Abstract:Objective To investigate the prevalence of nutritional risk and perioperative nutritional support status in the gastrointestinal cancer surgical patients.Methods Adult inpatients from the gastrointestinal cancer surgery department of our hospital were consecutively enrolled from May to September 2010.Nutritional Risk Screening 2002 ( NRS 2002) was performed at admission,and data of the nutritional support and complications during hospitalization were collected.The relationship between nutritional risk and postoperative complications was analyzed.Results A total of 195 patients were eligible.At admission,43.6％ (85/195) patients were "at risk" Of the patients "at risk",totally 11.7％ (10/85) patients received preoperative nutritional support,and 100％ (85/85) patients received postoperative nutritional support; of the patients "not at risk",no patients received preoperative nutritional support,and 84.5％ (93/110) patients received postoperative nutritional support.The overall rates of postoperative complications were 19.1％ ( 13/68 ) in the patients "at risk" while 7.1％ (9/127 )in the patients "not at risk" ( P =0.02).Conclusions Nutritional risk in gastrointestinal cancer surgical patients was high.The application of nutritional support was inappropriate in these patients.The nutritional risk is associated with higher postoperative complications.Dissemination of evidence-based guidelines should be enhanced.

Abstract:Objective To investigate nutritional risk and nutritional support condition for inpatients with cancer.Method Take screening on nutritional risk of 916 inpatients with cancer from 8 hospitals of Zhejiang.Assess body mass index and serum albumin to evaluate patients' nutritional condition and study nutritional support condition for inpatients with nutritional risk.Result Score of 916 patients' nutritional risk is from 0 to 7.334 patients have nutritional risk(≥3),among which 167 have malnutrition and 121 receive nutritional support.Conclusion Inpatients with cancer have nutritional risk.Management should be enhanced and nutritional support should be standardized.

Abstract:ObjectiveThis study aimed to assess the prognostic value of the Nutritional Risk Score 2002 (NRS2002) and patient-generated subjective global assessment (PG-SGA) for post-operative infections in patients with gastric cancer (GC) and colorectal cancer (CRC) who underwent curative surgery.MethodsThis prospective study included 1,493 GC patients and 879 CRC patients who underwent curative surgery at 18 hospitals in China between April 2017 and March 2020. The NRS2002 and PG-SGA were performed on the day of admission. The relationship between the nutritional status of patients before surgery and post-surgical incidence of infection was analyzed using univariate and multiple logistic regression analyses.ResultsAccording to NRS2002, the prevalence of nutritional risk was 51.1% in GC patients and 63.9% in CRC patients. According to the PG-SGA, 38.9% of GC patients and 54.2% of CRC patients had malnutrition. Approximately 4.4% of the GC patients and 9.9% of the CRC patients developed infectious complications after surgery. The univariate and multiple logistic regression analyses showed that the risk of infections was significantly higher in GC patients with a high nutritional risk score (NRS2002 ≥5) than in those with a low score (NRS2002 <3), and the PG-SGA score was identified as a predictor of post-operative infection complications of CRC.ConclusionThe pre-operative nutritional status of patients with GC or CRC has an impact on post-operative infection occurrence. NRS2002 ≥5 was a risk factor for post-operative infection in patients with GC, and the PG-SGA B/C was a predictor of infections in patients with CRC.

Abstract:The scored Patient-Generated Subjective Global Assessment (PG-SGA) is considered to be the most appropriate tool for detecting malnutrition in cancer patients. In particular, malignant tumours derived from the gastrointestinal tract may impair nutrient intake and absorption and cause malnutrition. We carried out a cross-sectional study to assess the nutritional status and related factors of patients with gastrointestinal cancer. Nutritional status was determined using the scored PG-SGA in patients (n 498) with advanced gastrointestinal cancer admitted to the Gastrointestinal Medical Oncology Unit at Beijing Cancer Hospital between 1 August 2012 and 28 February 2013. The possible related factors including age, sex, hospitalisation frequency and pathology were explored. We found that 98% of the patients required nutrition intervention and 54% of the patients required improved nutrition-related symptom management and/or urgent nutritional support (PG-SGA score ≥9). Factors related to malnutrition were age (r 0.103, P<0.01), hospitalisation frequency (r -0.196, P<0.01) and sex (the prevalence of malnutrition was higher in men than in women (9.88 v. 8.54, P<0.01)). Patients with rectal cancer had a lower risk of malnutrition than patients with other types of gastrointestinal cancer (F=35.895, P<0.01). More attention should be paid to the nutritional status of gastrointestinal patients, especially those at a higher risk of malnutrition, such as elderly patients, those hospitalised for the first time, male patients and those with other types of gastrointestinal cancer except rectal cancer. The nutritional status of these patients should be evaluated and they should be given proper nutrition education and nutritional support in a timely manner.

