Abstract:Background:The optimal treatment strategy for patent ductus arteriosus (PDA) in extremely preterm infants is currently highly controversial. This study aimed to evaluate the association between PDA treatment and short-term outcomes among extremely preterm infants.Methods:This cohort study included all extremely preterm infants (≤27 and 6/7 weeks) who were admitted to hospitals participating in the Chinese Neonatal Network from January 2019 to December 2021, and were diagnosed to have PDA by echocardiogram. PDA treatment was defined as medical treatment and/or surgical ligation of PDA during hospitalization. Short-term outcomes included death, bronchopulmonary dysplasia (BPD), death/BPD, retinopathy of prematurity, necrotizing enterocolitis, and severe brain injury. Multivariate logistic regression was used to evaluate the association between PDA treatment and outcomes. Subgroup analysis were performed among infants with different respiratory support on 3 and 7 days of life.Findings:A total of 2494 extremely preterm infants with the diagnosis of PDA were enrolled, of which 1299 (52.1%) received PDA treatment. PDA treatment was significantly associated with lower risk of death (adjusted odds ratio, 0.48; 95% confidence interval, 0.38-0.60). The decreased risk of death was accompanied by increased risk of BPD and death/BPD. In subgroup analysis according to respiratory support, PDA treatment was associated with lower risk of death among infants who required invasive ventilation. However, the beneficial effect on death was not significant among infants who did not require invasive ventilation.Interpretation:PDA treatment was associated with reduced mortality in extremely preterm infants, but this beneficial effect was mainly present among infants who required invasive ventilation.Funding:This study was funded by the Shanghai Science and Technology Commission's Scientific and Technological Innovation Action Plan (21Y21900800) and the Canadian Institutes of Health Research (CTP87518).
Effects of Hypothermic Cardiopulmonary Bypass on Internal Jugular Bulb Venous Oxygen Saturation, Cerebral Oxygen Saturation, and Bispectral Index in Pediatric Patients Undergoing Cardiac Surgery: A Prospective Study.

Abstract:The objective of this study was to evaluate the effect of hypothermic cardiopulmonary bypass (CPB) on cerebral oxygen saturation (rSO2), internal jugular bulb venous oxygen saturation (SjvO2), mixed venous oxygen saturation (SvO2), and bispectral index (BIS) used to monitor cerebral oxygen balance in pediatric patients.Sixty American Society of Anesthesiologists Class II-III patients aged 1 to 4 years old with congenital heart disease scheduled for elective cardiac surgery were included in this study. Temperature, BIS, rSO(2), mean arterial pressure, central venous pressure, cerebral perfusion pressure (CPP), and hematocrit were recorded. Internal jugular bulb venous oxygen saturation and SvO(2) were obtained from blood gas analysis at the time points: after induction of anesthesia (T0), beginning of CPB (T1), ascending aortic occlusion (T2), 20 minutes after initiating CPB (T3), coronary reperfusion (T4), separation from CPB (T5), and at the end of operation (T6). The effect of hypothermia or changes in CPP on rSO(2), SjvO(2), SvO(2), and BIS were analyzed.Compared with postinduction baseline values, rSO(2) significantly decreased at all-time points: onset of extracorporeal circulation, ascending aortic occlusion, 20 minutes after CPB initiation, coronary reperfusion, and separation from CPB (P < 0.05). Compared with measurements made following induction of anesthesia, SjvO(2) significantly increased with initiation of CPB, ascending aortic occlusion, 20 minutes after initiating CPB, coronary reperfusion, and separation from CPB (P < 0.05). Compared with induction of anesthesia, BIS significantly decreased with the onset of CPB, aortic cross clamping, 20 minutes after initiating CPB, and coronary reperfusion (P < 0.05). Bispectral index increased following separation from CPB. There was no significant change in SvO(2) during cardiopulmonary bypass (P > 0.05). Correlation analysis demonstrated that rSO(2) was positively related to CPP (r = 0.687, P = 0.000), with a low linear correlation to temperature (r = 0.453, P = 0.000). Internal jugular bulb venous oxygen saturation was negatively related to temperature (r = -0.689, P = 0.000). Bispectral index was positively related to both temperature (r = 0.824, P = 0.000) and CPP (r = 0.782, P = 0.000). Cerebral oxygen saturation had a positive linear correlation with CPP and a low linear correlation to temperature. Internal jugular bulb venous oxygen saturation had a negative linear correlation to temperature.Pre-and and early postbypass periods are vulnerable times for adequate cerebral oxygenation. Anesthetic management must aim to optimize the supply and demand relationship.
Impact of Manual Sustained Inflation vs Stepwise PEEP on Pulmonary and Cerebral Outcomes in Carotid Endarterectomy Patients

