Arthroscopic release around the greater trochanter in gluteal muscle contracture

Abstract:Objective Gluteal muscle contracture is common to be seen after repeated intramuscular injections. Surgery is necessary in severe cases. Massive release was always needed on the basis of the extent of contractile band, which would result in muscular weakness, wound hematoma, long-time surgery, even neurovascular injuries. The study reported arthroscopic release around the greater trochanter of the femur, which is a new minimally invasive method in arthroscopic surgery. Methods We retrospectively reviewed 175 patients with bilateral gluteal muscle contracture ( 64 males, 111 females ) with the mean age of ( 25.1 ± 5.4 ) years ( range: 17 - 40 years ). Each patient had the history of repeated intramuscular injections. Patients were divided into 2 group according to the type of surgery: group A included 91 patients who underwent the release around the greater trochanter, group B included 84 patients who underwent the release of the contracture band in the gluteal muscle. The following signs were positive in all the patients before the surgery: squatting and crouching disability, difficulty in crossing the leg, Ober's sign, clicking sound during the rotation of the hip. In the arthroscopic release around the greater trochanter, the iliotibial band was transversely released nearly under the greater trochanter at first, then interior and superior of the greater trochanter were reaeased according to the clicking sound and muscular tension. The released target area shaped as "C". Gluteal muscle contracture disability scale ( GDS ) was evaluated before the surgery and at the latest follow-up. Results The average operating time for one side of gluteal muscle contracture under the arthroscope was ( 12.0 ± 3.5 ) min in group A, and ( 21.0 ± 5.8 ) min in group B ( P < 0.05 ). All the patients were followed up for a minimum of 1 year after the operation. At the latest follow-up, all could crouch with both knees close to each other, sit with their legs crossed, had no Ober's sign or clicking sound during the rotation of the hip. There were no neurovascular injury,recurrent contracture of the hip abductor, residual hip pain. There was 1 case of wound hematoma in group A and 4 cases in group B. One patient developed muscular weakness ( level IV ) in group A and 6 cases in group B. GDS was 23 - 71 ( mean, 49.0 ± 16.2 ) in group A and 24 - 69 ( mean, 48.2 ± 14.8 ) in group B before the surgery ( P > 0.05 ), 82 - 100 ( mean, 90.7 ± 6.5 ) in group A and 83 - 98 ( mean, 89.3 ± 5.0 ) in group B ( P > 0.05 ) at the latest follow-up. Conclusions Arthroscopic release around the greater trochanter has advantages of less interference with the muscle, shorter operation time, and it is a new minimally invasive method with arthroscope.
