Experimental Study on MR Virtual Colonoscopy of Simulated Mass Lesions in Pig Colon

Abstract:Objective To study the techniques and diagnostic value of MR virtual colonoscopy (MRVC) by comparing with other postprocessing techniques of MR hydrography and colonoscopy.Materials and Methods Two segments of pig colon about 25cm long were resected in vitro, and 10 mass lesions with the size of 3~12mm in diameter were created. Using T 2weighted fast spinecho pulse sequences (HT 2FSE) and thin and thick singleshot fast spinecho pulse sequences (SSFSE), MR scanning was performed after the loop was properly distended by water. Five sets of postprocessed images were performed by one experienced doctor with an Advantage Window workstation using navigator software based on source imaging data. One segment underwent electron colonoscopy.Results MRH, especially the postprocessing images of MIP and SSD, displayed the colon quite similar to the appearance of single contrast barium exam, and MRVC showed colon morphology similar to colonoscopy. The source imaging data of thick SSFSE sequence couldn't be postprocessed. The average postprocessing time of MRVC, SSD, targetMIP, MIP and MPR was about 5′,30″,20″,18″, and 15″, respectively. The sensitivity of MRVC for detection of colon mass over 3mm in diameter with SSFSE or HT 2FSE sequences was 20% and 100%, respectively, while the accuracy was 11.1% and 83.3%, respectively. When combined the fourinone multiview images, source images and other reformations, especially MPR, the MRVC could accurately demonstrate the lesion site and make differentiation with small air bubbles, and the accuracy would be increased to 100%. Compared with colonoscopy, MRVC was not good for depicting the surface details of the colonic mucosa or the lesions. TargetMIP could detect lesions over 3mm in diameter, while SSD and MIP couldn't.Conclusion MRVC can provide noninvasive, endoscopelike display for the colon, need not to insert an instrument, can identify mass lesion as small as 3mm in diameter. The HT 2FSE sequence is very effective for postprocessing of MRVC.