Abstract:BACKGROUND Recruitment maneuvers (RMs) are used to reduce pulmonary atelectasis in patients under general anesthesia, but they can lead to a decrease in cerebral hemodynamics. MATERIAL AND METHODS Thirty patients undergoing carotid endarterectomy were randomized to a manual sustained inflation (SI) group or a stepwise increase in PEEP (IP) group. During both RMs, the peak airway pressure (Ppeak) was maintained at 30 cmH₂O for 30 s. Electrical impedance tomography was used to evaluate pulmonary aeration changes. Mean velocity of blood flow in the middle cerebral artery (Vm) and cerebral oxygen saturation (rScO₂) was monitored intraoperatively. RESULTS IP improved lung aeration better at Ppeak=30 cmH₂O than SI (58.2±8.4% vs 46.0±8.3%, P=0.001) and this persisted until the end of surgery. Dorsal (dependent) ventilation 30 min after extubation in the SI group was lower than that before surgery (7.7±2.6% vs 9.9±3.8%, P=0.003). Vm and rScO₂ returned to baseline immediately after RM in the SI group, while it remained below baseline in the IP group (42.5±12.6 vs 50.9±18.8 cm/s, P<0.001 and 68.1±3.5% vs 70.6±3.7%, P=0.001). Heart rate declined significantly during RM only in the SI group (55.9±6.6 vs 52.2±6.9 bpm, P=0.008). CONCLUSIONS Compared with SI, IP performed better in improving lung aeration, with greater hemodynamic stability. IP resulted in slower recovery of cerebral blood flow and oxygenation.

Abstract:OBJECTIVE To analysis the effects of non-pulsating perfusion, flow, blood pressure(BP), temperature(T), hypothermia with circulatory arrest(HCA) and cerebral ischemia and reperfusion on bispectral index(BIS) and auditory evoked potentials(AEP).METHODS 20 cased were divided into two groups randomly: I group(high flow) and II group(low flow). During the stable state of anesthesia, BIS and AEP were recorded at 15 different times. Non-pulsating perfusion, flow, BP, T and hypothermia with circulatory arrest were also recorded simultaneously. RESULTS During cardiopulmonary bypass(CPB), BIS and AEP decreased markedly after aorta occlusion (p0.05)and in hypothermia with circulatory arrest (p0.01). BIS and AEP increased markedly after the recovery of circulation(p0.01)and after return of heart beat and the stop of CPB(p0.05).CONCLUSIONS Non-pulsating perfusion in CPB can markedly depressed the activity of cerebral electricity and consciousness. During CPB, low flow, hypotension, hypothermia can markedly depressed BIS and AEP. Hypothermia with circulatory arrest can induce δ waves of cerebral ischemia-reperfusion.
Prognostic Factors for Permanent Neurological Dysfunction after Total Aortic Arch Replacement with Regional Cerebral Oxygen Saturation Monitoring

