Abstract:Patients with gastrointestinal(GI) tract neoplasms usually suffer from malnutrition, which is associated with increased morbidity and mortality, reduction of treatment efficacy, increased risk of therapy-associated side-effects, increased length of hospital stay, and reduced quality of life. Abstract:Objective The aim of this study was to understand the nutritional status in patients with tumor and evaluate the impact of malnutrition on clinical outcomes, then provide scientiifc reference for nutritional support of patients.Methods Collect 802 patients with tumor in 12 hospitals in zhejiang province, during June to August in 2014. The rate of nutritional risk and malnutrition was respectively evaluated with NRS 2002 and 2015 ESPEN diagnostic criteria, biochemical indexes and nutrition therapy were recorded.Results The incidence rate of nutrition risk was 31.80% and malnutrition was 17.70%, which was higher in gastrointestinal tumor than others. Weight and BMI were signiifcantly lower in gastrointestinal tumor patients than in non-gastrointestinal tumor patients (P<0.05); The ratio of nutrition therapy in the group of NRS 2002≥3 was 28.24%, and 6.40% in NRS 2002<3 (χ2=71.809, P<0.01). Nutrition therapy with malnutrition was 26.06% and 10.61% in well-nourished patients (χ2=28.235,P<0.01). Patients at nutritional risk according to NRS 2002(score≥3) prolonged hospitalization days and costs (P<0.01) over which NRS 2002 score<3. It’s the same as malnutrition except costs. The length of ICU and hospitalization costs was no signiifcation between well-nourished and malnutrition patients.Conclusions The ratio of malnutrition and nutritional risk were signiifcantly higher in patients with tumor, especially in gastrointestinal cancer. However, the nation of nutritional support was low, which cancer caused to prolong hospitalization days and costs. We should assess nutrition status in patients with cancer regularly and modify nutritional support to improve the quality of their life.

Abstract:Immune nutrition refers to adding some special nutrients to the standard formula of nutrition in order to treat and regulate metabolism and immune function by its pharmacological effect.In recent years,immune nutrition,including glutamine,arginine,ω-3 PUFA,nucleotide etc,has been widely used in patients with gastrointestinal cancer.These nutrients show their different supporting functions through different mechanisms,and improve the clinical outcome of patients.Therefore,clinical nutrition has been expanded and upgraded from the traditional "nutrition" to "nutritional therapy".

Abstract:Objective: To investigate the nutritional status and nutrient intake of the gastrointestinal cancer patients with adjuvant chemotherapy in order to provide the basis for dietary care of the patients.Methods: The subjective global assessment(SGA) was used to investigate the overall nutritional status of 62 patients and the combined inquiry and the weighing method was used to conduct a survey on the nutrient intake of the patients on any 3 days before chemotherapy,1 ~ 3 days after chemotherapy and on 8 ~ 10 days after stopping chemotherapy.The indices of traditional nutritional evaluation(BW,BMI,Hb,TLC,and ALB) and nutrient intake(energy,protein,fat,carbohydrates,dietary fiber,vitamin A,vitamin C,vitamin E,calcium,phosphorus,iron,zinc,selenium) were compared by SGA standard between the well-nourished and malnourished patients.Results: 14 patients were well-nourished(22.6%) and 48 patients were malnourished(77.4%).The differences in the comparison of BMI,Hb,TLC and ALB except BW were statistically significant between the well-nourished and malnourished patients(P<0.01);the comparison of the intake of energy,protein,fat,carbohydrates,calcium and iron was significantly different(P<0.01,P<0.05).Conclusion: The malnutrition was commonly found in the gastrointestinal cancer patients with adjuvant chemotherapy,the analysis results showed that nutrient intake plays an important role in the improvement of the nutritional status of the patients.The nurses of tumor specialty should study more nutrition-related knowledge so as to provide the professional dietary care for the patients.

Abstract:Objective To relieve long-term malnutrition caused by a malignant tumor affecting the pylorus and to prolong the patient survival. Methods A patient presented with complete pyloric obstruction due to pyloric tumors, and achieved good therapeutic effects through nutritional support and effective chemotherapy. Results After about 40 days of treatment, all physiological indicators were improving. After chemotherapy, the pyloric tumor shrank and the complete obstruction was relieved. The patient was able to eat and drink normally. Conclusion Nutritional support, combined with chemotherapy, may effectively treat complete pyloric obstruction caused by advanced tumors.

