Effects of Different Anesthesia on Controlled Hypotension for Ligation of Patent Ductus Arteriosus

Abstract:Objective The aim of this study was to investigate the effects of different anesthetic drugs and techniques on controlled hypotension for ligation of Patent Ductus Arteriosus (PDA). Methods Thirty-six ASA Ⅰ~Ⅱ PDA patients (male 21, female 17) undergoing ligation of Patent Ductus Arterious were studied. Their ages ranged from 1~8 years. The patients were randomly divided into two groups: Group Ⅰ(n=18) received lsoflurane combined with caudal block-induced hypotension; group Ⅱ (n=18) had the same induction of general anesthesia as in group Ⅰ but sodium nitroprusside (SNP) was used to induce hypotension. Blood samples were taken from peripheral veins before anesthesia, during hypotension, after operation and the day after operation for determination of serum IL-6?TNF-α. BP?HR?SPO 2 levels. Results The serum IL-6?TNF-α levels were comparable before anesthesia and after operation in group Ⅰ, but in group Ⅱ these levels increased significantly after operation (P 0.05) and had significant differences between the two groups (P 0.05). The HR increased significantly during SNP-induced hypostension in groupⅡ (P 0.05). Conclusions Isoflurane combined with caudal block induces hypotension and can keep good levels of cytokines and maintain a stable hemodynamic state during the ligation of Patent Ductus Arteriosus.