Abstract:AbstractObjectiveTo explore the prognostic factors for permanent neurological dysfunction (PND) after total aortic arch replacement with regional cerebral oxygen saturation (rSO2) monitoring.MethodsThis retrospective study enrolled 98 type A aortic dissection aneurysm patients who underwent emergency total aortic arch replacement combined with deep hypothermic circulatory arrest and right axillary artery selective antegrade cerebral perfusion (SACP). Data such as age, gender, body mass index, preoperative coexisting disease, laboratory test results, intraoperative critical operation duration, and intraoperative rSO2 were collected, and the neurological prognoses in the hospital were recorded and grouped by severity. Multiple logistic regression analysis was performed on the statistically significant differences between the groups to screen the predictors of postoperative neurological complications in these patients.ResultsForty‐two patients had postoperative neurological complications, among which there were 29 cases (29.6%) of transient neurological dysfunction, and 13 cases (13.3%) of PND. Multiple logistic regression results showed that advanced age, preoperative low platelet count, prolonged hemostasis time and lowest relative rSO2 to baseline (ΔrSO2min) in each time period were risk factors for postoperative PND. The ROC curve measurement showed that the optimal cut‐off value of ΔrSO2min was 79.7%, and the area under the curve was 0.708 (95% confidence interval = 0.557–0.858), p = 0.016; the optimal cut‐off value of ΔrSO2min in SACP was 81.6%, and the area under the curve was 0.720 (95% confidence interval = 0.570–0.870), p = 0.011; the optimal cut‐off value of ΔrSO2min in cardiopulmonary bypass (CPB) was 80.8%, and the area under the curve was 0.697 (95% confidence interval = 0.554–0.840), p = 0.023.ConclusionIntraoperative ΔrSO2min that is lower than the basal level of about 80%, advanced age, preoperative low platelet count, and prolonged hemostasis time are predictors of PND after total aortic arch replacement.

Abstract:Abstract Background and Aims: Postoperative neurological dysfunction is a common noncardiac complication following cardiac surgery. Primary objective of our study was to evaluate the effects of cerebral oxygen desaturation on the neurological outcome in the postoperative period by monitoring regional cerebral oxygen saturation in off-pump coronary artery bypass grafting (OPCABG). Secondary objective was to study the correlation between regional cerebral oxygen saturation and mean arterial pressure (MAP) during grafting. Setting and Design: It was a prospective observational study in 100 patients of the American Society of Anesthesiologists physical status grading II-III of either sex, having triple vessel coronary artery disease undergoing OPCABG. Patient, having left ventricular ejection fraction (LVEF) <35%, carotid artery disease, on extracorporeal membrane oxygenator or intra-aortic balloon pump, preexisting neurological disorder with standardized mini-mental state examination (SMMSE) score <23, undergoing emergency surgery, re-do surgery, and co-existing valvular diseases were excluded from the study. Surgeries were conducted under general anesthesia with midazolam 0.1 mg/kg, fentanyl 5 μg/kg, and vecuronium 0.1 mg/kg. Anesthesia was maintained with intermittent boluses of fentanyl, vecuronium, and isoflurane. Materials and Methods: The SMMSE was performed preoperatively, 8 and 12 h after extubation. Patients with postoperative SMMSE score of 24–30 were allocated to Group A and patients with SMMSE score ≤ 23, were allocated to Group B. The cerebral oxygen saturation (rSO 2 ), heart rate, MAP, and arterial blood gas were monitored perioperatively. Statistical Analysis: The Chi-square test was used to compare categorical variables and independent Samples t -test was used to compare continuous variables. Pearson bivariate correlation test was used to correlate between the continuous variables. Results: The mean difference of right and left rSo 2 , between baseline value, and during saphenous vein graft (SVG) to obtuse marginal grafting were 8.70 ± 3.38, 8.78 ± 3.19 in group A and 18.8 ± 2.40, 18.91 ± 1.88, respectively, in group B and were found to be statistically significant. The mean difference of right and left rSO 2 , between baseline value and during SVG to posterior descending artery grafting were 6.48 ± 3.91, 7.10 ± 3.67 in group A and 18.1 ± 3.57, 17.9 ± 3.60 in group B and were found to be statistically significant. Mean difference and percentage variation from baseline of right and left rSo 2 and MAP from preinduction to grafting from left internal mammary artery to left anterior descending artery were found to be statistically insignificant. Conclusion: A decrease of regional cerebral saturation value by 25% from baseline or an absolute reduction of rSO 2 value by 50% and reduction in MAP from baseline, during the period of grafting was associated with postoperative neurocognitive dysfunction.

