Abstract:Objective To investigate the clinical features,microsurgical method of the extracranial-intracranial tumors in the anterior skull bases and the curative effect of microsurgery on them.Methods The clinical data of 23 patients with extracranial-intracranial tumors in the anterior skull bases,who underwent microsurgery through anterior skull bases approach in our department from January,2000 to February,2008,were analyzed retrospectively.Of 23 cases of the tumors,9 were cranionasal ones,9 cranio-orbital ones and 5 cranio-orbito-nasal ones.Results Of 23 patients with extracranial-intracranial tumors in the anterior skull bases,19 received total resection of the tumors,2 subtotal and 2 greatly part.All the patients were significantly improved in the clinical symptoms and signs.Conclusion The microneurosurgery through anterior skull bases approach can effectively enhance the rate of total resection of the extracranial-intracranial tumors in the anterior skull bases.
Microsurgical Resection of Sellar Tumors Via Lateral Supraorbital Approach:Clinical Analysis of 20 Cases

Abstract:Objective: To research the application of lateral supraorbital (LOS) approach for resecting sellar tumors. Methods: Clinical symptoms of 20 patients with surgery via LOS approach were analyzed retrospectively.According to the neuroradiological findings and the improvement of symptoms, and some score scale, the efficacy and safety of LOS were assessed for removing sellar tumors.There were tuberculum sellae meningioma (n=11), craniopharyngioma (n=4), epidermoid cysts (n=4) and pituitary adenoma (n=1). Results: Complete resection was achieved in seventeen patients(85%). The mean operating time was 144.0±54 minutes and the mean intraoperative blood loss was 96.5±51.1 ml.Four patients had post-operative fever, four had endocrine disorders, and one had seizure.No other complications were noted.Fifteen patients had visual function impairment before operation, seven of which were improved while one was aggravated after surgery.The median Karnofsky score (3-46 mon) was 95(0-100) and the Glasgow outcome scale (GOS) at follow-up (3-46 mon) was 4.7(0-5). In general, the outcomes were improved in 18 patients and the mortality was 2. Conclusion: The resection of tumors at sellar region via lateral supraorbital approach is effecient, simple and minimally invasive, which can make the removal of the tumors reliably and safely.It is worthy to be popularized clinically.
Surgical management of facial malignant tumors

Abstract:Objective To investigate the performance of surgical management in facial skin malignancies.Methods From January 2000 to December 2006,65 patients with facial skin malignancies,including47 cases of basal cell carcinoma.10 cases of squamous cell carcinoma,3 cases of dermatofibrosarocoma protuberans,2 cases of malignant melanomas,and one case of malignant acanthoma,hemangioendotheliosar-coma and sebaceous carcinoma,respectively,were collected and managed with wide resection followed by reconstruction.In order to achieve a thorough resection,frozen sections were prepared and subjected to pathological examination during the operation process to ensure the margins of resection were free of malignancy.Reconstruction was carried out by direct closure,or with local random flaps,extended flaps,free skin grafts.Resuits All defects were managed by one-stage reconstruction.The survival rate of skin flaps/grafts was 100%,and a satisfactory appearance and function was achieved.During the follow-up from 6 months to 5 years,local relapse was observed in one patient with basal cell carcinoma and one with squamous eell carcinoma,lymphatic metastasis in one with squamous cell carcinoma.Distant metastasis occurred in a patient with malignant melanoma.who died consequently.Conclusions Thorough resection is the key to prevent relapse of facial skin malignancies after surgery.Appropriate reconstruction may favor the restoration of facial appearance,and local random flaps appear to be the best reconstruction strategy.

