Risk factors for postoperative liver dysfunction in patients with cirrhosis and portal hypertension

Abstract:Objective  To explore risk factors for postoperative liver dysfunction in patients with cirrhosis and portal hypertension. Methods  The clinical data of 124 patients with cirrhosis and portal hypertension who were admitted to the 422 Hospital of PLA from April 2012 to June 2016 were collected and retrospectively analyzed. According to postoperative liver function recovery, they were divided into liver dysfunction group and better liver function group. Univariate and multivariate analyses were used for data analysis to explore risk factors of postoperative liver dysfunction in patients with cirrhosis and portal hypertension. Results  Liver dysfunction occurred in 26 patients (21.0%), and liver failure-caused mortality was 0.8% (1/124) among the 124 cirrhosis and portal hypertension patients who underwent operative management. Significant differences were observed between the two groups in preoperative total bilirubin (TBIL), preoperative albumin (ALB), preoperative prothrombin time (PT), the extent of ascites, indocyanine green retention rate at 15 minute (ICGR15), plasma clearance of indocyanine green (ICGK) and effective hepatic blood flow (EHBF, P<0.05). Multivariate logistic regression analysis results show that preoperative TBIL, the extent of ascites and ICGR15 had significant impact on the postoperative liver dysfunction in patients with cirrhosis and portal hypertension (P<0.05). Conclusions  Preoperative TBIL, the extent of ascites and ICGR15 are independent risk factors for liver dysfunction after surgery in patients with cirrhosis and portal hypertension. Precise preoperative assessment based on preoperative TBIL, the extent of ascites and ICGR15 and appropriate surgical timing are beneficial to reduce the incidence of liver dysfunction after surgery in patients with cirrhosis and portal hypertension and improve the postoperative effect.
