The primary focus of such research is to study the MRI contrast agents with organ-selectivity, high relaxivity and safety.

Abstract:Numerous advances have been made in X-ray technology in recent years. X-ray imaging plays an important role in the nondestructive exploration of the internal structures of objects. However, the contrast of X-ray absorption images remains low, especially for materials with low atomic numbers, such as biological samples. X-ray phase-contrast images have an intrinsically higher contrast than absorption images. In this review, the principles, milestones, and recent progress of X-ray phase-contrast imaging methods are demonstrated. In addition, prospective applications are presented.

Abstract:In recent years,the incidence of coronary artery disease in China has tended to increase.Early screening of patients with coronary artery disease may lead to the appropriate therapeutic intervention and reduce the incidence of serious adverse cardiac events.Nowadays,the common diagnostic imaging methods include invasive coronary angiography,echocardiography,multi-slice spiral CT,radionuclide imaging and magnetic resonance imaging.With the rapid development of CT imaging technology,in addition to coronary angiography,dual-source CT can conduct stress myocardial perfusion imaging.This "one-stop" examination can obtain coronary anatomical features and myocardial hemodynamic change at the same time,which provides the basis for proper clinical intervention.The research progress of clinical application of dual-source CT myocardial perfusion imaging is reviewed in this paper.

Abstract:Computed tomography (CT) is one of the most widely used noninvasive clinical imaging modalities because of its wide availability, low cost, high efficiency and high resolution. Although the resolution of CT is much higher than that of conventional radiography and many other imaging techniques, it is still difficult to distinguish subtle changes of soft tissues because of physical limitations. To increase the contrast of soft tissues and improve the accuracy of disease diagnosis, a variety of CT contrast agents have been developed. Small iodinated compounds have been widely used as clinical CT contrast agents, but their toxicity and short circulation time restrict their further application. To resolve these problems, nanosized CT contrast agents have been developed. In addition to iodine, nanoparticles based on heavy atoms such as gold, tantalum, lanthanides, and bismuth have been investigated as more efficient CT contrast agents than small iodinated compounds. These novel nanosized CT contrast agents have high biocompatibility and long circulation times, which are advantageous for visualization of blood vessels and tumors. Moreover, the combination of CT with other imaging techniques, such as magnetic resonance imaging and ultrasound imaging, could realize higher sensitivity and more personalized diagnostic information. In particular, theranostic CT contrast agents that integrate diagnosis with therapy in a single nanoplatform may serve as a nanomedicine capable of both noninvasive imaging and remote-controlled therapy. Herein, we introduce the principles of CT imaging and commonly used nanoscale CT contrast elements, including iodine, gold, bismuth, tantalum, and lanthanides. The recent advances, future development and existing problems of multifunctional nanosized CT contrast agents are reviewed.

Abstract:Science Progress, Volume 107, Issue 1, January-March 2024. X-ray computed tomography (CT) and magnetic resonance (MR) imaging are essential tools in modern medical diagnosis and treatment. However, traditional contrast agents are inadequate in the diagnosis of various health conditions. Consequently, the development of targeted nano-contrast agents has become a crucial area of focus in the development of medical image-enhancing contrast agents. To fully understand the current development of nano-contrast agents, this review provides an overview of the preparation methods and research advancements in CT nano-contrast agents, MR nano-contrast agents, and CT/MR multimodal nano-contrast agents described in previous publications. Due to the physicochemical properties of nanomaterials, such as self-assembly and surface modifiability, these specific nano-contrast agents can greatly improve the targeting of lesions through various preparation methods and clearly highlight the distinction between lesions and normal tissues in both CT and MR. As a result, they have the potential to be used in the early stages of disease to improve diagnostic capacity and level in medical imaging.

Abstract:诚如作者在综述中所述,近年来,CT设备在技术上实现了跨越式的发展,从单层螺旋CT到MSCT,扫描速度与完成一次设定范围的扫描时间均大大缩短.然而,作为CT检查的另一项重要技术:静脉增强的应用,却没有多少改变,如何更为合理、效果更佳却少有研究.作者在综述中提到了这一问题,并从碘对比剂的药代动力学到影响强化的不同因素对目前增强CT存在的问题进行了分析,的确应引起临床更大重视.然而,作者在论述中却忽视了增强CT的一个重要问题,即增强扫描的目的.实际上,增强CT扫描的目的主要可分为2个:血管结构的增强(包括CTA)与组织的强化,两者的强化目的不同,方法也不应一致。

Abstract:Different tissues and diseases have specific biological signals. The development of magnetic resonance imaging (MRI) contrast agents responding to these signals can not only improve the sensitivity of disease detection by MRI, but also increase the accuracy of disease diagnosis through molecular imaging of these disease signals. This paper gives a review of MRI contrast agents responding to albumin, pH, metal ion, enzyme, redox and other biological signals with the focus on the latest developments and perspectives.