Abstract:OBJECTIVE:To investigate the surgical treatment and related experience of lateral skull base lesions.METHODS:In a retrospective, clinical data analysis of 20 patients with lateral skull base diseases, 11 cases with temporal bone space-occupying lesions and intracranial invasion were treated by translabyrinthine surgery, mastoid cavity drainage, or transmastoid surgery to remove the lesion; two cases of cholesteatoma with destruction of tympanic cavity and tympanic sinus canopy with intracranial invasion were treated by extended radical mastoidectomy; seven cases with lateral skull base bone destruction with cerebrospinal fluid otorrhea caused by trauma and deformity were treated by translabyrinth and transmastoid repair.RESULTS:Eleven cases with temporal bone space occupying lesions were resected completely and were without recurrence after surgery. Two cases with intracranial infection secondary to cholesteatoma were rapidly relieved of symptoms without recurrence after radical mastoidectomy. The remaining seven cases of CSF otorrhea included two cases of Mondini malformation and five cases of temporal bone fracture. The leak was stemmed in all seven cases after surgery.CONCLUSION:Surgery is the main therapeutic option for the treatment of lateral skull base diseases. The surgical approach and the type of skull base reconstruction should be selected on an individual case-by-case basis.

Abstract:In clinics,lateral skull base comprises intradural cerebellopontine angle area,and the region of extradural temporal and infratemporal fossa.The modern lateral skull base surgery started from those two operations that were surgical removal of acoustic schwannoma via otological approach and the extirpation of glomus jugulare tumor through infratemporal fossa.The pioneers of these two classic surgeries were the otologists who developed the new field of otological surgery.Since then,the lateral skull base surgery has become a developed and academical platform of multiple related disciplines,such as neurosurgery,head-neck surgery and reconstructive surgery etc.The lateral skull base surgeries build upon the microsurgical techniques and innovative surgical approaches,which are available successfully to treat riskful disorders in the so-called inaccessible regions.Nevertheless,it is better to find the best equation between the preservation of skull base structures with their function and the vital safety during each decision of surgical strategy.Probably,minimal invasive surgery or precision treatments of the disorders in skull base would become a new horizon in the future.

Abstract:Objective To study the multidisciplinary collaberation for surgical treatment of skull base tumors with intra and extra - cranial involvement.Methods 78 cases of skull base tumors with intra and extra - cranial involvementwere analyzed retrospectively.The relationship between surgical approaches and the origin and extension of the tumors was analyzed.The main complications and avoidance was discussed.Results Total tumor removal was performed in 48 cases,subtotal in 25 and partial in 5.The operation - related complications occurred in 36 patients without death. Conclusions Multidisciplinary collaboration benefits the surgical therapy for skull base tumor with intra and extra - cranial involvement.The origin and extension of the tumor is the most important consideration in determining the surgical approaches.The injury of blood vessel and cranial nerves,cerebrospinal fluid leakage and intracranial infection are the main operative complications.

Abstract:OBJECTIVE:To investigate the capable use of transmaxillary approach for surgical removal of invasive skull base tumors, the indications and the key points of this approach.METHODS:From November 1998 to July 2001, 27 consecutive patients with skull base tumor were operated through transmaxillary approach, including 6 patients with nasopharyngeal carcinoma, 5 with nasopharyngeal angiofibroma, 5 with nasopharyngeal cystadenocarcinoma, 2 with olfactory neuroblastoma, 2 with poorly differentiated carcinoma, 2 with sarcoma, 1 with maxillary carcinoma, 2 with schwannoma, and 2 with chordoma. Most of them (18/27) were recurrent tumor and 17/27 tumors involved important intracranial structures. All patients were followed up 2 - 33 months (average 16 months) and the clinical data was reviewed.RESULTS:The tumors could be totally removed in all patients. There were no operative mortality and morbidity. After operation, 2 patients died of cancer recurrence in 5 and 8 months separately. One patient had metastasis to the lungs 11 months after operation. Two patients had local recurrence in 7 and 12 months postoperation seperately and live with the tumor now. The rest patients are back to their routine life.CONCLUSIONS:Transmaxillary approach facilitates the surgical removal of invasive skull base tumors. The exposure is wide. The lesion as well as the important anatomy structures can be viewed directly and clearly. The tumor removal could be done more thoroughly and safely. This approach is suitable for the patients in whom tumor involves the skull base extensively and may be difficult to deal with by other approaches.

