Abstract:PURPOSE:To compare and analyze the clinical effectiveness of forearm flap and anterolateral thigh flap in reconstruction of oral and maxillofacial defects.METHODS:Fifty-four cases undergoing oral and maxillofacial defect reconstruction with forearm flap and anterolateral thigh flap were reviewed.Thirty-three cases used forearm flap (group A),and 21 cases used anterolateral thigh flap after tumor resection.Operative duration,survival rate of flap,shape changes of recipient site,donor site and patients' satisfaction were analyzed between 2 groups with SPSS 19.0 software package.RESULTS:The operative duration of group A was (41.6±8.9) min,(47.2±10.2) min in group B,and the difference was statistically significant (P＜0.05).Flap survival rate was 93.9％ in group A and 90.5％ in group B,the difference was not statistically significant (P＞0.05).Shape changes of recipient site in group A were similar to group B.Donor site recovery was better in group B.The rate of permanent dysfunction in group A was 24.2％ and 0 in group B,and the rate of temporal dysfunction was 9.5％ in group B.CONCLUSIONS:Forearm flap was as useful as anterolateral thigh flap in reconstruction of postoperative defects of oral and maxillofacial tumors.In clinical practice,advantages and disadvantages should be considered according to individual conditions in selection of free flaps.

Abstract:Objective:To investigate the application of free anteromedial thigh (AMT) perforator flap in the reconstruction of tongue defect after radical resection of tongue carcinoma.Methods:From September 2010 to January 2015,57 cases with tongue carcinoma underwent radical resection, leaving tongue and mouth floor defects which were reconstructed by AMT perforator flaps at the same stage. These 57 eases included tongue carcinoma at lingual margin (n =36),at ventral tongue (n =15) and at mouth floor (n=6). .Results:The size of AMT perforator flap ranged from 5.5 cm ×4.0 cm to 7.5 cm × 5.5 cm, the thickness of flap ranged from 1.0-1.7 cm. The length of pedicle from descending branch of lateral circumferential femoral artery is (8.2 ± 0.6) cm, found in 28 cases. The length of pedicle from profunda femoral artery is (8.5 ±0.4) cm, found in 17 cases. The length of pedicle from femoral artery is (8.1 ± 0.7) cm, found in 12 cases. All 57 perforator flaps survived uneventfully, the defects at donor site were closed directly in all cases. All patients were followed up for 12-24 months with satisfactory esthetic and functional results in reconstructed tongue. No local recurrence happened. Only linear scar left in the donor sites, the function of thighs were not affectedConclusions:The free AMT perforator flap is an ideal choice for reconstruction of the tongue and mouth floor defect after radical resection of tongue carcinoma.

Abstract:OBJECTIVE:To compare the clinical application of lateral arm flap and radial forearm flap in reconstruction after oral cancer ablation.METHODS:The clinical data of 21 cases with lateral arm flap and 104 cases with radial forearm flap between 2007 and 2009 were reviewed. The flap harvest time, flap survival rate, donor site morbidity and postoperative oral function were compared and analyzed.RESULTS:The average flap harvest time of lateral arm flap and radial forearm flap were (46.4±7.6) min and (41.5±7.5) min (x±s) respectively. The flap survival rates of the two flaps were 90.5% (19/21) and 95.2% (99/104) respectively. There was no significant difference between the two flaps in the average flap harvest time and survival rate (P>0.05). The donor site of lateral arm flap could be primarily closed and radial nerve injury occurred in 2 patients. Skin graft was necessary for the donor site closure of radial forearm flap and conspicuous unattractive scar could be seen. All patients could have oral diet and no difficulty in speech intelligibility.CONCLUSION:The lateral arm flap is relatively safe and reliable alterative to the radial forearm flap for the defects reconstruction after oral cancer ablation.

