Abstract:Objective With the rapid growth of modern imaging studies,more and more small adrenal lesions had been diag-nosed.Retroperitoneoscopic adrenal surgery for small adrenal tumors(≤1cm) can be challenging owing to its features and difficulties.Sometimes,they can not be found during operation.Here we present our experiences of using retroperitoneoscopic surgery to treat small adrenal tumors.Methods We retrospectively reviewed 389 consecutive retroperitoneoscopic adrenalectomies from September 2005 to December 2008,96 of them were small adrenal tumors.All the surgeries were performed retroperitoneally.During surgery,the internal part of the adrenal gland closing to the retroperitoneum was liberated firstly,and the whole adrenal gland was dissected completely.We performed partial adrenalectomy for adrenal tumors and total adrenalectomy for adrenal hyperplasia.Results Conver-sions to open surgery were performed in 4 patients(3.5%).Reasons for conversions were: Missing of target in 2 patients,massive hemorrhage caused by central adrenal vein injury in 1 patient,severe adhesion in 1 patient.Average diameter of the adrenal tumors were 0.7cm(range 0.5-1.0) including 76 cases of primary hyperaldosteronism(69 unilateral adenomas;7 unilateral hyperplasia),11 nonfunctional adrenal adenomas,3 Cushing’s syndrome,2 pheochromocytoma,2 myelopiloma,1 adrenal metastasis,1 melanoma),The operative time in the initial 40 cases was signifigantly longer than that in the subsequent 56 cases(P<0.01).However,no signifi-cant correlation was observed between estimated blood loss and the number of procedures.Conclusion Although laparascopic a-drenalectomy is the goldern standard for benign adrenal diseases,retroperitoneoscopic surgery for small adrenal tumors can be chal-lenging for its features and difficulties.Liberating the internal part of the adrenal gland closing to the retroperitoneum firstly and ex-ploring the whole adrenal tissue intraoperatively are the key points during the operations.
