A Study on Pulmonary Blood Distribution in Patients Undergoing Extracardiac Total Cavopulmonary Connection

Abstract:Objective: To study blood distribution in extracardiac total cavopulmonary connection (ETCPC). Methods: To combine a bidirectional cavopulmonary anastomosis with a Gore-Tex extracardiac conduit interposition between the inferior vena cava and the main pulmonary artery, and to evaluate the changes of pulmonary blood distribution with SPECT in all surviving patients. Results: The cournts of pulmonary radionuclide was (313.7±40.1)×10 3 increased significantly after surgery (t=2.23, P0.05). The fraction of right lung/left lung blood perfusion in postoperation (1.14±0.15) was higher than the preoperation (1.02±0.12). And the upper/lower lobe perfusion ratio was 0.23±0.09, decreased significantly in all postoperative patients (t=3.19, P0.01). Conclusion: With ETCPC, there is a relatively dominant blood perfusion in the right lung. The decreased upper/lower lobe perfusion ratio in the postoperative period may be of clinical importance in correlation with the development of pulmonary arteriovenous fistula and the activity intolerance.
