Efficacy of Local Anesthesia for Prostate Needle Biopsy by Periprostatic Lidocaine Injection Guided by Transrectal Ultrasound:A Prospective Randomized Clinical Trial

Abstract:Objective To assess the safety and efficacy of lidocaine periprostatic local anesthesia during transrectal ultrasound guided prostate biopsy. Methods A total of 150 consecutive patients who would undergo transrectal ultrasound guided 10-core needle biopsy of the prostate were randomly divided into 3 groups.Of them 50 cases each received an injection of 1% lidocaine (group 1)or normal saline(group 2)in the lateral and apical periprostatic region;the other 50 cases without any injection served as controls(group 3).Immediately after biopsy the intra-procedure pain was assessed by visual analogue scale (VAS) and the complications were recorded and evaluated.At the 7th day the complications and their severity were evaluated again. Results The pain scores in group 1,2 and 3 were 1.96,3.98 and 3.70,respectively,which showed that the pain was significantly reduced in lidocaine group ( P 0.05).There was no significant difference of pain scores between group 2 and 3 ( P 0.05 ).In the lidocaine group,no infection occurred,the amount of rectal bleeding was (3.96±0.39)ml.The rate of gross hematuria,difficult voiding,urinary retention,mild vasovagel episode in group 1 was 76%(38/50),12%(6/50),2%(1/50) and 10%(5/50),respectively.There was no significant difference of the incidence of complications among the 3 groups( P 0.05 ). Conclusions Lateral and apical periprostatic anesthesia can significantly diminish the pain and discomfort in those undergoing transrectal prostate biopsy without increasing the incidence of complications.We recommend that this procedure be routinely applied to all patients undergoing transrectal prostate biopsy.