Abstract:Abstract BACKGROUND Chondromyxoid fibromas (CMFs) are benign tumors that occur rarely in the skull base. OBJECTIVE To conduct a preliminary exploration of the diagnosis and treatment of cranial CMFs. METHODS A retrospective analysis of 19 cases of CMFs in the base of the skull between 2009 and 2014 in our hospital was conducted. The clinical manifestations, imaging characteristics, pathology, treatment strategies, and outcomes were examined. RESULTS The study cohort included 7 women (36.8%) and 12 men (63.2%), and symptom duration ranged from 1 mo to 5 yr. Of the 19 intracranial CMF cases examined, 15 (78.9%) conformed with the diagnostic criteria for extracranial CMF. Resection operations yielded subtotal removal of 13 tumors (68.4%) and partial removal of 6 tumors (31.6%). Postoperative pathological analysis demonstrated that the tumors were characterized by spindle-shaped or stellate cells arranged in a myxoid matrix without mitoses or permeation. Follow-up (range 2-7.3 yr; mean, 4.4 ± 1.7 yr) revealed that symptoms improved postoperatively in 15 cases (78.9%), were maintained in 2 cases (10.5%), and worsened in 2 cases (10.5%). Imaging follow-up revealed that residual tumors were stable in 18 cases (94.7%) and enlarged in 1 case (5.3%). CONCLUSION An accurate diagnosis should involve comprehensive consideration of clinical, radiological, and pathological features. The treatment strategy for CMFs consists of maximizing tumor removal while protecting adjacent key structures. Postoperative stereotactic radiotherapy is appropriate for residual tumors.

Abstract:Background: Communicating occupying lesions of the infratemporal fossa and mid-cranial fossa represent challenges for neurosurgeons and maxillofacial surgeons. Lesions of the skull base, with complex anatomy, are extremely varied in their histopathology. They often spread via normal anatomic pathways or bony destruction to involve adjacent structures, especially in the case of malignant neoplasms. However, with the development of operative skills and reconstruction techniques, total or near total resection of many such lesions can be accomplished with a low morbidity rate.Methods: The combined frontotemporal-preauricular infratemporal approach, frontotemporal-orbitozygornatic approach, combined frontotemporal and transmandibular approach, transmandibular approach, and transmaxillary approach were used to resect the lesions using microsurgical skills. The plastic reconstruction included reconstruction of the dura defects, osteosynthesis, and obliteration of the operative cavity.Results: Thirty-three patients with communicating lesions invading both the infratemporal fossa and the middle skull base underwent microsurgery. Among them, lesions were totally resected in 23, subtotally resected in 6, and partially resected in 4. In 29 patients, clinical status improved significantly. Complications were encountered in 7 cases, and morbidity was not remarkable.Conclusion: The surgical approach should be selected depending on the size, location, histopathology, and invading extension of the lesion, which can provide excellent visualization and allow for an aggressive and safe resection of lesions involving this region. Familiarity with skull base anatomy, careful microsurgical manipulation, protection of vital structures, reliable plastic reconstruction, intraoperative nerve monitoring, as well as consideration of functional and esthetic outcomes, will all contribute to successful surgery and satisfactory results. Abstract:Objective To explore the efficiency and complications of a new approach,maxilla-fronto-temporal approach of our own design,for removal of tumors involving infratemporal fossa.Methods Clinical data of 12 patients with infratemporal fossa tumors resected via maxilla-fronto-temporal approach were analyzed retrospectively.Results Tumors got successfully resected in all cases via this approach with minor complications.Conclusion With advantages of adequate protection of facial nerve,extended operation field and filling of dead space with exposed temporal muscle,the maxilla-fronto-temporal approach is especially useful in removal of tumors involving infratemporal fossa.

Abstract:Surgery of skull base tumors is a multidisciplinary team work with high difficulty and risk, which is also demonstrated in its history. Great advancements have been achieved in the surgery of skull base tumors in the recent 40 years, which were mainly manifested in tools, techniques, knowledge and therapy philosophy. Based on the four aspects mentioned above in the surgery of skull base tumors, we evaluated the current situation and looked into the future. Surgery of skull base tumors in China started late but developed well currently, the main obstacles and countermeasures were proposed.
Favorable Effects of Open Surgery on Patients with Extensive Skull Base Osteoradionecrosis Through a Personalized Sequential Approach: A Case Series