Abstract:The incidence of malnutrition in patients with malignant tumors is high, especially in patients with gastrointestinal cancer. In addition to the consumption of tumor itself and the release of specific factors, gastrointestinal tumors are often caused by inadequate or excessive loss of nutritional intake due to obstruction of the digestive tract, which further aggravates the degree of malnutrition in the patients. Perioperative malnutrition can increase the incidence of postoperative complications, prolong hospital stay, reduce quality of life, increase unplanned second surgical rates, and ultimately increase mortality. By using of effective nutrition screening tool for scanning patients with malnutrition, providing accurate and effective nutritional support treatment in the perioperative period, improving the nutritional status of patients with tumor, it can improve the tolerance of patients with tumor, reduce the adverse reaction and complication rate, improve curative effect, shorten hospitalization time, reduce hospitalization costs, thereby improving the clinical outcome of patients, and improve the quality of life and prolong the survival time. Therefore, perioperative nutrition screening and accurate nutrition therapy for gastrointestinal cancer patients are of great importance to improve the prognosis of patients with gastrointestinal cancer. From the aspects of the causes of malnutrition in patients with gastrointestinal cancer, the assessment and screening of nutritional status, the precision nutrition therapy, and the choice and application of the nutritional preparations, this paper illustrates the current research progress.

Abstract:Objective To study the nutritional status and investigate its effect on quality of life among digestive tract cancer patients treated with chemotherapy.Methods The subjective global assessment(SGA),anthropometric measurements and laboratory testing were taken in the nutrition assessment of 77 patients with digestive tract cancer.The patients' quality of life(QOL)was assessed by using EORTC QLQ-C30,and the QOLs between the patients in good nutrition and the patients in malnutrition were compared.Results According to SGAT scores,there 65 patients(84.4%)who were in a good nutritional condition and 12 patients(15.6%)who were in a malnutrition condition.The comparison between patients in the two groups showed statistically significant differences in physical function,fatigue,nausea and vomiting,pain,dyspnea,appetite loss,general health status/QOL(P<0.05).Conclusion A lager percentage of the patients with digestive tract cancer are in a malnutrition condition.The QOLs between the patients in good nutrition and in malnutrition are different,so the nutrition assessment of the patients should be paid more attention to,in order to improve the nutritional status and QOL of the patients.

Abstract:Objective:To investigate the nutritional risks,undernutrition,and nutritional support in 127 hospital-ized patients with malignant tumors,analyzing the relationship between the mutritional risks and disease stage,age,he-moglobin,albumin,serum prealbumin.Methods:A total of 127 patients admitted to our department were evaluated the nutrition risk and undernutrition by NRS2002 in the 2nd day after admission and recorded age,height,weight,disease staging,hemoglobin,treatment,calculating the MBI and NRS2002 scores.To study the relationship between the nutri-tion risk and above indexes.Results:25 patients were undernutrition in 127 patients with malignant tumors,accounting for 19.69%.72 patients were NRS2002 score ≥3 which have nutritional risk,accounting for 56.69%.23 patients were given nutrition support therapy,of which 3 patients'NRS2002 score were 1 ~2.Twenty patients whose NRS2002 score ≥3 were given nutrition support therapy,accounting for 27.78%(5 patients were terminal stage).The main ap-plications were intralipid,amino acid,glucose.Statistical analyses showed that the older of age the higher risk of nutri-tion.The incidence of anemia increased risk of nutrition.The decline of serum and prealbumin will increase risk of nu-trition.Disease stages had not relationship with nutrition risk.Conclusion:Nutrition risk is high in hospitalized pa-tients.Nutrition support therapies do not take seriously attention which have inappropriate and insufficiency therapy.In addition to nutritional therapy,treating anemia and hypoproteinemia actively were important in cancer patients.

Abstract:Objective: To investigate the current status of nutritional support application in patients within gastrointestinal tumor medicine of our hospital. Methods: Gastrointestinal cancer patients who discharged during the first half of 2012 were under investigation and 102 qualified patients who had ever received nutritional support were analyzed. Results: All 102 of patients were eligible. Nutrition risk screen had not been included in routine work. The percentage of nutritional support was 8. 2%. The median duration of nutritional support was 4 days,in which 51. 96% of patients was less than 5 days. The average energy supply by nutritional support was( 61. 0 ± 28. 0) kJ /( kg·d),and 21. 1% of patients was fed 41. 8 kJ( 1 0 kcal) /( kg·d). The average protein supply was 0. 4 ± 0. 2 g /( kg·d),and 56. 7% of patients was fed lower than 0. 6 g /( kg·d). The ratio of EN: PN was 1∶ 2. 9. In the patients recieved PN,47. 9% fed by personalized all-in-one preparations,43. 3% fed by single bottle sequential preparations,and 9. 6% was fed by industrialization three cavity bag. In the patients recieved EN,52. 7% fed by tube,and 47. 3% fed by oral. The nutraceuticals' s cost accounted for 17. 38% and 10. 80% of all drug costs and all medical treatment charge respectively. Per capita nutrition support cost was RMB $ 2436. 2 ± 1756. 2. Conclusion: The application of nutritional support to gastrointestinal cancer patients was inappropriate in part of patients. Nutrition risk screen had not been included in routine work, the percentage of underfeeding was high and single bottle infusion was common. A nutritional support team should be organized to help improve the prescriptive application of nutritional support therapy.