Abstract:A hypothermic circulatory arrest is usually used to correct thoracic aorta pathologies. The emergency treatment of acute type A aortic dissection and elective repair of aortic arch pathologies are the most common indications for using hypothermic circulatory arrest. A hypothermic circulatory arrest can also be used for surgical pulmonary thromboendarterectomy in patients with chronic thromboembolic pulmonary hypertension. Intervals with total circulatory arrest offer a clear surgical field for thrombus and emboli removal from the pulmonary artery branches. The price to pay for intermittent circulatory arrest during pulmonary thromboendarterectomy is postoperative neurological dysfunction due to brain hypothermia and hypoperfusion. A noninvasive method for cerebral monitoring during cardiac surgery is real‐time regional cerebral oxygen saturation (rSO2). Liu et al. report that continuous monitoring of rSO2 during surgical pulmonary thromboendarterectomy may reduce the long cerebral hypoperfusion time and prevent postoperative neurological dysfunction.

Relationship Between Cerebral Oxygen Metabolism During Coronary Artery Bypass Graft and Postoperative Mental Disorders in Geriatric Patients

Abstract:Objective To investigate the relationship between cerebral 0, metabolism during coronary artery bypass graft (CABG) performed with cardiopulmonary bypass (CPB) and postoperative mental disorders (POMD) in the elderly.Methods Thirty patients (13 male,17 female) aged 65-76 yr undergoing CABG performed with CPB were studied. Patients with senile dementia were excluded. The patients were premedicated with scopolamine 0.3 mg and pethidine 50mg. Anesthesia was induced with midazolam 0.05-0.1 mg·kg_(-1), propofol 0.2-0.5 mg·g_(-1), fentanyl 10-15μg·g_(-1) and vecuronium 0.1-0.3 mg·g_(-1) and maintained with isoflurane inhalation and intermittent iv boluses of fentanyl. Nasopharyngeal temperature (T) was maintained at 26℃-28℃) during CPB. Radial artery was cannulated for BP monitoring and blood sampling.Swan-Ganz was placed via right internal jugular vein (UV).Another catheter was inserted into right UV and threaded retrogradely until J point for blood sampling. Blood samples were taken from artery and UV simultaneously for blood gas analyses and determination of blood glucose and lactate concentration before anesthesia (T_1 ),immediately after induction of anesthesia (T_2), during splitting of sternum (T_3) when T was reduced to 28℃ (T_4) and rewarmed to 35℃(T_5 ) and at the end of bypass (T_6 ).Cerebral O2 delivery (CDO2 ),cerebral oxygen consumption (CMRO2),cerebral glucose consumption (CMRglu) and lactic acid production (ADVL) were calculated. POMD was assessed using CAM-ICU chart.Results (1)Seven patients developed POMD (7/30 or 23.3% ).(2) Low jugular bulb 02 saturation (SjO2 ≤50% or PjO2≤25% mm Hg) was observed in 8 patients during rewarming. The incidence was significantly higher in POMD patients (3/7 or 42.96%) than that in non-POMD patients (5/23 or 21.74%). (3) CBF/CMRO2 was significantly lower in POMD patients than in non-POMD patients during rewarming.(4) CMRO2/CMRglu was significantly lower but ADVL was significantly higher in POMD patients than in non-POMD patients during hypothermia, rewarming and at the end of CPB.(P0.05 or 0.01).Conclusion POMD is closely related to abnormal cerebral O2 metabolism during CABG performed under CPB in the elderly.
[Monitoring Cerebral Oxygenation Using Near Infrared Spectroscopy During Cardiopulmonary Bypass Surgery].

Abstract:To avoid cerebral hypoxia caused by the imbalance between cerebral oxygen supply and consumption, regional cerebral oxygenation of patients need to be monitored at real time during cardiopulmonary bypass (CPB) surgery, and the physiological parameters can be regulated and emergent treatment can be used according to it. Using the near infrared (NIR) instrument developed by our group, cerebral oxygenation of the patients under cardiac surgery was monitored. The instrument consists of a two-wavelength near infrared light source and two near infrared detectors. Hemoglobin concentration changes of regional cerebral tissue were calculated, and by steady-state spatially resolved spectroscopy (SRS) algorithm, regional cerebral oxygen saturation (rSO2) was also calculated. Physiological parameters of patients, such as mixed venous oxygen saturation (SvO2), were measured by another monitor during CPB. Hemoglobin concentration changes were easily disturbed, but the anti-disturbance ability of rSO2 was good. The value of rSO2 could be detected all over the surgeries, but SvO2 could be detected only during CPB. There were positive correlations between rSO2 and SvO2 in most of the patients, but the correlation coefficients were not very high. This was because SvO2 reflects the saturation of the main venous, but rSO2 reflects regional cerebral oxygenation. So the physiological meaning of rSO2 and SvO2 is different. The results indicate that cerebral oxygenation of patients can be reflected by rSO2 during CPB, while only monitoring SvO2 is not enough.
Cognitive Function Deterioration After Cardiopulmonary Bypass: Can Intraoperative Optimal Cerebral Regional Tissue Oxygen Saturation Predict Postoperative Cognitive Function?

