Comparative Study Between Segmental Chemoembolization and Conventional Chemoembolization for Hepatocellular Carcinoma

Abstract:Objective To evaluate the efficiency of segmental transcatheter arterial embolization (TAE) for hepatocellular carcinomas(HCCs) and its influence on liver function as compared with conventional TAE Methods Segmental TAE was performed in 38 cases of HCCs and conventional TAE was done in 63 cases No remarkable differences were seen in the size of tumor, position, number, liver function, associated cirrhosis, clinical staging,blood supply of tumors,dose of anticancer drugs and number of treatment between two groups Surgical resections were carried out 4 to 8 weeks after chemoembolization in 6 cases of segmental group and in 10 cases of conventional group All patients were followed up for 1 to 3 years Results The 1 ,2 , and 3 year survival rates were 96 9%,77 3% and 60 0% in segmental group, respectively, and 69 8%, 45 3% and 26 4% in conventional group respectively Significant difference or very significant difference were found between the two groups Comparative study of pathology of the resected specimens showed that remarkable differences were also seen in the necrosis rates of tumors and invaded capsules although there were no statistical significances in extent of tumor shrinkage of tumors and thickness of capsules between two groups Observation of liver function in 6 months after embolization revealed more serious deterioration with extraordinary remarkable difference in conventional group than in segmental group Conclusion Segmental TAE possesses markedly stronger anticancerous effect but much less damage to liver function and hence is more superior in long term effectiveness than conventional TAE