Abstract:The present study aimed to investigate outcomes following open surgery for extensive skull base ORN. Open surgery through a personalized sequential approach was employed to deal with five cases of extensive skull base ORN. Two patients with mild cases underwent regional debridement and sequestrectomy, and three patients with severe cases underwent extensive resection with reconstruction using free anterolateral thigh (ALT) flap. Biological glues and vascularized flaps were used for obturation of the skull base bony defect to prevent postoperative cerebrospinal fluid (CSF) leakage. The infections were controlled by antibiotic administrations which strictly followed the principles of antimicrobial stewardship (AMS). As results, both regional debridement plus sequestrectomy and extensive resection achieved satisfied outcomes in all patients. No severe complications and delayed hospitalization occurred. During the follow-up period (8-19 months), all patients were alive, pain free, without crusting or purulent discharge, and no sequestration or CSF leakage occurred. In conclusion, a personalized sequential approach including open surgery, pedicled/vascularized free flap reconstruction and AMS was advocated for patients with extensive skull base ORN.
Clinical Analysis of 18 Cases of Chordomas in the Skull Base

Abstract:Objective: To investigate the diagnosis and treatment of chordomas in the skull base.Methods: The clinical data,including image presentation,operative finding,histological diagnosis and prognosis of 18 patients with chordomas in the skull base who underwent surgery 1996-2004,were reviewed.Results: No distinctive symptoms occurred in these patients. Total resection was achieved in 5 cases,and subtotal resection in 13 cases.17 cases were typical chordomas and 1 case was chondroid chordomas.14 patients were followed up,4 died,10 survived.Conclusion: Chordomas in the skull base is an rare tumor and difficult to perform total resection.Microsurgery combined with radiotherapy or(γ-knife) therapy is recommended.The prognosis is very poor.
The Surgical Management of Nasal Skull Base Schwannoma under Endonasal Endoscope: a Retrospective Review of 52 Cases

Abstract:Objective: To discuss the surgical technique and outcome of nasal skull base schwannoma treated under endonasal endoscope. Methods: Fifty-two patients with nasal skull base nonvestibular schwannoma were treated under endonasal endoscope from May 2006 to June 2017 in Shanghai E&ENT Hospital. Of the patients, there were 21 men and 31 women. The age of the patients ranged from 33 to 71 years.Schwannoma mainly came from trigeminal nerve.Clinical symptoms included facial numbness, facial pain, nasal obstruction, headache, hypopsia, diplopia and tinnitus. Surgical approaches included pure endonasal endoscope approach (18 cases) and endoscopic endonasal resection asissted with sublabial transmaxillary approach (34 cases). Results: Total tumor resection was achieved in 42 patients(80.8%), subtotal resection in 8 patients, and partial resection in 2 patients. The relief rate of facial numbness, facial pain, nasal obstruction, headache, hypopsia, diplopia and tinnitus were 68.8%, 45.5%, 100.0%, 90.0%, 60.0%, 40.0% and 100.0%, respectively. During surgery, cerebrospinal fluid leakage was observed in 5 cases. All cases were successfully repaired with a nasoseptal flap or autologous fascia lata and fat obtained from thigh. Four cases with recurrence were observed in the follow-up period (6-134 months). Conclusion: Endonasal endoscopic approach is a safe and feasible procedure for schwannoma in nasal cavity, paranasal sinus, orbital apex, pterygopalatine fossa, infratomporal fossa, cavernous sinus and Meckel cave.

Abstract:Background The current study presents the effort of a global collaborative group to review the management and outcomes of malignant tumors of the skull base worldwide. Patients and methods A total of 28 institutions contributed data on 3061 patients. Analysis evaluated clinical variables, survival outcomes, and multivariable factors associated with outcomes. Results The median age was 56 years (IQR 44–67). The open surgical approach was used in 55% (n = 1680) of cases, endoscopic resection was performed in 36% (n = 1087), and the combined approach in 9.6% (n = 294). With a median follow‐up of 7.1 years, the 5‐year OS DSS and RFS were 65%, 71.7% and 53%, respectively. On multivariable analysis, older age, comorbidities, histology, dural/intracranial involvement, positive margins, advanced stage, and primary site were independent prognostic factors for OS, DSS, and RFS. Adjuvant RT was a protective prognostic factor. Conclusion The progress across various disciplines may have contributed to improved OS and DSS in this study compared to previous reports.

