Hypercholesterolemia is an Aggravating Factor of Contrast Media-Induced Nephrotoxicity

Abstract:Objective To evaluate the effects of hypercholesterolemia on contrast media-induced changes in renal function and histopathology. Methods Rats were fed either a normal rodent diet(N group) or high cholesterol supplemented diet (H group, 4% cholesterol and 1% cholic acid). At the end of 4 weeks and 8 weeks, 6 rats from each diet feeding were then given a tail vein injection of either 60% diatrizoate (6 mL/kg, NC group and HC group) or vehicle. Toltal serum cholesterol, triglyceride, serum creatinine, creatinine clearance rate and the fractional excretion of sodium and potassium were determined on the second day following contrast media administration. Renal blood flow was determined by the Color Doppler flow imaging and pulsed-mode Doppler. Renal histopathology was observed by light microscopy. Results Total serum cholesterol increased significantly at the end of 4 weeks and 8 weeks. The creatinine clearance rate decreased and the fractional excretion of potassium increased in the rats of group HC at the end of 4 weeks. Increased serum creatinine and fractional execretion of sodiunm besides the same changes as that at the end of 4 weeks were also found in group HC at the end of 8 weeks. Only increased fractional excretion of potassium was found in the rats of group NC at the end of 4 weeks and 8 weeks. Creatinine clearance rate decreased in the rats of group H at the end of 8 weeks. The resistance index of the rats in group HC at the end of 4 weeks and in group H and group HC at the end of 8 weeks increased significantly. The production of nitric oxide in renal cortex decreased in rats of group H and group HC at the end of 8 weeks. Focal necrosis?diffuse necrosis were found in group HC at the end of 4 weeks and 8 weeks respectively. Only degeneration was found in group NC and group H at the end of 4 weeds and 8 weeds. Conclusion Hypercholesterolemia is an aggravating factor of contrast media-induced nephrotoxicity. Reduction of the nitric oxide production may mediate the development of acute failure induced by contrast media.