Abstract:Past decades of research into contrast media injections and optimization thereof in radiology clinics have focused on scan acquisition parameters, patient-related factors, and contrast injection protocol variables. In this review, evidence is provided that a fourth bucket of crucial variables has been missed which account for previously unexplained phenomena and higher-than-expected variability in data. We propose how these critical factors should be considered and implemented in the contrast-medium administration protocols to optimize contrast enhancement.

Abstract:PET/CT which combines the functional metabolic information provided by PET with the anatomic morphological information obtained from CT, can simultaneously show both patho- physiological and structural changes, and therefore significantly increase the accuracy of the diagnosis. Abstract:The barium sulfate has been playing an important role as the contrast medium in gastrointestinal radiography and disease diagnosis. However, its application has been gradually reduced due to the limitation of its imaging effect and the progress of other imaging techniques. Here, the liquid metal gallium was proposed as an improved contrast agent, which was applied in the in vivo digestive tract radiography of the mice for the first time. Under the CT scanning, the gallium produced excellent contrast effect intuitively. According to the records of discharge time, the tissue sections of organs, the survival state and body weight, the liquid metal was proven to be capable and safe for gastrointestinal radiography. Further, the mixture of the gallium and the barium sulfate has been tested, which showed better performance in both contrast and detail. Therefore, with the characteristics of better imaging contrast effect and acceptable safety, the gallium and its mixture with the barium sulfate might be useful as potential candidates for digestive tract contrast agent in animal experiments, even possibly as alternative contrast agents for clinical use.

Abstract:Magnetic resonance imaging(MRI) is a powerful and non-invasive imaging technique that provides very high resolution.It shows both physiological and anatomical details to a certain extent and has no ionizing radiation.However,its resolution cannot meet the clinical needs in some cases,so contrast agents should be used to achieve higher resolution MR images.Data show that about 30% of MRI exams need the use of contrast agents.Gadolinium chelates is one of the most common clinical MRI contrast agents.Traditional gadolinium contrast agents lack specific targeting property,have short half-life and low relaxivity,and cause side effects when large doses of them are used.Recently,the field of nanomedicine-based contrast agents holds great promise for overcoming many traditional hurdles of contrast agents including specific targeting property,half-life,relaxivity,and toxicities.These nanomedicine-based contrast agents mainly include liposomes,dendrimers,micelles,nanoparticles,nanotubes and fullerenes.

Abstract:In recent times, magnetic resonance imaging (MRI) has emerged as a highly promising modality for diagnosing severe diseases. Its exceptional spatiotemporal resolution and ease of use have established it as an indispensable clinical diagnostic tool. Nevertheless, there are instances where MRI encounters challenges related to low contrast, necessitating the use of contrast agents (CAs). Significant efforts have been made by scientists to enhance the precision of observing diseased body parts by leveraging the synergistic potential of MRI in conjunction with other imaging techniques and thereby modifying the CAs. In this work, our focus is on elucidating the rational designing approach of CAs and optimizing their compatibility for multimodal imaging and other intelligent applications. Additionally, we emphasize the importance of incorporating various artificial intelligence tools, such as machine learning and deep learning, to explore the future prospects of disease diagnosis using MRI. We also address the limitations associated with these techniques and propose reasonable remedies, with the aim of advancing MRI as a cutting-edge diagnostic tool for the future.

Abstract:Magnetic resonance imaging (MRI) as one of the most important imaging methods is widely used in clinical diagnosis and basic life science research, with advantages including noninvasiveness, high/multiple soft tissue contrast, parametric imaging and the richness of information. During recent years, there have been many advances in the instrument technology and biomedical application of MRI. Advanced techniques, such as ultra-high field, ultra-fast and ultra-sensitive imaging, are now available for routine clinical usage and research settings. Also, the MRI technology is rapidly developing via incorporating with multiple cutting-edge disciplines including big data technology, artificial intelligence and theranostics. Currently, MRI is playing an indispensable role in tumor molecular imaging, functional brain imaging and brain connectome studies. This paper introduces the history and basic principles of MRI, and summarizes the important problems in the research of MRI instrument, methodology and application, providing a perspective on the directions of future MRI research in China.