Abstract:Objective The aim of this study was to understand the nutritional status in patients with tumor and evaluate the impact of malnutrition on clinical outcomes, then provide scientiifc reference for nutritional support of patients.Methods Collect 802 patients with tumor in 12 hospitals in zhejiang province, during June to August in 2014. The rate of nutritional risk and malnutrition was respectively evaluated with NRS 2002 and 2015 ESPEN diagnostic criteria, biochemical indexes and nutrition therapy were recorded.Results The incidence rate of nutrition risk was 31.80% and malnutrition was 17.70%, which was higher in gastrointestinal tumor than others. Weight and BMI were signiifcantly lower in gastrointestinal tumor patients than in non-gastrointestinal tumor patients (P<0.05); The ratio of nutrition therapy in the group of NRS 2002≥3 was 28.24%, and 6.40% in NRS 2002<3 (χ2=71.809, P<0.01). Nutrition therapy with malnutrition was 26.06% and 10.61% in well-nourished patients (χ2=28.235,P<0.01). Patients at nutritional risk according to NRS 2002(score≥3) prolonged hospitalization days and costs (P<0.01) over which NRS 2002 score<3. It’s the same as malnutrition except costs. The length of ICU and hospitalization costs was no signiifcation between well-nourished and malnutrition patients.Conclusions The ratio of malnutrition and nutritional risk were signiifcantly higher in patients with tumor, especially in gastrointestinal cancer. However, the nation of nutritional support was low, which cancer caused to prolong hospitalization days and costs. We should assess nutrition status in patients with cancer regularly and modify nutritional support to improve the quality of their life.

Abstract:Background: Patients with gastrointestinal tumors often suffer from poor nutritional status during treatment. Surgery is the main treatment for these patients, but the long postoperative recovery period is often accompanied by digestive and absorption dysfunction, leading to further deterioration of the nutritional status. Early enteral nutrition support is hypothesized to be helpful in improving this situation, but the exact effects have yet to be studied in depth. Aim: To observe the effect of early enteral nutritional support on postoperative recovery in patients with surgically treated gastrointestinal tract tumors, with the expectation that by improving the nutritional status of patients, the recovery process would be accelerated and the incidence of complications would be reduced, thus improving the quality of life. Methods: A retrospective analysis of 121 patients with gastrointestinal tract tumors treated in our hospital from January 2020 to January 2023 was performed. Fifty-three of these patients received complete parenteral nutrition support as the control group for this study. The other 68 patients received early enteral nutritional support as the observation group of this study. The clinical indicators comparing the two groups included time to fever, time to recovery of postoperative bowel function, time to postoperative exhaustion, and length of hospital stay. The changes in immune function and nutritional indexes in the two groups were compared. Furthermore, we utilized the SF-36 scale to compare the changes in the quality of life between the two groups of patients. Finally, the occurrence of postoperative complications between the two patient groups was also compared. Results: The postoperative fever time, postoperative bowel function recovery time, postoperative exhaustion time, and hospitalization time were all higher in the control group than in the observation group (P < 0.05). The levels of CD3+, CD4+, immunoglobulin (Ig) A, IgM, and IgG in the observation group were significantly higher than those in the control group at 1 d and 7 d postoperatively, while CD8+ was lower than in the control group (P < 0.05). Total protein, albumin, prealbumin, and transferrin levels were significantly higher in the observation group than in the control group at 7 d postoperatively (P < 0.05). The SF-36 scores of patients in the observation group were significantly higher than those in the control group (P < 0.0001). The overall incidence of adverse reactions after the intervention was significantly lower in the control group than in the observation group (P = 0.021). Conclusion: We found that patients with gastrointestinal tumors are nutritionally vulnerable, and early enteral nutrition support programs can improve the nutritional status of patients and speed up postoperative recovery. This program can not only improve the immune function of the patient and protect the intestinal function, but it can also help to improve the quality of life of the patient. However, this program will increase the incidence of complications in patients. Caution should be taken when adopting early enteral nutrition support measures for patients with gastric cancer. The patient's condition and physical condition should be comprehensively evaluated and closely monitored to prevent possible complications.
Impact of Malnutrition and Nutritional support to reduce infections in elderly hospitalized cancer patients: a multicenter survey in China

