-seven patients with maxillary tumors underwent maxillectomy and free fibula flap reconstruction in our department between 2011 and 2013. Virtual planning and surgical navigation were used for eight patients, and conventional surgery was performed in 19 patients. The three-dimensional fibular positions were evaluated in the two groups. Differences between the postoperative position of the fibular segments and the virtual plans were evaluated in the computer-assisted surgery group.Results: The three-dimensional position of the fibula flap in the computer-assisted surgery group, including the vertical distance (p = 0.013), horizontal position (p = 0.019) and extension of the posterior end (p = 0.041), was significantly more accurate and nearer to the ideal position than that in the conventional surgery group. The average difference between the actual postoperative position of the fibular segments and the virtual plan in the computer-assisted surgery group was <5 mm.Conclusion: Application of computer-assisted techniques such as virtual planning and surgical navigation significantly improve the clinical outcomes of maxillary reconstruction with free fibula flaps. A New Classification of Maxillary Defect and Simultaneous Accurate Reconstruction

Abstract:Objective:To select the preferred flaps for the reconstruction of different maxillary defects and to propose a new classification of maxillary defects.Methods:A total of 219 patients (136 males and 83 females) underwent the simultaneous reconstruction of maxillary defects in the Beijing Tongren Hospital, Capital Medical University, between January 2005 and December 2018 were reviewed. Age ranged from 16 to 78 years. Based on the proposed new classification of the maxillary defects, 22 patients with class Ⅰ defects (inferior maxillectomy), 44 patients with class Ⅱ defects (supperior maxillectomy), 132 patients with class Ⅲ defects (total maxillectomy) and 21 patients with class Ⅳ defects (extensive maxillectomy) were enrolled. Survival rate, functional and aesthetic outcomes of flaps were evaluated. Survival analysis was performed in 169 patients with malignant tumor, Kaplan-Meier method was used to calculate the survival rate, and Log-rank method was used to compare the difference of survival rate in each group.Results:A total of 234 repairs for maxillary defects were performed in 219 patients. Fibula flaps were used in 4/13 of class Ⅰ defects; temporal muscle flaps (11/24, 45.8%) and anterolateral thigh flaps (6/24, 25.0%) used in class Ⅱ defects; temporal muscle flaps (71/128, 55.5%), anterolateral thigh flaps (6/24, 25.0%) and fibula flaps (12/128, 9.4%) used in class Ⅲ defects; and anterolateral thigh flaps (8/20, 40.0%) and rectus abdominis flaps (8/20, 40.0%) used in class Ⅳ defects. The success rate of local pedicled flaps was 95.6% (109/114) and that of free flaps was 95.8% (115/120). Thrombosis(10/234,4.3%) was a main reason for repair failure. Among the followed-up 88 patients, swallowing and speech functions recovered, 82 (93.2%) of them were satisfied with appearance, and 75 (85.2%) were satisfied with visual field. The 3-year and 5-year overall survival rates were 66.5% and 63.6%, and the 3-year and 5-year disease-free survival rates were 57.1% and 46.2%, respectively, in the 169 patients with malignant tumors.Conclusion:A new classification of maxillary defects is proposed, on which suitable flaps are selected to offer patients good functional and aesthetic outcomes and high quality of life.

Abstract:Objective:To demonstrate the reconstruction of maxillary Ⅱ defect using free anterolateral thigh myocutaneous flap through modified lateral lip-submandibular approach.Methods:Between May 2012 and December 2016,twelve patients suffering from subtotal maxillary defects,who underwent maxillary reconstruction using free anterolateral thigh flap through modified lateral lip-submandibular approach,were recruited in this study.Results:Healing courses were uneventful in twelve patients.The patients had good speech function without facial paralysis.They were satisfied with face appearance except of one patient,who encountered the necrosis of the flap.Conclusion:Reconstruction of maxillary Ⅱ defect using free anterolateral thigh myocutaneous flap is a feasible and acceptable technique.

