Abstract:Sir:FigureWe read with great interest the article entitled “Postoperative Enoxaparin Prevents Symptomatic Venous Thromboembolism in High-Risk Plastic Surgery Patients” published in Plastic and Reconstructive Surgery in November of 2011.1 We totally agree with the authors' conclusion that in high-risk plastic surgery patients, postoperative enoxaparin prophylaxis is protective against 60-day venous thromboembolism. However, recently, a 60-year-old female patient who suffered Marjolin ulcer at the sacrum experienced postoperative stomach bleeding, which warned us of the dangers and the risks of anticoagulation therapy. The patient was injured in a car accident 8 years previously and received bilateral hip arthroplasty immediately. She recovered 2 weeks later, but a chronic wound at the sacrum was left. Gradually, the ulcer turned into an enlarged tumor, 10 × 10 cm (Fig. 1). The biopsy result indicated that the tumor was squamous cell cancer. She also suffered burn injury 30 years previously, which left multiple scars on her back and bilateral thighs. The patient denied any history of gastric ulcer. She underwent tumor resection and bilateral pedicled superior gluteal artery perforator flap reconstruction. Preoperative risk stratification was performed using the Caprini risk-assessment model, and her score was 6 (60 to 74 years old, malignancy, and operative time >45 minutes). Enoxaparin (10 mg subcutaneously) twice daily was prescribed to prevent venous thromboembolism starting at 6 hours postoperatively. The patient's body mass index was 22.5.Fig. 1: A 10 × 10-cm Marjolin ulcer on the sacrum.On the fourth day postoperatively, the patient vomited three times, with bloody substances. The patient's hemoglobin level dropped from 92 g/liter on the first day postoperatively to 71 g/liter. The patient received blood transfusion, and enoxaparin injection was stopped immediately. Upper gastrointestinal endoscopy on the sixth day postoperatively revealed a superficial cardia ulcer (Fig. 2). The patient was treated conservatively with intravenous proton pump inhibitors and parenteral nutrition. The bleeding did not recur.Fig. 2: Gastrointestinal endoscopy revealed a cardia ulcer.Stress-related gastric bleeding is rarely seen in flap surgery patients; thus, the use of prophylactic drugs is uncommon unless a history of gastric ulcer is present. Demirtas et al. reported postoperative upper gastric bleeding after rhinoplasty2 and emphasized the importance of obtaining a detailed medical history. According to the therapeutic guidelines concerning stress ulcer prophylaxis published by the American Society of Health-System Pharmacists in 1999 and recent updates,3–5 anticoagulant therapy has been identified as a risk factor for gastrointestinal bleeding, and the use of acid-suppressive therapy for prevention in non–intensive care unit patients is currently not recommended or supported. The American Society of Health-System Pharmacists guideline also defined “high-risk factor” to include mechanical ventilation, coagulopathy, low platelet count, and other factors. However, the selection of an optimal pharmacologic agent for stress ulcer prophylaxis continues to be debated. We share the same point of view with Dr. Pannucci that the optimal duration of chemoprophylaxis in plastic surgery patients remains unknown. Although rare, stress-related stomach bleeding is one of the severe complications for plastic surgery patients, especially those who have undergone flap surgery and were given anticoagulant therapy.