Abstract:Objective: To review our single-institution experience in the surgical management of complex skull base tumors using multimodal image fusion technology. Methods: From October 2019 to January 2022, 7 cases of complex skull base tumors that performed preoperative multimodal image fusion in Zhuhai People’s Hospital neurosurgery department were involved in this study. The image data were uploaded to the GE AW workstation. Corresponding image sequences were opened in the workstation to complete registration fusion and 3D reconstruction. We retrospectively reviewed the clinical and imaging data, and surgical strategy, respectively. Results: one case of recurrent C2 schwannoma, 1 case of recurrent spindle cell tumor of the left cranio-orbital communication, 1 case of lobular malignant tumor of the left infratemporal fossa, 1 case of central giant cell repairing granuloma, 1 case of mesenchymal malignant tumor in left pharyngeal process, 1 case of meningioma in jugular foramen, and 1 case of hemangioblastoma with vascular malformation in fourth ventricular. All cases underwent preoperative multimodal image fusion for the surgical plan and all cases had gross total resection. Except for one case of mesenchymal malignant tumor in left pharyngeal process that had dysphagia and one case of hemangioblastoma that had discoordination, others cases were without postoperative complication. Conclusions: Preoperative multimodal image fusion and surgical approach simulation benefit complex skull base tumor surgical treatment. Abstract:Objective: To review our single-institution experience in the surgical management of foramen magnum tumors via a far-lateral approach using an oblique straight incision. Methods: From October 2023 to January 2024, four cases of tumors in the foramen magnum area treated at the Capital Medical University-affiliated XuanWu hospital neurosurgery department were involved in this study. All cases were managed with a far-lateral approach using an oblique straight incision. We retrospectively reviewed the clinical and imaging data, as well as the surgical strategies employed. Results: Three cases of foramen magnum meningiomas and one case of glioma of the ventral medulla. All cases underwent a far-lateral approach using an oblique straight incision; all cases had a gross total resection, and the wounds healed well without cerebral fluid leakage or scalp hydrops. Except for one case of right foramen magnum meningioma, which had dysphagia and pneumothorax, the other cases were without any postoperative complications. Conclusion: A far-lateral approach using an oblique straight incision can preserve muscle integrity and minimize subcutaneous exposure, allowing for complete anatomical reduction of muscles. This craniectomy method is simple and replicable, making it worthy of further clinical practice.

Minimal Invasive Trans-Eyelid Approach to Anterior and Middle Skull Base Meningioma: a Preliminary Study of Shanghai Huashan Hospital.

Abstract:Transpalpebral or trans-eyelid approach is a modified trans-orbital access to lesions of anterior cranial fossa and sellar region. But whether this approach is also suitable for tumors extending laterally to the temporal lobe or middle cranial fossa is not clarified. We would like to share our experiences from the cadaveric anatomy study to clinical operations. We used 5 cadavers to study trans-eyelid approaches in a step-by-step fashion. And then assisted by an experienced ophthalmologist for incisions, we treated 3 female patients via this approach: One with spheno-orbital meningioma, one with sellar tuberculum meningioma, and the other with medial sphenoidal wing meningioma. After studying the cadavers, we made several revisions to the previously reported approach: 1) move the incision close to the edge of the eyelid, which resembled the double-eyelid incision. 2) A vascularized periosteum flap was dissected for repairing the opened frontal sinus and reconstruction of the skull base. 3) The dura was sutured up with a slice of temporalis muscle. Then we treated 3 patients by this approach. All tumors were totally resected as Simpson Grade I. Complications included orbital apex syndrome and transient oculomotor paralysis because of tumor invasion into orbit and cavernous sinus. No cerebrospinal fluid leakage. We find that trans-eyelid approach is suitable for lesions not only at anterior cranial base or sellar region, but also extending to middle cranial base, especially around sphenoidal wings within 2 cm range or spheno-orbital region. Thus, we propose whether it appropriate to nominate this approach as 'trans-eyelid pterional approach', since it may treat some anterior and middle cranial fossa lesions with a mini-craniotomy around pterion.
