Comparative Study of Effects of the Modified Salter Osteotomy with Classical Procedure in the Treatment of Congenital Dislocation of Hip

Abstract:Objective To compare the effects of modified Salter osteotomy and Batchelor plaster with that of the classical Salter procedure on the treatment of CDH. Methods 112 hips in 89 pediatric patients aged between 1.5 and 7 years with congenital hip dislocation were divided into two groups: ① The modified group included 42 cases(51 hips) and the osteotomy site for bone graft was devised to be located over the iliac crest 4cm away from the spina iliac anterior superior, the modified method conserved only part of the cortex of incisura ischiadica major with a chisel anter-oposteriorly. The double layer of illiac bone graft was inserted to the osteotomic site and the modified Batchelor plaster was used postoperatively with the hips fixed. ② The classical group of 47 cases (61 hips) was treated completely with the classical Salter osteotomy and Batchelor plaster. Results All patients were followed up for 1 to 11 years (average 7 years). According to Ji Shi-jun's criterion of the treatment of CDH, the good outcome rate of the modified group was 94.1%, while that of the classical group was only 77%. The two groups were compared with a chi-square test, P0.05. Conclusion The modified Salter osteotomy and Bathchelor plaster are more advantageous than the classical method in the treatment of CDH.