Abstract:Objective Cognitive impairment is a common neurologic complication after cardiac surgery with cardiopulmonary bypass (CPB). This study evaluated postoperative cognitive function to determine predictors of cognitive dysfunction, including intraoperative cerebral regional tissue oxygen saturation (rSO 2 ). Design A prospective observational cohort study. Setting At a single academic tertiary-care center. Participants A total of 60 adults undergoing cardiac surgery with CPB from January to August 2021. Interventions None. Measurements and Main Results All patients underwent Mini-Mental State Examination (MMSE) and quantified electroencephalography (qEEG) 1 day before cardiac surgery, 7 days postoperatively (POD7), and POD60. Intraoperative cerebral rSO 2 was monitored continuously. For MMSE, no significant decrease in MMSE score was found on POD7 versus preoperatively (p = 0 09), but POD60 scores showed significant improvement compared with both preoperative (p = 0.02) and POD7 scores (p < 0.001). On qEEG, relative theta power on POD7 was increased versus preoperatively (p < 0.001), but it was decreased on POD60 (POD7 versus POD60, p 0.99). Baseline rSO 2 was an independent factor for postoperative MMSE. Both baseline and mean rSO 2 showed a significant influence in postoperative relative theta activity, whereas mean rSO 2 was the only predictor for the theta-gamma ratio (p = 0.04). Conclusions The MMSE in patients undergoing CPB declined at POD7 and recovered by POD60. Lower baseline rSO 2 indicated a higher potential for MMSE decline at POD60. Inferior intraoperative mean rSO 2 was related to higher postoperative relative theta activity and theta-gamma ratio, implying subclinical or further cognitive impairment.

Abstract:Degradation of wheat straw (WS) and alkaline hydrogen peroxide (AHP)-treated wheat straw (AHPWS) by Ruminococcus albus 8 and Ruminococcus flavefaciens FD-1 was determined by measuring the growth (OD600) of each bacterium and determining DM disappearance (DMD) of the substrate. Complex medium and defined medium with or without the addition of phenylpropanoic acid (PPA) and phenylacetic acid (PAA) were used. Tubes were incubated at 39 degrees C for 8 d. Both OD600 and DMD indicated that AHPWS was degraded to a much greater extent by either bacterium (R. flavefaciens FD-1, 60.8 +/- 1.8% and R. albus 8, 42.3 +/- 3.5%) vs untreated WS (R. flavefaciens FD-1, 16.5 +/- 1.8% and R. albus 8, 8.6 +/- 6%) in the complex medium. Most degradation occurred between d 1 and 4. With the complex medium, addition of PPA and PAA did not stimulate degradation by either bacterium. When the defined medium was used, the addition of PPA and PAA enhanced (P less than .05) degradation of AHPWS (39.6 +/- 2.6%) vs AHPWS with no added PPA and PAA (24.9 +/- 7.6%) by R. albus 8. There was no synergistic effect on degradation when the two species were co-cultured with either WS or AHPWS as the substrate. No effect of PPA and PAA on disappearance of AHPWS was observed for R. flavefaciens FD-1 or when the two bacteria were grown together. Dry matter disappearance analysis showed that R. flavefaciens FD-1 degraded AHPWS more rapidly (6.1 mg/d) than R. albus 8 did (4.2 mg/d) in complex medium.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract:BACKGROUND Urinary sodium excretion and angiotensin II (Ang II), which are linked in a physiological feedback mechanism, have both been described to be blood pressure-independent determinants of left ventricular hypertrophy in essential hypertension. We conducted a study to investigate the interaction of sodium excretion with Ang II and its potential impact on myocardial hypertrophy. METHODS AND RESULTS Sixty-eight patients (46 men and 22 women; mean age, 52 +/- 10 years) with untreated World Health Organization stage I to II essential hypertension were examined in a cross-sectional study. Left ventricular structure and function (two-dimensionally guided M-mode echocardiography), dietary sodium intake (as estimated by 24-hour urinary sodium excretion), and noninvasive ambulatory blood pressure over 24 hours (Spacelab 90207) were determined in parallel with plasma renin activity and plasma Ang II and serum aldosterone concentrations (radioimmunoassay). Twenty-four-hour urinary sodium excretion emerged as a strong correlate of relative wall thickness independent of 24-hour ambulatory blood pressure (partial r = .49, P < .001). Ang II concentrations were weakly correlated with septal wall thickness (r = .27, P < .05) and left ventricular mass (r = .25, P < .05). Patients with high Ang II concentrations in relation to sodium excretion had a greater left ventricular mass (318 +/- 77 versus 257 +/- 54 g, P < .02), posterior wall thickness (11.8 +/- 1.9 versus 10.5 +/- 0.8 mm, P < .02), and septal wall thickness (13.6 +/- 1.8 versus 11.9 +/- 1.3 mm, P < .01) than those with "relatively low" Ang II levels in relation to sodium excretion. CONCLUSIONS Impaired suppression of the renin-Ang II system appeared to act as a stimulus for myocardial hypertrophy in hypertensive patients.

