A Phase II Trial of Postoperative Radiation Therapy of Gastric Cancer Patients Basing on a New Method of Lymph Node Target Volume Delineation: the Study Protocol, Toxicity and Efficacy

Abstract:The current lymph node target volume delineation for post-operative radiation therapy of gastric cancer is built on the traditional division system of stomach and the 2D treatment methods. In this study, we proposed a new delineation approach for lymph nodes station for postoperative gastric cancer. In addition, irradiation indications of each station were also provided. Furthermore, we conducted a phase II clinical trial to evaluate the efficacy and safety of this strategy. Fifty four Patients with D2 lymph node dissection in gastric cancer received two cycles of FOLFOX4 (Oxaliplatin 85 mg/m2 IV infusion day 1; LV 200 mg/m2 IV infusion days 1-2 in 2 hours; 5-FU 400 mg/m2 IV bolus days 1-2, followed by 5-FU 600 mg/m2 continuous IV infusion in 22 hours days 1-2 every two weeks). Then they received concurrent chemo-radiation therapy (CCRT: 45 Gy at 1.8 Gy per fraction, 5 fractions per week, for 5 weeks) with 5-FU/Leucovorin regiment (5-FU 400 mg/m2 IV bolus + leucovorin 20 mg/m2 IV bolus per day on the first 4 and the last 3 days of radiation therapy), followed by four additional cycles of FOLFOX4. Target volume included remnant stomach, anastomosis site, tumor bed and selective regional lymph nodes basing on the methods we proposed by taking gastric arteries as reference. The most common Grade 3 to 4 adverse events were neutropenia (14.8%). Neutropenia, anemia and nausea were common Grade 1 to 2 toxicities. No treatment related death occurred during the treatment. The 3-year locoregional recurrence-free survive (LRFS) was 91.1%. Eight patients developed peritoneal or distant metastases. The 3-year DFS and OS were 70.2% and 81.6%, respectively. It was feasible and well tolerated to delineate the target volume of post-operative lymph nodes stations in patients with gastric cancer after D2 resection based on a new delineation method and irradiation indication.
