Treatment for patients with recurrent paroxysmal atrial fibrillation after pulmonary veins isolation

Abstract:Objective:To report the results of treatment for recurrent paroxysmal atrial fibrillation (AF) in 49 patients who underwent pulmonary veins isolation procedure.Methods:Anti-arrhythmia drugs (AADs) as initial therapy were administered to all patients for at least 3 months. Re-ablation as the secondary choice was considered only in two situations as follows: ①AADs was proven ineffective which defined as the frequency of AF attacks and duration of AF episodes reduced less than 50 per cent as compared with that before ablation, and ②AADs produced serious side effects.Results:After 3 months of AADs treatment, freedom from AF was achieved in 12 patients ( 24.5%), and 5 of them without AADs. Recurrent AF still occurred, but was well-controlled in 17 ( 34.7%) patients. AADs was ineffective in 20 ( 40.8%) patients and re-ablation session was performed in 5 ( 10.2%). After the second pulmonary veins ablation session, AF recurred in 2 patients, and AADs therapy was effective for them.Conclusion:Recurrent AF after pulmonary veins isolation can be eliminated or well-controlled by AADs with adjunctive re-ablation strategy in majority patients.