Abstract:Background: Modified ultrafiltration (MUF) can be performed in infants with ventricular septal defects (VSDs) after cardiopulmonary bypass (CPB) to reduce haemodilution and its potential adverse effects. High-flow MUF might reduce ultrafiltration duration and hasten the necessary correction of haemodilution during CPB. However, its influence on brain oxygenation remains controversial. Objective: This non-randomized, prospective, pilot study aimed to investigate the influence of high-flow MUF on brain oxygenation in infants with VSDs. Methods: High-flow MUF (20 mL/kg/min) was performed in twenty infants. Brain oxygen saturation (rSO(2)) and tissue haemoglobin index (tHI) were non-invasively and continuously measured intraoperatively using near-infrared spectroscopy (NIRS). Transcranial Doppler non-invasively detected the mean flow velocity of the middle cerebral artery (Vmean). Results: rSO(2) increased significantly during MUF, as did tHI, Vmean, mean arterial pressure and haematocrit (all p<0.05). No correlation was found between changes in rSO(2) and changes in other parameters (all p0.05). Conclusion: In infants with ventricular septal defects managed with CPB during VSDs repair, high-flow MUF did not reduce brain oxygenation.
Effects of the Cardiopulmonary Bypass on Cerebral Vasomotor Changes

Abstract:Objective To study the cerebral vasomotor changes after different cardiopulmonary bypass time and modes. Methods Thirty SD rats were equally divided into 5 groups,which were subjec-ted to CPB for 60 min and 120 min,deep hypothermie low flow bypass 60 min, deep hypothermie circulato-ry arrest 60 min, and sham respectively. The right middle cerebral artery was harvested and prepared for assessment of the myogenic tone with different transmural pressure, the endothelial cell response induced by different concentration of acetylcholine, and the vascular smooth muscle cell response induced by the so-dium nitroprusside and serotonin. The hippocampus tissue was harvested to observe the ultramicrostructure of endothelial cells in capillary by electron microscope. Results The CPB was performed successfully in all the rats. The deep hypothermic bypass altered the results of myogenic tone test with different transmural pressure compared with the sham group [(34.62±2.36) % and (33.83±2.28) % vs (37.75± 1.92) %, P<0.05]. Acetylcholine caused a dose-dependent vasodilation in the sham group that was ab-sent in animals undergoing CPB [(24.26±1.90) % vs (8.59±1.48) %, P<0.05]. Significantly, this was apparent after only 60 min of CPB and the vasodilation response of endothelial cells was worse in DHLF and DHCA groups than in simple CPB. The vascular smooth muscle cell response induced by the so-dium nitroprusside and serotonin was intact in all the rats. Conclusion The specific loss of acetylcholine-induced vasodilation suggests endothelial cell dysfunction,which appeares early after the onset of CPB. The deep hypothermic bypass can cause the dysfunction of cerebrovascular autonomic regulation. And these cer-ebral vasomotor changes may contribute to the neurologic injuries after CPB.
Closure of Patent Ductus Arteriosus with or Without Beating Heart under Cardiopulmonary Bypass