Abstract:Objective To review 12 cases with free flap transfer in the oral and maxillofacial region. Methods From October 2001 to October 2006,12 cases with free flap transfer were performed in our hospital. Among them,8 had fibula flaps and 4 had radial forearm flaps. Results Five mandiblular defects were reconstructed,3 maxillary defects were reconstructed,and 4 tongues were reconstructed with the free flaps All the free flaps survived completely with 100% success rate. Conclusions Free flap is reliable for the reconstruction of oral and maxillofaicial defect.

Abstract:The purpose of this study was to assess the clinical features and therapeutic efficacy of the intraoral defects reconstruction with 3 types of anterolateral thigh flaps.The clinicopathologic data of 39 cases with oral tumors were obtained from the School of Stomatology, Nanjing University Medical Center, from December 2008 to June 2010. These patients underwent the simultaneous tumor resection and intraoral defects reconstruction with free anterolateral thigh flaps.There were 22 male and 17 female patients; the ratio of male to female was 1.3:1; the mean age was 56.1 years. Twenty-two of the anterolateral thigh flaps were musculocutaneous flaps (56.4%), 8 were fasciocutaneous flaps (20.5%), and 9 were ultrathin flaps (23.1%). Five patients required operative exploration in the perioperative period. Three flaps were thrombotic events, 1 flap was hematoma, and 1 flap was twisting of perforator. After the salvages, 1 flap was partial failure, 1 flap was total failure, and the other 3 flaps were complete survival.The free anterolateral thigh flap was the ideal soft tissue flap in the intraoral defects reconstruction. This flap presents good functional results at the receiving site with the additional advantages of minimal donor-site morbidity and a high level of patient satisfaction.
Maxillary Reconstruction with Free Flaps:A Review of 65 Consecutive Cases

Abstract:Objective To evaluate the use and indications of forearm flap,rectus abdominus myocutaneous flap and fibula osteocutaneous flap used for reconstructing the maxillary defects.Methods Sixty-five consecutive cases of maxil-lary reconstruction with free flaps were reviewed.All the clinical data including primary diseases,the type of maxillary de-fect,the select of free flap,perioperative complications and the follow-up were analyzed.Results All the cases under-went the three free flaps reconstruction of the maxillary defects.50cases with malignant tumors,10cases with benign tu-mors,2cases with radioosteonecrosis,2cases with trauma and1case with inflammation.Only one flap failure was noted.The successful rate was98.5%.All patients had oral diet with good separation of the oral and sinonasal cavities,and had no difficulty in speech intelligibility.The age and the type of maxillary defects in different free flap groups were analyzed statistically,the results indicated there were significant difference in the three free flap groups(P0.05).Conclusions The three free flaps can be used to reconstruct maxillary defects,which can improve patient's outlook and function and contribute to the overall quality of life.The forearm flap is suitable for the old patients with small maxillary defect.The fibula osteocutaneous flap is an ideal choice for ClassⅠandⅡmaxillary defect reconstruction.The rectus abdominus myocutaneous flap is fitted for large maxillary defects.

Abstract:OBJECTIVE:To evaluate the application, indications and outcomes of free flaps for ora-maxillofacial traumatic defects reconstruction.METHODS:Twenty consecutive cases of ora-maxillofacial the traumatic defects reconstruction with free flaps were reviewed. All clinical data including causes of injuries, the type of defects, selection of free flaps, perioperative complications and the follow-up were analyzed.RESULTS:All the cases underwent free flap reconstruction for ora-maxillofacial traumatic defects: 8 cases with soft tissue defects, 12 cases with soft and hard tissue defects. Fifteen patients received two-stage operation and 5 patients underwent primary reconstruction at the time of debridement or fracture reduction. Twenty free flaps were applied for the reconstruction, 11 cases with fibula flap, 1 case with iliac crest free flap, 7 cases with radial forearm flap and 1 case with scapula flap. No flap failure occurred. The successful rate of free flaps transfer was 100%.CONCLUSIONS:The free flaps transfer is reliable and can reconstruct the ora-maxillofacial traumatic soft and hard tissue defects. Fibula and radial forearm free flap are the most common used flaps. Early aggressive surgery with free flaps transfer for traumatic defects can prevent the scar contracture and tissue displace, which can shorten the treatment period and improve the final outcome.