Abstract:Current CT oral contrast agents improve the conspicuity and confidence for bowel and peritoneal findings in many clinical scenarios, particularly for outpatient and oncologic abdominopelvic imaging. Yet, existing positive and neutral oral contrast agents may diminish the detectability of certain radiologic findings, frequently in the same scans in which the oral contrast agent improves the detectability of other findings. With ongoing improvements in CT technology, particularly multi-energy CT, opportunities are opening for new types of oral contrast agents to further improve anatomic delineation and disease detection using CT. The CT signal of new dark oral contrast agents and of new high-Z oral contrast agents promise to combine the strengths of both positive and neutral oral CT contrast agents by providing distinct CT appearances in comparison with bodily tissues, iodinated IV contrast agents, and other classes of new CT contrast agents. High-Z oral contrast agents will unlock previously inaccessible capabilities of multi-energy CT, particularly photon-counting detector CT, for differentiating simultaneously administered IV and oral contrast agents; this technique will allow generation of rich 3D, intuitive, perfectly co-registered, high-resolution image sets with individual contrast-agent "colors" that provide compelling clarity for intertwined intraabdominal anatomy and disease processes.
A comparison of the use of contrast media with different iodine concentrations for enhanced computed tomography

Abstract:Objective: In this study, we compared the enhancement of blood vessels and liver parenchyma on enhanced computed tomography (CT) of the upper abdomen with two concentrations of contrast media (400 and 300 mg I/mL) based on similar iodine delivery rate (IDR) of 0.88 and 0.9 g I/s and iodine load of 450 mg I/kg. Methods: We randomly assigned 160 patients into two groups: iomeprol 400 mg I/mL (A group) and iohexol 300 mg I/mL (B group). The CT attenuation values of the main anatomical structures in the two groups with different scanning phases were measured and the image quality of the two groups was analyzed and compared. The peak pressure and local discomfort (including fever and pain) during contrast medium injection were recorded. Results: The mean attenuation value of the abdominal aorta was 313.6 ± 29.6 in the A group and 322.4 ± 30.1 in the B group during the late arterial phase ( p = 0.8). Meanwhile, the mean enhancement values of the portal vein were 176.2 ± 19.3 and 165.9 ± 24.5 in the A and B groups, respectively, during the portal venous phase ( p = 0.6). The mean CT values of liver parenchyma were 117.1 ± 15.3 and 108.8 ± 18.7 in the A and B groups, respectively, during the portal venous phase ( p = 0.9). There was no statistical difference in image quality, peak injection pressure (psi), and local discomfort between the two groups ( p > 0.05). Conclusion: When a similar IDR and the same iodine load are used, CT images with different concentrations of contrast media have the same subjective and objective quality, and can meet the diagnostic needs.

Abstract:X-ray computed tomography (CT) imaging can produce three-dimensional and high-resolution anatomical images without invasion, which is extremely useful for disease diagnosis in the clinic. However, its applications are still severely limited by the intrinsic drawbacks of contrast media (mainly iodinated water-soluble molecules), such as rapid clearance, serious toxicity, inefficient targetability and poor sensitivity. Due to their high biocompatibility, flexibility in preparation and modification and simplicity for drug loading, organic nanoparticles (NPs), including liposomes, nanoemulsions, micelles, polymersomes, dendrimers, polymer conjugates and polymeric particles, have demonstrated tremendous potential for use in the efficient delivery of iodinated contrast media (ICMs). Herein, we comprehensively summarized the strategies and applications of organic NPs, especially polymer-based NPs, for the delivery of ICMs in CT imaging. We mainly focused on the use of polymeric nanoplatforms to prolong circulation time, reduce toxicity and enhance the targetability of ICMs. The emergence of some new technologies, such as theragnostic NPs and multimodal imaging and their clinical translations, are also discussed.

Charting the Path Forward: CT Image Quality Assessment -- An In-Depth Review

Abstract:Computed Tomography (CT) is a frequently utilized imaging technology that is employed in the clinical diagnosis of many disorders. However, clinical diagnosis, data storage, and management are posed huge challenges by a huge volume of non-homogeneous CT data in terms of imaging quality. As a result, the quality assessment of CT images is a crucial problem that demands consideration. The history, advancements in research, and current developments in CT image quality assessment (IQA) are examined in this paper. In this review, we collected and researched more than 500 CT-IQA publications published before August 2023. And we provide the visualization analysis of keywords and co-citations in the knowledge graph of these papers. Prospects and obstacles for the continued development of CT-IQA are also covered. At present, significant research branches in the CT-IQA domain include Phantom study, Artificial intelligence deep-learning reconstruction algorithm, Dose reduction opportunity, and Virtual monoenergetic reconstruction. Artificial intelligence (AI)-based CT-IQA also becomes a trend. It increases the accuracy of the CT scanning apparatus, amplifies the impact of the CT system reconstruction algorithm, and creates an effective algorithm for post-processing CT images. AI-based medical IQA offers excellent application opportunities in clinical work. AI can provide uniform quality assessment criteria and more comprehensive guidance amongst various healthcare facilities, and encourage them to identify one another's images. It will help lower the number of unnecessary tests and associated costs, and enhance the quality of medical imaging and assessment efficiency.

The development, principles and primary applications of modern medicalimage equipment such as digital subtraction angiography, computed radiography, digital radiography, spiral CT,positron emission tomography/CT, and phase-contrast imaging are also described. Image post-processing is a veryimportant part of modern medical imaging equipment. Combined with medical a priori knowledge, modern CTscanners have even more powerful capabilities in computer-aided diagnosis.