Abstract:Objective To introduce the experiences of surgical treatment on patent ductus arteriosus (PDA) with or without beating heart under cardiopulmonary bypass.Methods All 27 PDA patients whose ductus were from 0.4 to 2.8 cm,were subjected to suturing closure of the PDA opening through pulmonary artery cut under cardiopulmonary bypass.Of them,9 PDA patients underwent deep hypothermia and low perfusion flow,10 PDA patients hypothermia and 8 PDA patients mild hypothermia on beating heart.The complicated intra-cardiac malformation was repaired simultaneously.Results Only in 1 case of PDA,low cardiac output syndrome developed after operation,which was cured by drug therapy.Only in 1 case of PDA,hoarseness developed and recovered 4 weeks later.All patients recovered well without cardiac murmur,ductus re-opening and residual shunt.Conclusion To the complicated cases of PDA complicated with severe pulmonary hypertension or with other intra-cardiac malformation which needs to be repair simultaneously,satisfactory results can be obtained by suturing closure of the PDA intra-opening by way of pulmonary artery cut under cardiopulmonary bypass with or without beating heart (especially with beating heart).
The Impact of BIS-Monitored Depth Regulation on Prognosis: Improving Outcomes for Patients with Chronic DoCs

Abstract:Abstract Background: The value of the bispectral index (BIS) for monitoring anesthesia in patients with disorder of consciousness (DoC) has not yet been determined. Objective: To investigate the effect of BIS-monitored depth regulation on the prognosis of patients with chronic DoCs undergoing spinal cord stimulation (SCS) implantation. Methods: Ninety-two 92 patients with DoC who underwent SCS implantation under general anesthesia were followed up for three months after surgery. Among the cohort, 83 patients met the inclusion criteria and were divided into the BIS (n = 45) and non-BIS (n = 38) groups. Relevant information was collected, along with patient characteristics. Coma recovery scale-revised (CRS-R) scores and outcomes were assessed preoperatively and three months postoperatively (CRS-R3m). Results: In the BIS group, the CRS-R3m score was higher than the preoperative CRS-R score; however, in the non-BIS group, there was no significant difference between the scores. The CRS-R3m score in the BIS group was higher than that in the non-BIS group. Consciousness improvement was evaluated in a multifactor logistic regression analysis, which revealed that dexmedetomidine was a high-risk factor for consciousness improvement. During follow-up, two patients in the BIS group experienced a decrease in consciousness, while two patients in the non-BIS group died. Conclusion: For patients with DoC undergoing SCS implantation under general anesthesia, intraoperative BIS monitoring to control the depth of general anesthesia sedation contributes to better postoperative outcomes. It is advisable to avoid the use of dexmedetomidine during the perioperative period, because it affects patient outcomes.

Abstract:Perfusion, Ahead of Print. IntroductionPostoperative delirium (POD) has a major impact on patient recovery after cardiac surgery. Although its pathophysiology remains unclear, there could be a correlation between cerebral blood flow (CBF) variations during cardio-pulmonary bypass (CPB) and POD. Our study aimed to evaluate whether variations in on-pump CBF, compared to pre-anesthesia and pre-CPB values, are associated with POD following coronary artery bypass grafting (CABG) surgery.MethodsThis prospective observational cohort study included 95 adult patients undergoing elective on-pump CABG surgery. Right middle cerebral artery blood flow velocity (MCAV) was assessed using Transcranial Doppler before anesthesia induction, before CPB and every fifteen minutes during CPB. Pre-anesthesia and pre-CPB values were chosen as baselines. Individual values, measured during CPB, were converted as percentage changes relative to these baselines and named as %MCAV0 and %MCAV1, respectively. POD was assessed using the Confusion Assessment Method for ICU (CAM-ICU) during the first 48 post-operative hours and with the 3-Minute Diagnostic Interview for Confusion Assessment Method (3D-CAM) on the fifth post-surgical day.ResultsOverall POD incidence was 17.9%. At 30 minutes of CPB, %MCAV0 was higher in POD group than in no-POD group (p = .05). %MCAV0 at 45 minutes of CPB was significantly higher in POD group (87 (±17) %) than in no-POD group (68 (±24) %), p = .04. %MCAV1 at 30 and 45 minutes of CPB were higher in POD group than in no-POD group, at the limit of statistical significance. We found %MCAV1 > 100% in POD group, but not in no-POD group.ConclusionsSignificant differences in %MCAV0 became evident after 30 minutes of CPB, whereas differences in %MCAV1 at 45 minutes of CPB were at limit of statistical significance. In POD group %MCAV1 was higher than 100% at 30 and 45 minutes of CPB, which is supposed to be a sign of cerebral hyperperfusion. Monitoring CBF during CPB could have prognostic value for POD.

