Abstract:Objective:To discuss the clinical significance of endocope and neuronavigation in transnasal transsphenoidal approach for the treatment of the tumor in the sellar region. Methods:From Jan. 2007 to Nov. 2008,68 patients underwent pure transnasal transphenoidal endocopic surgery for sellar region tumors. The neurovaigation was used in 6 cases. There were 24 males,44 females. Aged from 16-75 years old. CT scan and MRI examaination were preformed in all patients. Among them,there were 22 cases of microadenoma,29 cases of macroadenoma,5 cases of invasive pituitary adenoma,5 cases of pituitary apoplexy and 7 cases of Rathke's cleft cyst. Comprehensive endocrinologic measurement revealed 28 cases with hyperprolactinemia,4 cases with GH hypersecretion,21 cases with hypopituitarism and 3 cases met the endocrinologic criteria for Cusing's disease.There were 12 cases with normal result. Results:The tumor was totally removed in 55 cases,subtotally removed in 11 cases and partially removed in 2 cases. After operation,the vision of patients was improved in 28 cases,no change in 4 cases and worsen in 2 cases during the period of hospitalization.In patients with Cusing's disease,the plasma cortisol and 24 hours urinary free cortisol of 2 cases decreased to a level much lower than normal state and need hormone replacement therapy in the early stage postoperation. In the left 1 case,the plasma cortisol and 24 hours urinary free cortisol decreased to normal state and the replacement therpay was not needed. The clinical presentation of all these 3 cases were improved apparently. CSF leakage occurred in 3 cases which were cured with contiunous spinal CSF drainage.Acute visual loss occurred in 1 case because of intrasellar excessive packing. The vision was improved with another decompression surgery. In an invasive pituitary adenoma case,an intracavernous internal carotid artery pseudoaneurysm was found two months after operation. No complication such as nasal septal perforation and atrophic rhinitis occurred. Conclusion:The advantage of endoscopic transnasal transsphenoidal approach for the treatment of the tumor in the sellar region are the minimal injury to nasal cavity,no packing of the nasal cavity is needed after operation and provision of a wide view,especailly in the suprasellar region which can not be directly visualized under the tradiontional microcopic surgery. Incorporated with image-guided technique,the safty of the surgery and the rate of total tumor remove were improved.