Abstract:Background Malnutrition is common among elderly cancer patients which greatly affects patients' quality of life. Malnutrition is a key factor in the development of systemic chronic low-grade inflammation that can exacerbate. Methods A total of 633 elderly cancer patients from 29 tertiary public hospitals in 14 cities in China were selected as the research subjects. Physical examination, questionnaire survey, medical history collection, and blood index detection were performed on the research subjects to analyze the relationship between malnutrition, nutritional support and infection status. Results There was no significant difference in infection rate among elderly hospitalized cancer patients with different cancer types, cancer stages or therapy methods. ( P >0.05). The proportion of vomiting, loss of appetite, depression, and physical fatigue in patients with good nutritional status were lower than that in patients with moderate or severe malnutrition( P<0.05 ). Nutritional support is crucial to the nutritional status of elderly cancer patients, patients without special nutritional guidance are more likely to develop infection ( P<0.05 ). After adjusting for confounding variables, logistic regression analysis showed that ONS (OR 0.088, 95% CI 0.015-0.514), PN(OR 0.091, 95% CI 0.011-0.758), and individualized nutritional guidance (OR 0.026, 95% CI 0.003-0.202) were all protective factors associated with cancer infection. Conclusion Many elderly hospitalized cancer patients have local or systemic infections, and good nutrition is associated with lower rates of infection. Receiving nutritional guidance or support is associated with lower infection rates in elderly cancer patients.

Abstract:Background: Patients with gastrointestinal cancer often suffer from malnutrition during tumor progression. Malnutrition is associated with postoperative complications and decreased quality of life. Supporting cancer patients with proper nutrition is vital for improving their prognoses.Method: Google scholar and PubMed database searches were performed. Selection criteria included gastrointestinal cancer, surgery, ω - 3 fatty acids, randomized clinical trials from 2007 to August 2022.Conclusion: Nutritional therapy includes nutritional counseling, enteral nutrition, parenteral nutrition, and oral nutritional supplements. Immune nutrients like glutamine and ω-3 fatty acid have been demonstrated with benefits in reducing inflammatory responses and postoperative complications, regulating immune function and improving prognosis.

Abstract:The negative influence of malnutrition on patients with malignant tumors runs through the whole course of a disease. To the best of our knowledge, the nutritional status of cancer patients has not yet been reported in a large sample size. Therefore, this study aimed to investigate the status of malnutrition and nutritional support therapy in a large sample of patients with common malignant tumors in China. In this observational multicenter study, 80 tertiary hospitals in 22 major provinces and cities were selected using a two-stage random sampling method. A total of 47,488 hospitalized patients with 16 kinds of common malignant tumors were enrolled in this study. The nutritional status of the patients was assessed by the scored Patient-Generated Subjective Global Assessment (PG-SGA). The severity of malnutrition was identified by a quantitative assessment where 0–1 was labelled as no malnutrition and 2–3, 4–8, and ≥9 were defined as suspected/mild, moderate and severe malnutrition. The PG-SGA scores of patients with different tumor characteristics were compared by a variance analysis. The incidence rate of moderate and severe malnutrition was 58.2%, of which 32.1% and 26.1% fell under the category of moderate and severe malnutrition, 22.2% were suspected/mild and only 19.6% of the sample were not malnourished. Abstract:Objective: To investigate the current status of nutritional support application in patients within gastrointestinal tumor medicine of our hospital. Methods: Gastrointestinal cancer patients who discharged during the first half of 2012 were under investigation and 102 qualified patients who had ever received nutritional support were analyzed. Results: All 102 of patients were eligible. Nutrition risk screen had not been included in routine work. The percentage of nutritional support was 8. 2%. The median duration of nutritional support was 4 days,in which 51. 96% of patients was less than 5 days. The average energy supply by nutritional support was( 61. 0 ± 28. 0) kJ /( kg·d),and 21. 1% of patients was fed 41. 8 kJ( 1 0 kcal) /( kg·d). The average protein supply was 0. 4 ± 0. 2 g /( kg·d),and 56. 7% of patients was fed lower than 0. 6 g /( kg·d). The ratio of EN: PN was 1∶ 2. 9. In the patients recieved PN,47. 9% fed by personalized all-in-one preparations,43. 3% fed by single bottle sequential preparations,and 9. 6% was fed by industrialization three cavity bag. Nutrition risk screen had not been included in routine work, the percentage of underfeeding was high and single bottle infusion was common. A nutritional support team should be organized to help improve the prescriptive application of nutritional support therapy.
Perioperative Artificial Nutrition in Malnourished Gastrointestinal Cancer Patients