Abstract:Introduction/objective: To study the impact of hemodynamically significant patent ductus arteriosus (hsPDA), using near infrared red spectroscopy (NIRS) regional cerebral (SrO2), renal (RrO2) saturation measurements, and fractional tissue oxygenation extraction (FTOE) before and after medical and/or surgical treatment.Methods: Prospective cohort study of very low birth weight (VLBW) preterm infants with hsPDA requiring treatment from March 2014 to December 2016 in a tertiary Neonatal Intensive Care Unit in Singapore. NIRS was applied at diagnosis until 24–48 h after the last dose of ibuprofen or postsurgical ligation. All PDAs were documented to be closed by echocardiography. Post-treatment values (control group) were compared against pretreatment values.Results: Twenty-nine infants were studied, with mean gestational age of 26.7 weeks and birth weight of 956 g. Median day of life of PDA diagnosis was 4. Seven infants (24.1%) underwent PDA ligation. There was significant reduction in FTOE before treatment till after last dose of medication or ligation by a mean difference of 7.27% (p < .05). Reduction in FTOE was also found between the first dose of medication till after the last dose or ligation. Cerebral SrO2 significantly increased between first dose of medication till last dose of medication by a mean difference of 3.09% (p = .034). RrSO2 values were not significantly affected. No correlation between NIRS values and PDA size was found.Conclusions: Significant increase in cerebral SrO2 and reduction in FTOE by NIRS post PDA closure in hemodynamically significant PDAs suggest that PDA closure may reduce cerebral hypoxia burden.
Assessment of postoperative risk factors for EEG abnormalities in routine clinical management after paediatric cardiopulmonary bypass

Abstract:Objectives: The postoperative risk factors for electroencephalogram(EEG) abnormalities after paediatric cardiopulmonary bypass (CPB) remain to be identified. We investigated the characteristics of EEG abnormalities and risk factors in routine clinical management post-CPB. Methods: EEG and cerebral oxygen saturation (ScO2) were monitored in 96 patients (aged 3 days, 37 months, median 5 months) for 72 h post-CPB. Clinical measurements included 4-hourly arterial and central venous pressure, arterial blood gases, doses of inotropic and vasoactive drugs, daily C-reactive protein (CRP) and NT-proB-type Natriuretic Peptide (NT-proBNP). Demographics, STAT categories and outcomes (duration of mechanical ventilation,CICU stay) were recorded. Un. Results: Seizures occurred in 20 patients (20.8%) beginning at 0-48 hand lasting 10 min-31 h; background abnormalities occurred in 67 (69.8%) beginning at 0-8 h and lasting 4-48 h. Patients with EEG abnormalities had worse outcomes. In univariable regression, seizures positively correlated with STAT categories, CPB time, temperature, blood pressure, central venous pressure, NT-proBNP, CRP, lactate and epinephrine, negatively with ScO2 and PaCO2 (P < 0.001 for lactate and epinephrine, P < 0.1 for the remaining). The degree of background abnormalities positively correlated with STAT categories, CPB time, operative time, central venous pressure, milrinone, negatively with blood pressure (P = 0.0003-0.087); it negatively correlated with lower dose of epinephrine (P < 0.001) and positively with higher dose (P = 0.03l). In multivariable regression, seizures positively correlated with epinephrine, lactate and temperature; the background abnormality correlations remain significant except for milrinone and operative time (P < 0.001 for epinephrine, P < 0.05 for the remaining). Conclusions: Numerous perioperative risk factors are associated with EEG abnormalities post-CPB. The most significant and consistent risk factor is epinephrine.
