Subclassification of Idiopathic Membranous Nephropathy According to the Molecular Pathologic Features

Abstract:Objective:To study the molecular pathogenetic characteristics of idiopathic membranous nephropathy (IMN) and to investigate the role of subclassification of IMN on directing the therapeutical strategies and assessing the prognosis. Methodology:Twenty-one patients with IMN were enrolled in this study. Immunofluorescence double-staining and confocal microscopy were used to examine the configuration of subepithelial immune complex、C5b-9 and WT1 around the glomerular basement membrane(GBM). Immunohistochemical method was used to detect the distribution of IgG subclasses in renal tissues. Results:①The renal tissues of twenty-one patients were demonstrated to have two different types, named IMN of GBM proliferation type and GBM non-proliferation type. ②The deposition of IgG4 was stronger in GBM proliferation type. ③In GBM proliferation type, the deposition of C5b-9 was more frequent and stronger than that in GBM non-proliferation, and the density of podocytes was decreased in the former. Conclusion: Based on the different configuration of subepithelial immune complex and changes of GBM, We could divide IMN into two subtypes: GBM proliferation type and GBM non-proliferation type. These two subtypes of IMN had different molecular pathologic features.