Abstract:OBJECTIVE:To study technique of one-stage reconstruction of oral and maxillofacial soft tissue defect by using forearm free flap.METHODS:The microsurgery procedure was used in the treatment of 4 oral cancer patients with oral and maxillofacial soft tissue defect after surgery.RESULTS:The forearm free flap was survived, for 4-18 months the oral function was well, and the appearance was satisfactory.CONCLUSION:This is a good approach to the reconstruction of oral and maxillofacial soft tissue defect by using forearm free flap technique.

Abstract:OBJECTIVE:To compare the thin anterolateral flap with forearm flap in tongue defect repairing, and to introduce our methods and experiences in the tongue reconstruction with the thin anterolateral flap.METHODS:The clinicopathologic data of 46 cases with tongue carcinoma were obtained from School of Stomatology, Nanjing University Medical Center, Nanjing University from December 2009 to December 2011. To compare two methods of incidence of vascular crisis, tongue shape, language and swallowing functional recovery.RESULTS:46 patients with tongue carcinoma were performed the tongue reconstruction in 12 month, which 12 cases were used the thin anterolateral flap and 34 cases were used the forearm flap. In the thin anterolateral flap group, All cases were succeeded. 1 case occurs vascular crisis. In the forearm flap group, 33 cases were succeeded, and 1 case occurs necrosis. 3 cases occurs vascular crisis. The results of comparing two methods showed that: no obvious differences in the tongue shape, and no obvious differences in the function of language and swallowing.CONCLUSIONS:There no obvious differences in the reconstruction of tongue defect between the thin anterolateral flap and the forearm flap. The thin anterolateral flap have some advantages: little influence is on the donor site, the flap extent is abundant, the donor site is not spectacular. The thin anterolateral flap should be piror method for the tongue defect repairing.
[Repair and reconstruction of oral and maxillofacial defect--clinical analysis of 1973 cases]

Abstract:Objective: To compare the reconstructive method of oral and maxillofacial defect with free tissue flaps. Methods: The clinical materials were collected from 1 973 reconstructive cases between January 2000 and June 2004 and analyzed in terms of the distribution of age, gender, disease type, defect location, reconstructive method and the incidence of vascular crisis of free flaps as well as success rate of free flap respectively. SAS 6. 12 was adopted for statistical analysis. Results: A total of 1973 reconstructive cases included 764 in middle age (>45 years to < or =60 years, 38.72%), 527 in old age (>60 years, 26.71%), 450 young adults (>28 years to < or =45 years, 22.81%), 187 in young age (>14 years to < or =28 years, 9.48%) and 45 children (< or =14 years, 2.28%). The ratio of male to female was 1.5 : 1. The ratio of benign to malignancy lesion was 1 : 1.94. The tongue defect accounted for 20.63%, followed by mandibular defect(17.38%), parotid defect(13.74%), buccal defect(12.72%), maxillary defect (8.16%), oral pharynx defect (7.60%), floor of mouth defect (5.68%) and others (14.09%). Vascular free flap transfers accounted for 45.82%(904), followed by axial flap(38.17%, 753), random flap (10.19%, 201), a vascularized bone graft (1.52%, 30) and others(4.30%, 85). The most frequently used flap was the forearm flap(594 cases), followed by the fibula free flap(143 cases) and the pedicled pectoralis major myocutaneous flap(369 cases); these three flaps accounted for 56.06% (1106/1973). In 47 free tissue flaps (5.20%) having vascular crisis, 30 were saved (63.83%). The success rate of total free tissue flaps was 98.19% (923/940). Conclusion: The majority of reconstructive cases of oral and maxillofacial defects is the middle aged and the old aged male patients with malignancy. The tongue defect accounts for about one fifth of all the cases. The vascularized free flap has a high success rate, so it is a main method for reconstruction of oral and maxillofacial defects. The forearm flap, the fibular free flap and the pedicled pectoralis major myocutaneous flap are the main management for repairing oral and maxillofacial defects.

