Intra-abdominal hypertension impairs multiple organs and functions after liver transplantation (report of seven cases)

Abstract:Objective To investigate the causes of intra-abdominal hypertension after liver transplantation and its impairment to multiple organ functions in 7 cases. Methods The intra-abdominal pressure was monitored indirectly by intravesical pressure (intravesical pressure20mmHg was regarded as indicoting intr-abdominal hypertension). Blood creatinine, blood urea nitrogen levels and heart rate, mean arterial pressure , central venous pressure, pulmonary artery pressure ,arterial oxygen partial pressure / fraction of inspired oxygen, time of mechanical ventilation were also recorded for 3 days. Results Five cases presented acute renal impairment, and 5 cases had hypoxemia, the parameters of hemodynamics were abnormal in some cases. One of the cases died of multiple organ dysfunction syndrome on the 3rd postoperative day. Conclusions Patients undergoing liver transplantation are frequently at risk of intra-abdominal hypertension, which impairs multiple organ functions, and especially the renal and respiratory functions. It is beneficial to monitor the intra-abdominal pressure in patients undergoing liver transplantation.
