Antegrade catheterization in comprehensive interventional treatment of chronic deep venous thrombosis in lower extremities

Abstract:Objective To evaluate the clinical effectiveness of antegrade catheterization in combined interventional treatment of chronic lower extremity deep venous thrombosis (LEDVT). Methods Forty patients with thrombosis in longer segment, whole segment or multi-segments of the iliac and (or) femoral veins were included in the study. Inferior vena cava filters (IVCF) were placed in all patients via the normal femoral veins. Antegrade puncture and catheterization was carried out through the popliteal veins in the thrombotic side. Combined interventional therapies were accomplished, including intravascular thrombolysis, thrombosis aspiration, thrombosis amplification with Amplatz Thrombectomy Device (ATD), angioplasty, and implantation of stent. Results The successful rate of percutaneous catheterization in the popliteal veins was 87.5% (35/40). In the other five unsuccessful cases, the catheterization in the popliteal veins was achieved successfully after exposing the popliteal veins by operations. Total effective rate after combined interventional treatment was 100%. No serious complications were observed. Conclusion Combined interventional treatments of chronic LEDVT through the popliteal veins is safe and effective.
