Influences of Different Anticoagulant Intensity on Coagulation Activation in Patients after Mechanical Valve Replacement

Abstract:Objective: To study the influence of different intensities of anticoagulant on coagulative activation in patients with mechanical heart valve prostheses. Methods: 162 patients with cardiac valve replacement were randomized into 4 groups, according to anticoagulant intensity. Group A (INR1 5), 25 patients; group B (INR 1 5~1 99), 44 patients; group C (INR 2 0~2 99), 61 patients; group D (INR 3 0~4 5), 32 patients. The control group included 20 health persons. Prothrombin fragment 1+2 (F 1+2 ) and D dimer were measured by ELISA technique. Results: In group A, B, C, D and control group, F 1+2 values were (2 48± 1 32), (1 23±0 65), (0 81±0 45), (0 73±0 39) and (1 11±0 35) nmol/L, respectively. D dimer values were (0 45±0 24), (0 30±0 17), (0 24±0 10), (0 25±0 11) and (0 21±0 09) mg/L, respectively. F 1+2 values in group A were significantly higher than that in normal controls (P0 01), and in group C and D were significantly lower than that in normal controls (P0 05), while there was no significantly difference between group B and controls, as well as group B and group C. D dimer values in group A and B were markedly higher than that in normal group (P0 05), group C and D were no different from normal controls (P0 05). Conclusion: Anticoagulation therapy following the guideline of INR 2 0~3 0 significantly inhibited against blood coagulative activation in patients with mechanical heart valve prostheses.