Abstract:AIM:To investigate the potential role of perioperative nutrition in reducing complications and mortality in malnourished gastrointestinal cancer patients.METHODS:Four hundred and sixty-eight elective moderately or severely malnourished surgical patients with gastric or colorectal cancers defined by the subjective global assessment (SGA) were randomly assigned to 7 d preoperative and 7 d postoperative parenteral or enteral nutrition vs a simple control group. The nutrition regimen included 24.6+/-5.2 kcal /kg per d non-protein and 0.23+/-0.04 g nitrogen /kg per d. Control patients did not receive preoperative nutrition but received 600+/-100 kcal non-protein plus or not plus 62+/-16 g crystalline amino acids postoperatively.RESULTS:Complications occurred in 18.3% of the patients receiving nutrition and in 33.5% of the control patients (P=0.012). Fourteen patients died in the control group and 5 in those receiving nutrition. There were significant differences in the mortality between the two groups (2.1% vs 6.0%, P=0.003). The total length of hospitalization and postoperative stay of control patients were significantly longer (29 vs 22 d, P=0.014) than those of the studied patients (23 vs 12 d, P=0.000).CONCLUSION:Perioperative nutrition support is beneficial for moderately or severely malnourished gastrointestinal cancer patients and can reduce surgical complications and mortality.

Abstract:Objective To understand the postoperative nutritional status and symptoms of gastric cancer patients,and to analyze the correlation between nutritional status and symptoms.Methods A total of 1 1 0 postoperative gastric cancer patients in Peking University Cancer Hospital were enrolled from April to December 201 4 and selected using convenience sampling.We assessed the patient′s nutritional status by human body measurement,lab test and subjective global assessment (SGA).Memorial symptom assessment scale (MSAS) was used to evaluate these patients′symptoms after operation.Results Based on different nutrition indicators, rate of malnutrition in patients after surgery was from 6.3% to 52.7%.About 77 patients (70.0%)suffered from multiple symptoms,51 (46.4%)patients with physical symptoms,34 (30.9%)with psychological symptoms.The top 1 0 symptoms included difficulty in falling asleep,numb,abdominal distension,weight loss, nausea,diarrhea,poor appetite,thirst,anenergia,and hair loss.According to the SGA result,patients in worse nutritional status had higher symptom scores than those with normal nutrition (P <0.01 ),such as PHYS (physical symptom subscale score),PSYCH (psychological symptom subscale score),GDI (global distress index),and TMSAS (total MSAS score).Conclusions Our results show that postoperative malnutrition is common and the majority of patients may experience symptoms.Nutritional status is positively correlated with symptoms during recovery.Thus,when assessing nutritional status and performing nutrition intervention,we should consider patients′symptoms,and manage their symptoms appropriately as well.

Methods: 56 gastric carcinoma patients were measured with Inbody 3.0, ( Biospace Co.)and automatic biochemistry analyzerfor BMI,FFMI,BFMI,ALB,PA,FN,TF,LC and IBW%.The relation between malnutrition and postoperative infection or emptying disability was analyed. Results: PA,FN,BFMI,FFMI and BMI were significantly decreased in malnutrition gastric carcinoma patients and had a high correlation with postoperative infection and emptying disability. Conclusion: Malnutrition in gastric carcinoma patients correlates significantly with postoperative infection and emptying disability.