Abstract:Objective To summarize the experience in the repair of maxillofacial and head-neck defects by forearm free flap.Methods The clinical records of 22 cases of forearm free flap for the maxillofacial and head-neck reconstruction were summed up and analyzed.Results With the overall success rate of 95.5%,the forearm free flap is satisfied with repairing of defects in maxillofacial and head-neck area after operation by variational application.It has the advantage that meets the requirements for repairing defects of maxillofacial and head-neck area.Its disadvantage is that the handsome shape of provide area was destroyed because the radial artery was victim.Conclusion The forearm free flap is a multifunctional flap with good texture suitable to be used to repair soft tissue defects in various maxillofacial and head-neck regions,and with good long-term effect.It is the preferred flap for repairing maxillofacial and head-neck regions.
A Clinical Study on Reconstruction of Maxillofacial Soft Tissue Defects After Oral Cancer Operation by Free Forearm Flap

Abstract:Objective To discuss the method and experience of the reconstruction of maxillofacial soft tissue defects after oral cancer operation by free forearm flap.Methods Thirty eight patients with oral cancer recruited in our hospital from March 2005 to March 2011 were treated by the method of the reconstruction of maxillofacial soft tissue defects after oral cancer operation by free forearm flap and the post-operative survival of transplant and the occurrence of post-operative complications were analyzed.Results Free forearm flaps for 38 cases in this group all survived,with the success rate of 100%.1 case had the vascular crisis,which was caused by the blockage of blood coagulation in the drainage tube because of the unsmooth drainage of the rinse solution over the wound.It became better after stopping the rinse,replacing the drainage tube and cleaning off the blood coagulation.The incidence of post-operative vascular crisis was 2.6%(1 /38).1 case had the local chapped skin at the wound edge and thus received the treatment of changing dressings.The incidence of post-operative complications was 2.6%(1 /38).Conclusion It is a relatively ideal method for the reconstruction of maxillofacial soft tissue defects after oral cancer operation by free forearm flap.

Abstract:This study aims to compare flaps at different sites in treating soft tissue defects after oral cancer surgery and improving patients’ quality of life (QoL). Databases were searched until September 2023. The extracted data included the scores of chewing, swallowing, speech, mood, and appearance based on the University of Washington QoL questionnaire, version 4. Two types of free flaps and 2 types of pedicled tissue flaps were included. The free flaps were the forearm free flap (FFF) and anterolateral thigh flap, and the pedicled tissue flaps were the submental artery island flap and pectoralis major myocutaneous flap (PMMF). Compared with FFF, there was no significant difference in the scores of chewing, swallowing, speech, and mood among anterolateral thigh, submental artery island flap, and PMMF, and PMMF generally had a higher score than FFF only in terms of appearance, with statistical significance. There is no significant difference in chewing, swallowing, speech, and mood between flaps from different sites in repairing postoperative soft tissue defects of oral cancer. Therefore, the widely used FFF may be the preferred choice considering the QoL of patients after oral cancer surgery.

Abstract:Objective: To compare the clinical application results of the FPTF (free posterior tibial artery perforator flap) and RFFF (radial forearm free flap) for reconstruction of head and neck defects. Methods: A retrospective analysis of 27 cases treated with FPTF (19 males and 8 females, aged 14-69 years) and 24 cases with RFFF (11 males and 13 females, aged 22-69 years) for head and neck defect reconstruction at Beijing Tongren Hospital of Capital Medical University from January 2015 to December 2020 was conducted. Flap size, vascular pedicle length, matching degree of recipient area blood vessels, preparation time, total operation time, hospital stay, recipient area complications, donor area complications and scale-based patient satisfaction were compared between two groups of patients with FTPF and RFFF. SPSS 26.0 statistical software was used for statistical analysis. Results: There was no statistically significant difference between the two groups of patients in tumor T staging (P=0.38), primary sites (P=0.05) and mean flap areas ((53.67±29.84) cm2 vs. (41.13±11.08) cm2, t=-1.472, P=0.14). However the mean vascular pedicle length of FPTF was more than that of RFFF ((11.15±2.48)cm vs. (8.50±1.69)cm, t=-4.071, P<0.01). The donor sites of 4 patients in FPTF group could be sutured directly, while all the 24 patients in RFFF group received skin grafts from the donor sites. There was no statistically significant difference in the recipient area arteries between two groups of flaps (P=0.10), with more commonly using of the facial artery (RFFF: FPTF=21∶27), but there was significant difference in the recipient area veins (P<0.01), with more commonly using of the external jugular vein in RFFF (14/24) than FPTF (4/32) and the posterior facial vein in FPTF (27/32) than RFFF (9/24). There were 10 recipient complications and 3 donor complications in RFFF group; no recipient complication and 3 donor complications occurred in FPTF group. With patient's subjective evaluation of the donor site at 12 months after surgery, FPTF was better than RFFF (χ²=22.241, P<0.01). Conclusions: FPTF is an alternative to RFFF in head and neck reconstruction and has unique advantages in aesthetics and clinical application.