Abstract:[Objective] To evaluate the nutritional status and the incidence of nutritional risk of tumor patients treated with chemotherapy in hospital,analyze the factors that cause the nutritional risk of patients,provide reference for patients with malnutrition and nutrition risk,so as to they can have clinical nutrition support therapy in timely.[Method] Nutritional risk screening (NRS2002) was used to screen 236 patients who had been hospitalized for chemotherapy.The nutritional status of the patients were assessed by body mass index,serum hemoglobin,albumin and total protein levels The correlation analysis was carried out with NRS2002 score.[Result] Of the 236 patients who treated with chemotherapy,114 cases were male,accounting for 48.3％.The number of female patients was 122,accounting for 51.7％.Among them,142 patients had nutritional risk,accounting for 60.2％.The incidence of nutritional risk was 66.7％,69.0％,61.9％,66.7％,33.3％ and 56.0％respectively in respiratory system,digestive system,head and neck cancer,lymphoid hematopoietic system,gynecological system and urinary system There was a higher risk of nutritional risk in patients with digestive tumor disease.Nutritional risk was positively correlated with age (r =0.042,P＜0.05),and negatively correlated with HGB (r =-0.015,P ＜0.05),and negatively correlated with ALB (r =-0.103,P ＜0.05),and negatively correlated with BMI (r =-0.086,P ＜ 0.05).Sort by the size of normalization coefficient,the effect of the dependent variable nutrition risk was ALB,BMI,age,HGB.[Conclusion] Tmnor patients treated with chemotherapy have high incidence of nutritional risk,which associated with tumor patient's disease,age,anemia,albumin and BMI,we should strengthen the nutritional assessment,support nutritional treatment in time,so as to improve their clinical outcome.

Abstract:Background: Patients with gastrointestinal cancer often suffer from malnutrition during tumor progression. Malnutrition is associated with postoperative complications and decreased quality of life. Supporting cancer patients with proper nutrition is vital for improving their prognoses.Method: Google scholar and PubMed database searches were performed. Selection criteria included gastrointestinal cancer, surgery, ω - 3 fatty acids, randomized clinical trials from 2007 to August 2022.Conclusion: Nutritional therapy includes nutritional counseling, enteral nutrition, parenteral nutrition, and oral nutritional supplements. Immune nutrients like glutamine and ω-3 fatty acid have been demonstrated with benefits in reducing inflammatory responses and postoperative complications, regulating immune function and improving prognosis.

Abstract:Objective To investigate the effect of nutritional status on the adverse reactions of chemotherapy in patients with gastrointestinal cancer after surgery.MethodsRetrospective analysis of 48 cases of gastrointestinal cancer patients with postoperative chemotherapy and the effects of chemotherapy adverse reactions.Results 12 patients with good nutritional status, 36 patients with malnutrition, malnutrition and malnutrition in patients with adverse reactions were not statistically significant (P>0.05).Conclusion After the operation of gastrointestinal cancer chemotherapy patients, most of which are with malnutrition, good nutrition and malnutrition in patients with adverse reactions of chemotherapy compared to smaller differences, and chemotherapy and the number of samples and individual differences.

Abstract:The negative influence of malnutrition on patients with malignant tumors runs through the whole course of a disease. To the best of our knowledge, the nutritional status of cancer patients has not yet been reported in a large sample size. Therefore, this study aimed to investigate the status of malnutrition and nutritional support therapy in a large sample of patients with common malignant tumors in China. In this observational multicenter study, 80 tertiary hospitals in 22 major provinces and cities were selected using a two-stage random sampling method. A total of 47,488 hospitalized patients with 16 kinds of common malignant tumors were enrolled in this study. The nutritional status of the patients was assessed by the scored Patient-Generated Subjective Global Assessment (PG-SGA). The severity of malnutrition was identified by a quantitative assessment where 0–1 was labelled as no malnutrition and 2–3, 4–8, and ≥9 were defined as suspected/mild, moderate and severe malnutrition. The PG-SGA scores of patients with different tumor characteristics were compared by a variance analysis. The incidence rate of moderate and severe malnutrition was 58.2%, of which 32.1% and 26.1% fell under the category of moderate and severe malnutrition, 22.2% were suspected/mild and only 19.6% of the sample were not malnourished.