Abstract:Obj ective:To systematically evaluate the influence of postoper-ative intestinal immune nutritional support on postoperative infectious complications and hospitalization time.Methods:The CNKI,Wangfang datebase,VIP datebase,PubMed,Medline and Cochran Library were searched by computers.Two investigators used Cochrane system evalua-tion method to evaluate and extract data,and all the data were analyzed with RevMan5 .1 .Software.Results:A total of nine randomized controlled trials involving 800 patients met the inclusion criteria.The results showed that there were no significantly statistical differences in the rate of pulmo-nary infection[OR=0.61,95%CI(0.35,1.06)]、incision infection[OR=0.71,95%CI(0.37,1.36)]、urinary tract infection[OR=0.39,95%CI(0.14, 1.14)]、abdominal infection[OR=0.48,95%CI(0.22,1.07)]be-tween both groups (P>0.05).There was significantly statistical differ-ence in length of stay in hospital[WMD=-3.13,95%CI(-5.54,-0.71)] (P<0.05).Conclusion:The postoperative application of enteral im-mune nutrition support for gastrointestinal cancer patients may not reduce the incidence of postoperative common infectious complica-tions,but it maybe shrink the length of hospital stay.Limited to the study,more high quality RCTs and Meta analysis are required to ver-ify the effects.

Abstract:Malnutrition is a common complication in cancer patients. It often accelerates disease progression and affects treatment outcomes. Thus, in the early census of cancer patients, examination for possible nutritional risks and correcting potential causes of malnutrition are needed to improve patients’ quality of life. Our study included 375 patients diagnosed with cancer in Henan province and analyzed the relationship between nutritional risk and indicators like age, serum albumin, serum prealbumin, serum hemoglobin, tumor stage, tumor type, and inflammatory factors. We found that age, hemoglobin, and presence of gastrointestinal tumors were independent risk factors for nutritional risk. We also found significant correlation between inflammatory factors and nutritional risk in cancer patients, so as to provide new prediction indexes for clinical management of nutritional risk and dynamic changes of nutritional status.

Abstract:Objective This study aimed to explore factors contributing to malnutrition in gastric cancer(GC)patients.Methods A cross-sectional study was conducted on GC patients admitted between January and May 2019 at a Grade A tertiary hospital in Shanghai,China.Data on demographic,disease-related and nutritional-related information were collected.Specifically,nutrition status was assessed using patient-generated subjective global assessment,and dietary intake was assessed by food frequency questionnaire.Variables with P value less than 0.1 on univariate analysis went on to be included in multiple logistic analysis to identify the risk factors to malnutrition.Results A total of 160 GC patients were analyzed in this study.Overall,70.6%of patients were malnourished.21.9%of GC patients did not reach energy intake target,while 32.5%of GC patients did not reach protein intake target.The results of logistic regression showed that age 65 or greater(OR =3.62,P =0.005)and protein intake less than 2 g/(kg·d)(OR =3.01,P =0.018)were contributing factors to GC patient malnutrition.Conclusion The study results suggested that malnutrition is still prevalent in GC patients.Age and under target protein intake were the potential risk factors to malnutrition.Clinician should pay attention to dietary protein intake when providing nutritional counselling for gastric cancer patients with malnutrition.
Occurrence of Malnutrition in Cancer Patients During Radiotherapy

Abstract:Objective? To investigate the malnutrition status of cancer patients undergoing radiotherapy, and to provide research basis for later nutrition intervention. Methods? Using convenient sampling method, 380 patients with cancer treated by radiotherapy in Shanghai and Zhejiang provincial ClassⅢ Grade A hospitals from February to November 2018 were selected as the research subjects. The incidence of malnutrition, laboratory indicators on admission and discharge, and the incidence of significant weight loss during hospitalization were investigated. Malnutrition was assessed by Patient-Generated Subjective Global Assessment Scale (PG-SGA). Results? Among 380 cancer patients receiving radiotherapy, 48.9% (186/380) suffered from or suspected malnutrition at admission, of which 7.6% (29/380) suffered from severe malnutrition. At discharge, 64.2% (244/380) of radiotherapy patients developed or suspected malnutrition, of which 18.9% (72/380) suffered from severe malnutrition. In addition, 56.3% (214/380) patients lost weight during radiotherapy, and 12.4% (47/380) patients showed significant weight loss. Compared with that at admission, the levels of serum albumin and hemoglobin decreased significantly (P＜0.05). Conclusions The incidence of malnutrition is high in cancer patients during radiotherapy. Early nutritional assessment should be given at admission and early nutritional intervention should be given to malnutrition patients.
