Lipoprotein glomerulopathy:a pathological anallysis of six cases in China

Abstract:Objective:Lipoprotein glomerulopathy(LG)has been reported in recent years as a rare glomerular disorder in which the pathological findings are essential for the establishment of the diagnosis.In this study,we reported 6 cases of LG and focused on the pathological features under light microscopy,immunohistochemisty and electronic microscopy. Methodology:This is a retrospective analysis of the pathological data of 6 cases of LG patients whose main clinical manifestation was glomerular proteinuria,or nephrotic syndrome,and prominent histological feature was capillary clot under light microscopy.The biopsy specimen were processed routinely for light microscopy(2μm sections);immunohistochemistry for immunoglobulins,complement components,apoproteins(apoA,apoB and apoE)(5μm sections);and electronic microscopy.PAP assay was applied in the detection of apoA,apoB and apoE by using monoclonal antibodies on cryostatic sections. Results:In all of the six cases,increments of glomerular size and capillary lumen space were observed.Prominent lucent casts were found extensive in the capillary lumen,that were stained as pale mesh like substance,called lipoprotein thrombi.With immunohistochemistry,these casts were stained strongly positive for apoA,apoB and apoE.In the mesangial area,these apoproteins were also stained positive.Under electronic microscopy,granules and vacuoles of various size were observed in these capillary luminal casts. Conclusion:Lipoprotein glomerulopathy is pathologically characterized by extensive glomerular capillary casts which are stained positive for apoA,apoB and apoE.
