Effects of Sufentanial PCEA Analgesia on Plasma Levels of Endothelin-1 and Atrial Natriuretic Peptide in Patients with Hypertension after Operation

Abstract:ObjectiveTo observe the effects of sufentanil percutaneous epidural auto-regulatory analgesia (PCEA) on plasma levels of endothelin-1 (ET-1) and atrial natriuretic peptide (ANP) in patients with hypertension after operation.Thirty-six cases with hepertension intended to undergo upper abdominal operation were divided randomly into 3 groups: ①Group A, served as control, pethidine im. was injected intermittently after operation according to requirement;② Group B, PCEA was carried out after operation; ③Group C, PCEA plus sufentanial 50ug in local anesthetic. The plasma levels of ET-1 and ANP before operation, at the end of operation, 24h and 48h after operation were estimated by radioimmunoassay(RIA). Their heart rate (HR) and blood pressure (BP) were monitored and VAS were assessed.The plasma levels of ET-1 and ANP during 24h after operation in groups B and C were significantly lower than those in group A (P0.01), the amplitude of decline in group C was more prominent than that in group B (P0.01). HR, systolic BP(SBP), diastolic BP(DBP) and VAS after operation in groups B and C obviously decreased as compared with those of the same period in group A (P0.05), group C showed figures even lower than those of group B (P0.01).[Conclusions]Postoperative PCEA,especially supplemented with sufentanial, can effectively reduce the pain of patients and improve their circulatory function. Lowered ET-1 and ANP levels in patients with hypertension may decrease the postoperative complications.