In these 34 patients, 26 flaps were musculocutaneous perforators (76.5%) and 8 flaps were septocutaneous perforators (23.5%). Partial loss of flap occurred in 2 patients (5.9%).The free thin anterolateral thigh flap was the ideal soft tissue flap in intraoral defect reconstruction. This thin flap presents good functional results at the receiving site with the additional advantages of minimal donor-site morbidity and a high level of patient satisfaction.

Abstract:Objective To analyze the reliability and safety of radial forearm free flap for reconstruction of defects in head and neck following tumorectomy.Methods 24 patients receiving reconstruction of head and neck defects with radial forearm free flap from 2008 to 2013 were involved in this retrospective study.The flaps were used for reconstructing the defects of palate,cheek,tongue,floor of mouth,tempus.The survival rates and postoperative complications were evaluated.Results The flaps kept alive in 23 cases with an overall survival rate of 95.83%.The total complication rate of both donor and receiving areas was 12.5%.The vessel thrombosis rate was 4.17%.Conclusion With strict flap choice,careful flap preparation,skillful vascular anastomosis and effective postoperative monitoring,the radial forearm free flap is reliable and versatile in reconstruction of head and neck defects after tumorectomy.

Abstract:Objective To analyse the rationale and feasibility of maxillary reconstruction using free flaps.Methods Twenty consecutive cases of maxillary reconstruction using free tissue transfers were reviewed.Data concerning the operation included date of surgery,defect description and site,stage and histology of tumor,flap type,recipient vessel and complication.Results Three types of free flaps were used for maxillary reconstruction.The rectus abdominis flap was most commonly used,followed by free fibula flap and free radial forearm flap.All the flaps survived with the overall success rate of 100%.The overall complication rate was 30 percent.Apart from two hernia in our earlier cases,all the complications were minimal and neglectable.Conclusion Maxillary reconstruction using free tissue transfer is safe and reliable.

Abstract:To assess the clinical features and therapeutic effect of free thin anterolateral thigh flap for the reconstruction of intraoral defects.The clinicopathologic data of 34 cases with oral carcinoma were obtained from Institute and Hospital of Stomatology, Nanjing University Medical School, from December 2008 to December 2011. These 34 patients underwent the simultaneous tumor resection and intraoral defects reconstruction with free thin anterolateral thigh flaps. The defects were located at tongue, buccal, mouth floor, and so on. The subcutaneous fat thickness of the flap at the site of the perforator is usually measured by ultrasound before the operation. If the thickness of the subcutaneous fat at the site of the perforator exceeded 1.5 cm, the patient was excluded from the study.There were 16 male and 18 female patients. The mean age was 55.4 years. Among the 34 patients, 26 flaps with musculocutaneous perforators and 8 flaps with septo-cutaneous perforators were used. Partial flap necrosis occurred in 2 patients.The free thin anterolateral thigh flap is the ideal soft tissue flap for the intraoral defects reconstruction. Satisfactory functional results can he achieved at recipient area with minimal morbidity at the donor site area.