Abstract:Abstract: Reconstruction of complete bilateral maxillary defects (CBMDs) can be challenging due to the extensive loss of bone and soft tissues. This is a retrospective case series of 46 consecutive patients with CBMDs that were reconstructed with different micro-vascular free flaps. The authors aimed to evaluate the surgical outcomes and discuss the different reconstruction options in this case series. Thirty-six patients underwent reconstruction following ablation surgery for malignant tumors, 6 for benign tumors, 3 patients were treated for osteomyelitis, and 1 patient underwent free flap reconstruction for posttraumatic defects. Free fibula flap (n = 26) is the most commonly used reconstruction method in this case series, which was used in all defect types. This is followed by anterolateral thigh flap (n = 10), 5 rectus abdominis myocutaneous free flap, 3 radial forearm free flaps, and 2 composite free flaps. In this series, 44 free flaps survived, whereas only 2 flaps were lost. All patients could resume a soft diet postoperatively. Reconstruction of CBMDs with vascularized free flaps is a safe and reliable procedure.

Abstract:Objective: To evaluate the application of computer-assisted design and three-dimensional printing technique in reconstruction of maxillary defects with individual free fibular flap. Methods: A total of 13 patients, 7 males and 6 females with age from 12 to 55 years old, underwent the reconstruction of maxillary defects after subtotal or total maxillectomy for benign or malignant tumors between January 2016 and December 2018 were reviewed. Ther were 4 cases of subtotal maxillectomy and 9 cases of total maxillectomy. Before operation, osteotomy line was planned on three-dimensional images. A three-dimensional individual resin fibula model based on mirror images of the healthy side maxilla was obtained to fabricate an anatomically adapted osteomyocutaneous fibula free flap using computer-assisted design and forming. Oral, nasal, and eye functions and facial appearance were evaluated. Results: The 13 cases were followed up for 5-40 months, all flaps were alive except one due to flap failure and all cases were tumor free confirmed by CT or MRI. Ten patients could eat normal food without nasal food return, and 3 cases had palatal fistula. Eleven cases showed good speaking functional results. All cases had normal eye positions, no cases with diplopia and loss of vision. With evaluation by VAS, an average score of 8 was obtained, and most of patients were satisfied with their postoperative facial appearances. Conclusion: Reconstruction of maxillary defects by free fibular flap designed by three-dimensional printing and computer-assisted technique not only restored oral, nasal and eye functions, but also showed satisfactory facial appearance.
Application of Free Flaps in Reconstruction of Head and Neck Soft Tissue Defects With Bone Exposure

Abstract:Background: Reconstruction of head and neck soft tissue defects with bone exposure is both challenging and technically demanding for plastic surgeon. Objectives in head and neck soft tissue defects with bone exposure reconstruction are consistent restoration of functionality while also improving appearance. This study retrospectively analyzed the results of head and neck reconstructions using various types of free flaps over the past 4 years. Methods: A retrospective analysis was conducted from June 2019 to June 2023 on 12 patients treated at our hospital for head and neck soft tissue defects with bone exposure due to various causes. These included 4 cases of trauma from car accidents, 1 burn case, and 7 postoperative malignant tumor removals. The defect sizes ranged from 4 × 6 to 15 × 45 cm. Different free flaps were used for repair based on the defect, including 6 anterolateral thigh flaps, 3 forearm flaps, 2 latissimus dorsi flaps, and 1 dorsalis pedis flap. Flaps were designed and harvested to match the defect size and transplanted via anastomosed vessels. Results: All 12 flaps survived successfully. One patient required flap thinning surgery postoperatively. All patients were followed up for over 3 months, showing good color and texture of the transplanted flaps, satisfactory healing, and significant aesthetic improvement. Donor sites showed significant scarring without functional impairment. Conclusion: Free flap repair for head and neck soft tissue defects with bone exposure is feasible and yields good results.

