The cause analysis and management of sudden drop in blood pressure and heart rate during interventional procedure

Abstract:Objective To analyze the causes and features of patients with sudden drop in blood pressure and heart rate during interventional procedures.Methods Between 1997 and 2002,a total of 5 472 patients received interventional therapy,among which 229 patients experienced sudden drop in blood pressure and heart rate(16 acute cardiac tamponade,213 vasovagal depression) when interventional procedures were enrolled in this study.The patients with vasovagal depression grouped as group 1,the others group 2.In the early stage of this study,we used atropine in 50 patients with sudden drop in blood pressure and heart rate and found that the efficacy of atropine is rather low,and since then 5～10 mg dopamine instead of atropine was given intravenously to the others.Pericardial drainage was performed in those patients whose blood pressure and heart rate could not be corrected to normal after drug therapy.Results The patients 'blood pressure and heart rate in group 2 had a good response to intravenous dopamine in 3 minutes,but group 1 had not.Conclusion The find-ing that patient did not have a good response to intravenous dopamine may indicate cardiac tamponade or other serious complications.The diagnosis of cardiac tamponade can be made if transparent area around the heart under LAO project fluorography.Urgent pericardial drainage or even pericardectomy is essential for life saving.