Abstract:Background: The maxillary defects left unreconstructed or inadequately reconstructed often result in significant functional and esthetic impairments. Adequate reconstruction of extensive maxillary defects requires a sufficient volume of hard and soft tissues. Methods: A 48-year-old male presenting bilateral extensive maxillary defects underwent secondary reconstruction with a flow-through fibula free flap in combination with an anterolateral thigh free flap. Results: The use of flow-through technique allowed minimizing the problem of limited recipient vessels and the length of free flap vascular pedicle usually encountered in secondary reconstruction. The bilateral maxillary defects were successfully reconstructed, and the postoperative outcomes were uneventful. The patient was satisfied with the treatment outcomes. He is being followed up and was referred to the implantology department for the placement of osseointegrated dental implants. Conclusions: The flow-through fibula free flap, in combination with the anterolateral thigh free flap, was found reliable and feasible for this case of secondary reconstruction of bilateral maxillary defects. This technique has provided satisfactory functional and esthetic outcomes and effectively improved the patient’s self-esteem.

Abstract:Objective To summarize the experience in the repair of maxillofacial and head-neck defects by forearm free flap.Methods The clinical records of 22 cases of forearm free flap for the maxillofacial and head-neck reconstruction were summed up and analyzed.Results With the overall success rate of 95.5%,the forearm free flap is satisfied with repairing of defects in maxillofacial and head-neck area after operation by variational application.It has the advantage that meets the requirements for repairing defects of maxillofacial and head-neck area.Its disadvantage is that the handsome shape of provide area was destroyed because the radial artery was victim.Conclusion The forearm free flap is a multifunctional flap with good texture suitable to be used to repair soft tissue defects in various maxillofacial and head-neck regions,and with good long-term effect.It is the preferred flap for repairing maxillofacial and head-neck regions.

Abstract:This study aimed to evaluate the usefulness of lateral thoracic flap in reconstructing maxillofacial sift tissue defects to improve the quality of life of the patients. Totally 17 cases with tissue defects in the maxillofacial region, caused by various disease, were collected during 2004–2006. The age of these patients ranged from 29–71 years (average, 48.4 years), the ratio of male to female was 12:5. The lateral thoracic flap or combined with the scapular flap were applied to reconstruct the defect of soft tissue and/or bone tissue. All the flaps survived except one case which presented necrosis of the epidermis resulting from the too fine supplying artery. After 3–32 months follow up, the morphology of the reconstructed region was satisfied, and the scar in the donate site was inconspicuous. The application of thoracic flap is a better choice to improve quality of life of the patients compared with other free flaps traditionally used in clinic.

Abstract:The aim of this study was to present a treatment protocol for the individual repair of post-traumatic maxillofacial bone defects with vascularized flaps assisted by digital techniques. This study reviewed 20 patients with post-traumatic maxillofacial bone defects who underwent reconstruction with composite vascularized bone flaps assisted by digital techniques between April 2009 and July 2019. Preoperative computed tomography (CT) data were imported into ProPlan CMF software to complete virtual fracture reduction and reconstruction. Surgical navigation, three-dimensionally (3D) printed surgical plates, and prefabricated titanium mesh/plates were used to guide the actual surgery. All patients underwent open reduction and internal fixation and reconstruction surgery in one stage. CT data obtained at 1 week postoperative were imported into Geomagic Control software to evaluate the accuracy of the virtual surgical plan. The mean follow-up interval was 24 months (range 6-96 months). Donor and recipient site morbidity and second-stage procedures to rehabilitate the dentition and cosmetic organs were recorded. The flap success rate was 100%. Nine patients had deep circumflex iliac artery flaps and eleven patients had fibula flaps. The accuracy of computer-assisted surgery was 4.4±0.8mm. There were no postoperative complications. This study is novel in presenting a treatment protocol for individual computer-assisted reconstruction for post-traumatic maxillofacial bone defects with vascularized flaps.
